Neurological problems presenting to an ophthalmic casualty department.

All patients presenting with neurological problems to an eye hospital casualty department over one year were prospectively studied. A total of 119 patients were identified. The most frequent diagnoses were retrobulbar neuritis (34; 28.5%), sixth cranial nerve palsy (22; 18.5%), third cranial nerve palsy (15; 12.6%) and Adie's tonic pupil (11; 9%). Cranial nerve palsies were most commonly due to diabetes or hypertension (16; 43.2%). Only one intracranial aneurysm was found. Symptoms included blurred vision (52; 43.7%), binocular diplopia (51; 42.8%), and eye pain (27; 22.7%). Fifty patients (42

Posttraumatic maxillary and ophthalmic herpes zoster in an immune-competent female - A unique presentation.

Herpes Zoster is a neuro-cutaneous disorder caused by reactivation of Varicella-Zoster Virus. A number of factors such as old age, psychological stress, low immune states, radiation exposure and physical trauma have been implicated for reactivation. Post traumatic herpes zoster involving maxillary and ophthalmic division of trigeminal nerve is very rare and has been reported in presence of Immune deficiency and following iatrogenic trauma (nerve block injection). We report a unique presentation of simultaneous ophthalmic and maxillary herpes zoster following a facial injury with a wooden stick

Contralateral and ipsilateral microsurgical approaches to carotid-ophthalmic aneurysms.

OBJECTIVE: The vicinity of carotid-ophthalmic aneurysms to the roof of the cavernous sinus, to the anterior clinoid process, and to the optic nerve or the optic chiasm requires well-defined surgical techniques. Although microsurgical techniques with ipsilateral direct approaches to these aneurysms have been described in detail, studies about contralateral strategies for the microsurgical treatment of carotid-ophthalmic aneurysms are rare and are mainly confined to case reports. The aim of this study is to describe how to decide on the ipsilateral and contralateral microsurgical approaches to s

Rare presentation of sarcoidosis with optic neuropathy and third nerve palsy.

Sarcoidosis is a systemic, idiopathic and granulomatous disease, which most commonly affects the skin, lungs and lymph nodes but can affect virtually any organ. Neurosarcoidosis can be the presenting or the only clinical manifestation accounting for 5%-15% of sarcoid diagnoses. In contrast to uveitis which is the most common ophthalmic manifestation, neuro-ophthalmic signs are uncommon in sarcoidosis. Optic neuropathy is the most common neuro-ophthalmic sign (70% in one series). Sarcoid-related optic neuropathy commonly presents with a picture similar to optic neuritis. Less commonly, optic ne

Evaluation and management of herpes zoster ophthalmicus.

Herpes zoster ophthalmicus occurs when the varicella-zoster virus is reactivated in the ophthalmic division of the trigeminal nerve. Herpes zoster ophthalmicus represents up to one fourth of all cases of herpes zoster. Most patients with herpes zoster ophthalmicus present with a periorbital vesicular rash distributed according to the affected dermatome. A minority of patients may also develop conjunctivitis, keratitis, uveitis, and ocular cranial-nerve palsies. Permanent sequelae of ophthalmic zoster infection may include chronic ocular inflammation, loss of vision, and debilitating pain. Anti

Microphthalmia, aphakia and agenesis of the optic chiasm and tract in a wild fox squirrel (Sciurusniger).

We describe gross and histopathological features of multiple ocular and neuro-ophthalmic abnormalities in a fox squirrel (Sciurus niger). Ophthalmic findings included severe bilateral microphthalmos, with the right eye more affected than the left. Histopathology confirmed severe microphthalmia, aphakia, disorganized retinal tissue and small optic nerves, as well as agenesis of the optic chiasm and optic tract. This combination of neuro-ophthalmic abnormalities has not been previously described in wild animals.

Neuro-ophthalmic sequelae of pediatric brain tumors.

