[Essential facial neuralgia with bilateral involvement: apropos of a case seen at the Cité Verte Hospital in Yaoundé].

Despite the fact that essential facial neuralgia is a well known clinical entity as relates to its evolution and treatment, its physiopathology is still a controversial issue. The form with bilateral evolution that we observed and that we are reporting here is extremely rare and its physiopathology is even more controversial. The case of our patient is all the more complicated in that, the pain topography is limited to the right to a sensitive region of the trigeminal nerve (superior maxillary) and to the left to the region of the mandible (inferior maxillary) A similar disease evolution is no

Trigeminal evoked potentials and sensory deficits in atypical facial pain--a comparison with results in trigeminal neuralgia.

Trigeminal evoked potentials (TEPs) and sensory deficits in eighty-three patients admitted for first surgical treatment of facial pain were retrospectively analysed. Thirty-seven patients suffered from trigeminal neuralgia (TN), 10 from symptomatic TN (sTN), and 36 from atypical facial pain (AFP). Eighteen percent of the TN patients reported sensory deficits on the pain side, but 35% had delayed ipsilateral N13 waves. Of the sTN patients, 60% had either sensory deficits or a pathological corneal reflex and 62.5% a pathological N13. Of the AFP patients, 61% complained of sensory deficits, but o

[Progressive facial hemiatrophy (Parry-Romberg disease). Report of two cases associated with trigeminal neuralgia and cramps].

The authors report two cases of progressive facial hemiatrophy (PFH) associated with cranial nerves hyperexcitability. One of them (Case 1), a 41-year-old-woman noticed a slight atrophy on the left side of her face at age of ten. For the last three months, she has been suffering from fits of neuralgic pains on the left side of her face which have not ceased by drug treatment In view of this, a surgery procedure was planned for the patient. The other patient (Case 2), a 33-year-old-woman has a 8 year history which started with cutaneous abnormalities (white and dark spots). Two years later she

Association of vascular compression in trigeminal neuralgia versus other "facial pain syndromes" by magnetic resonance imaging.

Magnetic resonance imaging was performed in 13 patients with trigeminal neuralgia and 18 control patients with facial pain of other types. Among trigeminal neuralgia patients, eight (62%) had vessels seen abutting, or immediately adjacent to, the trigeminal nerve on the side of their symptoms. Only three control patients (17%) had such vessels. The proportion of patients having a trigeminal region vessel associated with their symptoms was significantly higher in patients with trigeminal neuralgia than in controls (p = 0.0086). These findings provide evidence supporting the theory that trigemin

Facial Pain Mimicking Trigeminal Neuralgia Secondary to Low-Cervical Spondylosis: A Case Report and Comprehensive Literature Review.

Trigeminal neuralgia (TN) is usually associated with neurovascular compression at the root entry zone. However, atypical presentations may result from secondary underlying pathologies. Cervical spondylosis (CS) is a rare cause, and its pathophysiologic mechanism leading to facial pain remains poorly understood. Here, we present a case of a 53-year-old woman with a one-year history of migraine episodes refractory to multiple medications. She subsequently developed sudden, severe left-sided trigeminal-like facial pain. Brain magnetic resonance imaging (MRI) excluded vascular compression or intra

Manual and automated capture of analgesic-responsive facial pain behaviors of unrestrained mice in a trigeminal neuralgia model.

Trigeminal neuralgia (TN), a devastating neuropathic pain condition, profoundly affects human well-being. Despite an initial response to carbamazepine (CBZ), many patients become refractory to pharmacotherapy and subsequent invasive procedures. Although preclinical trigeminal neuropathic pain models following extracranial chronic constriction injury (CCI) of the trigeminal nerve have been developed, the injury-induced facial hypersensitivity is rarely CBZ-responsive, questioning the models' relevance to TN. In recent reports, CBZ-responsive facial pain can be induced by intracranial CCI that b

Olfactory Ensheathing Cells Alleviate Facial Pain in Rats with Trigeminal Neuralgia by Inhibiting the Expression of P2X7 Receptor.

Trigeminal neuralgia (TN) is a common facial neuropathic pain that is mainly characterized by spontaneous or induced needling or electric shock pain in the innervation area of the trigeminal nerve. It is also referred to as "the cancer that never dies". The olfactory ensheathing cell (OEC) is a special glial cell in the nervous system that has a strong supportive function in nerve regeneration. Cell transplantation therapy is a useful treatment modality that we believe can be applied in TN management. In this study, OECs were transplanted into the ligation site of the infraorbital nerve of rat

Inhibitory Effects of Palmatine on P2X7 Receptor Expression in Trigeminal Ganglion and Facial Pain in Trigeminal Neuralgia Rats.

