Selective rhizotomies for spinal root pain and neuralgia of the inguinal region.

Our recent experience stimulated a review of selective rhizotomies for the alleviation of localized pain. Three patients with postoperative neuralgia in the inguinal region and two with neoplastic root compression were treated. Results were good and long-lasting in two cases, moderate in two and poor in one case. The relief appeared to better, the more clearly the pain was localized. In lesions distal to the spinal root it is necessary to undertake selective paravertebral root blocks with local anaesthetics. Experimental data are discussed that help in an understanding of pain recurrence after

Trigeminal neuralgia: a comparison of the results of percutaneous rhizotomy and microvascular decompression.

Seventy-five patients were treated between March 1976 and June 1980 for classical idiopathic tic douloureux. Fifty-five patients underwent percutaneous trigeminal rhizotomy (PTR) and twenty-four had posterior fossa microvascular decompression (MVD) of the trigeminal nerve. Four patients had both procedures. In the PTR group, 4% were immediate failures, 42% had a delayed recurrence of pain, while 54% remained totally pain free with an average follow-up of 30 months. In the MVD group, 12% were immediate failures, 17% had a delayed recurrence of pain, and 71% have remained free of pain with a ave

Sectorial posterior rhizotomy, a new technique of surgical treatment for spasticity.

After their experience of Foerster's operation and its technical modifications in 80 cases the authors report a new concept of analysis and treatment of spasticity in lower limbs. Spasticity of the different muscle groups is classified either as "useful spasticity" or "handicapping spasticity". The first has to be preserved, the second must be reduced. In order to achieve this purpose a new technique is presented, based on operative sectorial identification of the posterior rootlets subserving the "handicapping spasticity" by electrophysiological stimulation, muscle testing, and E.M.G. studies

Spinal cord tethering after selective dorsal rhizotomy below the conus medullaris.

BACKGROUND: Two techniques for selective dorsal rhizotomy (SDR) involve stimulating and sectioning nerve rootlets either below the conus medullaris or above the entrances to their respective dural root sleeves. In general, both techniques lead to sustained improvements in lower extremity spasticity with low complication rates. To our knowledge, spinal cord tethering has not been previously reported as a complication of SDR using either technique. METHODS: We review the presentation, treatment, and outcome of two patients who developed symptomatic spinal cord tethering after SDR below the conu

Biplane Fluoroscopic-Guided Balloon Rhizotomy for Trigeminal Neuralgia: A Technical Note.

BACKGROUND: The classic percutaneous technique used to cannulate the foramen ovale for the treatment of trigeminal neuralgia can place important anatomic structures, such as the distal cervical internal carotid artery, at risk. OBJECTIVE: To use fixed anatomic landmarks to safely and reliably locate the foramen ovale on anteroposterior (AP) fluoroscopy. METHODS: Locating the foramen ovale was initially tested using AP fluoroscopy on cadaveric skulls in the neurosurgical simulation lab. Fluoroscopic landmarks were identified and utilized to assist in successfully locating the foramen ovale du

MRI-based radiation-free method for navigated percutaneous radiofrequency trigeminal rhizotomy.

UNLABELLED: BACKGROUND/STUDY AIMS: Percutaneous radiofrequency trigeminal rhizotomy (RTR) is a standardized treatment for trigeminal neuralgia, yet it has been associated with serious complications related to the cannulation of the foramen ovale. Some of these complications, such as carotid injury, are potentially lethal. Neuronavigation was recently proposed as a method to increase the procedure's safety. All of the techniques described so far rely on pre- or intraoperative computed tomography scanning. Here we present a simple method based on magnetic resonance imaging (MRI) (radiation free)

Gamma Knife rhizotomy-induced histopathology in multiple sclerosis-related trigeminal neuralgia.

OBJECT: In this report, the authors describe the pathological changes in the human trigeminal nerve after Gamma Knife radiosurgery. METHODS: Three trigeminal nerves of patients with multiple sclerosis (MS)-related trigeminal neuralgia (MSTN) after Gamma Knife radiosurgery and other ablative procedures were examined by a neuropathologist. These cases were compared with 3 patients with typical TN who underwent partial surgical rhizotomy following recurrent symptoms after gasserian injury procedures, as well as with autopsy specimens from patients with and without MSTN. RESULTS: The three irrad

Long-term results of selective dorsal rhizotomy for hereditary spastic paraparesis.

Pure hereditary spastic paraparesis usually presents with progressive weakness and spasticity of the legs, which is similar to spastic cerebral palsy. In this study selective dorsal rhizotomy (SDR) was performed to improve the spasticity of pure hereditary spastic paraparesis and the long-term results were followed. A series of four patients with pure hereditary spastic paraparesis diagnosed by a multidisciplinary team received SDR. The dorsal rootlets from the L2 to S1 levels were selectively resected under electrophysiological monitoring. The patients were followed up for more than 2 years t

Corticospinal sprouting occurs selectively following dorsal rhizotomy in the macaque monkey.

