Quality of Life Outcomes Following Pelvic Exenteration for Gynaecological Malignancies: A Systematic Review.

BACKGROUND/AIM: Pelvic exenteration (PE) remains one of the most radical procedures in gynaecological oncology and is associated with substantial morbidity and long-term complications. We aimed to synthesise evidence on short and long-term QoL outcomes following PE for advanced or recurrent gynaecological malignancies. METHODS: A systematic review was conducted using a prospectively registered protocol and reported in accordance with PRISMA guidelines. MEDLINE, Embase, PubMed and the Cochrane Library were searched from inception to October 2025. Eligible studies reported on one or more patien

Minimum radial margin in pelvic exenteration for locally advanced or recurrent rectal cancer.

PURPOSE: The aim of this study was to clarify the suitable radial margin (RM) for favourable outcomes after pelvic exenteration (PE), focusing on the discrepancy between the concepts of circumferential resection margin (CRM) and traditional R status. METHODS: Seventy-three patients with locally advanced (LARC, n = 24) or locally recurrent rectal cancer (LRRC, n = 49) who underwent PE between 2006 and 2018 were retrospectively analysed. Patients were histologically classified into the following 3 groups; wide RM (≥1 mm, n = 45), narrow RM (0-1 mm, n = 10), and exposed RM (n = 18). The analysis

Post-operative mobilisation and mobility outcomes after pelvic exenteration surgery for colorectal cancer.

BACKGROUND: Total pelvic exenteration surgery varies in the level of resection performed and requirement for reconstructive surgery. This study aimed to describe the recovery of mobility in the post-operative period and how it varies according to the surgery performed. METHODS: A retrospective audit of 104 patients was conducted, reflecting surgery conducted over a 7-year period. Data was collected until hospital discharge. Mobility was measured using the Intensive Care Mobility Scale. Results were analysed based on three levels of perineal resection (pelvic exenteration without abdominoperin

Surveillance After Pelvic Exenteration: A Systematic Review of Oncological and Functional Follow-Up.

BACKGROUND: Surveillance following cancer surgery with curative intent serves the purpose of permitting early detection of recurrent disease in addition to monitoring and management of post-resection functional sequelae. Although recommendations exist for most primary cancers, in the context of radical or re-operative surgery such as pelvic exenteration (PE), whether these strategies are appropriate is unclear. The purpose of this review was to evaluate the oncological and functional follow-up strategies after PE. METHODS: A comprehensive literature search was conducted across PubMed, Embase,

Modified posterior pelvic exenteration combined with ileocecal resection for locally advanced endometrial cancer.

PURPOSE: Chronic testicular pain remains an important challenge for urologists. At present there are many treatment modalities available for chronic orchialgia. Some patients remain in pain despite a conservative treatment. Microsurgical denervation of spermatic cord appears to be successful in relieving pain in patients who fail conservative management. We assessed the long-term efficacy, complications and patient perceptions of microsurgical denervation of the spermatic cord in the treatment of chronic orchialgia. MATERIALS AND METHODS: A prospective study was conducted from January 2007 to

Laparoscopic posterior pelvic exenteration for clear cell adenocarcinoma arising in an episiotomy scar.

Malignant degeneration of endometriosis is a very rare event, especially when it develops in an episiotomy scar. A 53-year-old woman with an enlarged perineal mass presented to the hospital. She had undergone vaginal delivery with episiotomy twice. Imaging analyses showed a mass involving the levator ani muscle apart from the rectum, with lymph node metastases to the right inguinal and internal iliac regions. A biopsy specimen of the right inguinal lymph node revealed poorly differentiated adenocarcinoma. She underwent neoadjuvant chemotherapy according to the treatment strategy of anal fistul

Robotic total pelvic exenteration for rectal cancer: case report and review of literature.

We describe a case report of a robotic total pelvic exenteration (TPE) performed for a locally advanced rectal cancer at our institution in August 2018. Technical details and comparison with published literature are discussed. A 62-year-old patient with a locally advanced low rectal cancer T4N1cM0 with extramural vascular invasion (EMVI) underwent an elective robotic pelvic clearance performed by the urology and colorectal teams. He received neoadjuvant long-course chemo-radiotherapy to downstage the rectal cancer. The primary tumour was T4N1c with involvement of the bladder (trigone area) and

Anterior Pelvic Exenteration for Chemo-irradiated Non-diethylstilbestrol Exposed Clear Cell Vaginal Cancer.

BACKGROUND/AIM: Clear cell vaginal adeno-carcinomas are rare tumors occurring in women which are usually treated by chemo radiotherapy with good outcomes. However, in certain cases, this treatment is not associated with complete response and a further surgery is needed. CASE REPORT: We present the case of a 38-year-old patient diagnosed with stage IVA clear cell vaginal cancer who had been previously submitted to radio chemotherapy and in whom the lesion persisted after the oncological treatment; therefore, the patient was proposed for surgery with curative intent. The tumor was resected by p

Incidental Gynecologic Tract Neoplasms in Women Undergoing Anterior Pelvic Exenteration for Urothelial Carcinoma.

Urothelial carcinoma (UC) invasive into the muscularis propria or tumors unresponsive to treatment are indications for cystectomy. In females, with the goal of achieving complete cancer eradication and for concerns of UC extension into the adjacent pelvic organs, this may also warrant resection of the gynecologic organs. This study is aimed to assess the prevalence of unanticipated gynecologic neoplasms in anterior pelvic exenteration specimens. A retrospective review of pathology reports to identify women undergoing anterior pelvic exenteration for UC was performed (N=221), and incidentally d

Systematic review of health-related quality of life in patients undergoing pelvic exenteration.

BACKGROUND: Pelvic exenteration is a radical operative treatment reserved for the management of a number of advanced primary and recurrent pelvic malignancies, including, rectal, gynaecological and urological. The advances in radiological staging, surgical techniques and greater use of chemotherapeutic agents haves translated clinically into improvements in the overall survival of this cohort of patients, irrespective of underlying disease pathology. Consequently, a greater proportion of the surviving population will present to healthcare professionals with a range of physical and psychologica
