Deep hypothermic circulatory arrest versus non-deep hypothermic beating heart strategy in descending thoracic or thoracoabdominal aortic surgery.

OBJECTIVES: The ideal cardiopulmonary bypass (CPB) strategy during open surgical repair of the descending thoracic aorta (DTA) or thoracoabdominal aorta (TAA) is controversial. This study aimed to compare the clinical outcomes between deep hypothermic circulatory arrest (DHCA) and non-deep hypothermic beating heart CPB (non-DHCA) for DTA or TAA replacement. METHODS: From January 1994 to August 2011, 259 patients underwent DTA or TAA replacement. Of these, 212, who were judged to be suitable for both DHCA (n = 79) and non-DHCA (n = 109), were analysed. In-hospital outcomes were compared using

American Heart Association cardiopulmonary resuscitation quality targets are associated with improved arterial blood pressure during pediatric cardiac arrest.

AIM: To evaluate the association between cardiopulmonary resuscitation (CPR) quality and hemodynamic measurements during in-hospital pediatric cardiac arrest. We hypothesized that AHA recommended CPR rate and depth targets would be associated with systolic blood pressures≥80mmHg and diastolic blood pressures≥30mmHg. METHODS: In children and adolescents <18 years of age who suffered a cardiac arrest with an invasive arterial catheter in place, a CPR monitoring defibrillator collected CPR data which was synchronized to arterial blood pressure (BP) tracings. Chest compression (CC) depths were co

Predictors and Outcomes of Sudden Cardiac Arrest in Heart Failure With Preserved Ejection Fraction: A Nationwide Inpatient Sample Analysis.

Sudden cardiac arrest (SCA) is the leading cause of cardiovascular mortality in heart failure with preserved ejection fraction (HFpEF), contributing to around 25% of deaths observed in pivotal HFpEF trials. However, predictors and outcomes of in-hospital SCA in HFpEF have not been well characterized. We queried the Nationwide Inpatient Sample (2016 to 2017) to identify adult hospitalizations with a diagnosis of HFpEF. Patients with acute or chronic conditions associated with SCA (e.g., acute myocardial infarction, acute pulmonary embolism, sarcoidosis) were excluded. We ascertained whether SCA

Smartwatch based automatic detection of out-of-hospital cardiac arrest: Study rationale and protocol of the HEART-SAFE project.

Out-of-hospital cardiac arrest (OHCA) is a leading cause of mortality. Immediate detection and treatment are of paramount importance for survival and good quality of life. The first link in the 'chain of survival' after OHCA - the early recognition and alerting of emergency medical services - is at the same time the weakest link as it entirely depends on witnesses. About one half of OHCA cases are unwitnessed, and victims of unwitnessed OHCA have virtually no chance of survival with good neurologic outcome. Also in case of a witnessed cardiac arrest, alerting of emergency medical services is o

Acute Effects of Sodium Bicarbonate in Children with Congenital Heart Disease with Biventricular Circulation in Non-cardiac Arrest Situations.

Despite the controversy, sodium bicarbonate is a commonly used medication in critically ill patients of all ages. There is a lack of data on the acute impact on hemodynamic parameters, biomarker indicators of cardiac output, and changes in vasoinotropic support after sodium bicarbonate therapy. In our retrospective study on children with biventricular circulation in pediatric cardiac intensive care unit receiving bicarbonate therapy: we analyzed its effects on arterial blood gases, heart rate, blood pressure (BP), central venous pressures (CVP), cerebral and renal near-infrared spectroscopy (N

Comparison of left ventricular assist and intra-aortic balloon counterpulsation during early reperfusion after ischemic arrest of the heart.

In most centers, intra-aortic balloon counterpulsation and inotrope infusion are used for patients who require support to be weaned from cardiopulmonary bypass at the end of a cardiac surgical procedure. Where available, early institution of a left ventricular assist device is an alternative with possible advantages. In a canine model of left ventricular failure caused by 45 minutes of normothermic ischemic arrest, these two methods of support were instituted after an initial 30-minute reperfusion period. Both methods provided adequate support of the circulation (cardiac output greater than 2

Downregulation of TCF19 and ATAD2 causes endothelial cell cycle arrest at the transition from cardiac hypertrophy to heart failure.

