Compression therapies for the treatment of venous leg ulcers: a mixed method process evaluation in a randomised controlled trial, VenUS6.

BACKGROUND: Strong compression treatment is an evidence-based first-line treatment that helps reduce the healing time of venous leg ulcers. VenUS 6 is a three-arm RCT conducted in the UK that aimed to compare 4-layer bandages and 2-layer hosiery with compression wraps and 2-layer bandages. The study found that compression wraps are unlikely to reduce time to venous leg ulcer healing compared to the other treatments evaluate, although there was some uncertainty with this estimate. We nested a mixed-methods process evaluation within VenUS 6 to gain a more in-depth understanding of trial particip

Evaluating the Role of Lower Extremity Elastic Bandage in Patients with Acute Decompensated Heart Failure.

BACKGROUND: Lower extremity compression therapy has been proposed as an adjunct to mitigate diuretic resistance during acute decompensation of heart failure (ADHF). We evaluated the effect of lower leg compression with elastic bandages on diuretic efficiency in patients with diuretic-resistant ADHF. METHODS: Historical cohort study enrolling adult cardiac ICU admissions with ADHF who used elastic bandages (EB) at Mayo Clinic in Minnesota from 2007 to 2017. The diuretic efficiency before and after applying EB was the primary outcome. RESULTS: A total of 176 patients with heart failure who rec

Effectiveness of Mobiderm® bandages in the treatment of cancer-related secondary lymphedema: A pilot study.

Secondary lymphedema is a clinically incurable disease that commonly occurs following surgical cancer treatment and/or radiation. One of the most common forms of lymphedema treatment is complete decongestive therapy (CDT). This study aimed to investigate the clinical effects of new compression bandages (Mobiderm® bandages) in patients with secondary lymphedema after cancer treatment. This study included 17 patients with ipsilateral limb lymphedema after cancer treatment (one male and 16 female patients; age, 45-80 years). Patients were divided into the Mobiderm® bandage group (n = 9) and class

Survey of registered nurses' selection of compression systems for the treatment of venous leg ulcers in the UK.

BACKGROUND: Venous leg ulceration is common in older adults in the United Kingdom. The gold-standard treatment is compression therapy. There are several compression bandage and hosiery systems that can be prescribed or purchased, but it was unclear what types of compression systems are currently being used to treat venous leg ulceration within the UK. This online scoping survey of registered nurses sought to (1) to identify what compression systems are available across the UK, (2) how frequently these are in use and (3) if there are any restrictions on their use. RESULTS: The results showed t

Design and evaluation of a device for measurement of interface pressure.

An electropneumatic device has been designed to measure the interface pressure profile under compression bandages. The device uses commercially available pneumatic sensors (Talley Group Ltd, SJ235/3) and measures interface pressure at up to eight sites simultaneously along the lower limb, with an accuracy of +/- 3 mmHg. Measurements can be made in one of two modes: continuously at a rate of up to three samples per second with the results output to a suitable display device, or single measurements of interface pressure can be made and displayed on a digital display incorporated in the device. Cohesive short stretch bandages in the treatment of venous leg ulceration.

These two case studies report how a cohesive short-stretch bandage was employed to promote venous leg ulcer healing. The two patients were obese and in addition had champagne bottle shaped legs. The cohesive short-stretch bandage was applied following the shape of the leg. By using a cohesive short-stretch bandage this provided bandage stability. This sustained compression resulted in oedema and pain reduction and promoted wound healing.

Evaluation of Pressure-Applying Accuracy in Multilayer Bandaging Among Physiotherapists Who Treat Lymphedema.

BACKGROUND: Compression therapy is the only conservative treatment that has been shown to be effective for patients with lymphedema in randomized controlled trials. A commonly used classification system divides pressure into four categories ranging from mild to very strong. For lower limb edema, bandages with strong pressure were shown to be most effective. Recent studies indicate that many health care professionals use inadequate pressure. No study thus far has focused on therapists who are trained in lymphatic treatment. METHODS AND RESULTS: Thirty-seven physiotherapists trained in lymphati

[Adverse effects of compression in treatment of limb lymphedema].

INTRODUCTION: Limb lymphedema, whether primary or secondary, is a chronic disease. Compression is the cornerstone of therapy and includes multilayer low-stretch bandages and elastic garments. Compression is usually well-tolerated. The aim of our study was to identify all the different types of adverse effects of compression. MATERIALS AND METHODS: Since January 2005, we have recorded all adverse events occurring in outpatients and inpatients consulting in a single lymphology department, spontaneously reported by patient during consultations or physical examinations, and noted the type of comp

Implementing a Locally Made Low-Cost Intervention for Wound and Lymphedema Care in Western Kenya.

In Western Kenya, the burden of chronic wounds and lymphedema has a significant impact on functionality and quality of life. Major barriers to provision of care include availability, affordability, and accessibility of bandages. At the Academic Model Providing Access to Healthcare, dermatologists and pharmacists collaborated to develop a 2-component compression bandage modeled after the Unna boot, using locally available materials, that is distributed through a revolving fund pharmacy network. In partnership with nursing, use of these bandages at a national referral hospital and a few county f

Fibre Bragg Grating Based Interface Pressure Sensor for Compression Therapy.

Compression therapy is widely used as the gold standard for management of chronic venous insufficiency and venous leg ulcers, and the amount of pressure applied during the compression therapy is crucial in supporting healing. A fibre optic pressure sensor using Fibre Bragg Gratings (FBGs) is developed in this paper to measure sub-bandage pressure whilst removing cross-sensitivity due to strain in the fibre and temperature. The interface pressure is measured by an FBG encapsulated in a polymer and housed in a textile to minimise discomfort for the patient. The repeatability of a manual fabricat

Feasibility and effectiveness of basic lymphedema management in Leogane, Haiti, an area endemic for bancroftian filariasis.

BACKGROUND: Approximately 14 million persons living in areas endemic for lymphatic filariasis have lymphedema of the leg. Clinical studies indicate that repeated episodes of bacterial acute dermatolymphangioadenitis (ADLA) lead to progression of lymphedema and that basic lymphedema management, which emphasizes hygiene, skin care, exercise, and leg elevation, can reduce ADLA frequency. However, few studies have prospectively evaluated the effectiveness of basic lymphedema management or assessed the role of compressive bandaging for lymphedema in resource-poor settings. Velcro compression wraps as an alternative form of compression therapy for venous leg ulcers: a review.

The first-line treatment for venous leg ulcers (VLUs) is compression therapy, most commonly, with compression bandages. A similar treatment measure is used for lymphoedema in the form of Velcro compression wraps (VCWs). However, the use of VCWs for VLUs is less evident, and a direct comparison to compression bandaging is not evident. This review explores the evidence to support the use of VCWs for the treatment of VLUs in order to raise awareness of alternative forms of compression therapy. Nine primary research studies were analysed, from which four key themes emerged: quality of life, cost o
