A Case Report of Humerus Shaft Osteomyelitis Secondary to Local Skin Infection Treated with Saucerization.

INTRODUCTION: Focal myositis is a rare condition first described by Heffner et al., in 1977, as a self-limiting condition of unknown aetiology. It presents as an inflammatory pseudo tumour in skeletal muscle and can present diagnostic difficulty, being commonly mistaken for tissue of vascular, inflammatory, or neoplastic origin. Diagnosis is traditionally confirmed by muscle biopsy. We present a case where magnetic resonance imaging (MRI) was used to confirm the diagnosis without need for biopsy. CASE PRESENTATION: A 19-year year-old female presented with a two2-year history of intermittent s

Can intraoperative neurophysiologic monitoring during cervical spine decompression predict post-operative segmental C5 palsy?

BACKGROUND: C5 nerve root palsy is a known complication after cervical laminectomy or laminoplasty, characterized by weakness of the deltoid and bicep brachii muscles. The efficacy of intraoperative monitoring of these muscles is currently unclear. In the current prospective study, intraoperative monitoring through somatosensory (SSEPs), motor (TcMEPs) evoked potentials and real-time electromyography activity (EMG) were analyzed for their ability to detect or prevent deltoid muscle weakness after surgery. METHODS: One hundred consecutive patients undergoing laminectomy/laminoplasty with or wi

Humeral surface anatomy and percutaneous plate advancement: a cadaveric study.

OBJECTIVE: The aim of this study was to identify the anatomical obstacles on the humeral surface which can complicate subcutaneous plate advancement during surgical treatment of humeral fractures. METHODS: We dissected twelve upper extremities of six male cadavers, and measured the humeral length, which was defined as the distance between the greater tubercle and the lateral epicondyle. We performed a retrograde advancement of a 4.5 mm plate through the subbrachial tunnel and noted the mechanical obstacles during the procedure. In addition, we recorded the distances between the anatomic obsta

Shoulder muscle moment arms during horizontal flexion and elevation.

The instantaneous muscle moment arms of 10 shoulder muscles including the three portions of the deltoid and the rotator cuff and scapulohumeral muscle groups during four specified glenohumeral motions were calculated. Moment arm values were derived from a plot of tendon excursion versus glenohumeral joint rotation angle during horizontal flexion along the 90 degrees elevation plane and elevation in the sagittal, scapular, and coronal planes. The deltoid had the largest moment arm in elevation. The anterior deltoid has a larger moment arm in the anterior planes, whereas the midportion is greate

Changes in shoulder muscle activities and glenohumeral motion after rotator cuff repair: an assessment using ultrasound real-time tissue elastography.

BACKGROUND: Although rotator cuff repair is performed to restore the function of the rotator cuff muscles and glenohumeral (GH) joint motion, little has been known regarding the recovery process. The purpose of this study was (1) to investigate changes over time in activities of the supraspinatus and deltoid muscles assessed by ultrasound real-time tissue elastography (RTE) after rotator cuff repair and (2) to determine contributions of the activities of these muscles to the GH joint motion. METHODS: Twenty patients after rotator cuff repair and 13 control participants were enrolled in this s

Aging Accelerates Degradation of Human Neuromuscular Junction Following Peripheral Nerve Injury.

INTRODUCTION/AIMS: Clinical observations indicate differences between young versus older adults in outcomes of peripheral nerve injury (PNI). Here, we present an analysis of motor endplates (MEPs) from normal and denervated deltoid muscle biopsies from young adult (≤ 60 years) and older (> 60 years) patients. METHODS: Under an IRB-approved prospective cohort study protocol, 24 patients who underwent open surgery for either shoulder pathology or deltoid dysfunction secondary to axillary nerve injury, confirmed with electrodiagnostic studies, were evaluated. Deltoid muscle biopsies were procure

Moment arms of the shoulder musculature after reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty is known to increase the moment arm of the middle subregion of the deltoid during shoulder abduction; however, at present, comprehensive data regarding the shoulder muscle moment arm through the full range of abduction and flexion are not available. The purpose of this study was twofold: (1) to measure the instantaneous moment arms of thirteen subregions of major muscles spanning the glenohumeral joint during abduction and flexion of the shoulder after reverse total shoulder arthroplasty and (2) to compare these data with the muscle moment arms p

The impact of rotator cuff muscle volume on functional outcomes following reverse total shoulder arthroplasty: the use of a validated muscle segmentation software.

BACKGROUND: The indications for reverse total shoulder arthroplasty (rTSA) have progressively expanded, and it is now implanted for a wide range of clinical scenarios, including cases with an intact rotator cuff. The objective of this study was to use a validated computed tomography (CT) muscle segmentation software to evaluate the relationship of rotator cuff muscle volume and postoperative clinical outcomes. MATERIALS AND METHODS: A retrospective study was conducted using an institutional database including all patients who underwent rTSA between 2016 and 2022, with a minimum of two years o

Relief increases select shoulder muscle activity, following subacromial injection in subacromial pain syndrome.

