Humour and laughing in patients with prolonged incurable cancer: an ethnographic study in a comprehensive cancer centre.

PURPOSE: Most people are familiar with the expression 'laughter is the best medicine'. By enhancing cognitive flexibility and strengthening relationships, laughter can be considered a holistic care-approach. Yet, in medical oncology, especially the palliative phase, using humour can be considered inappropriate or taboo. We aimed to explore the acceptability and functions of humour and laughter in patients with prolonged incurable cancer. METHODS: This study was performed in a Dutch Comprehensive Cancer Centre. We included four short conversations with patients, eighteen in-depth patient-inter

How fathers and mothers make their children laugh: Associations with the security of parent-child attachment relationships.

OBJECTIVES: Bowlby (1969/1982) described an infant's smile and pleasant vocalizations as attachment behaviors. However, most research on the formation of attachment relationships centers on the role of the caregiver's response to child distress, neglecting the role of behaviors that promote proximity in a positive context. This study aimed to 1) explore fathers' and mothers' laughter-eliciting behaviors and children's laughter using a newly developed coding instrument and 2) explore associations between these behaviors, children's laughter, and child attachment security. DESIGN: A sample of 1

Urodynamic characterization of giggle incontinence in children.

AIMS: Giggle incontinence is a rare condition resulting in excessive urinary incontinence with laughter, where bladder function is otherwise "normal." Urodynamic descriptions of the condition to date are limited. We believe that giggle incontinence has characteristic urodynamic findings. We tested this hypothesis. METHODS: We retrospectively reviewed the urodynamic investigations of patients with giggle incontinence managed in a tertiary regional bladder unit between February 2014 and November 2019. RESULTS: We identified the studies of seven patients, median age 13.5 years (10.4-15.7) of wh

Humor, hypnosis and kids.

Bertrand Russell, philosopher and Nobel prize winner said "Laughter is the most inexpensive and most effective wonder drug. Laughter is a universal medicine." Humor as medicine may be just what the doctor ordered to keep us emotionally, mentally and physically fit. Children are not only wonderful hypnotic subjects, they love laughter and silliness. The therapeutic effectiveness of hypnotherapy with children and adolescents can be enhanced when humor is incorporated into the session. Laughter is fun and free and can be used to strengthen rapport and the therapeutic alliance between the clinicia

Successful stereoelectroencephalography-guided radiofrequency thermocoagulation in the treatment of a patient with mesial frontal region-originated mirthful gelastic seizure.

Gelastic seizures (GS), characterized by automatic bouts of unnatural stereotyped laughter and commonly recognized as the hallmark of hypothalamic hamartoma, is rarely associated with cortical epileptogenic focus. Whether there is a dissociation of the motor program of laughter and the experience of mirth and the symptomatogenic zone for ictal laughter with or without mirth are still unclear. We report a patient with drug-resistant mirthful GS who receive a comprehensive investigation of stereoelectroencephalography recording and electrocortical stimulation. Mirthful GS were confirmed to origi

A clinical evaluation of gelastic and dacrystic seizures: a multicenter study.

BACKGROUND: Gelastic seizures are extremely rare, short-lasting, unprovoked, and uncontrollable laughing attacks. We conducted this retrospective evaluation to determine whether these symptoms, manifesting in different forms, such as cheerful laughter, laughing, smiling, and sobbing had any value in terms of etiology or localization. METHODS: A total of 31 patients who exhibited bouts of laughing or crying and who were under follow-up between 2000 and 2019 at tertiary epilepsy centers were included in the study. Laughing seizures were divided into three groups in terms of semiology (i.e., lau

Tickle.

Tickle is a familiar sensation that may have two components: a light or feather-type noxious sensation termed by Hall and Allin as knismesis, and a heavy or laughter-associated sensation termed gargalesis. Studies on the sensation of tickle have generally been on one sensation or the other, but not on both. In pleasurable, laughter-associated tickle there are elements of touch and pleasure, but the sensation may be a synthesis of touch and itch, with receptors, neural pathways or central receptors different from knismesis. It is unclear why some skin sites are more ticklish than other sites. L

"Fou rire prodromique" heralding a brainstem stroke.

A patient manifested pathological laughter heralding the onset of brainstem stroke leading to a "locked-in" state. The pathological laughter did not recur. MRI revealed a bilateral ventral pontine infarct. The clinico-anatomical correlations of this rare phenomenon of fou rire prodromique are discussed.

Brainstem compression from a trigeminal schwannoma presenting with pathological crying.

Patients with pathological laughter and crying have episodes of uncontrollable laughter, crying or both. Pathological laughter is a well-described entity secondary to various conditions such as multiple sclerosis, pseudo-bulbar palsy, cerebello-pontine angle tumours, clival chordomas and brainstem gliomas. Pathological crying is rare and there have been no previous reports of brainstem compression causing this entity. We report a patient who presented with pathological crying caused by a trigeminal schwannoma with a tumor-associated cyst indenting the pons. This case report confirms the involv

Ictal semiology of gelastic seizures.

