The cricothyroid muscle does not influence vocal fold position in laryngeal paralysis.

The status of the cricothyroid muscle, which is innervated by the superior laryngeal nerve, is believed to influence the vocal fold position in laryngeal paralysis. It is believed that isolated lesions of the recurrent laryngeal nerve generally result in the paralyzed vocal fold assuming a paramedian position but that with lesions of both the superior and recurrent laryngeal nerves, a more lateral (intermediate or cadaveric) vocal fold position can be expected. Twenty-six consecutive patients with unilateral vocal fold paralysis underwent transnasal fiberoptic laryngoscopy (TFL) and laryngeal

Nasal air puff stimulations and laryngeal, thoracic and abdominal muscle activities.

In cats, we studied the activity of laryngeal, thoracic and abdominal muscles and the variations in oesophageal pressure in response to air puff stimulations of the nasal mucosa. Following single stimulations, inspiratory and laryngeal dilator muscles were transiently inhibited. During inspiratory inhibition, expiratory muscles and laryngeal constrictor were transiently activated. Repetitive air puff stimulations, which induced sneeze, evoked a similar pattern of transient activities during the inspiratory preparation of sneeze. This resulted in transient fluctuations of the oesophageal pressu

[Repair of the defect after laryngeal surgery using the sternohyoid muscle and fascia].

In order to achieve a radical excision of laryngeal cancer with preservation of its functions, the authors devised an operative procedure in which the defects of the larynx, oral pharynx, laryngeal pharynx and/or cervical esophagus were repaired with sternohyoid muscle and fascia. Since May 1981, this operation has been done on 52 patients with satisfactory results. The operative procedure and clinical materials are reported in this paper. This operation is simple, postoperative complications are rare, and it warrants further clinical trial.

Alteration in breathing of the awake rat after laryngeal and diaphragmatic muscle paralysis.

The respiratory rate (f), tidal volume (VT) and ventilation (V) were measured in 3 groups of rats: 10 rats before and after cutting both recurrent laryngeal nerves (RLNX), 10 rats before and after bilateral phrenicotomy (PNX) and 5 sham transected (SHAMX) rats. All rats were exposed to air and gas mixtures, deficient in O2 and/or enriched with CO2. The barometric method was used to measure ventilatory parameters. The sham operation did not affect breathing pattern or ventilation. In RLNX rats, breathing the various gas mixtures exhibited no changes in V because f uniformly increased as VT decl

Experimental laryngeal reinnervation by phrenic nerve implantation into the posterior cricoarytenoid muscle.

Under general anaesthesia, 5 dogs underwent sectioning of the right recurrent nerve followed by implantation of the phrenic nerve into the posterior cricoarytenoid (PCA) muscle. Some 6-7 months later the dogs were sacrificed after registration of vocal cord motility. Still photographs and movie film of the larynx were taken during quiet and forced respiration and at electrical stimulation of the implanted phrenic nerve. The PCA and vocal muscles were removed for histochemical studies. We found practically no abductory movement of the vocal cord on the reinnervated side, either during quiet or

Radiation Dose to Swallowing Muscles and Post-Radiotherapy Laryngeal Penetration or Aspiration in Head and Neck Squamous Cell Carcinoma.

BACKGROUND/OBJECTIVES: Radiation-induced dysphagia can cause life-threatening complications including aspiration pneumonia. Consequently, studies have attempted to limit the radiation dose to the swallowing muscles to prevent radiation-induced dysphagia. However, data identifying the specific swallowing muscles associated with post-radiation dysphagia are lacking. Therefore, this case-control study aimed to identify the swallowing muscles whose radiotherapy-related damage is likely to cause radiation-induced dysphagia. METHODS: This retrospective study included 53 patients with head and neck

Preventive effects of bortezomib on denervation-induced atrophy of the intrinsic laryngeal muscles: an experimental study in the rat.

CONCLUSION: Bortezomib was effective in attenuating atrophy of the posterior cricoarytenoid (PCA) muscle, but not the thyroarytenoid (TA) muscle. This was probably due to differences in the fiber composition of the two muscles. The PCA muscle is composed of a combination of fast- and slow-twitch fibers, and therefore is more resistant to atrophy than the TA muscle, which is composed solely of fast-twitch fibers. OBJECTIVES: To investigate the preventive effects of bortezomib on denervation-induced atrophy of the TA and PCA muscles in the rat. METHODS: Following transection of the left recurr

[Hyoid epiglottis combined with sternocleidomastoid muscle clavicle flap for repairing laryngeal defect after laryngeal trauma infection: a case report].

The initial treatment of open laryngeal trauma must be implemented immediately, with the primary focus on saving lives. However, in the later stages, various factors may cause changes in the structure and function of the larynx, which requires special attention. This article reports on the treatment process of a patient with depression who suffered from laryngeal trauma. Due to the late stage of laryngeal infection causing laryngeal defects, a hyoid epiglottis combined with sternocleidomastoid muscle clavicular flap repair was performed. Additionally, personalized functional exercise was perfo

Intraoperative Laryngeal Muscle and Heart Rate Responses to Implanted Vagus Nerve Stimulation.

