Antacid-induced phosphate depletion syndrome presenting as nephrolithiasis.

A 29-year-old insulin-dependent diabetic woman developed phosphate depletion, nephrolithiasis and bilateral ureteric obstruction due to antacid abuse. Unlike previous descriptions of chronic phosphate depletion, myalgia, weakness and bone pain were absent. Biochemical features included hypophosphataemia, hypercalciuria, hypophosphaturia, elevated plasma, 1,25-dihydroxyvitamin D and low plasma intact parathyroid hormone. These abnormalities were corrected when antacid ingestion was reduced and phosphate intake supplemented. We propose that phosphate depletion secondary to antacid abuse caused 1

Silent Obstructive Nephrolithiasis Resulting in a Non-functional Kidney.

Asymptomatic nephrolithiasis can lead to progressive and irreversible kidney damage. We report a 79-year-old woman with incidentally discovered bilateral kidney stones identified on low-dose computed tomography (CT) imaging performed for lung cancer screening. Imaging revealed a 24 mm left renal stone with severe cortical thinning, suggestive of markedly reduced left renal function, and an 8.7 mm right ureteral stone causing hydronephrosis in a functional kidney. The patient denied flank pain, discomfort, hematuria, or other urinary symptoms. Laboratory evaluation demonstrated preserved renal

[Carbapenem-resistant urinary tract infection in patients with nephrolithiasis].

INTRODUCTION: Carbapenems belong to reserve antibiotics; however, the frequency of carbapenem-resistant bacterial strains is increasing in hospitals. Resistance of nosocomial Gram-negative strains to carbapenems is a significant problem with a tendency to progress, necessitating close attention to it. OBJECTIVE: Analysis of the prevalence of carbapenem-resistant flora in a urology clinic, identifying the most significant factors associated with the resistance of pathogens to medications. MATERIAL AND METHODS: A single-center retrospective study was conducted analyzing 1,720 urine culture res

Extreme Reactive Thrombocytosis Caused by Obstructive Nephrolithiasis and Pyelonephritis.

Platelet counts in reactive thrombocytosis rarely exceed 1000 × 109/L. We present the case of a male patient, aged 80 years, with quiescent rheumatoid arthritis who was found to have a platelet count of 1011 × 109/L on routine laboratory testing. The patient was initially asymptomatic but developed leukocytosis to 23.1 × 109/L on hospital day 2. Diagnostic work-up revealed obstructive nephrolithiasis and pyelonephritis, and the thrombocytosis and leukocytosis gradually resolved with empiric antibiotic treatment and ureteral stent placement. Tests for myeloproliferative disorders, including JAK

Proton pump inhibitors use and risk of incident nephrolithiasis.

Proton pump inhibitors (PPIs) are widely prescribed medications that have effects on both enteric and urinary solute handling with an unknown effect on risk of nephrolithiasis. Our objectives were to examine the association between PPI exposure and incident nephrolithiasis and to determine its effect on 24H urine chemistry. We performed a single-center retrospective study on patients diagnosed with gastroesophageal reflux disease (GERD) without a history of kidney stones. Exposure to PPIs was abstracted, and then subsequent kidney stone diagnoses were identified. Multivariable Cox models with

Normocalcaemic primary hyperparathyroidism: is nephrolithiasis more common than osteoporosis?

BACKGROUND: Normocalcaemic primary hyperparathyroidism (NPHPT) is often under-recognised in clinical practice. AIM: To determine the prevalence and clinical significance of NPHPT in an unselected sample in an acute hospital setting. METHODS: Patients aged >18 years who had measurement of an elevated serum parathyroid hormone (PTH ≥ 7 pmol/L) during 12 months from 1 January 2017 to 31 December 2017 were retrospectively studied. NPHPT was defined by the presence of elevated serum PTH with normal albumin-corrected serum calcium on two or more occasions after excluding secondary causes. Patients

Patient-Reported Outcomes in Nephrolithiasis: Can We Do Better?

PURPOSE: Nephrolithiasis contributes significantly to the worldwide healthcare burden. Patient-reported outcome measures (PROMs) are widely recognized as important outcome measures for patient-centered care. We sought to summarize what is currently known about PROMs in urinary stone disease and to characterize use of PROMs in randomized controlled trials (RCTs) for nephrolithiasis. MATERIALS AND METHODS: Health-related quality of life (HRQOL) in patients with nephrolithiasis is reviewed. A search of both MEDLINE and EMBASE databases for RCTs in nephrolithiasis was performed to assess utilizat

Acetazolamide-induced nephrolithiasis: implications for treatment of neuromuscular disorders.

Carbonic anhydrase inhibitors can cause nephrolithiasis. We studied 20 patients receiving long-term carbonic anhydrase inhibitor treatment for periodic paralysis and myotonia. Three patients on acetazolamide (15%) developed renal calculi. Extracorporeal lithotripsy successfully removed a renal calculus in one patient and surgery removed a staghorn calculus in another, permitting continued treatment. Renal function remained normal in all patients. Nephrolithiasis is a complication of acetazolamide but does not preclude its use.

Pediatric Nephrolithiasis: A Changing Landscape Through Time and Space.

Pediatric nephrolithiasis is an ancient and complex disorder that has seen a significant rise in recent decades and the underlying causes contributing to stone formation in children may also be shifting. Historically, kidney stones have been linked to factors such as metabolic disorders, congenital abnormalities, and family history. However, the recent increase in incidence appears to be associated with new risk factors, including changes in lifestyle and diet, the growing prevalence of obesity, metabolic syndrome, diabetes, and even climate change. Given this evolving landscape, performing a

Patient-Reported Financial Toxicity Associated With Management of Nephrolithiasis.

OBJECTIVE: To characterize stone-related financial burden among adults with nephrolithiasis through validated questionnaires for financial toxicity. METHODS: We performed a cross-sectional survey of adults with history of nephrolithiasis at an outpatient clinic. The survey contained a measure of stone-related financial toxicity (COST score), and assessed demographics, stone event history, and burden of overall, ancillary, preventative, and insurance costs. A COST score ≤21 was classified as high financial toxicity. Multivariable logistic regression was used to evaluate associations with demog

Nephrolithiasis and Elevated Urinary Ammonium: A Matched Comparative Study.

OBJECTIVE: To describe the associations between elevated urinary ammonium and clinical characteristics of kidney stone formers. A 24-hour urine test is recommended in high-risk patients to identify urinary abnormalities and select interventions to reduce the recurrence risk. While elevations in urine ammonium may be seen in acidosis, diarrhea, high protein diets or due to pathogenic bacteria, the clinical characteristics of these patients have not been previously described. METHODS: We retrospectively identified adult patients with kidney stone disease who completed a 24-hour urine at our ins

Temporary Memory Steal: Transient Global Amnesia Secondary to Nephrolithiasis.

Transient global amnesia (TGA) is typified by an abrupt and transient anterograde amnesia, "with repetitive questioning and often variable retrograde amnesia persisting up to 24 hours." A 54-year-old male presented to our emergency department with paroxysms of left-sided flank pain, suggestive of renal colic. Computed tomography (CT) of the abdomen/pelvis revealed a three-millimeter left ureterovesicular-junction calculus. Pain control proved difficult, necessitating multiple doses of opioid and non-opioid analgesia. Subsequently, the patient developed repetitive questioning and perseveration
