[Two cases of lymph node recurrence after endoscopic mucosal resection of esophageal cancer].

Here, we reported two cases of lymph node recurrence after endoscopic mucosal resection of esophageal cancer. Case 1: A patient was a 49-year-old man. Endoscopic mucosal resection (EMR) was performed to the 0-IIc type esophageal cancer in October 2005. The pathological findings were moderately differentiated squamous cell carcinoma, pT1b-SM3, ly0, and v0. Therefore, additional therapy of definitive chemoradiation was conducted. However, he complained a difficulty in swallowing in June 2007 and computed tomography (CT) revealed a lymph node measuring 4 cm in diameter at the right side of cardia

Extensive (8 to 12 cm2) noncircumferential endoscopic mucosal resection for early esophageal cancer.

BACKGROUND: Endoscopic mucosal resection (EMR) is an appealing method for treating intramucosal esophageal cancer but must comply with the following stringent requirements: proper preoperative staging, complete resection of the lesion, obtaining a resected specimen for histologic analysis of safety margins, and squamous reepithelialization without stricture formation. METHODS: A rigid esophagoscope was created to resect up to 12 cm(2) of esophageal mucosa in a single specimen and at a constant depth through the submucosa. Under visual control, the esophageal mucosa is sucked into a transparen

Combined endoscopic mucosal resection and photodynamic therapy for esophageal neoplasia within Barrett's esophagus.

BACKGROUND: Endoscopic mucosal resection (EMR) and photodynamic therapy have been proposed as treatments for early stage cancers. EMR is limited by its focal nature whereas photodynamic therapy is dependent on precise staging. The combination of EMR and photodynamic therapy were studied in the treatment of superficial cancer in patients with Barrett's esophagus. METHODS: Seventeen consecutive nonsurgical patients with superficial cancers underwent EMR followed by photodynamic therapy with a porphyrin photosensitizer. Photoradiation was performed at 630 nm for a total dose of 200 J/cm of diffu

Pathological appearance of the stomach after endoscopic mucosal resection for early gastric cancer.

BACKGROUND: The pathological findings of the resected stomach after endoscopic mucosal resection (EMR) for early gastric cancer were reviewed. EMR was indicated when a lesion consisting of well or moderately differentiated adenocarcinoma had a diameter of less than 2 cm. METHODS: Of 39 patients with early gastric cancer were treated with EMR between 1990 and 1995, 11 required additional surgery. RESULTS: Malignant tissue in the gastric wall was completely removed in four patients, while cancer cells remained in the mucosa adjacent to the scar in five and infiltrated into the submucosa in two

Successful Management of Colonic Angiodysplasia With Endoscopic Mucosal Resection and Argon Plasma Coagulation.

BACKGROUND Colonic angiodysplasia is an acquired arteriovenous malformation that mainly occurs in older patients and is routinely managed using argon plasma coagulation (APC). Among the available endoscopic treatment options for angiodysplasia, APC is the most widely used. However, repeat endoscopy may be required because of post-treatment rebleeding after APC. Furthermore, the high prevalence of angiodysplasia in the right colon complicates treatment with APC because ablation in the right colon is associated with a higher risk of gastrointestinal perforation. A well-documented consensus on th

Gel immersion endoscopic mucosal resection for small gastric neoplastic lesions: A pilot study.

Gastric endoscopic mucosal resection is challenging due to the slippery mucosa, abundant blood vessels, and the presence of mucus. We developed gel immersion endoscopy to secure the visual field, even in a blood-filled gastrointestinal lumen in 2016. Clear gel with appropriate viscosity, instead of water, can prevent rapid mixture with blood and facilitate identification of the culprit vessel. We further optimized the gel for endoscopic treatment, and the resultant product, Viscoclear (Otsuka Pharmaceutical Factory) was first released in Japan in 2020. The viscosity of this gel has been optimi

Efficacy of Underwater Endoscopic Mucosal Resection for Superficial Non-Ampullary Duodenal Epithelial Tumor.

BACKGROUND/AIMS: Endoscopic resection (ER) for superficial non-ampullary duodenal epithelial tumors (SNADETs) is challenging. Conventional endoscopic mucosal resection (CEMR) is also problematic due to the anatomical features of the duodenum. We compared the safety and efficacy of underwater endoscopic mucosal resection (UEMR) with those of CEMR through a retrospective analysis. METHODS: Altogether, 44 consecutive patients with 46 SNADETs underwent ER (18 CEMR cases and 28 UEMR cases) between January 2016 and October 2019. We investigated the proportions of en bloc resection, R0 resection, co

Enterococcus faecalis Endocarditis After Endoscopic Mucosal Resection of a Large Sessile Colonic Polyp.

