Patient values and preferences regarding prognostic counseling in isolated REM sleep behavior disorder.

STUDY OBJECTIVES: Isolated REM sleep behavior disorder (iRBD) carries a high lifetime risk for phenoconversion to a defined neurodegenerative disease (NDD) including Parkinson disease, dementia with Lewy bodies, and multiple system atrophy. We aimed to examine iRBD patient values and preferences regarding prognostic counseling. METHODS: One hundred thirteen iRBD patient participants enrolled in the Mayo Clinic iRBD Patient Registry were sent an email survey concerning their values and preferences concerning NDD prognostic counseling and their experiences following diagnosis with iRBD. Motor Dysfunction in REM Sleep Behavior Disorder: A Rehabilitation Framework for Prodromal Synucleinopathy.

Parkinson disease (PD) and other related diseases with α-synuclein pathology are associated with a long prodromal or preclinical stage of disease. Predictive models based on diagnosis of idiopathic rapid eye movement (REM) sleep behavior disorder (iRBD) make it possible to identify people in the prodromal stage of synucleinopathy who have a high probability of future disease and provide an opportunity to implement neuroprotective therapies. However, rehabilitation providers may be unaware of iRBD and the motor abnormalities that indicate early motor system dysfunction related to α-synuclein pa

Physician and patient determinants of prognostic counseling in idiopathic REM sleep-behavior disorder.

OBJECTIVES/BACKGROUND: Prognostic counseling about the risk for developing overt neurodegenerative disorders for patients with idiopathic REM sleep-behavior disorder (iRBD) and isolated REM sleep without atonia (iRSWA) is difficult, given lack of disease-modifying interventions and uncertainty in accurate prognostication for individuals. We aimed to analyze patient and physician characteristics associated with documented prognostic discussions for patients with iRBD and iRSWA. PATIENTS/METHODS: We retrospectively reviewed the medical records for 138 (112 iRBD and 26 iRSWA) patients seen at th

Spinocerebellar ataxia type 31 associated with REM sleep behavior disorder: a case report.

BACKGROUND: Spinocerebellar ataxia type 31 (SCA 31) is a slowly progressive neurodegenerative disorder characterized by pure cerebellar ataxia. Unlike other CAG repeat diseases, sleep-related problems have not been reported in patients with SCA 31 so far. CASE PRESENTATION: A 67-year-old woman was admitted to our hospital with dysarthria and gait disturbance after onset age of 62 years. Neurological examination revealed pure cerebellar ataxia. Genetic analysis detected expansion of a TGGAA repeat in the coding region of the BEAN/TK2 gene on chromosome 16p22.1, confirming the diagnosis of SCA

Alpha-synuclein aggregates in the parotid gland of idiopathic REM sleep behavior disorder.

BACKGROUND: The neuropathological hallmark of Parkinson's disease (PD) is the presence of aggregates of phosphorylated alpha-synuclein (pAS) in the nervous system. METHOD: We report a patient with video-polysomnography-confirmed idiopathic REM sleep behavior disorder that underwent parotidectomy because of parotid gland cancer. Immunohistochemistry of the gland tissue revealed abundant pAS deposits. One year after surgery the patient was diagnosed with PD. Prompted by this observation we examined the parotid gland in 10 consecutive individuals that underwent elective parotidectomy irrespectiv

Smartphone motor testing to distinguish idiopathic REM sleep behavior disorder, controls, and PD.

OBJECTIVE: We sought to identify motor features that would allow the delineation of individuals with sleep study-confirmed idiopathic REM sleep behavior disorder (iRBD) from controls and Parkinson disease (PD) using a customized smartphone application. METHODS: A total of 334 PD, 104 iRBD, and 84 control participants performed 7 tasks to evaluate voice, balance, gait, finger tapping, reaction time, rest tremor, and postural tremor. Smartphone recordings were collected both in clinic and at home under noncontrolled conditions over several days. All participants underwent detailed parallel in-c

National validation and proposed revision of REM sleep behavior disorder screening questionnaire (RBDSQ).

