Respiratory and circulatory insufficiency during emergent long-distance critical care interhospital transports to tertiary care in a sparsely populated region: a retrospective analysis of late mortality risk.

OBJECTIVES: To test if impaired oxygenation or major haemodynamic instability at the time of emergency intensive care transport, from a smaller admitting hospital to a tertiary care centre, are predictors of long-term mortality. DESIGN: Retrospective observational study. Impaired oxygenation was defined as oxyhaemoglobin %-inspired oxygen fraction ratio (S/F ratio)<100. Major haemodynamic instability was defined as a need for treatment with norepinephrine infusion to sustain mean arterial pressure (MAP) at or above 60 mm Hg or having a mean MAP <60. Logistic regression was used to assess mort

Evaluating the effects of sodium glucose co-transporter -2 inhibitors from a renin-angiotensin-aldosterone system perspective in patients infected with COVID-19: contextualizing findings from the dapagliflozin in respiratory failure in patients with COVID-19 study.

Numerous studies demonstrate parallels between CVD, type 2 diabetes mellitus (T2DM) and COVID-19 pathology, which accentuate pre-existing complications in patients infected with COVID-19 and potentially exacerbate the infection course. Antidiabetic drugs such as sodium-glucose transporter-2 (SGLT-2) inhibitors have garnered substantial attention recently due to their efficacy in reducing the severity of cardiorenal disease. The effect of SGLT-2 inhibitors in patients with COVID-19 remains unclear particularly since SGLT-2 inhibitors contribute to altering the RAAS cascade activity, which inclu

Detection of SARS-CoV-2 by real-time PCR under challenging pre-analytical conditions reveals independence of swab media and cooling chain.

With global demand for SARS-CoV-2 testing ever rising, shortages in commercially available viral transport media pose a serious problem for laboratories and health care providers. For reliable diagnosis of SARS-CoV-2 and other respiratory viruses, executed by Real-time PCR, the quality of respiratory specimens, predominantly determined by transport and storage conditions, is crucial. Therefore, our aim was to explore the reliability of minimal transport media, comprising saline or the CDC recommended Viral Transport Media (HBSS VTM), for the diagnosis of SARS-CoV-2 and other respiratory viruse

Helicopter Air Transport of a Patient Using High Flow Nasal Cannula: A Case Report.

The classic out-of-hospital approach to a patient with severe acute respiratory failure involves both orotracheal intubation and invasive mechanical ventilation. The use of non-invasive methods for respiratory support has been shown to be beneficial in managing both acute and chronic respiratory failure. However, its use had not been previously considered for air medical transport due to concerns related to airway safety during flight, limited oxygen availability, and limited experience in this setting. We describe the successful inter-hospital helicopter transport of a patient with end-stage

Acute effects of parainfluenza virus on epithelial electrolyte transport.

Parainfluenza viruses are important causes of respiratory disease in both children and adults. In particular, they are the major cause of the serious childhood illness croup (laryngotracheobronchitis). The infections produced by parainfluenza viruses are associated with the accumulation of ions and fluid in the respiratory tract. It is not known, however, whether this accumulation is because of a direct effect of the viruses on ion and fluid transport by the respiratory epithelium. Here we show that a model parainfluenza virus (the Sendai virus), in concentrations observed during respiratory i

Evaluation of Nonintubated Analgesia Practices in Critical Care Transport.

OBJECTIVE: Current analgesia recommendations in the prehospital setting are not specific to critical care transport. Variation exists in the recommended agent and dosing strategies. Furthermore, there is a paucity of literature evaluating benzodiazepine and opiate coadministration, which may place patients at risk for respiratory decompensation. METHODS: This was a retrospective chart review of nonintubated adult critical care transport patients between July 1, 2020, and July 1, 2022, who received fentanyl or ketamine during transport. The primary outcome was the proportion of patients overse

Implementation of High-Flow Nasal Cannula and Noninvasive Ventilation During Pediatric Interfacility Transport: Effect on Intubation Rates.

OBJECTIVE: Previously, young children had limited respiratory support options during interfacility transport. Recently, high-flow nasal cannula (HFNC) and noninvasive ventilation (NIV) have become available for pediatric transport. We hypothesized that the implementation of HFNC and NIV on interfacility transport decreases the rate of intubation in infants and toddlers before and after transport to a tertiary-care pediatric intensive care unit (PICU). METHODS: We conducted a retrospective chart review of children aged 30 days to < 36 months transported to a tertiary-care PICU from a referring

[Effect of rhDNase on rheologic properties and transport capacity of mucus in cystic fibrosis].

Mucociliary transport is a major component of respiratory function and contributes greatly to the protection of the airways against infection. Cystic fibrosis is associated with severe abnormalities of the capacities of transport, due to the abnormal mucus and inflammatory changes of the epithelium. Recombinant human DNase I (rhDNase) has been demonstrated to reduce the viscosity of purulent cystic fibrosis respiratory mucus. To investigate the impact of rhDNase on transport capacities, we analysed ex vivo its effect on mucus transport by the ciliary activity and by the simulated cough. We mea

Calcium transport driven by a proton gradient and inverted membrane vesicles of Escherichia coli.

Calcium transport into inverted vesicles of Escherichia coli was observed to occur without an exogenous energy source when an artificial proton gradient was used. The orientation of the proton gradient was acid inside and alkaline outside. Either phosphate or oxalate was necessary for transport, as was found for respiratory-driven or ATP-driven uptake (Tsuchiya, T., and Rosen, B.P. (1975) J. Biol. Chem. 250, 7687-7692). Phosphate accumulation was found to occur in conjunction with calcium accumulation. Calcium transport driven by an artificial proton gradient was stimulated by dicyclohexylcarb

Oxygen transport and utilization in ARDS.

The process of tissue O2 transport and utilization is complex and incompletely understood. It depends on the successful interaction of the respiratory and cardiovascular system in order to deliver O2 in sufficient amounts and at a high enough capillary partial pressure to allow for adequate diffusion into the cell. Normally, adequate amounts of O2 are available and the system can easily augment transport to meet increased metabolic demands. Under these circumstances O2 utilization is the independent variable. In adult respiratory distress syndrome (ARDS) it appears that O2 transport is not suf

Malnutrition causes a reduction in alveolar epithelial sodium and chloride transport which predisposes to death from lung injury.

BACKGROUND: All forms of malnutrition have been associated with increased severity of pneumonia, an increased pneumonia associated mortality and an increased risk of pulmonary fluid overload. Malnutrition was found to be associated with increased sweat sodium and chloride concentrations. A reduction of systemic sodium and chloride transport reflected in sweat sodium and chloride levels has been linked to increased severity of pulmonary edema in children with septicemia. HYPOTHESIS: Malnutrition causes a reduction in alveolar epithelial sodium and chloride transport which predisposes to death
