Sex differences in electrolyte abnormalities indicating refeeding syndrome risk among hospitalized adolescents and young adults with eating disorders.

BACKGROUND: Refeeding syndrome is the gravest possible medical complication in malnourished patients undergoing refeeding in the hospital. We previously reported that males with malnutrition secondary to eating disorders required more calories and had longer hospital stays than females; however, sex differences in electrolyte abnormalities indicating refeeding syndrome risk remain unknown. The objective of this study was to assess differences in electrolyte abnormalities indicating refeeding syndrome risk among male and female adolescents and young adults with eating disorders hospitalized for

Hepatic steatosis due to total parenteral nutrition: the influence of short-gut syndrome, refeeding, and small bowel transplantation.

This study was undertaken to determine whether refeeding through the native small intestine or through a small bowel transplant would reverse hepatic steatosis induced by total parenteral nutrition (TPN), and of what influence a coexisting short-gut syndrome is. Three short-gut syndromes of different severity were established in Lewis rats (short-gut I, mild; short-gut II, moderate; short-gut III, severe). TPN was administered for 10 days and the animals were refed for 20 days. A liver biopsy after the TPN period confirmed a mild to moderate fatty infiltration of the liver in all groups. After

Refeeding syndrome and psychopharmacological interventions in children and adolescents with Anorexia Nervosa: a focus on olanzapine-related modifications of electrolyte balance.

This study aims to investigate the potential correlation between the use of olanzapine, a psychopharmacological intervention commonly prescribed in Anorexia Nervosa treatment, and the occurrence of Refeeding Syndrome. Despite the acknowledged nutritional and biochemical impacts of olanzapine, the literature lacks information regarding its specific association with Refeeding Syndrome onset in individuals with Anorexia Nervosa. This is a naturalistic, retrospective, observational study, reporting the occurrence of Refeeding Syndrome in children and adolescents with Anorexia Nervosa, treated or u

Incidence and risk factors of refeeding syndrome in inpatients undergoing enteral and parenteral nutrition hospitalized in a university medical hospital.

Refeeding syndrome (RS) encompasses electrolyte and metabolic disturbances, and its incidence depends on diagnostic criteria. We aimed to evaluate the incidence and risk factors for RS in inpatients at a university hospital in Brazil, who received enteral or parenteral nutrition. This retrospective study included all patients admitted to the hospital who received enteral or parenteral feeding after at least a 3-day fasting period. RS diagnosis was based on phosphorus, potassium, and magnesium changes in the first 3 days after diet reintroduction. Clinical, anthropometric, and other laboratory

A Case of Successful Neurological Critical Care for the Very-High-Risk Refeeding Syndrome With Central Nervous System Disorders.

Refeeding syndrome potentially causes death from refeeding-induced central nervous system disorders, such as hypoglycemic encephalopathy, Wernicke encephalopathy, and hypophosphatemic encephalopathy. However, there is no established management for malnourished patients with this refeeding-induced central nervous system syndrome. We report a case of successful neurological critical care for refeeding-induced central nervous system syndrome. A 15-year-old female with anorexia nervosa presented to our hospital after fainting. Her body mass index (BMI) was 8.8 kg/m². Upon arrival, her Glasgow Coma

Management of Severe Hypomagnesemia as the Primary Electrolyte Abnormality with a Delayed Onset of Clinical Signs as a Result of Refeeding Syndrome in a Cat.

This case report describes severe hypomagnesemia in a cat attributed to refeeding syndrome with an onset of clinical signs from the magnesium deficiency apparent on the twelfth day following initiation of feeding. The patient initially presented in a state of cachexia from apparent malnutrition after missing from the owners care for five months. The patient was initially discharged five days after the initiation of feeding with only a mild hypokalemia apparent and requiring supplementation and returned for outpatient management. The patient presented through the emergency department on the twe

Refeeding syndrome in surgical patients post initiation of artificial feeding, a prospective cohort study in a low-income country.

BACKGROUND & AIMS: South Africa's inequitable history has contributed to a malnourished population, further aggravated by high levels of violence and economic uncertainty culminating in a population suffering the sequelae of poverty. The perceived notion is that malnutrition places the South African population at greater risk for development of refeeding syndrome. This study aimed to identify the incidence of refeeding syndrome in the South African population. METHODS: All patients admitted into the surgical intensive care unit from 1 November 2019 to 30 September 2020, were screened using th

Neonatal Refeeding Syndrome and Clinical Outcome in Extremely Low-Birth-Weight Babies: Secondary Cohort Analysis From the ProVIDe Trial.

