Urothelial cancer of the renal pelvis in percutaneous nephrolithotomy patients.

INTRODUCTION: The association between staghorn calculus of the kidney long-standing and urothelial tumors of the renal pelvis is well documented. We describe 3 patients with urothelial cancer in a kidney who underwent percutaneous nephrolithotomy (PCNL). PATIENTS AND METHODS: Retrospective analysis of our series of more than 500 PCNLs revealed 3 patients in whom urothelial cancer of the renal pelvis was diagnosed during or following percutaneous stone removal. Preoperative assessment included IVP, renal sonography and renal scan using DTPA in all 3 patients, and CT in 2 patients. Imaging did

Transgluteal CT-Guided Percutaneous Renal Access for Percutaneous Nephrolithotomy in a Pelvic Horseshoe Kidney.

CT-guided percutaneous renal access has been described as a safe and effective access technique in patients with complex anatomy, including ectopic kidney, retrorenal colon, spinal dysraphism, hepatomegaly, and splenomegaly. In comparison to conventional intraoperative fluoroscopic-guided access, CT imaging allows for delineation of surrounding structures that are at risk for injury during percutaneous access. However, previous reports indicate that pelvic kidneys might be inaccessible percutaneously without laparoscopic assistance. Herein, we present a novel transgluteal route to renal access

Kidney Parenchymal Volume Loss and Functional Loss After Percutaneous Nephrolithotomy.

PURPOSE: Renal stone is a common global disease. Percutaneous nephrolithotomy (PCNL) is the standard treatment for large stones but may cause renal parenchymal injury. Most studies have focused on global renal function rather than localized renal changes. This study evaluated renal parenchymal volume and renal function after PCNL, hypothesizing modest ipsilateral volume loss with preserved global glomerular filtration rate (GFR). MATERIALS AND METHODS: This study included 80 patients who underwent PCNL. All patients had preoperative and postoperative computed tomography (CT), with follow-up C

Percutaneous nephrolithotomy: Three-needle technique on two planes. Cury's technique.

INTRODUCTION: Percutaneous nephrolithotomy is the main type of surgery indicated for kidney stones larger than 2 cm. The present study describes a three-needle technique for percutaneous nephrolithotomy on two planes. SURGICAL TECHNIQUE: The patient is first placed in the lithotomy position for cystoscopy, which guides the ureteral and urethral catheter. Next, the patient is placed in ventral decubitus for the three-needle technique. With the aid of the nephroscope, the first needle is positioned in the projection of the renal pelvis, the second needle is placed in the most posterior of the i

Initial Clinical Experience with Swiss LithoClast Trilogy During Percutaneous Nephrolithotomy.

Introduction and Objective: Current available lithotrites have clinical stone clearance rates averaging 24 to 32 mm2/minute. The objective of this study was to critically evaluate the initial experience with the Swiss LithoClast® Trilogy lithotrite during percutaneous nephrolithotomy (PCNL). Methods: We prospectively enrolled patients with a minimum of 15 mm of stone in axial diameter at three locations (Indiana University, University of California Davis, and University of California San Diego) scheduled to undergo PCNL for nephrolithiasis over a 60-day trial period. We assessed objective meas

Oxygen Venous Embolism After Hydrogen Peroxide Use During Percutaneous Nephrolithotomy.

Background: Hydrogen peroxide (H2O2) is a common antiseptic that is available without a prescription in the United States, and it is indicated for minor dermal abrasion; mouth, gum, or dental irritations; and removal of oral secretion. Several other medical uses have also been described, including clot dissolution during endoscopic gastrointestinal evaluation, cleansing of orthopedic surgical sites, and bladder irrigation. However, these uses of H2O2, as well as high-dose ingestion, have been associated with a wide variety of medical complications, including but not limited to air pulmonary em

Renal pelvic stones: choosing shock wave lithotripsy or percutaneous nephrolithotomy.

Introduction of minimally invasive techniques has revolutionized the surgical management of renal calculi. Extracorporeal shock wave lithotripsy and percutaneous nephrolithotomy are now both well-established procedures. Each modality has advantages and disadvantages, and the application of each should be based on well-defined factors. These variables include stone factors such as number, size, and composition; factors related to the stone's environment, including the stone's location, spatial anatomy of the renal collecting system, presence of hydronephrosis, and other anatomic variables, such

Complications of Percutaneous Nephrolithotomy: Experience From a Tertiary Care Center.

Introduction Renal stones are mineral concretions in the pelvicalyceal system. Their prevalence and recurrence are increasing globally. Percutaneous nephrolithotomy (PCNL) is a minimally invasive procedure for the removal of kidney stones. It is a safer technique offering the highest stone-free rates. However, a few complications may still occur. We aimed to evaluate our experiences of PCNL and classify the complications as intraoperative, early postoperative, and late postoperative events. We also aimed to find the predictors of complications in PCNL. Methods A single-center prospective obser

Predictors of Persistent Residual Stones After Second Look Percutaneous Nephrolithotomy.

Introduction: Second look flexible nephroscopy (SLFN) is often offered after percutaneous nephrolithotomy (PCNL). However, even after SLFN, residual fragments (RFs) occasionally persist. We sought to determine the incidence of persistent RFs in patients who undergo SLFN for RFs after PCNL and to identify parameters predictive of persistent RFs. Methods: We identified all patients at our institution with RFs on post-PCNL computed tomography (CT) that underwent SLFN between 1/2016 and 5/2020. Patient demographics, stone characteristics, and perioperative parameters were analyzed for each study p

Mini versus ultra-mini percutaneous nephrolithotomy in a paediatric population.

OBJECTIVE: To evaluate whether there would be a difference in outcome when the smaller ultra-mini 12 Fr sheath was used instead of the mini 16 Fr sheath for percutaneous nephrolithotomy (PCNL) in paediatric patients for stones less than 25 mm. METHODS: This was a prospective cohort study of patients who underwent PCNL in our hospital in a 2-year period from July 2016 to June 2018 by a single surgeon. PCNL was performed in a prone position and tract was dilated to the respective size using single step dilatation. Laser was used to fragment the stone. Stone-free outcome was defined as absence o

Erector Spinae Plane Block for Perioperative Analgesia after Percutaneous Nephrolithotomy.

Erector spinae plane block was recently introduced as an alternative to postoperative analgesia in surgical procedures including thoracoscopies and mastectomies. There are no clinical trials regarding erector spinae plane block in percutaneous nephrolithotomy. The aim of our study was to test the efficacy and safety of erector spinae plane block after percutaneous nephrolithotomy. We analyzed 68 patients, 34 of whom received erector spinae plane block. The average visual analogue scale score 24 h postoperatively was the primary endpoint. The secondary endpoints were nalbuphine consumption and

Unplanned flexible ureteroscopy during percutaneous nephrolithotomy in the prone position.

BACKGROUND: Over time, the prone position has been shown to be effective and safe for performing percutaneous nephrolithotomy. One of its disadvantages, however, is the inability to simultaneously perform an ureteroscopy after positioning the patient, which is feasible in the supine position. Our objective is to demonstrate the feasibility of implementing flexible ureteroscopy with the patient in the prone position. MATERIAL AND METHODS: We present a case of percutaneous nephrolithotomy of partial staghorn calculi in the prone position during which access to the middle calyx was not possible.
