Improving trends estimates for specific work-related ill-health conditions when excess zeros are present in a voluntary health reporting scheme.

BACKGROUND: Trends in occupational disease incidence are estimated in voluntary reporting schemes such as The Health and Occupational Reporting (THOR) Network in the UK. Voluntary reporting schemes request responses even if no cases are observed to reduce uncertainty in non-response. This may result in false zeros that bias trends estimates. Analysis using zero-inflated models is unsuitable for specific health outcomes due to overestimates of the excess zeros. Here, we attempt to account for excess zeros while investigating condition-specific trends. METHODS: Zero-inflated negative binomial (

Evolution of work conditions for employees driving for work in France based on data from the 2003, 2010 and 2017 SUMER surveys (Surveillance Médicale des expositions aux risques Professionnels).

INTRODUCTION: The occupational road-accident risk on public roads and the work conditions for professional driving is still an important issue in occupational health despite lower road-accident rates. This study presents the evolution over time of the work-related constraints for these employees based on the Sumer surveys carried out in 2003, 2010 and 2017. METHOD: Data from the 2010 and 2017 surveys were restricted to match the scope of the 2003 survey in order to enable prevalence data to be compared in equivalent populations. The main variable of interest was "driving (car, truck, bus, and

Review of Diagnostic Challenges in Occupational Asthma.

PURPOSE OF REVIEW: Occupational asthma (OA) is one of the most frequent occupational diseases and its diagnosis is often difficult. This review summarizes its current diagnostic challenges. RECENT FINDINGS: OA is associated with significant health and socio-economic burden. It is underdiagnosed and physicians need to adopt a stepwise approach to confirm the diagnosis. Although early removal from exposure to the offending agent is associated with a better prognosis, physicians should try to confirm the diagnosis of work-related asthma before taking a worker off work. A proper occupational and

[Hand and occupational diseases].

Hand is frequently the site of work accidents or occupational diseases. The musculoskeletal upper limb is the first recognized occupational disease and carpal tunnel syndrome is the most common of them. The most common location of occupational dermatoses is the hand. Their causes are often multifactorial, involving chemical irritants, physical, allergens and endogenous factors (mainly atopic dermatitis). Occupational exposure to microtrauma and iterative use of vibrating tools may also be the cause of hypothenar hammer syndrome and acrosyndromes. The frequent chronicity and functional impairme

Distinct Burnout Profiles in Physicians: Relationships with Occupational Satisfaction, Professional Identification, and Intellectual Humility.

Physician burnout represents a major occupational health concern associated with impaired wellbeing, reduced job satisfaction, and decreased quality of patient care. Although burnout is frequently examined using variable-centered approaches, less is known about distinct burnout profiles among physicians and their associations with professional identification and intellectual humility. The present study investigated relationships between burnout, job satisfaction, professional social identification, and intellectual humility among physicians using both variable-centered and person-centered appr

Inhalation accidents reported to the SWORD surveillance project 1990-1993.

Inhalation accidents are the fifth most common cause of illness reported by occupational and chest physicians to the SWORD surveillance scheme, with an estimated total of 1180 cases in the 5-year period 1989-1994. Questionnaires were sent to all physicians still participating, to record the circumstances of each inhalation accident they had reported between January 1990 and July 1993, covering case management and any resultant effects on health or employment; 93% of physicians returned the forms completed with adequate data on 85% of cases. From the information provided, 30% of the accidents r

The Management of Obstructive Sleep Apnea in Primary Care.

Approximately 30 million Americans suffer from sleep disorders. The incidence of and mortality rates associated with obstructive sleep apnea (OSA) have been increasing in recent years in the United States. OSA is associated with various health problems, including depression and hypertension, and it adversely affects occupational and academic performance. Hence, OSA is a major public health concern. Sleep specialists may be consulted for the evaluation and treatment of OSA. Continuous positive airway pressure (CPAP) is the mainstay of OSA treatment. The role of primary care physicians in such a

Seroprevalence of Leishmania infantum in outdoor workers from southern Italian endemic regions.

Visceral leishmaniasis is a zoonotic vector-borne disease caused by Leishmania infantum. The infection often remains asymptomatic, though clinical forms may occur in immunosuppressed individuals. Although data on leishmaniasis in humans are available worldwide, the exposure to L. infantum of workers conducting outdoor activities has been scantly investigated, and it is limited to military personnel operating in endemic regions. This study aimed to assess the seroprevalence of L. infantum in different groups of outdoor workers and the occupational risk factors. The cross-sectional study was per

[When the one who heals dies. The physician at the end of life].

A physician's death represents a critical point of tension between the values of modern medicine and the ethical limits of care. This review article analyzes how physicians face their own end of life and the type of care they receive compared with the general population. A narrative review of literature published between 2014 and 2025 was conducted using PubMed, Scopus, and SciELO databases with the MeSH and DeCS descriptors: "Terminal care", "Palliative care", "Attitude to death", "Health personnel", "Right to die". Findings show that although physicians express a preference for a dignified d

Workplace risk assessment criteria for pregnant workers exposed to physical exertion and biological and chemical hazards.

Workplace risk assessment criteria for pregnant workers in the EU remain inconsistent and poorly harmonised, with notable gaps in practical guidelines for occupational health physicians (OHPs). This regulatory ambiguity could lead to either insufficient protection or unnecessary exclusion of pregnant workers from the workplace, with significant implications for maternal and foetal health, as well as healthcare and social security systems. The aim of this study was to propose common, harmonised criteria for workplace risk assessment in healthy pregnant workers exposed to physical exertion and b

Contact urticaria from rubber gloves: an occupational skin disorder for health care workers.

Rubber allergy is an important occupational health problem for an increasing number of patients. It may produce a type I urticarial reaction, or more commonly, a type IV delayed eczematous dermatitis. The risk groups include health care workers, rubber industry workers, and children with spina bifida (meningomyelocele) and urogenital abnormalities. In this report, we describe a physician with a relevant history who was found to have type I hypersensitivity to rubber by prick test and use test. Our aim is to increase awareness among physicians, discuss diagnosis and management, and review the l
