BACKGROUND: Hallux valgus (HV) is one of the most common forefoot deformities, and its prevalence increases with age. HV has been associated with poor foot function, difficulty in fitting footwear and poor health-related quality of life. The aims of this study were to design and develop an easy-to-use measurement device for measuring hallux valgus angle (HVA) in patients with HV and to assess the measurement reliability of the newly designed measurement device.
METHODS: A manual measurement device for measuring HVA was designed and developed to test on patients with HV. Two measuring methods,

Introduction An awareness of the anatomical parameters of the foot such as the position and orientation of the sesamoid bones can be of great value for the etiology and diagnostic approach to patients with hallux valgus (HV). The purpose of this study was to evaluate the basic anatomical features and measurements related to the characteristics of HV in cadaveric material. Materials and methods The study sample included 12 cadaveric limbs with HV and 10 cadaveric limbs without HV as a control group. We measured the HV angle (HVA), the first to second intermetatarsal angle (IMA), and the first t

Diabetic foot ulceration of the plantar hallux is a challenging condition and can require partial hallux amputation when complicated by infection. Lower extremity biomechanics play an important role in the development of hallux ulcers, and hallux rigidus (HR) could influence the outcomes after partial hallux amputation. We hypothesized that radiographic evidence of HR in patients with diabetes would be associated with greater ulcer recurrence and reamputation rates after partial hallux amputation. We performed a retrospective review of all patients with diabetes who had undergone a partial hal

STUDY DESIGN: Case control study.
OBJECTIVE: To explore the validity of the assumptions underpinning the Hubscher maneuver of hallux dorsiflexion in relaxed standing, by comparing the relationship between static and dynamic first metatarsophalangeal (MTP) joint motions in groups differentiated by normal and abnormal clinical test findings.
BACKGROUND: Limitation of motion at the first MTP joint during gait may be due to either structural or functional factors. Functional hallux limitus (FHL) has been proposed as a term to describe the situation in which the first MTP joint shows no limitatio

Hallux valgus, also known as a bunion, is one of the most frequent deformities of the forefoot. The prevalence is 23% in people between the ages of 18 and 65 years and 36% in people over 65 years of age. Hallux valgus deformity normally begins between the ages of 30 and 60 years. Women are overall more frequently affected than men. Hallux valgus is characterized by the valgus deviation of the big toe and the varus deviation of the first metatarsal bone (MT-I), also known as metatarsus primus varus. This deformity can lead to a pressure point on the medial MT‑I head due to shoe conflict and can

Recurrence is a common complication after surgical correction of hallux valgus deformity. The reported incidence of recurrence after hallux valgus surgery has been shown to be as high as 64.7%. It has now received acceptance that a hallux valgus angle of more than 20° and an intermetatarsal angle of more than 10° should be classified as a recurrence. For the adequate correction, a number of different techniques need to be considered. According to the literature for more severe deformities, the arthrodesis of either the tarsometatarsal joint or the metatarsophalangeal joint seems to be the tech

Hallux rigidus is a common degenerative condition of the hallux metatarsophalangeal joint (MTPJ) characterized by pain, swelling, stiffness, and limited range of motion with characteristic corresponding clinical, physical examination, and radiographic findings. Many historical risks factors including trauma and family history and patient factors including hallux valgus interphalangeus and inflammatory arthropathies have a well-substantiated etiologic role in the disease process. The purpose of this section is to review the normal and pathologic anatomy and biomechanics of the hallux MTPJ while

First metatarsophalangeal joint (MTPJ) arthroplasty provides hallux rigidus patients with pain relief and preserved motion, offering an alternative to arthrodesis. Recent advancements in implant technology and surgical techniques have broadened treatment options. Although good outcomes have been documented in the literature, concerns persist regarding increased complications, uncertain long-term efficacy, and challenges in managing failed arthroplasties. Addressing bone loss resulting from the procedure further complicates salvage procedures. Larger cohorts and extended studies are necessary t

Hallux rigidus can be treated with a variety of surgical procedures, including joint preserving techniques, arthrodesis, and arthroplasty. The most commonly reported complications for joint preserving techniques consist of progression of arthritis, continued pain, and transfer metatarsalgia. Although good outcomes have been reported for arthrodesis overall, careful attention must be paid to technique and positioning of the toe to avoid nonunion or malunion. Arthroplasty preserves motion but in the case of failure can present the additional challenge of bone loss. In these scenarios, the author

This review summarises the present knowledge of diagnosing and treating hallux rigidus in Denmark. Hallux rigidus is a degenerative disease of the first metatarsophalangeal joint. The pathology of the joint can be visualised on standing radiograph of the foot. If conservative treatment measures fail to provide adequate pain relief, surgery will be an option for most patients. Several procedures are adequate for the treatment of hallux rigidus, and especially surgery show good or excellent results for most patients.