Gelastic seizures are rare epileptic manifestations characterized by laughter or a smile. The main etiology is represented by hypothalamic hamartoma, but also focal localization of the epileptogenic zone is described. We reviewed a group of patients with gelastic seizures to describe the semiology and to establish any difference related to diverse epilepsy etiologies. Thirty-five seizures from 16 patients (6 females) were reviewed. The study confirms that hypothalamic hamartoma is the more frequent etiology associated with gelastic seizures. Laughter represented the majority of gelastic ictal

They are laughing at me: cerebral mediation of cognitive biases in social anxiety.

The fear of embarrassment and humiliation is the central element of social anxiety. This frequent condition is associated with cognitive biases indicating increased sensitivity to signals of social threat, which are assumed to play a causal role in the maintenance of social anxiety. Here, we employed laughter, a potent medium for the expression of acceptance and rejection, as an experimental stimulus in participants selected for varying degrees of social anxiety to identify cerebral mediators of cognitive biases in social anxiety using functional magnetic resonance imaging in combination with

[Pseudobulbar affect in the prodromal phase of psychosis: a case report].

Laughter and crying are fundamental human expressions integral to social interaction and emotional communication. In certain neurological conditions or psychiatric disorders, these behaviors may become pathological, leading to significant impairments in interpersonal and social functioning. We report a patient presenting with uncontrollable episodes of laughter and crying who developed a psychotic episode three weeks later. We review the epidemiology, pathophysiology, diagnostic considerations, and the emerging therapeutic option for pathological laughter and crying (pseudobulbar affect), such

Emotion stimulus processing in narcolepsy with cataplexy.

Reported brain abnormalities in anatomy and function in patients with narcolepsy with cataplexy led to a project based on qualitative electroencephalography examination and analysis in an attempt to find a narcolepsy with cataplexy-specific brain-derived pattern, or a sequence of brain locations involved in processing humorous stimuli. Laughter is the trigger of cataplexy in these patients, and the difference between patients and healthy controls during the laughter should therefore be notable. Twenty-six adult patients (14 male, 12 female) suffering from narcolepsy with cataplexy and 10 healt

Bridging the Communication Gap: Pragmatics and Interactional Dynamics in Deaf and Hard of Hearing Children.

PURPOSE: Children who are deaf and hard of hearing (DHH) often face challenges in everyday social communication, even when they achieve good auditory recovery through hearing technologies. These difficulties, particularly in pragmatic and interactional domains, can affect peer relationships, educational outcomes, and quality of life. This study aims to characterize interactional and discourse-level challenges in DHH children with functional auditory access to inform targeted interventions. METHOD: We analyzed dyadic child-adult interactions involving nine DHH and 13 typically hearing (TH) chi

Epilepsy and cataplexy in Angelman syndrome. Genotype-phenotype correlations.

BACKGROUND: Angelman syndrome (AS) is a neurogenetic disorder characterized by intellectual disability, epilepsy, and low threshold for laughter. AIMS: We investigated the occurrence and severity of epilepsy and laughter-induced loss of postural muscle tone determined by the different genetic subtypes. METHODS: This study included 39 children with AS. Deletion breakpoints were determined by high resolution CGH microarray (1×1M Agilent). Clinical data were based on a parent interview and medical record review. RESULTS: All patients with AS based on a deletion had epilepsy. Epilepsy was prese

'Fou rire prodromique' as the heralding symptom of lenticular infarction, caused by dissection of the internal carotid artery in a 12-year-old boy.

A 12-year-old, right-handed boy experienced a pathological fit of laughter before a sudden right hemiplegia. Magnetic resonance imaging showed a left basal ganglia infarction, induced by a left internal carotid dissection. Arteriography revealed an underlying fibromuscular dysplasia. This case study demonstrates that cerebral artery dissection can occur in children and that a basal ganglia infarction may be preceded by pathological laughter called 'fou rire prodromique'. The clinical and anatomical relationship of this paroxysmal event are discussed.

Pathological laugher and crying in motor neuron diseases: a matter of bulbar and neurobehavioral involvement with sex imbalance.

BACKGROUND: Emotional lability (EL), also known as pathological laughter and crying, is a common but understudied symptom in motor neuron diseases (MND): amyotrophic lateral sclerosis and primary lateral sclerosis. This study aimed to investigate the prevalence of EL in MND and to explore the independent frequency components of laughter and crying in relation to motor, cognitive, neuropsychiatric, and neuroimaging factors. METHODS: A total of 198 incident MND patients were enrolled. The Centre of Neurological Study-Lability Scale was used to measure EL. Associations between EL and motor funct

Gelastic epilepsy: video-EEG, MRI and SPECT characteristics.

Gelastic epilepsy, or ictal laughter, is a relatively uncommon type of seizure which may occur singly or, more frequently, with other types of convulsions. Gelastic seizures have been observed to be associated with many different conditions, mainly hypothalamic hamartomas. We report on a patient whose ictal laughter was the only neurologic disturbance. Ictal video-EEG demonstrated seizure arising from the left frontal region with subsequent involvement of the contralateral homologous area and secondary generalization. MRI showed an enlarged left frontal horn of the lateral ventricle. Postictal