Subacromial pain syndrome (SAPS) is commonly linked to disrupted neuromuscular function, yet the effects of pain reduction on shoulder muscle activity remains unclear. This study examined how immediate and long-term (six weeks) decreases in shoulder pain influence muscle activation during humeral elevation in individuals with SAPS. Twenty-one participants with clinically diagnosed SAPS completed three sessions of humeral elevation at: baseline (T1), immediately after a subacromial injection (T2), and after six weeks of physical therapy (T3). Electromyography (EMG) was recorded from eight shoul

The sub-deltoid approach to the metaphyseal region of the humerus.

In a new surgical approach to the proximal part of the humerus, used in eighteen patients, the distal part of the tendon of the deltoid muscle was divided and the whole muscle was lifted up to give an excellent view of the proximal one-third of the diaphysis, the metaphysis, the surgical neck, and both tuberosities of the humerus. The exposure allowed visualization and protection of the axillary nerve. Postoperatively the deltoid remained functional and only the preoperative condition of the joint seemed to prevent full recovery of function of the muscles in a few patients.

Atrophy, inducible satellite cell activation, and possible denervation of supraspinatus muscle in injured human rotator-cuff muscle.

The high frequency of poor outcome and chronic pain after surgical repair of shoulder rotator-cuff injury (RCI) prompted this study to explore the potential to amplify muscle regeneration using nitric oxide (NO)-based treatment. After preoperative magnetic resonance imaging (MRI), biopsies of supraspinatus and ipsilateral deltoid (as a control) were collected during reparative surgery for RCI. Muscle fiber diameter, the pattern of neuromuscular junctions observed with alpha-bungarotoxin staining, and the γ:ε subunit ratio of acetylcholine receptors in Western blots were examined in tandem with

In vivo pediatric shoulder muscle volumes and their relationship to 3D strength.

In the pediatric shoulder, injury and pathology can disrupt the muscle force balance, resulting in severe functional losses. As little data exists pertaining to in vivo pediatric shoulder muscle function, musculoskeletal data are crucially needed to advance the treatment of pediatric shoulder pathology/injury. Therefore, the purpose of this study was to develop a pediatric database of in vivo volumes for the major shoulder muscles and correlate these volumes with maximum isometric flexion/extension, internal/external rotation, and abduction/adduction joint moments. A methodology was developed

Electromyographic study of the upper extremity during bilateral sanding: unresisted and resisted conditions.

Electromyographic information was obtained from seven right shoulder complex muscles in nine subjects (three normal, three paraplegic, and three quadriplegic) during the occupational therapy activity of bilateral sanding on an incline board, an activity that has been recommended as a treatment modality to strengthen the triceps brachii. Electromyography revealed that the anterior and middle portions of the deltoid were the muscles most responsible for the subjects' arm movements during both resisted and unresisted bilateral sanding . Triceps lateralis and medialis were also active during the u

Biomechanical comparison of 3 latissimus dorsi transfer sites for reverse total shoulder arthroplasty in the absence of teres minor.

BACKGROUND: Reverse total shoulder arthroplasty is often performed to treat rotator cuff tear arthropathy with irreparable rotator cuff tears. Patients with full-thickness tears involving the posterior cuff and teres minor specifically lose active external rotation of the humerus, limiting activities of daily living. A latissimus dorsi transfer has been described as one potential solution, but few studies have compared different latissimus dorsi transfer sites. This study assesses the biomechanics of 3 latissimus dorsi transfer sites, examining external rotation, deltoid muscle forces, and for

Transfer of axillary nerve branches to reconstruct elbow extension in tetraplegics: a laboratory investigation of surgical feasibility.

In spinal cord injuries at the C6 level, elbow extension is lost and needs reconstruction. Traditionally, elbow extension has been reconstructed by muscle transfers, which improve function only moderately. We have hypothesized that outcomes could be ameliorated by nerve transfers rather than muscle transfers. We anatomically investigated nerve branches to the teres minor and posterior deltoid as donors for transfer to triceps motor branches. In eight formalin-fixed cadavers, the axillary nerve, the teres minor branch, the posterior deltoid branch, the triceps long and upper medial head motor b

Bipolar latissimus dorsi flap transfer for reconstruction of the deltoid.

Four patients with complete paralysis of the deltoid muscle with irrepairable axillary nerve lesion and one with traumatic destruction of the shoulder cap were treated with a bipolar latissimus dorsi muscle transfer. The latissimus dorsi muscle was prepared as an island flap, turned over and sutured to the trapezius proximally and to the original deltoid insertion on the humerus distally. Function and cosmesis were excellent in two patients; in one case the goal was only to achieve soft-tissue coverage, and was successfully achieved. One patient only had partial survival of the muscle and in o
