Electrical Impedance Tomography-Informed Regional Functional Phenotypes in Sepsis-Associated Acute Respiratory Distress Syndrome.

Sepsis-associated acute respiratory distress syndrome (SA-ARDS) is a heterogeneous syndrome in which systemic inflammation, vascular dysfunction, altered fluid balance, chest-wall loading, and respiratory effort may produce regional abnormalities not captured by global respiratory variables. Electrical impedance tomography (EIT) provides continuous bedside assessment of regional ventilation and, increasingly, regional perfusion. This review summarizes EIT-derived measures relevant to SA-ARDS and examines how key sepsis-associated mechanisms may influence regional ventilation, perfusion, and me

[Spinal operations performed in prone position: pulmonary gas exchange, hemodynamics, and possible complications].

Invasive and non-invasive studies have been used to study the cardiac and respiratory systems in 93 patients during prolonged prone surgical interventions into the vertebral column. Pronounced hemodynamic changes have been found to be absent at the stage of anesthetic anesthesia and while turning the patient with to the prone position. Gas exchange is determined by the increase in alveolar dead space, PaCO2, and PaO2. The position complications during these interventions are caused by errors while placing the patient on an operating table.

The impact of instrumental dead-space in volume-targeted ventilation of the extremely low birth weight (ELBW) infant.

BACKGROUND: Volume-targeted ventilation is increasingly used in neonatal ventilation to reduce the risk of volutrauma and inadvertent hyperventilation. However, normative data for appropriate tidal volume (V(T)) settings are lacking, especially in extremely low birth weight (ELBW) infants in whom the added dead space (DS) of the flow sensor may be important. OBJECTIVE: To quantify the effect of instrumental dead-space (IDS) on ventilation and to obtain normative data for initial V(T) associated with normocapnia in ELBW infants ventilated with volume guarantee (VG) ventilation. DESIGN/METHODS

Physiological dead space during exercise in patients with heart failure with preserved ejection fraction.

Heart failure with preserved ejection fraction (HFpEF) is associated with cardiopulmonary abnormalities that may increase physiological dead space to tidal volume (VD/VT) during exercise. However, studies have not corrected VD/VT for apparatus mechanical dead space (VDM), which may confound the accurate calculation of VD/VT. We evaluated whether calculating physiological dead space with (VD/VTVDM) and without (VD/VT) correcting for VDM impacts the interpretation of gas exchange efficiency during exercise in HFpEF. Fifteen HFpEF (age: 69 ± 6 yr; V̇o2peak: 1.34 ± 0.45 L/min) and 12 controls (70

The reflex respiratory effects on cats of breathing through a tube.

1. In anaesthetized cats, breathing air through an external dead space (a tube, 20-40 ml.) stimulated ventilation in excess of that expected from the end-tidal gases. 2. The stimulatory effect remained after bilateral vagotomy. 3. The stimulatory effect was reduced or abolished after bilateral section of the carotid sinus nerves. 4. The stimulatory effect was abolished by bilateral section of both the vagi and the sinus nerves. 5. Recording from chemoreceptor afferents in the sinus nerve showed that, although the tube increased mean discharge frequency, this increase was insufficient to accoun

Effect of hemorrhagic shock on intrapulmonary right-to-left shunt (QS/QT) and dead space (VD/VT).

The effect of hemorrhagic shock on intrapulmonary right-to-left shunt and dead space ratio was investigated in spontaneously breathing dogs under barbiturate anesthesia. Intrapulmonary right-to-left shunt was reduced sufficiently to offset the marked increase in arteriovenous oxygen difference and PaO2 did not fall. Dead space ratio was increased markedly; therefore, in shock, sufficient CO2-elimination depends on severe hyperventilation. According to this empirical findings, a decrease in PaO2 in shock must not be interpreted as a consequence of the shock itself, but it implies an additional

A mathematical model of the human respiratory system.

A model of the human respiratory system is proposed which has a satisfactory performance under different physiological conditions. The model comprises a continuous plant and a discrete controller which generates and updates the drive signal to the plant at the end of every breath to represent the Hering-Breuer reflex. Arterial and central medullary sensors are included. The lung volume, dead space volume, cardiac output and cerebral blood flow are time varying. The respiratory work is minimized. The model is examined and simulation results of its performance in hypercapnia, hypoxia, periodic b

[Analysis of pulmonary vasomotor responses to alveolar hypoxia and hypercapnia].

In acute experiments on cats responses of the lung vessels to hypoxic and hypercapnic stimuli and the latters effect on minor circle hemodynamics were studied as well as the role of changes of the blood acid-alkaline balance and volume of ventilation in potentiation and blocking of the above responses. The alveolar hypoxia and hypercapnia being induced a reversed dependence between the relative mass of the lung area and reduction of its blood flow vasdseined Hyperventilation with hypoxic gas mixture inhibited the hypoxic vasoconstriction in respective lung area which seems to suggest a shift t

Pulmonary blood flow and tissue volume. Assessment of a non-invasive technique of measurement.

