Shockwaves delivery for aortic valve therapy-Realistic perspective for clinical translation?

Calcific aortic valve disease (CAVD) is the most frequent valvular heart disorder, and the one with the highest impact and burden in the elderly population. While the quality and standardization of the current aortic valve replacements has reached unprecedented levels with the commercialization of minimally-invasive implants and the design of procedures for valve repair, the need of supplementary therapies able to block or retard the course of the pathology before patients need the intervention is still awaited. In this contribution, we will discuss the emerging opportunity to set up devices t

The Nuss procedure for repair of pectus excavatum: 20 error traps and a culture of safety.

In 1998, Donald Nuss changed the way the world thought about the surgical repair of pectus excavatum. The new operation adheres to sound orthopedic fundaments, but has a significantly higher learning curve than other operations in pediatric surgery. Variations in pectus excavatum type, severity, symmetry and chest wall pliability bring challenges. This article will detail common error traps and ways to avoid them when performing the Nuss procedure. As recent publications have shown, an operation done more than 50,000 times across the world may bring to light infrequent but devastating outcomes

Synchondral Fracture of the Posterior "Hemiarch" of Pediatric Atlas with Cerebrospinal Fluid Fistula following a Penetrating Neck Injury.

INTRODUCTION: As such, the incidence of spinal fractures in young children is less than that of adults due to an increased pliability of the immature bones. The presence of unfused synchondroses in these children predisposes them to an infrequent pattern of fractures that traverse through ossification centers. Such synchondral injuries are uncommonly reported in the C1 and C2 vertebrae. Those that have been occasionally described in C1 involved the anterior synchondrosis. Furthermore, penetrating injuries to a pediatric spine are relatively rare. CASE PRESENTATION: In this context, we present

Hyaluronic acid: A key molecule in skin aging.

SKIN AGING IS A MULTIFACTORIAL PROCESS CONSISTING OF TWO DISTINCT AND INDEPENDENT MECHANISMS: intrinsic and extrinsic aging. Youthful skin retains its turgor, resilience and pliability, among others, due to its high content of water. Daily external injury, in addition to the normal process of aging, causes loss of moisture. The key molecule involved in skin moisture is hyaluronic acid (HA) that has unique capacity in retaining water. There are multiple sites for the control of HA synthesis, deposition, cell and protein association and degradation, reflecting the complexity of HA metabolism. Th

The lung and pediatric trauma.

Thoracic trauma is relatively frequent in children and causes considerable mortality. This is mainly due to the multiorganic nature of the trauma. The lung is more often affected even in the absence of rib fractures because of the considerable pliability of the chest wall that allows direct transfer of energy to this organ. Injuries to the heart, the aorta, the esophagus, and the diaphragm are rare. Lung contusion and laceration cause parenchymal hemorrhage and consolidation sometimes accompanied by pneumothorax and/or hemothorax. Tracheobronchial disruption is rare but life-threatening. Most

A newly developed shunt system for carotid surgery: T-shaped silicone shunt tubes and clamping devices: technical note.

BACKGROUND: Although the use of a shunt during carotid endarterectomy has been controversial, a convenient shunt system is still required. METHODS: We have devised an intraluminal shunt system, T-shaped silicone shunt tubes, and clamping devices for carotid surgery. The tube has enough pliability and length to be used in a looped or U-shaped mode and constructs a T-shape with a side arm available for monitoring blood pressure in the shunt system. Clamping devices, such as modified Sugita ring clips and encircle type bulldog clamps, make it possible to hold the tube by a simple maneuver and to

Fiber reinforcement of a resin modified glass ionomer cement.

OBJECTIVES: Understand how discontinuous short glass fibers and braided long fibers can be effectively used to reinforce a resin modified glass ionomer cement (RMGIC) for carious lesion restorations. METHODS: Two control groups (powder/liquid kit and capsule) were prepared from a light cured RMGIC. Either discontinuous short glass fibers or braided polyethylene fiber ribbons were used as a reinforcement both with and without pre-impregnation with resin. For the former case, the matrix was the powder/liquid kit RMGIC, and for the latter case the matrix was the capsule form. Flexural strength w

Impact of esophageal flexion level on the surgical outcome in patients with sigmoid esophageal achalasia.

