The sesamoid index in psoriatic arthropathy.

OBJECTIVE: The sesamoid index was originally described as an aid to the diagnosis of acromegaly. We performed this study to assess the value of the thumb sesamoid index in the diagnosis of psoriatic arthropathy. DESIGN: Retrospective measurement of the sesamoid index (length x width of the medial thumb sesamoid), along with the age and sex were recorded for patients as described below. Patients with psoriasis were subdivided into those with or without radiographic evidence of hand arthropathy. PATIENTS: Fifty-nine consecutive patients attending rheumatology clinics with arthralgia and psoria

Arthroscopic surgery of the metatarsophalangeal first joint.

Arthroscopic surgery of the metatarsophalangeal first joint was used to treat a dorsal impingement syndrome of the hallux by removing the dorsally located osteophytes. Also, osteochondritis dissecans, painful sesamoid bones resistant to conservative therapy, and hallux rigidus were arthroscopically treated in a prospective study enrolling 24 consecutive patients. Seventeen patients were high-level athletes. Pain, swelling, sports, and work involvement were recorded. The operative technique is described. There was one persistent loss of sensitivity of the hallux. In the dorsal impingement group

Transection of radial digital nerve of the thumb during trigger release.

Iatrogenic laceration of the radial digital nerve of the thumb during trigger release occurred in four patients. Histologic cross sections through the thumb in cadaveric specimens at the level of the metacarpophalangeal crease showed the radial digital nerve to be 2.19 mm beneath the dermis and 1.15 mm directly anterior to the radial sesamoid bone. In this position the nerve is vulnerable to transection when it is trapped between the sesamoid and the knife blade.

Sesamoid problems.

The understanding of the pathology and treatment of hallux sesamoid disorders has improved considerably over the last few decades. It is interesting to speculate what further improvements are on the horizon. Newer imaging methods and possibly an increased use of small joint arthroscopy may make it possible to better diagnose and treat conditions of these two small bones and to develop better biologic and artificial methods of surface replacement. In the meantime clinicians should only excise sesamoids if it is really essential.

Evaluation of skin sensitivity after shock wave treatment in horses.

OBJECTIVE: To evaluate the effects of shock wave treatment on cutaneous nerve function, compared with the effects of local nerve block and sedation. ANIMALS: 18 clinically sound Swiss Warmbloods. PROCEDURE: Horses were randomly allocated to 3 groups and received different amounts and types of shock waves (extracorporeal shock wave treatment [ESWT] or radial pressure wave treatment [RPWT]). Horses were sedated with xylazine and levomethadone. Shock waves were applied to the lateral palmar digital nerve at the level of the proximal sesamoid bones on 1 forelimb. Skin sensitivity was evaluated b

Sudden death caused by spinal cord injury associated with vertebral fractures and fetlock failure in a Thoroughbred racehorse.

The most prevalent causes of death in racehorses are musculoskeletal injuries, causing ~83% of deaths within the racing industry in California and elsewhere. The vast majority of these injuries have preexisting lesions that predispose to fatal injury. A 4-y-old Thoroughbred colt suffered an acute suspensory apparatus failure, including biaxial proximal sesamoid bone fractures of the right front fetlock, causing loss of support of the fetlock joint and consequent fall with fractures of the cervical and sacral spine. Cervical fracture caused spinal cord damage that resulted in sudden death. A pr

Distal Phalanx Fracture of the Toe With Sesamoid Entrapment in the Interphalangeal Joint: A Case Report.

Phalangeal fractures are common among foot injuries, with the majority amenable to conservative treatment. This case involved a fracture of the toe combined with intra-articular sesamoid entrapment. To prevent nonunion, surgical intervention was recommended. We present a rare case of distal phalanx fracture of the toe with interphalangeal joint sesamoid entrapment, review the intraoperative challenges encountered, and summarize postoperative insights with recommendations. We report a 41-year-old female construction worker who presented with a crush injury to her right great toe. Diagnosis was

Radiographic features of the hand in diffuse idiopathic skeletal hyperostosis (DISH): comparison with normal subjects and acromegalic patients.

Radiographs of the hands of 25 men and 9 women with diffuse idiopathic skeletal hyperostosis (DISH) were compared with age- and sex-matched controls and acromegalic patients. Characteristics of DISH included broadening and "arrowheading" of the distal phalangeal tufts, enlarged sesamoid bones, increased cortical width of tubular bones, prominent enthesopathy in the proximal phalanges, exostoses (particularly at the metacarpophalangeal heads), and new bone in the joint capsule. There was no consistent change in soft-tissue measurements or cartilage width between DISH and control patients. Acrom

Posttraumatic Pathologies of the First Metatarsophalangeal Joint.

