Drug-induced movement disorders.

This review covers the major categories of movement disorders secondary to the use of antipsychotic drugs and other pharmacologic agents that interfere with dopaminergic transmission at the basal ganglionic level. In the past year new syndromes were reported, old concepts were either challenged or supported by recent studies, and significant contributions were made in unraveling the complex pathophysiology of these disorders.

Cocaine-related movement disorders.

We describe four patients, two with Tourette's syndrome, one with the combination of idiopathic dystonia and essential-like tremor, and one with tardive dystonia, who noted marked exacerbation of their movement disorders after exposure to cocaine. These patients provide support for the hypothesis that dopaminergic preponderance plays an important role in the pathogenesis of certain hyperkinetic movement disorders. Cocaine should be regarded as an important cause or precipitant of hyperkinetic movement disorders.

[Genetics in movement disorders].

Genetic considerations in movement disorders are described. 1) Familial parkinsonisms are heterogeneous; genes for two of them, 'Lubag' and Waisman syndrome have been mapped to X chromosome, though genes for others do not have been mapped. 2) The responsible gene for Huntington's disease has been cloned recently and named huntingtin. A (CAG)n repeat longer than the normal range was observed in huntingtin gene. The (CAG)n repeat appears to be located within the coding sequence of a predicted approximately 348 kD protein that is widely expressed but unrelated to any known gene. The expansion of

[Seizures induced by movement].

Three cases with seizures induced by movement are presented. These patients were a 24-year-old man, a 15-year-old girl and a 15-year-old boy respectively. In each case, the onset began at the age of thirteen. The following characteristics were observed in all cases. The attacks were precipitated by sudden initiation of voluntary movement after rest. The attacks were brought on, for instance, by running quickly from a standing position, and were characterized by tonic spasm or choreoathetotic movement of unilateral extremities. They lasted only 5 - 10 seconds, without loss of consciousness and

Tetrabenazine and movement disorders.

Based on blind review of videotape recordings, we analyzed the ability of tetrabenazine to suppress abnormal movements. The recordings were done before treatment, after 3 weeks on a stable dose (up to 200 mg daily) of tetrabenazine, and then after 3 weeks on placebo. Among the patients who completed the trial, improvement occurred in 6 of 8 with Huntington disease, 6 of 10 with tardive dyskinesia, and 3 of 8 with dystonic disorders. The drug was usually tolerated well.

Neurophysiology of movement disorders.

The relation of the cortex, cerebellum, and basal ganglia and their participation in movement disorders is presented. Studies have shown that lesions to the cortex will produce contralateral paralysis while damage to the basal ganglia or cerebellum will result in movement abnormalities. These data provide support for the view that subcortical but not cortical structures initiate the control movement activity. It would appear that the basal ganglia and cerebellum receive signals from the auditory, visual, and somatosensory cortex integrate this information, and relay signals back to the motor c

Therapeutics of disordered movement.

Human motor behavior is organized around two major neurotransmitter systems in the basal ganglia--dopaminergic and cholinergic. Hypokinetic disease may result from hypofunction of the dopaminergic system or cholinergic hyperfunction. The reverse seems true for many hyperkinetic movement disorders. Drugs which facilitate dopaminergic neurotransmission or which block cholinergic transmission relieve many hypokinetic disorders; the opposite approach is useful in treating many hyperkinetic disorders.

Neuroscience of human ventral lateral thalamic nucleus related to movement and movement disorders.

The ventral lateral nucleus (VL) in the human ventral group exhibits activity associated with active and passive limb movements, and pathological movements found in patients with movement disorders. This group includes the anterior (VLa) and posterior (VLp) nuclei, which receive excitatory inputs from the deep cerebellar nuclei and inhibitory inputs from the internal segment of the globus pallidus (GPi). These nuclei primarily project to the primary and supplementary motor cortices. Despite differences in input sources, neurons in VLa and VLp often show similar activity patterns during active

Altered movement strategy during functional movement after an ACL injury, despite ACL reconstruction.

