BACKGROUND: We present the results of an alternative technique for decompression of a Morton intermetatarsal neuroma that is minimally invasive and does not require endoscopic instrumentation.
METHODS: Seventeen nerve decompressions were performed in 14 patients using an instrument designed to release the transverse carpal ligament for carpal tunnel syndrome. Nine women and five men (3 bilateral) had the procedure performed. The mean followup for the 17 decompression procedures was 25.92 months.
RESULTS: Eleven of the 14 patients had absence of the neuroma symptoms after the decompression. T

OBJECTIVE: To estimate the prevalence of "incidental" acoustic neuromas (ANs) in the population at large.
DESIGN: An intracranial magnetic resonance imaging (MRI) database of 46 414 patients presenting to the University of California, San Francisco (UCSF), without known audiovestibular complaints was searched retrospectively from July 1995 to February 2003. Seventy percent of these MRIs included gadolinium, and none was specifically targeted through the internal auditory canal. A medical chart review of 688 patients with acoustic neuromas presenting to UCSF between 1980 and 1999 was searched

This article reviews the Montreal experience of hearing preservation in acoustic neuroma surgery. The medical records since 1995 of 36 patients who underwent acoustic neuroma extirpation with the intent to preserve hearing were examined. Intraoperative monitoring was conducted using auditory brainstem response measurement with electrocochleography via a transtympanic electrode. The role of intraoperative monitoring in guiding surgical technique and its correlation with postoperative hearing outcome are discussed. A review of the literature regarding hearing preservation in acoustic neuroma sur

OBJECTIVE: To review complications that occur during the course of acoustic neuroma surgery.
STUDY DESIGN: Database and retrospective case review.
SETTING: Tertiary referral center, private neurotologic practice.
PATIENTS: A series of 1,687 patients undergoing acoustic neuroma surgery between 1987 and 1997. The 822 male and 865 female patients ranged in age from 10 to 87 years (mean age at time of surgery, 50 yr; standard deviation, 14 yr). The most common surgical approach was translabyrinthine (72.5%), followed by middle fossa (25.7%). The tumors ranged in size from 3 to 7 cm in diameter

OBJECTIVE: Evaluating patients who have had surgical management of acoustic neuroma has relied heavily on the surgeon's viewpoint for determining success. However, the perspective of the surgeon may be different from that of the patient. Thus, a recent increased interest in terms of quality of life has been documented by the literature on this specific topic essentially through the use of a questionnaire. The objective of this paper was to review this topic in our series of patients operated on for acoustic neuroma to ascertain the personal and social impact that surgery has had on their lifes

Patients older than 65 years who develop acoustic neuromas have the same signs and symptoms as younger patients. Age limits beyond which surgery for acoustic neuroma is currently not recommended are unreasonable. Untreated vertigo in older patients frequently results in falls that can cause fracture of the femur and significant morbidity and mortality. Surgical removal of acoustic neuromas in patients older than 65 produces results that are as good as those seen in younger patients. The patient's general medical condition, life expectancy, and factors other than chronological age should be con

Long-lasting severe headaches are reported to occur in up to 83% of patients who have undergone resection of acoustic neuroma, especially through a suboccipital approach. These headaches, however, are not well defined. The objective of this study was to assess the frequency and character of new-onset headaches after resection of acoustic neuroma by a suboccipital approach with cranioplasty. Review of the medical record was followed by a telephone interview with 48 patients (67% female; mean age, 52 years) who had undergone resection of an acoustic neuroma through a suboccipital craniotomy duri

The purpose of this investigation was to study the effects of translabyrinthine acoustic neuroma surgery on tinnitus in a consecutive sample of patients operated on between 1988 and 1994 in Uppsala (Sweden). A postal questionnaire was returned by 141 patients, yielding a 90% response rate without reminder. The results showed that tinnitus was experienced by 70% of the patients before surgery and 60% after surgery. In general, low degrees of tinnitus distress were found, which was confirmed by the questionnaire results. Ratings of tinnitus distress after surgery, using the Klockhoff and Lindblo

A 22-year-old male with unilateral facial, cochlear and glossopharyngeal cranial nerve dysfunction and ataxia that was slowly progressive over one year is described. Although clinically it was considered he probably had an acoustic neuroma, surgery revealed a pontine glioblastoma exophytic into the cerebellopontine angle and internal acoustic meatus. Clinicoradiological features that distinguish this extremely rare lesion of the hindbrain from acoustic neuroma are discussed.

To avoid misdiagnosis and mistherapy of acoustic neuroma, 104 cases of acoustic neuroma treated between 1986 and 1995 were retrospectively reviewed. Among them, 23 patients (24 ears, 23%) presented with sudden hearing loss as the starting symptoms. The clinical and audiological analysis demonstrated that 54.2% of the ears showed hearing loss over 71 dB HL, and all ears had deteriorated ABRs. -SP/AP was measured in nine ears, seven showed -SP/AP greater than 0.4. Acoustic reflex was negative in all 11 ears tested, and 94.4% of the ears showed abnormal ENG. The rate of positive CT scan was 88.8%

The purpose of this study was to establish the frequency and pattern of depressive disorders after surgery for acoustic neuroma, and to look for associations. Twenty seven patients with acoustic neuroma underwent thorough psychiatric assessment before surgery and at three and 12 months after surgery. Three patients had a depressive disorder in the preoperative assessment. Of the remaining 24 patients, nine (38%) had a depressive disorder at the three month check up. Deterioration of hearing was the only postoperative detriment associated with a depressive disorder (P = 0·024). All nine patient

Bilateral vestibular schwannomas (VSs) are the hallmark of neurofibromatosis type 2 and the crucial criteria for the diagnosis of this autosomal dominant disorder according to the criteria of the National Institutes of Health Consensus Statement. We describe three patients without VSs aged 47, 52 and 69 years, in whom a NF2 gene carrier status was diagnosed by spinal tumors, a cataract and schwannoma of cranial and peripheral nerves. In one case the diagnosis was ascertained by mutation analysis, which revealed a 163bp deletion in the NF2 cDNA, and by the fact that two daughters had the same d

We have been able to culture acoustic neuroma (AN) tissues by taking small tumor pieces during surgery from the cisternal or, preferably, metal part of the tumor where macroscopic tissue degeneration is not prominent. The AN pieces were transported in culture tubes to our cell laboratory where they were sliced and transferred to culture medium. The AN tissue demands a crude culture suspension containing laminin. We have been successful in culturing 15 successive ANs. The cells have different morphological features with characteristics of type Anthony A and B. Four main cell types were recogniz

A series of 151 patients with 154 acoustic schwannomas have been operated upon in Manchester Royal Infirmary by a joint Otological and Neurosurgical team, employing either the translabyrinthine or the suboccipital approach. The perioperative mortality rate was 3 per cent. Anatomical preservation of the facial nerve was achieved in 89 per cent of tumour removals and a good to normal functional result in 79 per cent of cases. Attempts at hearing preservation have been unsuccessful, largely because of the small number of patients in the series in whom useful hearing was present preoperatively. Co

The authors present a case of seronegative rheumatoid arthritis and symptomatic Morton's neuroma. The thought process of the differential diagnosis is explored extensively with substantial support and review of existing literature.