The corticospinal tract in the macaque and human forms the major descending pathway involved in volitional hand movements. Following a unilateral cervical dorsal root lesion, by which sensory input to the first three digits (D1-D3) is removed, monkeys are initially unable to perform a grasp retrieval task requiring sensory feedback. Over several months, however, they recover much of this capability. Past studies in our laboratory have identified a number of changes in the afferent circuitry that occur as function returns, but do changes to the efferent pathways also contribute to compensatory

Spinal deformity after selective dorsal rhizotomy in patients with cerebral palsy.

Selective dorsal rhizotomy is used widely as a means of treating spasticity associated with cerebral palsy. Little is known regarding the effect of the procedure on the development or progression of spinal deformity. The authors reviewed six patients with progressive deformity after rhizotomy. Prerhizotomy and postrhizotomy records of physical examinations and radiographs were reviewed retrospectively in an attempt to identify risk factors for development of and/or rapid progression of, spinal deformity. Detailed preoperative and postoperative evaluation of spinal alignment should be undertake

Gait changes in children with spastic diplegia after selective dorsal rhizotomy.

Twenty-six ambulatory children underwent preoperative and 1-year postoperative assessments after selective dorsal rhizotomy. These included spasticity, passive range of motion, tone, three-dimensional motion analysis, and electromyography. Independent and dependent ambulators were evaluated separately. A decrease in spasticity was found in all lower extremity muscle groups. An increase in passive range of motion was found only at the hip for both independent and dependent ambulators. Gait changes included increases in velocity and stride length in the independent ambulators. An improvement in

Continuous epidural morphine/butorphanol infusion following selective dorsal rhizotomy in children.

The authors prospectively evaluated 15 patients who had undergone selective dorsal rhizotomy who were given a continuous morphine/butorphanol infusion, to determine whether variations in the postoperative pain control and side effects seen using a bolus technique could be reduced. Patients had an epidural catheter placed at the end of the operative procedure through which 50-60 micrograms/kg preservative-free morphine and 15-20 micrograms/kg butorphanol was administered. A continuous epidural infusion of 5 micrograms/kg h morphine and 1.2 micrograms/kg h butorphanol was then initiated. Postope

Urinary continence in spinal injury patients following complete sacral posterior rhizotomy.

Complete sacral posterior rhizotomy was carried out in 15 spinal injury patients in conjunction with implantation of sacral anterior root stimulators. All patients were incontinent pre-operatively and had video-pressure cystometry before and at regular intervals after surgery. Detrusor hyper-reflexia was totally abolished in all but 1 patient following rhizotomy and 87% no longer require any form of incontinence appliance. Deafferentation produced adverse changes in vesicourethral function and even when rhizotomy was complete, continence could not be guaranteed. The pre-operative state of the

Cervicothoracic dorsal rhizotomy for the management of upper limb spasticity: a systematic review.

Lumbar selective dorsal rhizotomy (SDR) is a well-established surgical procedure for managing lower limb spasticity. However, its application for upper limb spasticity remains limited. This systematic review aims to analyze and synthesize the existing literature on cervicothoracic SDR for upper limb spasticity, focusing on surgical techniques, its effectiveness, and associated outcomes. A systematic literature review was conducted in PubMed, Scopus, and the Cochrane Library from inception to 2024, following PRISMA guidelines. Studies reporting at least one new case of cervicothoracic SDR for u

Comparison of EMG Waveforms versus Degree of Spread in Selective Dorsal Rhizotomy.

Selective dorsal rhizotomy (SDR) is a treatment for lower-extremity spasticity in disorders such as cerebral palsy (CP). "Selective" refers to sectioning nerve rootlets with the most abnormal responses on electromyography (EMG) upon intraoperative stimulation. EMG abnormalities can be classified by waveform appearance or by degree of spread throughout lower extremity muscles. We examine the relationship between different EMG waveforms and grades of spread. Intraoperative SDR EMG records from November 2009 through December 2021 were analyzed for waveform types and degrees of spread. Irregular,

A Preliminary Assessment on Reproducibility of Triggered Electromyography During Selective Dorsal Rhizotomy.

BACKGROUND AND OBJECTIVES: Selective dorsal rhizotomy (SDR) reduces spasticity in children with spastic cerebral palsy (CP). Intraoperative neurophysiology, triggered electromyography (trigger-EMG) is crucial in guiding sensory rootlet selection, but its reproducibility during SDR surgery has not been fully investigated. The objective of this study was to evaluate the reproducibility of trigger-EMG during SDR performed in children with spastic CP. METHODS: A retrospective review was performed for cases where dorsal roots were stimulated twice 1 to 2 minutes apart during SDR using a specific p

Frameless navigation-guided percutaneous rhizotomy of the trigeminal nerve: an appraisal of the literature.

Percutaneous rhizotomy is a cornerstone of trigeminal neuralgia treatment. The procedure is classically performed under intermittent fluoroscopic guidance. While frameless navigation has been advanced to overcome potential difficulties and risks of the technique, literature on the subject is limited, and a gap between actual use and published series is likely. We have assessed all available studies of percutaneous rhizotomy of the trigeminal nerve performed under frameless navigation. Technical and clinical data that has been reviewed included clinical outcome, type of navigation employed, typ
