Impact of specialist consultations on inpatient admissions for dermatology-specific and related DRGs.

BACKGROUND: Studies of dermatologic inpatients are important, given the rise in the number of admissions and of Medicare spending for dermatology-specific and dermatology-related diagnosis related groups (DRGs) in recent years. Yet inpatient studies of patients admitted for skin conditions have mainly focused on dermatology consults, which neglect the experiences of patients not seen by dermatology. Identifying patients based on DRG codes includes all patients admitted for skin conditions and therefore allows for a more comprehensive analysis of the dermatologic care delivered. OBJECTIVES: Ou

Association between colonoscopy and colorectal cancer mortality in a US cohort according to site of cancer and colonoscopist specialty.

PURPOSE: We designed this study to evaluate the association of colonoscopy with colorectal cancer (CRC) death in the United States by site of CRC and endoscopist specialty. METHODS: We designed a case-control study using Surveillance, Epidemiology, and End Results (SEER)-Medicare data. We identified patients (cases) diagnosed with CRC age 70 to 89 years from January 1998 through December 2002 who died as a result of CRC by 2007. We selected three matched controls without cancer for each case. Controls were assigned a referent date (date of diagnosis of the case). Colonoscopy performed from Ja

A cost-effectiveness and cost-benefit analysis of contingency contracting-enhanced methadone detoxification treatment.

We examined treatment costs in an ongoing study in which 102 opioid-addicted patients had been randomly assigned to either 180-day methadone detoxification or the same treatment enhanced with contingency contracting. In the latter condition, study participants received regular reinforcers contingent on negative urine toxicology screens and breath analyses for a range of drugs and alcohol. Both conditions involved psychosocial treatment, and all participants were stabilized to a daily methadone dose of approximately 80 mg during the first 4 months, followed by a 2-month taper. Individuals parti

Cost-Effectiveness of HPV Self-Testing Options for Cervical Cancer Screening.

IMPORTANCE: Mailing human papillomavirus (HPV) self-sampling kits to underscreened individuals increases cervical cancer screening and can be cost-effective. However, cost-effectiveness has not been evaluated across other screening histories. OBJECTIVE: To conduct an economic evaluation of mailed HPV self-sampling among members of a US health care system with adherent, overdue, or unknown screening histories. DESIGN, SETTING, AND PARTICIPANTS: This economic evaluation was a cost-effectiveness analysis (CEA) and budget impact analysis (BIA) based on results of a randomized clinical trial (RCT

A NOVEL METHOD FOR HANDLING PRE-EXISTING CONDITIONS IN PREDICTION MODELS FOR COVID-19 DEATH

ABSTRACT Objective To derive a predicted probability of death (PDeathDx) based upon complete sets of ICD-10 codes assigned to patients prior to their diagnosis of COVID-19. PDeathDx is intended for use as a summary metric for pre-existing conditions in multivariate models for COVID-19 death. Methods Cases were identified through the COVID-19 Shared Data Resource (CSDR) of the Department of Veterans Affairs. The diagnosis required at least one positive nucleic acid amplification test (NAAT). The primary outcome was death within 60 days of the first positive test. We retrieved all diagnoses ente

Comparative Analysis of Outcomes in Medicare-Eligible Patients with a Hospital Stay Less than Two-Midnights versus Longer Length of Stay following Total Knee Arthroplasty: Implications for Inpatient-Outpatient Designation.

The Centers for Medicaid and Medicare Services (CMS) removed primary total knee arthroplasty (TKA) from the inpatient-only list in January 2018. This study aims to compare outcomes in Medicare-aged patients who underwent primary TKA and had an in-hospital stay spanning less than two-midnights to those with a length of stay greater than or equal to two-midnights. We retrospectively reviewed 4,138 patients ages ≥65 who underwent primary TKA from 2016 to 2020. Two cohorts were established based on length of stay (LOS), those with an LOS <2 midnights were labeled outpatient and those with an LOS ≥

Chatbot Outreach in Value-Based Preventive Care: Retrospective Analysis.

BACKGROUND: As health care delivery shifts toward value-based care, proactive strategies to close preventive care gaps are essential. However, patient engagement remains suboptimal due to logistical, behavioral, and socioeconomic barriers. Traditional outreach methods, such as phone calls, emails, and postal mail, have long been used, but emerging digital approaches, such as chatbot-based messaging, offer potential advantages in scalability and personalization. Their comparative effectiveness, however, remains underexplored. OBJECTIVE: This study aimed to evaluate the effectiveness of chatbot

Operative vs Nonoperative Treatment of Acute Cholecystitis in Older Adults With Multimorbidity.

