Pycnodysostosis- a rare disorder with distinctive craniofacial dysmorphia. A case report.

Pycnodysostosis is a rare autosomal recessive condition caused by the mutation of CTSK gene. CTSK regulates the activity of Cathepsin K which is responsible for osteoclast-mediated bone resorption. This mutation causes the bones to become dense, sclerotic, brittle, and thus, prone to fracture. Affected individuals have normal cognitive development and life expectancy, however, the quality of life depends on the early diagnosis of the condition. The patient presents with striking clinical (short stature, brachydactyly) and radiological (frontal and parieto-occipital bossing, open sutures, and f

Novel mutation and white matter involvement in an Indian child with pycnodysostosis.

Pycnodysostosis (OMIM # 265800) is an inherited lysosomal disorder due to affection of cathepsin K gene, localised to 1q21. Pycnodysostosis can present with both skeletal and extraskeletal features. The index patient presented with cardinal features of short stature, dental and digital anomalies with history of multiple fractures. He, in addition had an unreported finding of white matter hyperintensity suggesting dysmyelination on neuroimaging. Molecular analysis revealed a homozygous insertion of single nucleotide in exon 5 of the CTSK gene that produces the substitution of phenylalanine inst

A child with bone fractures and dysmorphic features: remember of pycnodysostosis and craniosynostosis.

Accidental bony injuries are common in children. Children may also present with bony injuries following non-accidental injuries. Pathological fractures, though extremely rare, are an important entity and constitute fractures that occur in abnormal bones, usually after minor trauma. Pycnodysostosis is a rare skeletal dysplasia characterised by a clinical phenotype that includes short stature, skull deformities, osteosclerosis, acroosteolysis and bone fragility. Often the disease is diagnosed at an early age as a result of the investigation of short stature. However, the diagnosis is sometimes d

[Ultrastructural abnormalities of the chondrocytes in pycnodysostosis. Their relation to a disorder of lipid metabolism].

The ultrastructural study of the growth cartilage of pycnodysostosis reveals the presence of abnormal inclusions in the majority of the chondrocytes. The inclusions are single membrane bound and contain granular material and lamellar irregularly interwoven structures which at very high magnification appear to be made up of dense parallel bands. These vacuoles displace adjacent structures and some of them appear to be closely related to the Golgi apparatus. In addition, appearances are sometimes seen which suggest the expulsion of the vacuoles into the cell capsules. The abnormal chondrocyte in

Healing fragile bones: a case report on hyperbaric oxygen therapy in pycnodysostosis.

Pycnodysostosis is a rare lysosomal storage disorder characterised by short stature, craniofacial dysmorphisms, dental anomalies, and increased bone fragility due to osteoclast dysfunction caused by cathepsin K gene mutations. This case report describes a 43-year-old female pycnodysostosis patient with recurrent subtrochanteric fractures and delayed bone healing following multiple surgical interventions, including femoral osteotomy and bone grafting. Despite these efforts, bony union was not achieved. The patient underwent 39 sessions of hyperbaric oxygen therapy (HBOT), administered at 243.2

When Bones Tell a Story: Diaphyseal Femoral Fracture in a Child With Pycnodysostosis.

Pycnodysostosis (PYCD) is a rare genetic disorder marked by generalized bone sclerosis and a high risk of fractures. On average, children with PYCD experience approximately 0.2 fractures per year, often complicated by delayed healing and poor bone remodeling. Surgical treatment of long bone fractures in patients with PYCD is scarcely documented in the literature, and orthopedic surgeons play a crucial role in managing the affected children. In this article, we present the case of an 11-year-old boy diagnosed with PYCD who presented to the pediatric surgery department for the management of a fe

A Mutation in CTSK Gene in an Autosomal Recessive Pycnodysostosis Family of Chinese Origin.

Pycnodysostosis is a rare autosomal recessive skeletal dysplasia characterized by short stature, osteosclerosis, acro-osteolysis, frequent fractures, and skull deformities. Mutation in the gene encoding cathepsin K (CTSK), which is a lysosomal cysteine protease, has been found to be responsible for this disease. Here we reported a consanguineous Chinese family with 1 affected individual demonstrating autosomal recessive pycnodysostosis with recurrent kidney stone, a new clinical manifestation which has not been reported in patients of pycnodysostosis before. To identify the pathogenic mutation

Radiographic studies of upper airway obstruction with cor pulmonale in a patient with pycnodysostosis.

The purpose of this article is to present radiographic aspects of obstructive anatomical alterations of the upper airway before and during sleep in a 4 1/2-year-old boy affected with pycnodysostosis. The striking length of the uvula and the inspiration-exspiration changes in its position was an important factor in the obstruction of the upper airway. For better and earlier understanding of the actual mechanism of upper airway obstruction, the radiological data should be obtained during the same circumstances that the patient is experiencing his maximum discomfort.

Case Report: A Novel Homozygous Variant of the CTSK Gene in Rare Pycnodysostosis.

Pycnodysostosis (PD) is a rare autosomal recessive skeletal dysplasia from impaired bone resorption due to osteoclastic dysfunction. The features of PD are deformity of the skull, maxilla, and phalanges; osteosclerosis; and bone fragility. We describe the case of a patient with complaints of multiple fractures of the lower extremities in the anamnesis and pain in the lower extremities, cervical spine, and shoulder girdle during physical exertion. Genetic testing revealed a novel homozygous variant c.704T>C (p.Leu235Pro) in the CTSK gene. Biallelic pathogenic variants in this gene lead to PD. T

Multiple Fractures and Impaired Bone Fracture Healing in a Patient with Pycnodysostosis and Hypophosphatasia.

Pycnodysostosis (PYCD) is a rare recessive inherited skeletal disease, characterized by short stature, brittle bones, and recurrent fractures, caused by variants in the Cathepsin K encoding gene that leads to impaired osteoclast-mediated bone resorption. Hypophosphatasia (HPP) is a dominant or recessive inherited condition representing a heterogeneous phenotype with dental symptoms, recurrent fractures, and musculoskeletal problems. The disease results from mutation(s) in the tissue non-specific alkaline phosphate encoding gene with reduced activity of alkaline phosphatase and secondarily defe

A rare case of pycnodysostosis: Technical difficulties in managing long bone fractures.

Pycnodysostosis is a rare inherited disorder of autosomal recessive trait causing cathepsin K deficiency, leading to failure of osteoclastic activity. Brittle and sclerotic bones which are prone for frequent fractures is the characteristic feature of this congenital disorder. Despite good healing potential there are few issues in the management of fractures in pycnodysostosis patients. In this article we report the challenges faced in managing a fracture of the femoral shaft in a 12 year old girl with pycnodysostosis. For early rehabilitation and to avoid deformity and shortening, we opted for
