Cerebral protection devices for transcatheter aortic valve replacement.

Stroke is a devastating, potential complication of any cardiovascular procedure including transcatheter aortic valve implantation (TAVI). Even clinically silent lesions as detected by magnetic resonance imaging have been associated with poor long-term cognitive outcomes. As a result, extensive efforts have been focused on developing stroke preventative strategies including the development of novel embolic protection devices. These devices aim to reduce this risk by capturing or deflecting emboli away from the cerebral circulation. Areas covered: This review provides an insight into the inciden

Neurologic Complications after Transcatheter Aortic Valve Replacement: An Updated Review of Incidence, Current Treatments, and Future Directions.

Transcatheter aortic valve replacement (TAVR) is the standard treatment for severe aortic stenosis, yet stroke remains a persistent complication with significant morbidity. This review summarizes the incidence, timing, mechanisms, and predictors of TAVR-related stroke, highlighting procedural embolization and patient comorbidities. We examine evidence for cerebral embolic protection devices, pharmacologic strategies, and left atrial appendage closure in mitigating risk. Despite advances in technology, randomized trials have not shown consistent stroke reduction with embolic protection. Optimal

Endovascular recanalization of the chronically occluded brachiocephalic and subclavian arteries: technical considerations and an argument for embolic protection.

OBJECTIVE: We described our endovascular techniques for the recanalization of chronic occlusions of the brachiocephalic and subclavian arteries. Given their large caliber, origins from the aorta, and proximity to the carotid and vertebral arteries, various complex endovascular techniques are required to achieve revascularization. Navigation of distal embolic protection devices into the carotid and vertebral arteries mandates varied catheter approaches. METHODS: Four women harboring chronic occlusions of the brachiocephalic and subclavian arteries were evaluated for their presenting symptoms,

"No-reflow" phenomenon during percutaneous carotid angioplasty in a scleroderma patient.

Atherosclerosis remains the most common etiology of carotid artery stenosis, but other systemic diseases can also cause this disease. Percutaneous carotid angioplasty and stenting (CAS) with and without distal embolic protection devices is gaining acceptance as the treatment of choice. No-reflow phenomenon due to distal embolization has been previously described mainly during percutaneous intervention of degenerated saphenous vein graft. We describe a patient with systemic sclerosis who underwent CAS with distal embolic protection device with occurrence of no-reflow phenomenon during the proce

Retrieval of Thrombosed Inferior Vena Cava Filter and Mechanical Thrombectomy of Associated Caval Thrombosis Using FlowTriever and ClotTriever Systems With the ClotTriever as Embolic Protection.

A 55-year-old Chinese male with anti-phospholipid syndrome presented with left lower limb deep venous thrombosis (DVT) on warfarin and had an inferior vena cava filter (IVCF) implanted before a radical resection of a perineal sarcoma. The inferior vena cava (IVC) filter removal was attempted 2 weeks after the surgery, but thrombus was found at the filter. It was decided to leave the filter and to continue with anticoagulation. He presented with progressive right lower limb swelling, erythema and pain, with a CT venogram showing caval thrombosis extending from the IVC filter to the right below-

Proximal versus distal embolic protection for carotid artery stenting: a national cardiovascular data registry analysis.

OBJECTIVES: The aim of this study was to compare the stroke/death rates between proximal embolic protection devices (P-EPDs) and distal filter embolic protection devices (F-EPDs) in elective carotid artery stenting (CAS). BACKGROUND: P-EPDs have theoretical advantages that may make them superior to F-EPDs for stroke prevention during CAS. METHODS: We examined 10,246 consecutive elective CAS procedures performed with embolic protection in the NCDR CARE registry between January 2009 and March 2013. We analyzed crude and propensity-matched rates of in-hospital combined death/stroke in patients

Feasibility and Safety of Distal and Proximal Combined Endovascular Approach with a Balloon-Guiding Catheter for Subclavian Artery Total Occlusion.

BACKGROUND: Symptomatic subclavian artery total occlusion is widely treated with an endovascular procedure that often results in distal vertebral artery embolism. Therefore, protection devices are important. Establishing a filter or balloon device in the vertebral artery can protect against this distal embolism. However, the use of embolic protection devices is not easy, and it makes the procedure more complicated. Here, we report a case of symptomatic subclavian artery total occlusion that was treated successfully with a balloon-guiding catheter and the pull-through technique. CASE DESCRIPTI

Enhancing Stroke Prevention in Transcatheter Aortic Valve Replacement: The Role of F2, a Novel Neuroprotection Device.

PURPOSE: Transcatheter aortic valve replacement (TAVR) is an established treatment for severe aortic stenosis; however, it carries the risk of periprocedural strokes. Current cerebral embolic protection (CEP) devices, such as the Sentinel, provide partial protection but are limited by inadequate anatomical coverage and inability to capture smaller emboli effectively. This study aimed to evaluate the effectiveness of a novel CEP device, the F2 filter with a 28 μm pore size and full cervical vessel coverage, in preventing emboli from entering the cerebral circulation. METHODS: The Sentinel and

Effect of percutaneous coronary interventions for in-stent restenosis in degenerated saphenous vein grafts without distal embolic protection.

