The role of nitrate compounds in the therapy of pancreatic diseases is discussed in this review. A short overview is given about the physiological background of the treatment: the regulation of Oddi's sphincter function and its pharmacology. Noninvasive hemodynamic and clinical studies were carried out in 24 patients with congestive heart failure refractory to digitalis, diuretics and vasodilators. Nitroglycerin was given by intravenous infusion for 5-7 days. The results showed that the symptoms, signs and hemodynamic parameters were markedly improved. The overall clinical effective rate was 96% and no serious side effect was noted.

To determine whether nitroglycerin is as effective as nifedipine in lowering the blood pressure in severe hypertension and hypertensive crisis, two groups of 20 patients received in random sequence either 1.2 mg nitroglycerin sublingually or a 10-mg nifedipine capsule, which was chewed and swallowed. The blood pressure fell after 5 min in the nitroglycerin group from 211/122 mmHg to 171/95 mmHg and after nifedipine from 210/118 to 185/102 mmHg. The effect of intravenous (IV) nitroglycerin (NTG) on perioperative myocardial ischemia as detected by single pass radionuclide angiocardiography was studied in 20 patients scheduled for elective coronary artery bypass grafting (CABG). The loss of nitroglycerin (glycerol trinitrate (GTN), Perlinganit) when infused at various rates through polyvinyl chloride (PVC) tubing was measured against time by continuously monitoring the GTN concentrations at the outflow of the tubing using a direct photometric method. Little is known of the clinical significance of myocardial bridges, which may be recognized angiographically as systolic coronary artery narrowing (SCAN). A retrospective review of a 1 year's experience (313 consecutive coronary arteriograms) revealed 5 patients with SCAN, an incidence of 1.6%. PURPOSE: Identification of the Adamkiewicz artery before aortic surgery is important for preventing postoperative complications due to spinal cord ischemia. The Adamkiewicz artery is difficult to identify due to its small diameter.
