Use of PEAK PlasmaBlade in implant-based breast reconstruction and radiotherapy: new strategy to reduce complications.

BACKGROUND: Implant-based breast reconstruction in the setting of radiotherapy often leads to higher complications rates (mainly capsular contracture and wound dehiscence) and poor cosmetic outcomes. We hypothesized that the combination of pulsed-electron avalanche knife (PEAK) PlasmaBlade (a pulsed radiofrequency electrosurgery) and acellular dermal matrix Veritas® in postmastectomy radiotherapy implant-based breast reconstruction could result in lower complications rate, better reconstructive results, and patient satisfaction. METHODS: A prospective observational study focused on the use of

A rare case report of grade III gingival enlargement associated with chronic periodontitis: Comparison of two treatment techniques.

Idiopathic gingival enlargement (GE) is a rare entity characterized by massive enlargement of the gingiva. It may be associated with other diseases/conditions characterizing a syndrome, but rarely documented in literature occurring along with chronic periodontitis. This case report describes a rare case of long standing massive GE in a systemically healthy, nonsyndromic young female involving both the arches, thereby posing a diagnostic dilemma. Furthermore, in this case, we compared two surgical approaches, that is, scalpel and electrosurgery for the convenience as well as the postoperative c

New technique using combined pulsed dye laser and fractional resurfacing for treating facial angiofibromas in tuberous sclerosis.

BACKGROUND AND OBJECTIVE: Tuberous sclerosis (TS) is a well-described genetic disorder that is classically associated with up to hundreds of facial angiofibromas. Due to the progressive nature of the skin lesions and the early clinical presentation, a safe and effective technique for treating these disfiguring skin lesions is needed. STUDY DESIGN/PATIENTS AND METHODS: We report a combinatorial technique for treating the angiofibromas of TS. We present a case series of three TS patients treated with this technique combining pinpoint electrosurgery, pulsed-dye laser treatment, and ablative frac

The effect of laparoscopic salpingectomy for ectopic pregnancy on ovarian reserve.

BACKGROUND: Salpingectomy may damage ovarian reserve by direct vascular interruption to the ovary or thermal vascular injury from electrosurgery. It is plausible that this risk may increase in the context of salpingectomy conducted for ectopic pregnancy due to the distension of the fallopian tube and vascular changes associated with pregnancy. AIM: To report anti-Müllerian hormone (AMH) concentrations before and after laparoscopic salpingectomy for ectopic pregnancy as an indicator of change in ovarian reserve. MATERIALS AND METHODS: Women aged 18-44 years scheduled for salpingectomy for tub

Risk factors for uterine rupture after laparoscopic myomectomy.

Case reports for uterine rupture subsequent to laparoscopic myomectomy were reviewed to determine whether common causal factors could be identified. Published cases were identified via electronic searches of PubMed, Google Scholar, and hand searches of references, and unpublished cases were obtained via E-mail queries to the AAGL membership and AAGL Listserve participants. Nineteen cases of uterine rupture after laparoscopic myomectomy were identified. The removed myomas ranged in size from 1 through 11 cm (mean, 4.5 cm). Only 3 cases involved multilayered closure of uterine defects. Electrosu

Clip Liberation to Facilitate Transcatheter Tricuspid Valve Replacement (CLEFT).

BACKGROUND: Transcatheter tricuspid valve edge-to-edge repair (T-TEER) can be effective for patients with tricuspid regurgitation (TR), but TR may recur, requiring transcatheter tricuspid valve replacement (TTVR). Although TTVR has been described in patients with single leaflet device attachment of edge-to-edge devices, implantation in presence of well-attached devices has not been described. CASE SUMMARY: We present a patient with recurrent TR after T-TEER. Transesophageal echocardiography revealed that 3 previously placed MitraClip XTW devices were well attached to the tricuspid leaflets, w

A uniQue wAy of Transcatheter mAss extraction: The QATAR Procedure.

BACKGROUND: Intracardiac masses traditionally require surgical resection, but transcatheter techniques offer a less invasive alternative. CASE SUMMARY: A 57-year-old woman with rheumatic heart disease and severe mitral stenosis was evaluated for percutaneous mitral commissurotomy. Imaging revealed a mobile right atrial mass near the inferior vena cava, posing a risk for embolization. Given the need for mass removal before the procedure, we performed the QATAR technique using the Penumbra Lightning 16 system, a loop snare, and electrocautery. The mass was successfully extracted without complic

Effect of pre- and post-weaning high-fat dietary manipulation on intestinal microflora and alkaline phosphatase activity in male rats.

Recurrent respiratory papillomatosis (RRP) is a rare disease, characterized by recurrent proliferation of benign squamous cell papillomas in the larynx as well as in the other parts of the aerodigestive tract. We have compared different treatment options for RRP of the aerodigestive tract including surgical, conservative and combined approaches. A total of 43 patients with papillomatosis that received a combined therapy were followed in the period from 2009 to 2013. The treatment included electrosurgery and CO2 laser surgery alongside with either immunotherapy with Bacillus Calmette-Guerin (BC

Laparoscopic Radical Hysterectomy: A Preliminary Experience.

From June 1993 through September 1993, we had performed four cases of laparoscopic radical hysterectomy. All these patients had early cervical squamous cell carcinoma of less than 4 cm in diameter. We first developed the left paravesical and pararectal space with the suction-irrigator probe and Endoretractor (Autosure, USA). Then we desiccated the left uterine artery at its origin. The left ureter was dissected from the point it enters the pelvis to the ureteric canal. The left cardinal ligament was completely divided twice by the EndoGIA (Autosuture, USA). The rectovaginal space was opened an

INFLUENCE OF A VARIETY OF SUTURE MATERIAL ON THE ANAL CANAL WOUNDS HEALING AFTER COMBINED OPERATIONS CONCERNING THE COMBINED ANORECTAL PATHOLOGY WITH USING OF MODERN TECHNOLOGIES.

The urgency of the problem of combined pathology of the anal canal and rectum is quite high due to the lack of a unified approach to surgical treatment of this category of patients. The aim of the study was to conduct a comparative morphological assessment of postoperative wound healing in patients with combined anorectal pathology after combined operations using different types of suture material, as well as modern high-frequency electrosurgery and radio-wave surgery devices. The dynamics of the wound process under the influence of caprosin (3/0) and polysorb (3/0) was performed on 60 patient

Predictors for Using Electricity During Hysteroscopic Removal of Retained Products of Conception.

Background: Retained products of conception (RPOC) can be managed via hysteroscopic removal using mechanical or electrosurgical techniques. Electrosurgery introduces greater technical complexity and may reflect more adherent or vascular tissue, yet preoperative predictors for its necessity remain poorly defined. Objective: The objective of this study was to evaluate clinical outcomes and identify preoperative predictors associated with the use of electrosurgery during hysteroscopic removal of RPOC. Methods: In this retrospective cohort study conducted at a single tertiary center, we reviewed 5

Managing Different Spectrums of Tuberous Sclerosis Facial Angiofibroma: A Report of Two Cases.

Tuberous sclerosis is one of the rarest genetically linked disorders that can affect a multitude of body systems in various forms. Patients with facial angiofibroma may face issues arising from the various modalities and approaches that can be applied. Moreover, surgeons also face challenges in preparing patients for specific interventions. Here, we are reporting the two spectrums of this condition that may present, along with how we managed patients in our center. The first case is a severe form of tuberous sclerosis involving the skin as well as neurological manifestation, while the second c