OBJECTIVES: Implanted cervical vagus nerve stimulation (VNS) is used to treat refractory epilepsy, depression, stroke sequelae, and rheumatoid arthritis. It is necessary to understand the recruitment of different neural elements in response to VNS to uncover the therapeutic mechanisms for these therapies. We quantified VNS-evoked physiologic responses in participants who underwent VNS implant surgery: laryngeal muscle activation (electromyogram [EMG]; indicating side effects; mediated by activation of large-diameter myelinated A-fibers) and changes in heart rate (HR; mediated by activation of

Use of laryngeal muscle evoked potential recording for experimental vagus nerve stimulation.

The laryngeal muscle evoked potential (LMEP) is a neurophysiological outcome parameter that guarantees integrity of the nerve-electrode interface during experiments with vagus nerve stimulation (VNS). This paper discusses a large series of minimally invasive LMEP recordings in 46 female Lewis rats, implanted with a custom-made VNS electrode around the left cervical vagus nerve. After a 3-week recovery, LMEPs were recorded twice in each animal, with swapping the anode and cathode positions of the VNS electrode (polarity inversion). A VNS-induced LMEP was identified as the initial negative peak

Use of Motion Capture Technology to Study Extrinsic Laryngeal Muscle Tension and Hyperfunction.

OBJECTIVES: Patients with primary muscle tension dysphonia (pMTD) commonly report paralaryngeal pain and discomfort, and extrinsic laryngeal muscle (ELM) tension and hyperfunction are commonly implicated. However, quantitative physiological metrics to study ELM movement patterns for the characterization of pMTD diagnosis and monitoring of treatment progress are lacking. The objectives of this study were to validate motion capture (MoCap) technology to study ELM kinematics, determine whether MoCap could distinguish ELM tension and hyperfunction between individuals with and without pMTD, and inv

Laryngeal Muscle-Evoked Potential Recording as an Indicator of Vagal Nerve Fiber Activation.

BACKGROUND: Vagus nerve stimulation (VNS) is an adjunctive therapy for drug-resistant epilepsy. Noninvasive evoked potential recordings in laryngeal muscles (LMEPs) innervated by vagal branches may provide a marker to assess effective vagal nerve fiber activation. We investigated VNS-induced LMEPs in patients with epilepsy in acute and chronic settings. MATERIALS AND METHODS: A total of 17 of 25 patients underwent LMEP recordings at initiation of therapy (acute group); 15 of 25 patients after one year of VNS (chronic group); and 7 of 25 patients were tested at both time points (acute + chroni

Clinical responses following inspiratory muscle training in exercise-induced laryngeal obstruction.

PURPOSE: Exercise-induced laryngeal obstruction (EILO) is relatively common in young people. Treatment rests on poor evidence; however, inspiratory muscle training (IMT) has been proposed a promising strategy. We aimed to assess laryngeal outcomes shortly after IMT, and to compare self-reported symptoms with a control group 4-6 years later. METHODS: Two groups were retrospectively identified from the EILO-register at Haukeland University Hospital, Norway; one group had received only information and breathing advice (IBA), and another additionally IMT (IBA + IMT). At diagnosis, all participant

Innervation of the human posterior cricoarytenoid muscle by the external branch of the superior laryngeal nerve.

BACKGROUND: The posterior cricoarytenoid muscle is the abductor muscle for the vocal cords and is innerved by the recurrent laryngeal nerve (RLN). The purpose of this study was to present our determination if the external branch of the superior laryngeal nerve (SLN) contributes to the motor innervation of the posterior cricoarytenoid muscle. METHODS: We performed electromyographies (EMGs) via needle electrodes on 47 posterior cricoarytenoid muscles from 28 patients during thyroidectomy (9 lobectomies and 19 total thyroidectomies) with intraoperative neural monitoring. The RLN, vagus nerve, an

Effects of Functional Electrical Stimulation on Denervated Laryngeal Muscle in a Large Animal Model.

Bilateral vocal fold paralysis (BVCP) is a life-threatening condition that follows injury to the Recurrent Laryngeal nerve (RLn) and denervation of the intrinsic laryngeal musculature. Functional electrical stimulation (FES) enables restoration and control of a wide variety of motor functions impaired by lower motor neuron lesions. Here we evaluate the effects of FES on the sole arytenoid abductor, the posterior cricoarytenoid (PCA) muscle in a large animal model of RLn injury. Ten horses were instrumented with two quadripolar intramuscular electrodes in the left PCA muscle. Following a 12-wee

Influence of muscle relaxation on neuromonitoring of the recurrent laryngeal nerve during thyroid surgery.

BACKGROUND: The influence of muscle relaxation on the intra-operative neuromonitoring of the recurrent laryngeal nerve during thyroid surgery is unclear. METHODS: In a prospective study involving 200 patients undergoing elective thyroid surgery, the influence of muscle relaxation on neuromonitoring of the recurrent laryngeal nerve was investigated. The patients received balanced anaesthesia with oxygen-nitrous oxide-isoflurane, and rocuronium bromide was used as the non-depolarizing neuromuscular blocking agent. The degree of relaxation was monitored continuously by accelerometry [twitch (% T