BACKGROUND: Refeeding syndrome (RS) following preterm birth has been linked to high intravenous (IV) protein intake in the presence of low electrolyte supply. In extremely low-birth-weight (ELBW) babies, we aimed to determine the incidence of RS and associations with birth characteristics and clinical outcomes. METHOD: Prospective cohort study of ELBW ProVIDe Trial participants in 6 New Zealand neonatal intensive care units. RS was defined as serum phosphate < 1.4 mmol.L-1 and total calcium > 2.8 mmol.L-1 . Relationships between RS and other factors were explored using 2-sample tests and logi

Medical nutrition therapy for ALS: Dietitians' approaches to diagnosing malnutrition, facilitating feeding tube discussions, and mitigating refeeding syndrome risk.

BACKGROUND: Persons living with amyotrophic lateral sclerosis (PALS) are at high risk for malnutrition because of disease-related factors such as dysphagia, self-feeding difficulty, and hypermetabolism. Nutrition interventions, including enteral nutrition (EN) initiation after gastrostomy tube (G-tube) placement, are integral to care but can introduce challenges, such as refeeding syndrome. This qualitative analysis explores how registered dietitians (RDs) at US-based outpatient amyotrophic lateral sclerosis (ALS) clinics assess malnutrition, facilitate discussions around G-tube placement, and

Case Report: A complicated case of neonatal refeeding syndrome following intrauterine growth restriction corrected by high dose thiamine supplementation.

Refeeding syndrome (RS), marked by severe electrolyte imbalances (e.g., hypophosphatemia, hypokalemia) and thiamine deficiency, poses significant risks during nutritional rehabilitation in intrauterine growth restriction (IUGR) neonates. This case report highlights the efficacy of high-dose thiamine (2 mg/kg IV) in resolving refractory RS in a preterm IUGR infant (34 weeks, 1,415 g) unresponsive to standard electrolyte correction. Despite gradual caloric reintroduction and parenteral supplementation, the infant exhibited persistent hypophosphatemia (3.0 mmol/kg/day IV requirement), hypokalemia

Refeeding Syndrome and Non-Alcoholic Wernicke's Encephalopathy in a Middle-aged Male Initially Presenting with Gallstone Pancreatitis: A Clinical Challenge.

Wernicke's Encephalopathy (WE) is a neurological condition characterized by ophthalmoplegia, ataxic gait, and altered mental status. It is an underdiagnosed yet severely limiting disease process precipitated by thiamine deficiency. Often times, it can occur in conjunction with other disease states like refeeding syndrome in which the underlying etiology is prolonged periods of malnutrition. We present a unique case of non-alcoholic WE in a middle-aged male who initially presented with gallstone pancreatitis complicated with severe metabolic derangements. This ultimately resulted in the develop

Refeeding syndrome and vitamin B1 deficiency in a young man with normal body mass index following starvation in the COVID-19 era.

COVID-19 has spread worldwide and significantly influenced economies. Refeeding syndrome (RFS) is a potentially fatal abnormalities of electrolytes and fluid that can occur in malnourished patients undergoing mechanical refeeding. Herein, we report the case of a man in his 20s with a normal body mass index who presented with RFS and vitamin B1 deficiency. Although it was uncommon under normal circumstances, it occurred because of the severe social situations that were prevalent in the COVID-19 era. In this era, physicians should carefully evaluate their patients' nutritional status to identify

Increased risk of refeeding syndrome-like hypophosphatemia with high initial amino acid intake in small-for-gestational-age, extremely-low-birthweight infants.

BACKGROUND: Recent nutrition guidelines for extremely-low-birth-weight infants (ELBWIs) recommend implementation of high initial amino acid (AA) supplementation in parenteral nutrition. OBJECTIVE: We sought to evaluate the influence of AA intake on refeeding syndrome-like electrolyte disturbances including hypophosphatemia in ELBWIs. STUDY DESIGN: Medical records of 142 ELBWIs were reviewed. Demographic, nutritional, outcome, and electrolyte data were compared between ELBWIs with initial low (1.5 g/kg/day) and high (3 g/kg/day) AA intake. Multivariate analysis was conducted to determine the