OBJECTIVES: This study was designed to investigate the impact of percutaneous coronary interventions (PCIs) in degenerated saphenous vein grafts (SVGs) without distal embolic protection. BACKGROUND: Distal embolic protection devices have been shown to reduce the incidence of no reflow/slow flow during PCI of de novo lesions in degenerated SVGs. It is unclear whether PCI of in-stent restenosis (ISR) lesions in degenerated SVGs is associated with no reflow/slow flow and whether distal embolic protection is beneficial in these cases as well. METHODS: We studied 54 consecutive patients with trea

Endovascular Extraction of an Aortic Arch Thrombus Using Dual Snares and Aspiration Thrombectomy.

BACKGROUND: Aortic mural thrombi are rare causes of embolic stroke and pose therapeutic challenges, particularly in critically ill patients. CASE SUMMARY: A 47-year-old man presented with middle cerebral artery infarction. Imaging revealed a large, mobile mass in the aortic arch, suspected as the embolic source. Surgical removal was contraindicated owing to cerebral hemorrhage, and an endovascular approach was chosen. Using a cerebral protection device, dual snares, and a large-bore aspiration catheter, the thrombus was successfully removed without procedural complications. Histology confirme

Successful TAVR for a Patient with Bicuspid Aortic Valve Stenosis and Previous Type A Aortic Dissection.

Whether transfemoral transcatheter aortic valve replacement (TAVR) can be accomplished in patients with a bicuspid aortic valve (AV) and previous type A aortic dissection remains rarely addressed. We report such a case in whom transfemoral TAVR was smoothly performed employing an extraordinary long sheath to bypass the dissected aortic segment, yet at the cost of perioperative left temporoparietal infarction owing to no suitable cerebral embolic protection device available in the presence of a remaining dissection in the aortic arch. Thus, for such patients, transfemoral TAVR is still feasible

Systematic review: comparative effectiveness of adjunctive devices in patients with ST-segment elevation myocardial infarction undergoing percutaneous coronary intervention of native vessels.

BACKGROUND: During percutaneous coronary intervention (PCI), dislodgement of atherothrombotic material from coronary lesions can result in distal embolization, and may lead to increased major adverse cardiovascular events (MACE) and mortality. We sought to systematically review the comparative effectiveness of adjunctive devices to remove thrombi or protect against distal embolization in patients with ST-segment elevation myocardial infarction (STEMI) undergoing PCI of native vessels. METHODS: We conducted a systematic literature search of Medline, the Cochrane Database, and Web of Science (J

Carotid stenting and transcranial Doppler monitoring: indications for carotid stenosis treatment.

BACKGROUND: Recently, angioplasty and stenting of carotid arteries (CAS) have taken the place of surgery. The aim of our study is to assess the role of transcranial Doppler (TCD) monitoring during CAS to address the embolic complications during the stages of the procedure, with or without embolic cerebral protection devices. METHODS: A total of 152 patients were submitted to carotid stenting. All patients were submitted to carotid arteries Duplex scanning. RESULTS: Neurological complications are related to TCD detection of corpuscolate signals in rapid succession. Even if no reduction of the

Percutaneous revascularization of subclavian artery chronic occlusion with dual cerebral artery protection.

Percutaneous intervention for symptomatic subclavian artery chronic occlusion is an occasionally performed, minimally invasive alternative to surgical bypass. Potential complications include stroke, perforation, and recanalization failure. We discuss a case of successful percutaneous revascularization of symptomatic subclavian artery chronic occlusion using coronary devices and dual cerebral embolic protection.

CaRotid Artery Filtering Technique (CRAFT): A Technique for Carotid Artery Stenting with Intrinsic Embolic Protection.

PURPOSE: Carotid artery stenting (CAS) is an established treatment for symptomatic carotid artery stenosis as an alternative to carotid endarterectomy. A variety of techniques and devices have been devised to minimise periprocedural stroke risk using either proximal or distal embolic protection. This study presents a method of embolic protection during CAS-the CaRotid Artery Filtering Technique (CRAFT). MATERIALS AND METHODS: The CRAFT technique employs aspects of both proximal and distal embolic protection. The CASPER RX stent (MicroVention, Tustin, CA, USA), which is a double-layered, close

An overview of modern embolic protection in percutaneous coronary intervention.

Coronary embolism due to atherosclerotic debris is a rather common cause of post-procedural complications. While evidence has shown that both arteriolar vasodilators and platelet glycoprotein inhibitors have proven ineffective against post- and peri-procedural embolism,5 mechanical interventional devices have been shown to improve (lower) 30-day MACE rates. These interventions include distal filtration, distal, and proximal occlusion balloons. The distal occlusion balloon was the first approach to embolic protection. The intervention involves placement of a low pressure (<2 atm) balloon distal

[Devices and Techniques for Transarterial Embolization of Dural Arteriovenous Fistulas: Current Trends in the Use of Liquid Embolic Agents and Microcatheters].

Transarterial embolization (TAE) of dural arteriovenous fistulas (DAVFs) has evolved significantly following insurance approval of liquid embolic agents such as OnyxTM and Histoacryl® (n-butyl-2-cyanoacrylate[NBCA]). Onyx allows prolonged injection and controlled penetration into the venous side, whereas NBCA offers rapid polymerization and thrombogenicity. The success of TAE depends on the selection of appropriate delivery devices. The ultra-fine microcatheter, DeFrictor® nano, provides superior distal access to small feeders near the shunt point, thereby facilitating effective embolization.