Cardiac hypertrophy is a key mechanism that allows the heart to adapt to increased load, but in the long term is associated with a higher risk for heart failure, arrhythmia, and death. During hypertrophic growth, cardiac myocytes signal to endothelial cells via vascular endothelial growth factor (VEGF) to promote angiogenesis and maintain myocardial oxygen supply. Insufficient angiogenesis leads to a decline in capillary density and drives the progression from compensated cardiac hypertrophy to heart failure. Here, we studied the time course of endothelial cell gene expression during heart fai

Normothermic ischaemic cardiac arrest of the isolated perfused rat heart: effects of trifluoperazine and lysolecithin on mechanical and metabolic recovery.

To evaluate the hypothesis that maintenance of the integrity of myocardial membrane systems and prevention of Ca2+ influx into the cell are significant in the survival of ischaemic tissue, the effect of trifluoperazine and lysolecithin, were tested on the recovery of globally ischaemic rat hearts. Trifluoperazine increases membrane stabilization, inhibits calmodulin and binds to other Ca2+-dependent proteins. Lysolecithin, on the other hand, has a detergent action on myocardial cell membranes and facilitates Ca2+ ingress in ischaemic tissue. With trifluoperazine (2.45 microM), added before ind

Nitric oxide supplementation during extracorporeal resuscitation drives oxidative-inflammatory signaling and metabolic suppression in the post-cardiac arrest heart.

BACKGROUND: Myocardial dysfunction is a major determinant of mortality after cardiac arrest, yet the molecular events driving post-resuscitation injury remain incompletely understood. Nitric oxide (NO) has been proposed as a cardioprotective adjunct during extracorporeal life support (ECLS), but its mechanistic impact on myocardial recovery is unclear. We investigated whether NO supplementation during ECLS modulates oxidative stress, metabolic pathways, and apoptotic signaling in the post-cardiac arrest heart. METHODS: Male Sprague Dawley rats underwent hypothermic cardiac arrest followed by

Mitochondrial Damage-associated Molecular Patterns as Potential Biomarkers in DCD Heart Transplantation: Lessons From Myocardial Infarction and Cardiac Arrest.

Heart transplantation with donation after circulatory death (DCD) has become a real option to increase graft availability. However, given that DCD organs are exposed to the potentially damaging conditions of warm ischemia before procurement, new strategies for graft evaluation are of particular value for the safe expansion of DCD heart transplantation. Mitochondria-related parameters are very attractive as biomarkers because of their intimate association with cardiac ischemia-reperfusion injury. In this context, a group of mitochondrial components, called mitochondrial damage-associated molecu

Exposure to ambient fine particulate matter and primary cardiac arrest among persons with and without clinically recognized heart disease.

The authors studied the association between incidence of primary cardiac arrest and daily measures of fine particulate matter (</=2.5 micro m) using a case-crossover study of 1,206 Washington State out-of-hospital cardiac arrests (1985-1994) among persons with (n = 774) and without (n = 432) clinically recognized heart disease. The authors compared particulate matter levels on the day of the cardiac event and the 2 days preceding the event with levels from matched reference days. The estimated relative risk for a 13.8- micro g/m(3) increase in fine particulate matter (nephelometry: 0.54 x 10(-

Successful management of biventricular heart failure due to cardiac arrest with isolated left ventricular assist device therapy: a case series.

BACKGROUND: The treatment of patients with biventricular heart failure using a left ventricular assist device (LVAD) remains a significant challenge. For patients with cardiac arrest (CA), biventricular assist device (BiVAD) support is necessary; however, it severely impacts their quality of life. We present three patients with CA who initially required BiVAD but were successfully weaned to isolated LVAD therapy. CASE SUMMARY: Three men with acute lymphocytic fulminant myocarditis developed cardiogenic shock and CA. After temporary mechanical circulatory support, they received extracorporeal

Cerebellar Infarction Following Pulseless Electrical Activity Arrest in Advanced Heart Failure With Reduced Ejection Fraction: A Post-resuscitation Diagnostic Pitfall.

