Immediate postoperative and mid-term survival in nonagenarians undergoing non-traumatic emergency surgery.

BACKGROUND: Nonagenarian people are increasingly using the emergency services, however, few studies have addressed this population. The goal of this prospective observational study is to determine relevant factors that independently reduce their immediate postoperative and mid-term survival. METHODS: We included all the nonagenarians who underwent non-traumatic emergency surgery between 2006-2011. The dependent variable, days of postoperative survival, was studied at 2 intervals: 0-30 days and 31 days to 1 year after surgery (immediate and mid-term respectively). Firstly, we calculated the Ka

Too old to operate? Retrospective outcomes of laparoscopic paraesophageal hernia repair in nonagenarians.

INTRODUCTION: The majority of paraesophageal hernias (PEH) present in patients > 65 years old. We have previously reported the safety of laparoscopic paraesophageal hernia repair (LPEHR) in these patients. However, information regarding outcomes of LPEHR in nonagenarians is scant. Surgical mortality in nonagenarians has been reported as high as 20%. Many referring providers are reluctant to refer elderly patients for LPEHR because they believe the surgical risk is prohibitive. We designed this study to evaluate LPEHR outcomes at our institution in nonagenarians, elective vs non-elective. METH

Gross hematuria in nonagenarians admitted to a urological ward: prevalence, predictors, and outcomes.

PURPOSE: To describe the trajectories of nonagenarians admitted to a urological ward and to identify risk factors for admission due to gross hematuria (GH) and its impact on discharge status. METHODS: A retrospective cohort study of nonagenarians admitted to a tertiary care center between 2014 and 2022 was conducted. Data on demographics, comorbidities, medication, and outcomes were collected. Frailty was assessed via the Canadian Study of Health and Aging Clinical Frailty Scale (CSHA-CFS). Multivariable Poisson regression was used to assess risk factors for GH-related admissions. Secondary e

Long-term Stroke and Mortality Risk in Nonagenarians After Transcatheter Aortic Valve Insertion.

BACKGROUND: Limited data exist on the long-term outcomes of transcatheter aortic valve insertion (TAVI) in nonagenarian patients. This study investigated the relationship between patient baseline comorbidity and frailty on the long-term outcome of the nonagenarian population. METHODS: A retrospective analysis was conducted of 187 consecutive nonagenarian patients who underwent TAVI from 2009 to 2020. Multivariable models were used to analyze the association between baseline patient and frailty variables and mortality, stroke, and repeat hospitalization. Long-term survival was compared with an

Surgical treatment for cervical spine injury in nonagenarians: A report of three cases.

Among the elderly, even minor injuries can cause cervical spine fractures. With the increasing number of nonagenarians, the opportunities for treatment of cervical spine injuries in nonagenarians are getting to be more common. Conservative therapy is often chosen in nonagenarians with cervical spine injuries because of high risk associated with surgical treatment; however, we present herein the cases of three patients in nonagenarians who underwent surgical treatment for cervical spine injury. After a fall, three cases of nonagenarians who lived alone and independent were diagnosed with a Jeff

Clinical outcomes of nonagenarians with acute myocardial infarction who undergo percutaneous coronary intervention.

BACKGROUND: With increases in life expectancy, percutaneous coronary intervention is being performed more often, even in elderly patients with acute myocardial infarction. However, the optimal management of nonagenarians with acute myocardial infarction is uncertain. This study sought to investigate clinical outcomes of nonagenarians who undergo percutaneous coronary intervention. METHODS: Of 2640 consecutive patients with acute myocardial infarction hospitalised within 24 hours after symptom onset in 2009-2018, we prospectively analysed 96 nonagenarians (median age 92 years; interquartile ra

Inaccuracy of standard geriatric scores in nonagenarians following hospitalization for various spinal pathologies.

Demographic trends make it incumbent on spine surgeons to recognize the special challenges involved in caring for older patients. The aim of this study was to identify variables that may predict early mortality in geriatric patients over the age of 90 following elective hospitalization for various spinal pathologies. Retrospective analyses of all patients over the age of 90 years, which were electively treated between 2006 and 2016 at out department for various spinal conditions, were performed. Patient characteristics, type of treatment, and comorbidities were analyzed with regard to the 30-d

[Prevalence of carotid artery stenosis in nonagenarians: Survey in a primary care hospital].