Pulmonary blood flow and tissue have been measured by a rebreathing technique in which the disappearance of soluble and insoluble gases from an inhaled mixture is followed continuously using a respiratory mass spectrometer. Although measurements made with diethyl ether are more reproducible than those made with freon-22, they are inaccurate because ether (the more soluble gas) is taken up by the tissues surrounding the respiratory dead space. Experiments carried out on four normal subjects demonstrate that measurements of both pulmonary blood flow and tissue volume are influenced by the volume

Assessment of Bohr and Enghoff Dead Space Equations in Mechanically Ventilated Children.

BACKGROUND: Recent findings suggest that using alveolar PCO2 (PACO2 ) estimated by volumetric capnography in the Bohr equation instead of PaCO2 (Enghoff modification) could be appropriate for the calculation of physiological dead space to tidal volume ratio (VD/VT Bohr and VD/VT Enghoff, respectively). We aimed to describe the relationship between these 2 measurements in mechanically ventilated children and their significance in cases of ARDS. METHODS: From June 2013 to December 2013, mechanically ventilated children with various respiratory conditions were included in this study. Demographic

[An experimental study on the pathophysiology of septic ventilatory depression].

Ventilatory depression and apnea are well-known early pulmonary responses of sepsis in infants, yet their underlying mechanisms are not understood. To further elucidate the pathophysiology, we induced Escherichia coli septicemia in piglets and studied the sequential changes in intrapulmonary shunt (QS/QT), physiological dead space (VD/VT), minute ventilation (VE), and blood gases for up to 6 hours of lethal sepsis. Lung lymph was also collected and extravascular lung water (EVLW) was measured. Histology confirmed that interstitial edema developed 1 hour after E. coli infusion. These data sugge

Determinants of the effect of extracorporeal carbon dioxide removal in the SUPERNOVA trial: implications for trial design.

PURPOSE: To describe the variability and determinants of the effect of extracorporeal CO2 removal (ECCO2R) on tidal volume (Vt), driving pressure (ΔP), and mechanical power (PowerRS) and to determine whether highly responsive patients can be identified for the purpose of predictive enrichment in ECCO2R trial design. METHODS: Using data from the SUPERNOVA trial (95 patients with early moderate acute respiratory distress syndrome), the independent effects of alveolar dead space fraction (ADF), respiratory system compliance (Crs), hypoxemia (PaO2/FiO2), and device performance (higher vs lower CO

Expiratory high-frequency percussive ventilation: a novel concept for improving gas exchange.

BACKGROUND: Although high-frequency percussive ventilation (HFPV) improves gas exchange, concerns remain about tissue overdistension caused by the oscillations and consequent lung damage. We compared a modified percussive ventilation modality created by superimposing high-frequency oscillations to the conventional ventilation waveform during expiration only (eHFPV) with conventional mechanical ventilation (CMV) and standard HFPV. METHODS: Hypoxia and hypercapnia were induced by decreasing the frequency of CMV in New Zealand White rabbits (n = 10). Following steady-state CMV periods, percussiv

Dead space ventilation in critically ill children with lung injury.

STUDY OBJECTIVE: In children with acute lung injury, there is an increase in minute ventilation (E) and inefficient gas exchange due to a high level of physiologic dead space ventilation (VD/VT). Mechanical ventilation with positive end-expiratory pressure, when used in critically ill patients to correct hypoxemia, may contribute to increased VD/VT. The purpose of this study was to measure metabolic parameters and VD/VT in critically ill children. DESIGN: A cross-sectional study. SETTING: Pediatric ICU of a university hospital. PATIENTS: A total of 45 mechanically intubated children (mean a

Prevention by ulinastatin of decreased carbonic anhydrase activity induced by PMN elastase in vitro.

Disturbed carbon dioxide (CO2) elimination in adult respiratory distress syndrome (ARDS) has been considered to result from the dead space created by microthrombosis and vasoconstriction. However, another disturbance factor in CO2 elimination has been reported; the chemical dead space resulting from the inhibition of carbonic anhydrase (CA). This experiment was conducted to quantify the inhibition of CA activity by polymorphonucleocyte (PMN) elastase, which increases in ARDS. Different flasks containing solutions of CA, buffer, elastase, ulinastatin (an elastase antagonist), CA with PMN elasta

Chronic ventilatory failure caused by abnormal respiratory pattern generation during sleep.

A 67-yr-old man presented with a 4-yr history of chronic ventilatory failure in the absence of any restrictive or obstructive ventilatory defect. Detailed neurologic investigations were unremarkable, and the response of minute volume of ventilation to inhaled CO2 was normal. During sleep there was no evidence of upper airway obstruction, and minute volume of ventilation averaged 6.7 L/min, compared with 7.2 L/min during quiet wakefulness. However, sleep was associated with a rapid and shallow pattern of breathing, resulting in high dead space ventilation, inadequate alveolar ventilation, hypox

Effect of breathing pattern on nasal airflow, air conditioning, and gas clearance in a patient specific upper airway model.

BACKGROUND: Breathing pattern influences not only pulmonary ventilation but also upper airway transport processes such as nasal heat exchange, sinus ventilation, and carbon dioxide clearance. This study used transient computational fluid dynamics to investigate how respiratory rate and waveform shape affect these processes in a single patient specific upper airway model. METHODS: A computed tomography (CT) derived upper airway model from one healthy adult male, extending from the nasal vestibule to the nasopharynx and including the maxillary sinuses, was used as a fixed anatomical model.
