Editorial Commentary: Arthroscopically Assisted Lower Trapezius Transfer: Promising Technique Still Searching for the Optimal Indications or Panacea for All Irreparable Rotator Cuff Tears?

Arthroscopic-assisted lower trapezius transfer (LTT) has gained in popularity as a technique to restore function and improve pain in large-to-massive irreparable posterosuperior rotator cuff tears. Surgeons who have adopted this technique have variable indications. While most agree that there is a limit of arthropathy, other factors appear to be contentious. Age, teres minor integrity, and degree of external rotation deficit remain debatable in terms of indications and contra-indications for LTT. The biomechanical narrative around LTT is sound and has been proven. Surgeons have recently raised

Comparison of upper trapezius muscle stiffness between female assembly workers with and without myofascial trigger points.

BackgroundMuscle stiffness in the upper trapezius (UT) muscle is a significant parameter for evaluating myofascial trigger points (MTrPs). Because the measuring position influences UT muscle stiffness, investigating UT muscle stiffness in different working postures would be useful for evaluating and planning effective interventions for MTrPs.ObjectiveTo identify differences in UT muscle stiffness measured in the resting and working postures among female assembly workers, both with and without MTrPs.MethodsEighteen female workers with right (Rt) side UT MTrPs and 18 workers without MTrPs partic

Posterior intercostal flap: an anatomical study and best flap design.

BACKGROUND: Posterior intercostal arteries perforators (PICAPs) and lateral intercostal arteries perforators (LICAPs) are great vascular pedicles. Between the 4th and 11th spaces, they arise from the thoracic aorta. These are large perforators that can be the basis of many flaps. Yet, these perforators are underrated as they are poorly studied and scarcely utilized in plastic reconstructions. METHODS: Twenty (ten males and ten females) adult cadaveric dissections were done on both sides to study the types, locations, and sizes of posterior intercostal perforators to help design flaps based on

Ultrasound-guided five-point injection of botulinum toxin for patients with trapezius hypertrophy.

OBJECTIVES: Our study aimed to explore the clinical therapeutic effects of ultrasound-guided five-point injection of botulinum toxin type A for patients with trapezius hypertrophy. METHODS: Twenty female patients diagnosed with trapezius hypertrophy were enrolled in this study. The thicknesses of the trapezius muscle were measured by using the ultrasound scanner to locate the thickest point of trapezius, followed by labelling the other four points around the first point. Botulinum toxin type A was injected bilaterally (50 IU/side, 5 points/side) in the trapezius muscle of these patients. The

[Neuromuscular dynamic scapular winging: Clinical, electromyographic and magnetic resonance imaging diagnosis].

Dyskinesia of the scapula is a clinical diagnosis and includes all disorders affecting scapula positioning and movement whatever its etiology. Scapular winging is a subtype of scapular dyskinesia due to a dynamic prominence of the medial border of the scapula (DSW) secondary to neuromuscular imbalance in the scapulothoracic stabilizer muscles. The two most common causes of DSW are microtraumatic or idiopathic lesions of the long thoracic nerve (that innerves the serratus anterior) or the accessory nerve (that innerves the trapezius). Diagnosis of DSW is clinical and electromyographic. Use of m

Adaptability of the load sharing between the longissimus and components of the multifidus muscle during isometric trunk extension in healthy individuals.

PURPOSE: Redundancy of the musculoskeletal system implies multiple strategies are theoretically available to coordinate back extensor muscles. This study investigated whether coordination between back muscles during a tightly constrained isometric trunk extension task varies within and between individuals, and whether this changes following brief exposure to activation feedback of a muscle. METHODS: Nine healthy participants performed three blocks of two repetitions of ramped isometric trunk extension in side-lying against resistance from 0-30% of maximum voluntary contraction over 30 s (forc

Comparing exercises with and without electromyographic biofeedback in subacromial pain syndrome: A randomized controlled trial.

BACKGROUND: Deficits in movement and muscle activation of scapulohumeral joint are related to Subacromial Pain Syndrome. Electromyography biofeedback during exercise may enhance muscle activation and coordination, and consequently improve pain and shoulder function. METHODS: This study compared the effects of an exercise protocol with and without using electromyographic biofeedback on pain, function and movement of the shoulder complex in subjects with Subacromial Pain Syndrome. A total of 24 patients with subacromial pain (mean age = 46.2 + 8.1;18 women) were randomized to either therapeutic

Influence of an upper limb exoskeleton on muscle activity during various construction and manufacturing tasks.

Musculoskeletal disorders (MSDs) significantly impact workers in the manufacturing and construction sectors. One solution that has gained interest to reduce MSDs incidence is the use of exoskeletons. In this study, the influence of an upper limb exoskeleton on muscle activity was investigated experimentally for three commonly performed tasks in the manufacturing and construction sectors. The tasks tested were overhead assembly, bricklaying, and box moving tasks. Eighteen males participated in the tests. The results showed a reduction in shoulder flexor muscle activation during all three tasks

Scapular Winging Caused by Combined Palsy of the Spinal Accessory Nerve and the Long Thoracic Nerve: A Case Report.

CASE: A 37-year-old man with scapular winging, caused by combined palsy of the spinal accessory nerve and the long thoracic nerve, was successfully treated with a pectoralis major transfer to substitute for the serratus anterior muscle, and with levator scapulae and rhomboid muscle transfers to substitute for the trapezius muscle. CONCLUSION: The serratus anterior paralysis was thought to have occurred secondary to traction of the long thoracic nerve by the unsupported scapula following the spinal accessory nerve palsy. Even with combined paralyses of the serratus anterior and trapezius muscl

Reconstruction with latissimus dorsi, external abdominal oblique and cranial sartorius muscle flaps for a large defect of abdominal wall in a dog after surgical removal of infiltrative lipoma.

This animal was presented with a large-sized infiltrative lipoma in the abdominal wall that had been noted for 4 years. This lipoma was confirmed by histological examination from a previous biopsy, and the infiltrative features were identified by a computerized tomography scan. The surgical removal created a large-sized abdominal defect that was closed by a combination of latissimus dorsi and external abdominal oblique muscle flaps in a pedicle pattern. A small dehiscence at the most distal end of the muscle flap resulted in a small-sized abdominal hernia and was repaired with cranial sartoriu

Trapezius Muscle Electromyographic Activity in Lawn-Mower Exercise: Standing Versus Quadruped.

CONTEXT: In shoulder rehabilitation, exercises that reduce upper trapezius (UT) activity and optimize the trapezius muscle activation rate reduce the symptoms of shoulder pathologies. One such exercise, the lawn-mower (LM) exercise, is frequently used in scapular rehabilitation protocols due to its multijoint, multiplanar, and kinetic chain-based nature. Design/Objective: This study aimed to compare trapezius muscle activation levels and ratios during the LM exercise in standing and quadruped positions through a randomized controlled trial. METHODS: Eighteen healthy volunteers (mean age [SD]

Muscle Pain Induces a Shift of the Spatial Distribution of Upper Trapezius Muscle Activity During a Repetitive Task: A Mechanism for Perpetuation of Pain With Repetitive Activity?

OBJECTIVE: An association exists between repetitive movements and the development or perpetuation of neck-shoulder muscle pain. The mechanisms underlying this association remain unclear. This observational study investigated the effect of upper trapezius muscle pain on the distribution of upper trapezius activity during repetitive lifting. It was hypothesized that nociception would change the distribution of activity resulting in activation of muscle regions which would not normally be active during the task. MATERIALS AND METHODS: Healthy men repeatedly lifted a box with a cycle time of 3 se