AIM: Carotid artery stenosis increases with age and may cause brain ischemia if arterial hypotension occurs. We performed a monocentric pilot study to investigate its prevalence in the very elderly and to assess its potential influence on blood pressure (BP) goals during antihypertensive treatment. METHODS: All patients≥90 years of a primary care medical ward were prospectively included over 15 months. Ultrasound exams of the precerebral arteries were offered to all elderly patients for routine evaluation of their cardiovascular risk. Frequencies of stenosed common, internal and external caro

Short- and mid-term results after transapical transcatheter aortic valve replacement in nonagenarians.

BACKGROUND: Recent reports have suggested that advanced age may preclude favorable outcomes in transcatheter aortic valve replacement (TAVR), particularly when performed via transapical (TA) access. However, detailed examinations of TA-TAVR in nonagenarian patients are lacking in the contemporary literature. We therefore describe our experience with 25 consecutive nonagenarians who underwent TA-TAVR and report their short- and mid-term outcomes. METHODS: We identified all patients 90 years old or greater who underwent TA-TAVR between 2009-2014 at our institution. Demographic, comorbidity and

Primary knee and hip arthroplasty among nonagenarians and centenarians in the United States.

OBJECTIVE: The number of individuals ages >or=100 years in the US is expected to increase considerably in the future along with the need for arthroplasties. This report focuses on the poorly studied epidemiology and mortality outcomes of arthroplasty among these individuals. METHODS: We describe the epidemiology of knee and hip arthroplasties among centenarians using data from a large hospital discharge database in the US (the Nationwide Inpatient Sample) during the period 1993 through 2002. We used nonagenarians as the comparison group with adjustment for differences in the prevalence of con

Middle meningeal artery embolization for chronic subdural hematoma in the nonagenarian population.

BACKGROUND: The estimated incidence of chronic subdural hematomas (cSDH) in the general population is projected to nearly double over the next decade, likely making it the most commonly treated cranial neurosurgical condition in adults by 2030. We investigated the outcomes of middle meningeal artery embolization (MMAe) as either a primary or adjunctive treatment for cSDH in nonagenarian patients. METHODS: We retrospectively reviewed all patients 90 years of age or older treated with middle meningeal artery embolization for cSDH from 2018 to 2024 at two academic institutions. Transcatheter Atrial Septal Defect Closure in a 'Nonagenarian' with Intractable Heart Failure.

A 92-year-old man was referred to our hospital with decompensated heart failure. He was treated with diuretics and inotropic agents, but the clinical response was unsatisfactory. Echocardiography incidentally revealed an atrial septal defect (ASD) with a significant left-to-right shunt and pulmonary-to-systemic-blood-flow ratio of 2.36. Because the ASD was considered to be the primary cause of his treatment-resistant heart failure, the patient underwent transcatheter ASD closure. Shortly after shunt closure, his hemodynamics significantly improved, and he was discharged nine days later. This i

Paradoxical Embolism of Stroke-Related Patent Foramen Ovale in a Nonagenarian Woman.

There are few clinical reports of elderly patients with paradoxical embolism in the current literature. Herein, we describe the case of a nonagenarian patient with paradoxical embolism of stroke-related patent foramen ovale (PFO). A 95-year-old woman was admitted to our hospital because of dysarthria. Her medical history included cerebral infarction, hypertension, and dyslipidemia. Magnetic resonance imaging performed in the emergency room revealed cerebral infarction of the left temporal lobe. After hospitalization in the neurosurgery department, we performed further clinical investigations t

Isolated testicular pain mimicking ruptured abdominal aortic aneurysm in a nonagenarian.

Ruptured abdominal aortic aneurysm commonly presents as acute abdominal or lower back pain and hemodynamic instability. We discuss the case of a 90-year-old patient who presented to the emergency unit with a 3-day history of left testicular pain. Ultrasound scan demonstrated and a computed tomography scan confirmed a 6 cm in diameter ruptured abdominal aortic aneurysm with enlargement and hematoma of the left psoas muscle causing the symptoms. This atypical presentation highlights the need for clinical vigilance and emergency physician–performed ultrasound scan in the older patients with seemi

Outcomes of nonagenarians undergoing hip fracture surgery: a multicentre observational study across Asia-Oceana.

BACKGROUND: Nonagenarians with hip fracture represent a rapidly growing, highly vulnerable subgroup, yet comparative data across diverse health‑care systems in Asia‑Oceania are scarce. We aimed to describe mortality, postoperative complications, and perioperative care patterns in nonagenarians undergoing hip fracture surgery in Australia and Malaysia, and to explore associations between key perioperative factors and mortality. METHODS: We conducted a binational, retrospective, multicentre observational study of consecutive nonagenarians (aged ≥ 90 to < 100 years) who underwent surgery for unil