IMPORTANCE: Acute cholecystitis in older patients with multimorbidity is associated with a high risk of morbidity and mortality. Debate exists as to whether operative or nonoperative treatment is the most appropriate approach. OBJECTIVES: To compare the effectiveness of operative and nonoperative treatment in older adults with multimorbidity who are hospitalized emergently with acute cholecystitis. DESIGN, SETTING, AND PARTICIPANTS: This was a nationwide retrospective comparative effectiveness research study conducted in the US from 2016 to 2018 that used both an inverse propensity weight an

Clinician validation of the Medicare measure for potentially avoidable hospital visits after chemotherapies.

BACKGROUND: Medicare's OP-35 measure (the 35th hospital outpatient quality measure) tracks unplanned hospital visits within 30 days of chemotherapy and defines a subset as "potentially avoidable" using approximately 300 diagnosis codes. Despite widespread adoption in policy and oncology quality reporting, the measure has not been validated with clinician review. METHODS: We identified 14 220 acute hospital visits within 30 days of chemotherapy (2016-2023) from 22 hospitals in 3 health systems (academic, safety-net, community). A 5% stratified random sample of 705 visits underwent blinded revi

Effects of Antihypertensive Class on Falls, Syncope, and Orthostatic Hypotension in Older Adults: The ALLHAT Trial.

Hypertension treatment has been implicated in falls, syncope, and orthostatic hypotension (OH), common events among older adults. Whether the choice of antihypertensive agent influences the risk of falls, syncope, and OH in older adults is unknown. ALLHAT (Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial) was a randomized clinical trial that compared the effects of hypertension first-step therapy on fatal coronary heart disease or nonfatal myocardial infarction (1994-2002). In a subpopulation of ALLHAT participants, age 65 years and older, we determined the relative

Repeat Large-Volume Paracentesis Versus Tunneled Peritoneal Catheter Placement for Malignant Ascites: A Cost-Minimization Study.

OBJECTIVE: The objective of this study was to determine the point in time at which tunneled peritoneal catheter placement becomes less costly than repeat large-volume paracentesis (LVP) for patients with malignant ascites. MATERIALS AND METHODS: Procedure costs were based on 2013 Medicare reimbursement rates. Rates for specific complications were obtained from the literature and were assigned costs. A decision tree-based Markov chain Monte Carlo model was designed with 11 cycles of 10 days, to simulate 4000 subjects per trial. Patients were grouped according to initial treatment decision (LVP

Impact of a standardized documentation template on hospital case mix index and reimbursement for patients with traumatic brain and spine injuries.

OBJECTIVE: The case mix index (CMI) measures the complexity and severity of hospitalized patients and is used to determine hospital reimbursement rates. Better documentation is associated with higher complexity assigned to cases as measured by CMI but remains understudied in the trauma population. The authors hypothesized that standardized templates for neurotrauma history and physical (H&P) notes are associated with increased CMI and reimbursement. METHODS: A multidisciplinary team consisting of trauma program professionals, trauma data specialists, neurosurgeons, revenue integrity specialis

Cardiovascular risk in long-term survivors of breast, prostate, colon, and rectal cancer.

BACKGROUND: Cardiovascular disease (CVD) is the leading cause of noncancer mortality in long-term cancer survivors. Population-level assessment of cancer-related exposures is limited with respect to long-term cardiovascular risk in older survivors who have additional aging-related risks. METHODS: This was a Surveillance, Epidemiology, and End Results-Medicare retrospective cohort study of long-term (5-year) survivors of breast, prostate, colon, and rectal cancers who were aged 66 years and older, diagnosed from 2003 to 2012, and received definitive treatment. The primary endpoint was late CVD

The Epilepsy-Cog study: Methods to establish a harmonized study of late onset epilepsy in a metacohort of six population-based cohorts in the United States.

OBJECTIVE: With the expected demographic shift toward those ≥65 years of age in the United States, late onset epilepsy (LOE) poses a significant public health issue, yet it has been historically understudied. We are undertaking an effort in the Epilepsy-Cog study to pool individual participant data from six United States-based prospective cohort studies. In this paper, we outline the process for ascertaining epilepsy, harmonizing, and pooling individual participant data across the six cohorts. METHODS: The Epilepsy-Cog study includes individual participant data from six United States-based lo

Cancer Drug Prices in the United States: Efficacy, Innovation, Clinical Trial Evidence, and Epidemiology.

OBJECTIVES: Rising cancer drug prices challenge patients and healthcare systems. Although prices are routinely assigned to original drug indications receiving US Food and Drug Administration (FDA) approval, the pricing of supplemental indication approvals remains uncertain. This study identifies and quantifies factors associated with cancer drug prices, distinctly analyzing original and supplemental indications. METHODS: Clinical trial evidence and epidemiologic data supporting new indications' FDA approval (2003-2022) were collected from the Drugs@FDA database, ClinicalTrials.gov, and Global