We validated the Italian version of the rapid eye movement sleep behavior disorder (RBD) screening questionnaire (RBDSQ) and calculated its cut-off value for discriminating RBD group from other sleep disorders and healthy controls (HC). 380 patients with sleep disorders and 101 HC were enrolled. RBDSQ achieved an acceptable Cronbach's α value of 0.787 and item 10 was the only one with a very low item-total biserial correlation (0.141). At ROC analysis, we obtained an AUC of 0.888, denoting a good performance of the RBDSQ total score for predicting the RBD status. The optimal cut-off value was

A case of REM sleep behavior disorder, narcolepsy-cataplexy, parkinsonism, and rheumatoid arthritis.

A patient is reported in whom signs and symptoms of REM sleep behavior disorder (RBD) and narcolepsy have been associated for almost two decades with a late development of parkinsonism and rheumatoid arthritis. A 78-year-old male patient in whom RBD was first diagnosed was followed-up by clinical examination, video-polysomnography, multiple sleep latency test, cerebral magnetic resonance imaging, and dopamine transporter imaging by single-photon emission computerized tomography. The patient was found to present for almost two decades, in addition to RBD, also narcolepsy. Moreover, a late devel

Changing demographics in REM sleep behavior disorder: possible effect of autoimmunity and antidepressants.

BACKGROUND: Rapid eye movement (REM) sleep behavior disorder (RBD) has been described predominantly in elderly men and in association with neurodegenerative disease. But an increasing proportion of cases in recent reports and in clinical practice do not fit this description; thus we sought to describe a current RBD population and possibly identify new subgroups with RBD. METHODS: Records of 115 consecutive patients with polysomnogram-confirmed RBD at an academic sleep center were retrospectively reviewed. RESULTS: Male to female ratio was 2:1, and 1.25:1 for early-onset (age <50) cases. Mean

Cardiac autonomic denervation in Parkinson's disease is linked to REM sleep behavior disorder.

BACKGROUND: Recent studies have suggested a close connection between autonomic dysfunction and rapid eye movement sleep behavior disorder, which differs in nature from other early-stage markers of Parkinson's disease. In this study we examined the relationship between rapid eye movement sleep behavior disorder and autonomic dysfunction in Parkinson's disease as measured by cardiac beat-to-beat variability. METHODS: In 53 patients with Parkinson's disease and 36 controls, electrocardiographic trace from a polysomnogram was assessed for measures of beat-to-beat RR variability including RR-stand

The clinical and pathophysiological relevance of REM sleep behavior disorder in neurodegenerative diseases.

REM sleep behavior disorder (RBD) is characterized by vigorous movements associated with unpleasant dreams and increased electromyographic activity during REM sleep. Polysomnography with audiovisual recording is needed to confirm the diagnosis of RBD and to exclude other sleep disorders that can mimic its symptoms including obstructive sleep apnea, nocturnal hallucinations and confusional awakenings. RBD may be idiopathic or related to neurodegenerative diseases, particularly multiple system atrophy, Parkinson's disease and dementia with Lewy bodies. RBD may be the first manifestation of these

Physiological and anatomical link between Parkinson-like disease and REM sleep behavior disorder.

Parkinson's disease (PD) is a progressive neurodegenerative disease that is caused by a loss of neurons in the ventral midbrain. Parkinsonian patients often experience insomnia, parasomnias, and daytime somnolence. REM sleep behavior disorder (RBD) is characterized by vigorous movements during REM sleep, and may also be caused by neuronal degeneration in the central nervous system (CNS); however, the site of degeneration remains unclear. Both Parkinsonism and RBD become more prevalent with aging, with onset usually occurring in the sixties. Recent findings show that many individuals with RBD e

Gait dysfunction as an early marker of phenoconversion in REM sleep behavior disorder.

