Radiation recall pneumonitis triggered by an immune checkpoint inhibitor following re-irradiation in a lung cancer patient: a case report.

BACKGROUND: Radiation recall pneumonitis (RRP) is unpredictable but associated with severe radiation damage in previously irradiated fields. Chemotherapy and targeted drugs have been reported to contribute to RRP. Here we report a case of a patient with non-small cell lung cancer (NSCLC) who developed RRP following administration of immune checkpoint inhibitor (ICI) 18 months after the end of re-irradiation. CASE PRESENTATION: A 69-year-old man received adjuvant chemoradiotherapy post-operatively. He underwent thoracic re-irradiation for oligometastatic NSCLC. On second recurrence, pembrolizu

Carotid blowout in a patient with nasopharyngeal carcinoma treated with SBRT re-irradiation for local recurrence using twice weekly treatment.

We present the case of a patient undergoing reirradiation for a T4 nasopharyngeal tumor 1 year after his initial radiation, treated with SBRT to a moderate dose with twice weekly fractionation. Despite the measures of caution employed, the patient had a fatal carotid blowout at 7 months following SBRT. This suggests that spacing apart fractions of SBRT alone may not be sufficient to obviate the risk of carotid blowout syndrome and other risk factors and interventions should be considered.

Cumulative dose analysis following elective pelvis re-irradiation at junction with primary prostate radiotherapy from the OLIGOPELVIS GETUG P07 trial.

OBJECTIVE: Re-irradiation for pelvic lymph node recurrences at junction with primary prostate radiotherapy remains challenging due to the risk of adverse events from excessive cumulative doses. Clear dosimetric constraints for pelvic OAR are currently lacking in this situation. METHODS: The OLIGOPELVIS GETUG P07 prospective Phase 2 trial evaluated elective nodal radiotherapy combined with androgen deprivation therapy in patients with oligorecurrent pelvic lymph nodes. We focused on patients with prior prostate radiotherapy. Biologically equivalent (EQD2) dose accumulation was performed using

Conventionally fully fractionated Gamma Knife Icon re-irradiation of primary recurrent intracranial tumors: the first report indicating feasibility and safety.

OBJECTIVE: With the incorporation of real-time image guidance on the Gamma Knife system allowing for mask-based immobilization (Gamma Knife Icon [GKI]), conventionally fully fractionated (1.8-3.0 Gy/day) GKI radiation can now be delivered to take advantage of an inherently minimal margin for delivery uncertainty, sharp dose falloff, and inhomogeneous dose distribution. This case series details the authors' preliminary experience in re-irradiating 7 complex primary intracranial tumors, which were considered to have been previously maximally radiated and situated adjacent to critical organs at r

Feasibility and Safety of High-Dose Proton Re-Irradiation in Recurrent Pediatric Central Nervous System Tumors: A Single-Institution Retrospective Study.

PURPOSE: Pediatric central nervous system (CNS) tumors often recur despite multimodality therapy. Although re-irradiation (re-RT) has historically been limited by concerns for severe late toxicities, modern techniques have renewed interest in this approach. Proton therapy provides dosimetric advantages that may enable curative re-treatment with reduced normal tissue exposure. METHODS: We retrospectively reviewed 54 pediatric patients who underwent proton re-RT for recurrent CNS tumors at our institution (2010-2024). Eligible patients had received prior CNS-directed RT at ≤20 years of age. Tox

A treatment planning comparison of photon stereotactic ablative radiotherapy and proton beam therapy for the re-irradiation of pelvic cancer recurrence.

BACKGROUND: Patients who experience a pelvic cancer recurrence in or near a region that received initial radiotherapy, typically have few options for treatment. Organs at risk (OAR) have often reached their dose constraint limits leaving minimal dose remaining for standard re-irradiation (reRT). However, photon based stereotactic ablative radiotherapy (SABR) has been utilised for reRT with promising initial results although meeting OAR constraints can be challenging. Proton beam therapy (PBT) could offer an advantage. MATERIALS AND METHODS: SABR plans used for treatment for ten pelvic reRT pa

A model to predict the risk of lethal nasopharyngeal necrosis after re-irradiation with intensity-modulated radiotherapy in nasopharyngeal carcinoma patients.

BACKGROUND: For patients with nasopharyngeal carcinoma (NPC) who undergo re-irradiation with intensity-modulated radiotherapy (IMRT), lethal nasopharyngeal necrosis (LNN) is a severe late adverse event. The purpose of this study was to identify risk factors for LNN and develop a model to predict LNN after radical re-irradiation with IMRT in patients with recurrent NPC. METHODS: Patients who underwent radical re-irradiation with IMRT for locally recurrent NPC between March 2001 and December 2011 and who had no evidence of distant metastasis were included in this study. Prognostic factors, patterns of failure and re-irradiation in hypofractionated stereotactic radiotherapy-treated brain metastases from non-small cell lung cancer.

BACKGROUND: Hypofractionated stereotactic radiotherapy (fSRT) is increasingly used for brain metastases (BMs) from non-small cell lung cancer (NSCLC). However, relevant data concerning treatment outcomes of fSRT and clinical utility of re-irradiation using fSRT (re-fSRT) remain scarce. METHODS: Consecutive NSCLC patients with fSRT-treated BMs from May 2018 to May 2022 were included. The cumulative incidence of intracranial local recurrence (iLR), intracranial progressive disease (iPD) and symptomatic radiation necrosis (sRN) were calculated from the initiation of fSRT, choosing death as the c

Carotid blowout syndrome in pharyngeal cancer patients treated by hypofractionated stereotactic re-irradiation using CyberKnife: A multi-institutional matched-cohort analysis.

BACKGROUND AND PURPOSE: Although reirradiation has attracted attention as a potential therapy for recurrent head and neck tumors with the advent of modern radiotherapy, severe rate toxicity such as carotid blowout syndrome (CBOS) limits its potential. The aim of this study was to identify the risk factors of CBOS after hypofractionated stereotactic radiotherapy (SBRT). METHODS AND PATIENTS: We conducted a matched-pair design examination of pharyngeal cancer patients treated by CyberKnife reirradiation in four institutes. Twelve cases with CBOS were observed per 60 cases without CBOS cases. Pr