Hallux varus is a rare foot deformity due to iatrogenic, post-traumatic, idiopathic, inflammatory, spontaneous, or congenital pathologies. Acquired hallux varus, in particular, iatrogenic type, is the commonest. The primary pathology is the abnormal musculotendinous forces secondary to soft tissue or bony imbalance exerting varus deforming force. Understanding the anatomy of the hallux stabilisers and the pathophysiology of hallux varus is vital in its management. It would be helpful to understand the potential surgical pitfalls leading to iatrogenic hallux varus. This literature review summar

Hallux varus is an uncommon condition and majority of the cases are iatrogenic. It can occur as a result of any type of hallux valgus correction surgery and in our cases scarf osteotomy is not an exception. Treatment of this complication can be challenging and it is important to understand the factors that cause this deformity before embarking on surgical correction. Four cases of hallux varus following Scarf osteotomy (1% of our total Scarf osteotomy cases) and discuss the salient features of these patients. The authors ascertained the factors that caused iatrogenic hallux varus and formulate

Hallux valgus deformity is a very common pathological condition which commonly produces painful disability. It is characterised as a combined deformity with a malpositioning of the first metatarsophalangeal joint caused by a lateral deviation of the great toe and a medial deviation of the first metatarsal bone.Taking the patient's history and a thorough physical examination are important steps. Anteroposterior and lateral weight-bearing radiographs of the entire foot are crucial for adequate assessment in the treatment of hallux valgus.Non-operative treatment of the hallux valgus cannot correc

Iatrogenic hallux varus is a relatively rare complication of corrective hallux valgus surgery that has multiple pathologic facets. It requires a comprehensive assessment that focuses on joint flexibility, joint integrity, soft tissue balance, and bony deformity. A step-wise treatment approach is used to address all elements of the deformity. The literature on hallux varus treatments consists mainly of retrospective case series, with several proposed procedures addressing various degrees of deformity. Comparison of these procedures is a challenging endeavor and each case should be considered on

Hallux valgus deformities in children and adolescents are attributed to various malformations. Meticulous assessment of clinical and radiological findings as well as age-dependent progress of hallux malalignment has to be taken into consideration to work out an individual therapeutic concept. Conservative treatment includes both night splints and exercises. Surgical therapy has to be strictly based on objective criteria, i.e. the size of the first intermetatarsal angle and correction of the distal metatarsal articular angle. Moderate deformities can be corrected with three-dimensional distal m

We undertook a prospective cohort study to assess risk factors associated with hallux ulceration, and to determine the incidence of healing or amputation, in consecutive patients with diabetes mellitus who were treated over the observation period extending from September 2004 to March 2005, at the Jabir Abu Eliz Diabetic Centre, Khartoum City, Sudan. There were 122 diabetic patients in the cohort (92 males and 30 females) with an overall mean age of 58 +/- 9 years. Fifty-three percent of patients had complete healing within 8 weeks and 43% healed within 20 weeks. The overall mean time to heali

Hallux rigidus or osteoarthritis of the first metatarsophalangeal joint is characterized by pain, stiffness of the joint, and alterations of gait. The appeal of joint arthroplasty for hallux rigidus is similar to its benefits in other joints in the body. The ideal implant should relieve pain, restore motion, improve function, and maintain joint stability. Numerous implants have been described for the hallux metatarsophalangeal joint. This article discusses various implant options along with clinical outcomes and complications.

A variety of metatarsal osteotomies has been described since the first report by Gernet in 1836. Many of these osteotomies were abandoned throughout the years. Because one procedure is not capable of correcting all types of hallux valgus deformities, an algorithm, as a guideline, is preferable. This article presents a choice of metatarsal osteotomies for correction of hallux deformities based on clinical and biomechanical research.

Replacement of the first metatarsophalangeal joint (MTP1) is still controversial when compared with MTP1 fusion in treating hallux rigidus and other disabling conditions of the MTP1 joint. Prospective studies concerning endoprosthetic replacement of the MTP1 joint are not available yet. Nevertheless, better understanding of biomechanics and tribology show favour towards total endoprosthesis. Some currently available models are described (Toefit-Plus, Roto-glide, ReFlexion, Bio-Action, Moje). Furthermore the authors report their results with 30 Toefit-Plus total endoprostheses. Five complicatio