PURPOSE: Esophageal achalasia can be roughly divided into non-sigmoid and sigmoid types. Laparoscopic surgery has been reported to be less than optimally effective for sigmoid type. The aim of this study was to examine the impact of the esophageal flexion level on the clinical condition and surgical outcomes of patients with sigmoid esophageal achalasia. METHODS: The subjects were 36 patients with sigmoid esophageal achalasia who had been observed for >1 year after surgery. The subjects were divided into sigmoid type (Sg) and advanced sigmoid type (aSg) groups based on the flexion level of th

Flexible ureteroscopy in children with von Willebrand disease.

BACKGROUND AND PURPOSE: Treatment options for urolithiasis include shockwave lithotripsy (SWL), percutaneous nephrostolithotomy (PCNL), and ureteroscopy. While these treatment options are becoming the standard of care in the pediatric population, children with a bleeding dyscrasia must be approached differently. We report our flexible ureteroscopic experience in children with von Willebrand disease (vWd). PATIENTS AND METHODS: A review of a prospectively maintained database of stone patients was performed. Children with a bleeding diathesis were identified. Stone burden, operative access, int

The effects of active release technique on hamstring flexibility: a pilot study.

OBJECTIVE: The aim of this study is to determine if active release technique (ART) significantly increases hamstring flexibility in healthy male participants. METHODS: Twenty physically active male participants with no current or previous history of lower extremity injury received ART on the origins and insertions of the hamstrings and dorsal sacral ligament. The sit-and-reach test was used before and after treatment to determine hamstring flexibility. Summary statistics were calculated, and pre and post hamstring flexibility scores were compared using a related samples t test. RESULTS: Ther

Efficacy of monitoring long-bone fracture healing by measurement of either bone stiffness or resonant frequency: numerical simulation.

Development of noninvasive mechanical tests to monitor fracture healing has been hindered because relationships between bone geometry, measurement conditions, and fracture callus strength are not well understood. Beam theory was used to analyze the effects of fracture length, fracture location, end conditions, and fracture callus stiffness on mechanical properties (resonant frequency, bending stiffness, and torsional stiffness) of healing bone. Actual bone mineral geometry from a human tibia, quantified every 1 mm, was used in the beam analysis. Geometry of the fracture callus segment was unif

[The "short Achilles tendon" syndrome: a new entity of the diabetic foot].

The "short Achilles tendon" syndrome, characterised by limited dorsiflexion of the tibiotarsal joint, is not well-known but could be a disposing factor for trophic disorders and plantar ulcer related to hypersupport in the diabetic subject. We report two cases of multicomplicated diabetic patients treated during several months for plantar ulcer superinfected with underlying osteoarthrits, whose course became rapidly favourable after tenotomy of the Achilles tendon. This simple surgical act can be performed in ambulatory conditions under local anaesthesia. Functional disability is negligible. T

Glottal area waveform analysis of benign vocal fold lesions before and after surgery.

Glottal area waveform (GAW) is the plot of relative glottal area versus time through 1 representative glottal cycle. It is derived from the quantitative analysis of the videostroboscopic image. A GAW analysis was performed on 24 patients before and after microlaryngeal phonosurgery. Patients with vocal fold polyps, polypoid degeneration, cysts, sulcus vocalis, and Reinke's edema were included. From each GAW, 5 parameters were determined and compared: maximum normalized glottal area, maximum opening rate, maximum closing rate, percent open time at 50% glottal opening, and glottal gap size. Flexibility enhancement with vibration: Acute and long-term.

INTRODUCTION: The most popular method of stretching is static stretching. Vibration may provide a means of enhancing range of motion beyond that of static stretching alone. PURPOSE: This study sought to observe the effects of vibration on static stretching to determine whether vibration-aided static stretching could enhance range of motion acquisition more than static stretching alone in the forward split position. METHODS: Ten highly trained male volunteer gymnasts were randomly assigned to experimental (N = 5) and control (N = 5) groups. The test was a forward split with the rear knee flex

The effects of cycling shoe stiffness on forefoot pressure.

Plantar pressure data were recorded in two different shoe types to determine the effect of cycling shoe stiffness on peak plantar forefoot pressure in cyclists. Two pairs of shoes of the same size and manufacturer, identical except for outsole material and stiffness, were tested. Shoe stiffness measurements were collected under controlled conditions and in two different configurations using a dynamic hydraulic tensile testing machine. Measurements of plantar pressure were done using Pedar capacitive-based sensor insoles while subjects pedaled in a seated position at a controlled power output.