Knee joint functional deficits are common after anterior cruciate ligament (ACL) injury, but different assessment methods of joint function seem to provide contradicting information complicating recovery monitoring. We previously reported improved perceived knee function and functional performance (forward lunge ground contact time) in patients with an ACL injury from pre to 10 months post ACL reconstruction without improvement in knee-specific biomechanics. To further investigate this discrepancy, we additionally analyzed knee extensor and flexor muscle strength, and movement quality in the f

Peripheral Visual Cues Contribute to the Perception of Object Movement During Self-Movement.

Safe movement through the environment requires us to monitor our surroundings for moving objects or people. However, identification of moving objects in the scene is complicated by self-movement, which adds motion across the retina. To identify world-relative object movement, the brain thus has to 'compensate for' or 'parse out' the components of retinal motion that are due to self-movement. We have previously demonstrated that retinal cues arising from central vision contribute to solving this problem. Here, we investigate the contribution of peripheral vision, commonly thought to provide str

The movement-induced self-reference effect: enhancing memorability through movement toward the self.

Much evidence suggests that bodily actions affect cognitive states. In particular, pulling owned objects toward the self improves memory for those objects compared to memory for objects pushed away from the self. Experiments 1 and 2 examined the effect of incidental joystick movement on static stimuli, hypothesizing that using the physical self (rather than a computer monitor) as a reference point would enhance memory for items categorized via a toward-the-self action but not toward-the-computer-monitor action. Experiment 3 examined whether movement toward an external representation of self, o

What affects the movement can be seen from the movement: Effects of optical information and dynamical constraints on movement production and perception.

IMPORTANCE: Over the past decades, the number of patients, especially in the older adult patient group, diagnosed with vestibular schwannoma (VS) has increased. Assuming that older adult patients have more comorbidities, a longer recovery period after surgery, a higher rate of surgical complications, and a higher mortality rate after VS surgery, a treatment strategy for this group of patients is warranted, based on clinical evidence on postsurgical survival. OBJECTIVE: To evaluate the survival after diagnosis of a VS in patients 70 years and older, treated with either observation or surgery,

Eye Tracking Insights Into Movement Preparation and Execution Under Nonstandard Visual Movement Feedback.

For eye-hand coordination, predictions of sensory movement consequences may already be issued, and adjusted, during action preparation. In this pre-registered study, we combined a delayed-movement paradigm with a virtual reality-based eye-hand tracking task to investigate the oculomotor correlates of planning and executing coordinated eye-hand movements under standard versus nonstandard visual hand movement feedback. We measured pupil dilation and gaze-hand tracking during action preparation and subsequent task execution, where visual movement feedback violated or matched cued expectations: Pa

Supplementary motor area contributes to carrying previous movement information over to current movement.

The purpose of the present study was to determine the cortical areas contributing to the influence of the previous movement on the current movement. Right-handed healthy human participants abducted and then adducted the left index finger in response to a start cue. Twenty consecutive trials with 10 s intertrial intervals were performed in each trial block. An odd-numbered trial was considered to be the previous trial, and a trial immediately after the previous trial (even-numbered trial) was the current trial. In each trial block, transcranial magnetic stimulation (TMS) was given over one of t

Rapid Eye Movement Sleep Behavior Disorder and Other Rapid Eye Movement Sleep Parasomnias.

PURPOSE OF REVIEW: The most common rapid eye movement (REM) parasomnia encountered by neurologists is REM sleep behavior disorder (RBD), and nightmares are so frequent that every neurologist should be able to differentiate them from the dream enactment of RBD. Isolated sleep paralysis is relatively common and is often mistaken for other neurologic disorders. This article summarizes the current state of the art in the diagnosis of RBD, discusses the role of specific questionnaires and polysomnography in the diagnosis of RBD, and reviews recent studies on idiopathic RBD as an early feature of a

The effects of muscle fatigue and movement height on movement stability and variability.

Performing repetitive manual tasks can lead to muscle fatigue, which may induce changes in motor coordination, movement stability, and kinematic variability. In particular, movements performed at or above shoulder height have been associated with increased shoulder injury risk. The purpose of this study was to determine the effects of repetitive motion-induced muscle fatigue on posture and on the variability and stability of upper extremity movements. Ten healthy subjects performed a repetitive task similar to sawing continuously until volitional exhaustion. This task was synchronized with a m