Neurologic deterioration following cardiac arrest is frequently attributed to global hypoxic-ischemic brain injury. However, focal ischemic stroke may occur concurrently and represents an important diagnostic pitfall in post-resuscitation care. We report the case of a 59-year-old woman with end-stage heart failure with reduced ejection fraction who developed acute neurologic impairment following prolonged pulseless electrical activity arrest. Early non-contrast CT of the head demonstrated a focal left cerebellar infarction without diffuse cerebral edema. MRI of the brain performed four days la

Association of non-cardiac depolarization-blocking drugs with first-registered heart rhythm and survival in out-of-hospital cardiac arrest.

OBJECTIVE: Out-of-hospital cardiac arrest (OHCA) is one of the leading causes of mortality worldwide. Survival following OHCA largely depends on whether the initial heart rhythm is shockable or non-shockable. Previous research has suggested that drugs with an off-target effect of blocking cardiac depolarization (non-cardiac depolarization-blocking (DB) drugs) may be associated with a higher likelihood of having a non-shockable rhythm. In this study, we examined whether the use of non-cardiac DB-drugs affects the initial rhythm and survival in OHCA. METHODS: We identified all patients with OHC

A Rare Case of Late Onset Familial Long QT Syndrome Presented with Recurrent Cardiac Arrest, Complete Heart Block, and NSTEMI.

Long QT Syndrome (LQTS) is a rare cardiac condition whose etiology is acquired or congenital. It has a wide range of clinical manifestations ranging from asymptomatic to sudden cardiac death due to malignant arrhythmia such as ventricular tachycardia. Congenital LQTS usually occurs at an early age in the form of prolonged QT interval in ECG examination, but such a condition may occur in later life. Therefore, QT interval should be assessed thoroughly to minimize the risk of iatrogenic ventricular tachycardia. A 72-year-old Javanese female with recurrent syncope episodes for 8 months was referr

ChatGPT's Performance in Cardiac Arrest and Bradycardia Simulations Using the American Heart Association's Advanced Cardiovascular Life Support Guidelines: Exploratory Study.

BACKGROUND: ChatGPT is the most advanced large language model to date, with prior iterations having passed medical licensing examinations, providing clinical decision support, and improved diagnostics. Although limited, past studies of ChatGPT's performance found that artificial intelligence could pass the American Heart Association's advanced cardiovascular life support (ACLS) examinations with modifications. ChatGPT's accuracy has not been studied in more complex clinical scenarios. As heart disease and cardiac arrest remain leading causes of morbidity and mortality in the United States, fin

Impact of Different Serum Potassium Levels on Postresuscitation Heart Function and Hemodynamics in Patients with Nontraumatic Out-of-Hospital Cardiac Arrest.

BACKGROUND: Sustained return of spontaneous circulation (ROSC) can be initially established in patients with out-of-hospital cardiac arrest (OHCA); however, the early postresuscitation hemodynamics can still be impaired by high levels of serum potassium (hyperkalemia). The impact of different potassium levels on early postresuscitation heart function has remained unclear. We aim to analyze the relationship between different levels of serum potassium and postresuscitation heart function during the early postresuscitation period (the first hour after achieving sustained ROSC). METHODS: Informat

Critical Care Management of Patients After Cardiac Arrest: A Scientific Statement From the American Heart Association and Neurocritical Care Society.

The critical care management of patients after cardiac arrest is burdened by a lack of high-quality clinical studies and the resultant lack of high-certainty evidence. This results in limited practice guideline recommendations, which may lead to uncertainty and variability in management. Critical care management is crucial in patients after cardiac arrest and affects outcome. Although guidelines address some relevant topics (including temperature control and neurological prognostication of comatose survivors, 2 topics for which there are more robust clinical studies), many important subject ar