A 71-year-old man with mitral regurgitation and apical cardiomyopathy underwent endoscopic mucosal resection of a 22-mm transverse colon tubulovillous adenoma with high-grade dysplasia. Six weeks later, he presented with fever, valvular vegetations, and positive blood cultures for Enterococcus faecalis. To our knowledge, this is the first reported case of endocarditis involving native heart valves after endoscopic mucosal resection.

Focal Intramucosal Adenocarcinoma Occurring in Gastric Hyperplastic Polyps Treated with Endoscopic Mucosal Resection.

Hyperplastic polyps are the most frequent benign epithelial gastric polyps. Although they are considered nonneoplastic, some cases have been reported with focal adenocarcinoma. We present the case of a 59-year-old woman with a sessile lesion of 15 mm on the distal gastric body associated with an extensive atrophic gastritis. Magnifying endoscopy with Fuji Intelligent Color Enhancement (FICE) revealed an irregular microsurface pattern at the apex, suggesting malignancy. A mucosectomy was performed. The histopathology revealed that the base corresponded to a hyperplastic polyp, where a tubular a

Underwater endoscopic mucosal resection of colorectal neoplasia is easily learned, efficacious, and safe.

BACKGROUND: Underwater endoscopic mucosal resection (UEMR) without submucosal injection is a novel endoscopic procedure. It is not known if UEMR can be easily taught and learned, and the efficacy and safety of UEMR has not been demonstrated at multiple medical centers. Our aims were to demonstrate that (1) UEMR is a technique that can be easily learned by an endoscopist trained in traditional EMR, (2) endoscopic ultrasound (EUS) may not be required before UEMR, and (3) UEMR is an efficacious and safe method for resection of large or flat neoplastic colorectal lesions. METHODS: An experienced

Feasibility of endoscopic mucosal resection for superficial pharyngeal cancer: a minimally invasive treatment.

BACKGROUND AND STUDY AIMS: New diagnostic techniques have recently been developed so detection of superficial pharyngeal cancer is dramatically increasing and endoscopic mucosal resection (EMR) can now be performed on an experimental basis. The aim of this study was to clarify the effectiveness of EMR for superficial pharyngeal cancer. PATIENTS AND METHODS: Between 2004 and 2007, 31 patients with 37 pharyngeal lesions underwent EMR at our hospital. EMR using a cap-fitted endoscope (EMR-C) was used on 34 lesions and strip biopsies on the remaining three. We retrospectively assessed the effecti

Pathological Resection Depth in Cold Snare Polypectomy and Underwater Endoscopic Mucosal Resection for Superficial Nonampullary Duodenal Epithelial Tumors.

INTRODUCTION: Cold snare polypectomy (CSP) and underwater endoscopic mucosal resection (UEMR) are used for small superficial nonampullary duodenal epithelial tumors (SNADETs). This study aimed to compare the resection depth of SNADETs removed by CSP versus UEMR. METHODS: Data from SNADETs resected with CSP or UEMR between April 2018 and July 2023 at Nagoya City University Hospital were collected retrospectively. An independent pathologist, blinded to clinical information, evaluated the resected specimens and categorized resection depth into four groups: mucosa only (M), incomplete muscularis

En bloc resection of intestinal-type adenoma extending to the minor duodenal papilla by underwater endoscopic mucosal resection with a forward-viewing endoscope.

We report successful en bloc underwater endoscopic mucosal resection (UEMR) of a lesion involving the minor duodenal papilla. A woman in her 60s presented with a 10-mm depressed lesion in the descending duodenum. White-light imaging and Magnifying endoscopy suggested a tumor involving a minor papilla. Although side-viewing endoscopes are conventionally used for the minor papilla, pre-procedural assessment indicated inadequate maneuverability for this specific lesion (ED-580T; Fujifilm Corporation, Tokyo, Japan). Consequently, we strategically used a forward-viewing dual-channel endoscope (GIF-

Efficacy and safety of precutting endoscopic mucosal resection versus endoscopic submucosal dissection for non-ampullary superficial duodenal lesions.

BACKGROUND: Endoscopic treatments for non-ampullary superficial duodenal lesions (NASDLs) are yet to be standardized. Endoscopic submucosal dissection (ESD) for NASDLs demands advanced techniques and a long procedure time to prevent perforation and bleeding. Precutting endoscopic mucosal resection (EMR) is a technical modification of ESD that overcomes the limitations of ESD. This study aimed to compare the efficacy and safety of precutting EMR versus ESD for NASDLs. METHODS: We conducted a retrospective analysis of patients with NASDLs treated with either precutting EMR or ESD from January 2