Brain tumors are a leading cause of mortality and morbidity in the pediatric population. Neuro-ophthalmic symptoms can be a consequence of the disease process, as well as a feature of long-term complications from treatment. Compressive lesions of the visual pathway lead to changes in visual acuity and field defects. Double vision and strabismus can be some of the earliest clues of underlying posterior fossa tumors, which frequently cause hydrocephalus and manifest as optic nerve edema on neuro-ophthalmic examination due to increased intracranial pressure. Persistent complaints of blurry vision

Optic nerve and dura mater involvement as the first sign of multiple myeloma.

PURPOSE: To report a case of optic nerve and dura mater involvement as the first sign of multiple myeloma. METHODS: Case report. RESULTS: A 43-year-old woman presented with a headache and decreased vision in both eyes. Ophthalmic examination revealed anterior uveitis and subretinal mass around the optic nerves with accompanying disc edema bilaterally. Magnetic resonance imaging showed dural and optic nerve infiltration with tram-track enhancement in the optic nerve sheath. The diagnosis of multiple myeloma was made as a result of systemic investigations. The patient underwent systemic chemot

Microsurgical anatomy of sympathetic fibres running inside the cavernous sinus.

A microanatomical study has been designed to obtain data concerning the pattern of organization of the sympathetic fibres running within the cavernous sinus. The course of the sympathetic fibres has been examined in 60 fresh specimens of parasellar region from autopsy cadavers. The sympathetic nerve or nerves running with the carotid artery provide a multitude of thin branches at irregular intervals along its course, but the largest residual usually joins the 6th cranial nerve and leaves it to join the first division of the 5th. Despite other anatomical descriptions, we have found a direct con

Contralateral mini-craniotomy for clipping of bilateral ophthalmic artery aneurysms using unilateral proximal carotid control and Sugita head frame.

OBJECTIVE: Conventional surgical treatment of bilateral ophthalmic aneurysms would require bilateral craniotomies and bilateral neck dissections for proximal control of the cervical internal carotid artery (ICA). We present a semiemergent case where bilateral ophthalmic artery aneurysms were clipped using a unilateral mini-pterional craniotomy and contralateral proximal cervical ICA control while employing the Sugita head frame. Visual disturbance: where do I look?

Visual disturbances often signal serious ophthalmic disease. Comprehensive general medical and ophthalmic histories are most helpful. Information about the animal's vision, based on its performance in familiar and unfamiliar environments, should be obtained from the owner. As bilateral involvement and visual disturbances are often associated with systemic diseases, a complete physical examination is indicated. The ophthalmic examination consists of a series of diagnostic procedures to isolate and define the ophthalmic disorder. Several clinical tests can be performed easily in the examination

Having the nerve to home: trigeminal magnetoreceptor versus olfactory mediation of homing in pigeons.

The ability of pigeons to find their way home from unfamiliar sites located up to hundreds of kilometers away is well known, but the mechanisms underlying this ability remain controversial. One proposed mechanism is based on the suggestion that pigeons are equipped with magnetoreceptors that can enable the detection of either the earth's magnetic field and/or magnetic field anomalies in the local terrain over which the pigeons fly. Recent reports have suggested that these magnetoreceptors are located in the upper beak where they are innervated by the ophthalmic branch of the trigeminal nerve.

Unilateral hypoplasia of the trigeminal ganglion.

CASE REPORT: We report a case of unilateral anaesthesia of the V1 (ophthalmic) division of the trigeminal cranial nerve presenting with persistent corneal erosions and ulceration secondary to trigeminal ganglion hypoplasia. The patient had a lifelong history of unexplained left-sided ophthalmic symptoms for which numerous diagnoses were provided. Cranial nerve testing demonstrated partial trigeminal dysfunction on the left side. Further investigation eliminated viral etiologies, and subsequent magnetic resonance imaging determined that the patient had a hypo-plastic left trigeminal ganglion.