Trigeminal Neuralgia (TN) refers to recurrent severe paroxysmal pain in the distribution area of the trigeminal nerve, which seriously affects the quality of life of patients. This research applied the chronic constriction injury of the infraorbital nerve (CCI-ION) approach to induce an animal model of TN in rats. The mechanical pain threshold of each group of rats was determined postoperatively; the expression of P2X7 receptor in trigeminal ganglion (TG) was assessed by qRT-PCR, immunofluorescence and Western blot; and the changes of the proinflammatory cytokines IL-1β and TNF-α in serum of r

Severe postherpetic neuralgia and facial paralysis in the oral and periauricular regions managed with acupuncture and electroacupuncture: a case report.

Postherpetic neuralgia (PHN) is a severe and persistent pain condition following herpes zoster infection. This case report details the analgesic effects of acupuncture combined with electroacupuncture in a 66-year-old male patient presenting with PHN and peripheral facial paralysis, who showed limited response to conventional treatment with corticosteroids and antiviral medications. Following a comprehensive treatment protocol, including pricking-cupping bloodletting, and targeted acupuncture, the patient experienced significant pain relief and improved facial nerve function. This report highl

Differentiating Trigeminal Neuralgia From Persistent Idiopathic Dentoalveolar Pain and Persistent Idiopathic Facial Pain in Oral and Maxillofacial Surgery.

The diagnostic distinction between trigeminal neuralgia (TN), persistent idiopathic dentoalveolar pain, and persistent idiopathic facial pain remains a challenge for oral and maxillofacial surgeons. While neurovascular compression is often viewed as a definitive indicator for surgical intervention, recent evidence suggests that neurovascular compression is frequently an incidental finding in patients with nociplastic pain. This paper explores the nociplastic signature of persistent idiopathic dentoalveolar pain and persistent idiopathic facial pain, emphasizing that central sensitization and c

Risk factors of facial herpes simplex after percutaneous microballoon compression for trigeminal neuralgia: A retrospective case-cohort study.

BACKGROUND: Percutaneous microballoon compression (PMC) is an important clinical technique for the treatment of trigeminal neuralgia (TN). Some studies have shown that patients may be infected with herpes simplex virus type 1 (HSV-1) after surgery. However, the prevalence and associated risk factors are unclear yet. This study aimed to explore the potential risk factors of facial herpes simplex (FHS) in patients with TN treated by PMC retrospectively. METHODS: A retrospective study included 181 patients with TN undergoing PMC treatment between September 2019 and August 2020 in Sichuan Cancer

Effect of early stellate ganglion blockade for facial pain from acute herpes zoster and incidence of postherpetic neuralgia.

BACKGROUND: The incidence of postherpetic neuralgia (PHN) has been reported to be 25% among those over the age of 50 years treated with antiviral medication. The role of sympathetic block in its prevention remains questionable. OBJECTIVES: The aim of this study is to determine whether early stellate ganglion blockade for acute herpes zoster of the face will reduce the intensity and duration of acute herpetic pain, and if the blockade has the potential to prevent or reduce the incidence and/or severity of PHN. STUDY DESIGN: Randomized, controlled, double blind trial. SETTING: Hospital, outpa

Analyzing the Effect of Intraoperative Stimulation Voltage on Facial Numbness Following Radiofrequency Thermocoagulation in the Treatment of Idiopathic Trigeminal Neuralgia.

INTRODUCTION: Radiofrequency thermocoagulation (RFT) effectively alleviates idiopathic trigeminal neuralgia (ITN); however, postoperative facial numbness poses a significant challenge. This issue arises due to the close proximity of high-temperature thermocoagulation, which not only ablates pain-related nociceptive fibers but also affects tactile fibers. Intraoperative sensory stimulation voltage (SV), which reflects the distance between the RFT cannula and the target nerve, potentially possesses the ability to prevent tactile fiber injury. This study aimed to investigate the influence of SV o

[Treatment of atypical post-traumatic and postoperative facial neuralgias by chronic stimulation. Apropos of 2 cases, with review of the literature].

This article describes the case of two patients suffering from deafferentation pain after surgery or traumatic lesions in the area of the peripheric trigeminal branches on the Gasserian ganglion. Chronic electrical stimulation by an electrode implanted in the Gasserian ganglion led to a good permanent result in both cases, within a follow-up period of one year and one year and a half, respectively. The pre-operative test was carried out percutaneously via the foramen ovale, the definitive implantation by surgery with subtemporal access. This method can only be used when at least part of the ga