Injuries to the first metatarsophalangeal joint are often missed and not adequately treated, resulting in secondary deformities and degenerative arthritis. The particular anatomy of the joint puts mainly soft tissue structures at risk, which is why conditions such as turf toe injury and valgus or varus deformities can occur. Because the sesamoid bones are receiving high pressures, fractures should be considered. Many conditions can be treated conservatively by offloading, immobilization, or taping. Surgical options for high instability, dislocated fractures, or advanced cartilage damage should

The sesamoids of the feet in humans: a systematic review and meta-analysis.

The prevalence and distribution of the sesamoid bones in the feet has been reported in the literature with a high degree of variability. This systematic review aims to provide a better estimate of the frequency of the sesamoids of the foot and their association with variables such as ancestry, gender, and side. Thirty-seven studies met the inclusion criteria and were submitted for meta-analyses, sensitivity analyses and proportion difference tests, whenever possible. At the metatarsophalangeal (MTP) joint of the hallux, sesamoids were nearly always present. At the interphalangeal (IP) joint, t

Biomechanical and Microstructural Properties of Subchondral Bone From Three Metacarpophalangeal Joint Sites in Thoroughbred Racehorses.

Fatigue-induced subchondral bone (SCB) injury is common in racehorses. Understanding how subchondral microstructure and microdamage influence mechanical properties is important for developing injury prevention strategies. Mechanical properties of the disto-palmar third metacarpal condyle (MCIII) correlate poorly with microstructure, and it is unknown whether the properties of other sites within the metacarpophalangeal (fetlock) joint are similarly complex. We aimed to investigate the mechanical and structural properties of equine SCB from specimens with minimal evidence of macroscopic disease.

Catastrophic musculoskeletal injuries in Thoroughbred racehorses on racetracks in Gauteng, South Africa.

The incidence and types of catastrophic musculoskeletal injuries in Thoroughbreds that resulted in euthanasia on selected racetracks in South Africa between 1998 and 2012 were investigated by an observational retrospective investigation. Data from the National Horseracing Authority of Southern Africa for these racetracks were used to calculate incidence rates in Thoroughbreds (n = 114) that sustained catastrophic musculoskeletal injuries during racing that required immediate euthanasia, based on the diagnosis made by the on-site veterinarian as well as on fetlock radiographs and dissections of

Nonsurgical Treatment of Osteomyelitis of the Hallux Sesamoids: A Case Series and Literature Review.

The aim of the present study was to determine the effectiveness of nonsurgical treatment for osteomyelitis of the hallucal sesamoids. Osteomyelitis of the hallucal sesamoids in young and healthy patients is rare and might originate from hematogenous spread or after a puncture wound. In diabetic patients with peripheral neuropathy, it often results from direct contiguous seeding from adjacent ulceration. The superiority of surgical versus nonsurgical therapy is still debated. In our institution, all patients presenting with osteomyelitis of the hallucal sesamoids are first treated nonsurgically

[Hallux valgus--several ways to treat].

Hallux valgus is a progressive exertional metatarsal disturbance altering the kinematics of walking. It may be accompanied by other problems of the foot, such as hammer toe or flatfoot syndrome, symptoms of metatarsalgia or arthrosis of the small joints of the foot. Conservative treatment aims to reactivate the muscles of the foot. Operatively, placement of the 1st metatarsal to its anatomical position over the sesamoid bones is attempted to support the action of the foot muscles. When choosing the operative technique the extent and severity of the disorder as well as the objectives and risk f

Delayed Presentation of a Locked Metacarpophalangeal Joint Secondary to Incarcerated Sesamoid with Associated Proximal Ulnar Collateral Tear in a 15-Year-Old Boy.

A 15-year-old right-handed boy presented 7 weeks after a hyperextension injury while playing basketball with a suspected metacarpophalangeal (MCP) joint dislocation to his right thumb and subsequent reduction by his mother at the time of injury. Initial radiographs were negative for acute fracture or dislocation. However, pain and stiffness persisted in the MCP joint, and subsequent magnetic resonance imaging demonstrated an incarcerated radial sesamoid and a proximal ulnar collateral ligament (UCL) injury with migration of the ulnar sesamoid under the proximal UCL, a finding not previously do