Clinical isolated REM Sleep Behavior Disorder (iRBD) is a prodromal stage of α-synucleinopathies such as Parkinson's disease (PD). Longitudinal changes of gait as a marker of phenoconversion in iRBD have not been systematically evaluated yet. Therefore, twenty-one iRBD patients, nineteen matched healthy controls and fourteen PD patients (H&Y stage I - III) were assessed at baseline using a pressure-sensor walkway (GAITRite). While normalized gait velocity and walking endurance were significantly decreased in iRBD patients compared to controls, gait rhythm parameters such as single support furt

REM sleep behavior disorder as a shared motor phenotype: A multidimensional clinical study.

BACKGROUND: REM sleep behavior disorder (RBD) is increasingly recognized as a heterogeneous condition that may arise in different etiological contexts, including an isolated form that is often a prodromal synucleinopathy, antidepressant exposure, and RBD occurring in the context of clinically diagnosed tau-spectrum neurodegenerative syndromes. The aim of this study was to compare clinical, cognitive, neuroimaging, and polysomnographic features of isolated RBD (iRBD), antidepressant-associated RBD (iatroRBD), and tauopathy-associated RBD (tauRBD) associated with Alzheimer's disease, progressive

Serum Ferritin and Inflammation in Isolated REM Sleep Behavior Disorder: A Longitudinal Study.

BACKGROUND AND OBJECTIVES: Isolated REM sleep behavior disorder (iRBD) is recognized as a prodromal stage of α-synucleinopathies. Iron metabolism and inflammation have been implicated in the pathogenesis of neurodegeneration. This study investigated the peripheral iron metabolism and inflammation in relation to prodromal clinical features and disease progression in iRBD. METHODS: This prospective observational cohort study recruited patients with iRBD at Sleep Medicine Center of West China Hospital. We measured peripheral iron metabolism markers and analyzed their associations with clinical c

Discordance Between Striatal Dopaminergic Imaging and Motor Performances in REM Sleep Behavior Disorder.

BACKGROUND AND OBJECTIVES: Idiopathic REM sleep behavior disorder (iRBD) is a marker of early neurodegenerative synucleinopathy. Nigrostriatal dopaminergic dysfunction is often considered the primary pathological mechanism behind motor symptoms; however, other mechanisms have been proposed. The study aim was to identify whether there were iRBD patients with a discordance between motor testing and abnormal nigrostriatal uptake, and to characterise those patients. METHODS: This multicenter study included 108 subjects with polysomnography-confirmed iRBD who underwent [123I]-Ioflupane SPECT (DaT-

Differences in intestinal microbiota in Parkinson's disease and isolated REM sleep behavior disorder.

Previous studies have shown differences in the microbiota of patients with Parkinson's disease (PD) compared to healthy controls (HC). To deduce a possible causality, it is highly relevant to examine changes in the prodromal phase. This study investigated the microbiome in stool samples of individuals with isolated REM sleep behavior disorder (iRBD, n = 32) compared to clinical PD (n = 23) and HC (n = 34) and showed significant changes of beta-diversity in PD and iRBD patients compared to HC (p = 0.025; p = 0.003), with an increase in proinflammatory species in iRBD and PD and decrease in SCFA

REM sleep behavior disorder and Prodromal Parkinson's Disease in patients with Essential Tremor.

Several studies suggested the presence of non-motor symptoms in Essential Tremor (ET), including REM sleep behavioral disorder (RBD). RBD is an essential criterion for Prodromal Parkinson's Disease (PPD), suggesting a link between ET and PD. Our objective was to assess the prevalence and features of ET patients with RBD and PDD. RBD was diagnosed by questionnaire screening, followed by polysomnography. PPD risk factors and prodromic markers were assessed with a structured protocol. Patients were characterized regarding tremor features. ET patients with RBD (ET-RBD) and PPD (ET-PPD) were compar
