Cochlear synaptopathy (or the loss of primary auditory synapses) remains a subclinical condition of uncertain prevalence. Here, we investigate whether it affects humans and whether it contributes to suprathreshold speech-in-noise intelligibility deficits. For 94 human listeners with normal audiometry (aged 12-68 years; 64 women), we measured click-evoked auditory brainstem responses (ABRs), self-reported lifetime noise exposure, and speech reception thresholds for sentences (at 65 dB SPL) and words (at 50, 65 and 80 dB SPL) in steady-state and fluctuating maskers. Based on animal research, we

In order to elucidate why many elderly listeners have difficulty understanding speech under reverberation, we investigated the relationship between word intelligibility and auditory brainstem responses (ABRs) in 28 elderly listeners. We hypothesized that the elderly listeners with low word intelligibility scores under reverberation would show degraded subcortical encoding information of reverberant speech as expressed in their ABRs towards a reverberant /da/ syllable. The participants were divided into two groups (top and bottom performance groups) according to their word intelligibility score

OBJECTIVE: This study examined the effect of intensity level of presentation on scaling of speech intelligibility in speakers with and without dysarthria.
PATIENTS AND METHODS: A total of 50 utterances produced by speakers with dysarthria and healthy speakers were played to 60 listeners in four conditions, which consisted of two different presentation levels (‘high’ vs. ‘low’) and equalization of levels across utterances (‘adjusted’ vs. ‘unadjusted’). Speech intelligibility was scaled by using a direct magnitude estimation technique with and without modulus.
RESULTS: A significant decrease i

OBJECTIVES: The intelligibility-based Repeat-Recall Test (i-RRT) seeks to compare secondary speech-in-noise (SiN) test outcomes, such as listening effort, against normative references measured at equal levels of SiN intelligibility. The present study evaluates the reliability, sensitivity, and accuracy of the easy speech reception threshold ("ezSRT") test, the first part of the i-RRT, a Bayesian-guided adaptive SiN test for quickly estimating a listeners' complete performance-intensity functions and deriving estimated SRTs.

OBJECTIVES: The purpose of this pilot study was to investigate (1) if the Speech Intelligibility Index (SII) could be used to identify ears that meet cochlear implant (CI) candidacy beyond the "60/60" referral guidelines and (2) investigate the relationship between SII from patients' own hearing aids (HAs) and SII from optimally fit clinic-stock HAs such that a patient's own HAs can act as a proxy for an optimally fit HA and thus as a referral metric.
DESIGN: Prospective study of 23 participants (≥18 years) with bilateral moderate to profound sensorineural hearing loss who were CI candidates

INTRODUCTION: Dysarthria, a motor speech disorder caused by muscle weakness or paralysis, severely impacts speech intelligibility and quality of life. The condition is prevalent in motor speech disorders such as Parkinson's disease (PD), atypical parkinsonism such as progressive supranuclear palsy (PSP), Huntington's disease (HD), and amyotrophic lateral sclerosis (ALS). Improving intelligibility is not only an outcome that matters to patients but can also play a critical role as an endpoint in clinical research and drug development. This study validates a digital measure for speech intelligib

BACKGROUND: More than 6,000,000 individuals worldwide are diagnosed with Parkinson's disease (PD). Nearly 90% develop speech signs that may substantially impair their speech intelligibility, resulting in losses in their communication and quality of life. Benefits of intensive speech treatment have been documented for a range of speech signs. However, the critical question of whether speech is more intelligible after treatment has not been investigated in a randomised controlled trial (RCT). We hypothesised that intensive speech treatment would improve speech intelligibility in PD.

Machine-learning based approaches to speech enhancement have recently shown great promise for improving speech intelligibility for hearing-impaired listeners. Here, the performance of three machine-learning algorithms and one classical algorithm, Wiener filtering, was compared. Two algorithms based on neural networks were examined, one using a previously reported feature set and one using a feature set derived from an auditory model. The third machine-learning approach was a dictionary-based sparse-coding algorithm. Speech intelligibility and quality scores were obtained for participants with

Producing speech that is clear, audible, and intelligible to others is a challenge for many children with cerebral palsy (CP) and children with Down syndrome (DS). Previous studies have demonstrated the effectiveness of using the Lee Silverman Voice Treatment (LSVT LOUD®) to increase vocal loudness and improve speech intelligibility in individuals with dysarthria secondary to Parkinson's disease (PD), and some research suggests that it also may be effective for individuals with dysarthria secondary to other conditions, including CP and DS. Although LSVT LOUD targets healthy vocal loudness, the

OBJECTIVE: Individuals with hearing impairments face challenges in their ability to comprehend speech, particularly in noisy environments. This study explores the effectiveness of audio-visual speech enhancement (AVSE) in improving the intelligibility of vocoded speech in cochlear implant (CI) simulations.
METHODS: We propose a speech enhancement framework called Self-Supervised Learning-based AVSE (SSL-AVSE), which uses visual cues such as lip and mouth movements along with corresponding speech. Features are extracted using the AV-HuBERT model and refined through a bidirectional LSTM. Experi

PURPOSE: Speech audibility refers to how much access a listener has to the acoustic cues that comprise spoken communication. The Speech Intelligibility Index (SII) is a measure of speech audibility that has had increasing use in pediatric audiology to quantify auditory access with and without hearing aids. This tutorial focuses on clinical applications of the SII in children who are deaf and hard of hearing (DHH).
METHOD: We discuss the calculation of the SII and the current clinical tools that calculate aided and unaided SII. We describe practical uses of the SII throughout the steps of audi

Aural rehabilitation with hearing aids can decrease the attentional requirements of cognitive resources by amplifying deteriorated-frequency sound in hearing loss patients and improving auditory discrimination ability like speech-in-noise perception. As aural rehabilitation with an intelligible-hearing sound also can be hopeful, the aim of this study was to evaluate the effectiveness of aural rehabilitation with intelligible-hearing sound for hearing loss patients. Adult native Japanese speakers (17 males and 23 females, 68.43 ± 9.23 years) with hearing thresholds exceeding 30 dB at any of the
