Transient tachypnea of the newborn (TTN): a role for polymorphisms of surfactant protein B (SP-B) encoding gene?

BACKGROUND: Transient tachypnea of the newborn (TTN) is usually a benign self-limiting respiratory disorder in the immediate neonatal period. The lipophilic surfactant-associated protein B (SP-B) was demonstrated to be the most relevant structural component of the surfactant system for immediate postnatal pulmonary adaptation. We hypothesized genetic variations of surfactant protein B (heterozygous 121 ins 2 mutation er intron 4 polymorphisms) to be related to TTN. PATIENTS AND METHOD: We screened genomic DNA of 83 healthy term neonates (gestational age: 39 (37 - 41) completed weeks [median a

[A simple method to assess dynamic hyperinflation outside a pulmonary function test laboratory: The metronome-paced tachypnea test].

Lung dynamic hyperinflation (DH) is one of the main determinants of dyspnea in chronic respiratory disease patients. Producing evidence of DH is critical during dyspnea assessment, the objectives being to explain the cause, to target treatments, and to monitor their efficacy. The gold standard method consists in repeated measurement of inspiratory capacity (IC) during cardiopulmonary exercise testing (CPET). Unfortunately, access to CPET is limited and assessment of IC during CPET can be challenging in some patients. An alternative method consists in assessment of IC during the testing known a

Thoracic fluid content (TFC) using electrical cardiometry versus lung ultrasound in the diagnosis of transient tachypnea of newborn.

This study aimed to evaluate TFC by EC versus lung ultrasound (LUS) findings for diagnosing and follow-up of TTN in late preterm and term neonates. This prospective observational study was conducted on 80 neonates with gestational age ≥ 34 weeks. TTN group included 40 neonates diagnosed with TTN, and no lung disease (NLD) group included 40 neonates without respiratory distress. LUS and EC were performed within the first 24 h of life and repeated after 72 h. There was a statistically significant increase in TFC in TTN group on D1 [48.48 ± 4.86 (1 KOhm-1)] compared to NLD group [32.95 ± 4.59 (1

Transient Tachypnea of Newborns Is Associated With Maternal Spinal Hypotension During Elective Cesarean Delivery: A Retrospective Cohort Study.

BACKGROUND: The risk for transient tachypnea of newborns, a common cause of respiratory distress in the neonatal period, is 2- to 6-fold higher during elective cesarean delivery compared to vaginal delivery. Here, we evaluated the association between transient tachypnea of newborns and the degree and duration of predelivery maternal hypotension during spinal anesthesia for elective cesarean delivery. METHODS: Demographic data, details of anesthetic management, blood pressure measurements, and vasopressor requirement preceding delivery were compared between transient tachypnea newborns (n = 30

Efficacy of Noninvasive Neurally Adjusted Ventilatory Assist (NIV-NAVA) for Neonates With Transient Tachypnea of the Newborn: A Retrospective Analysis.

BACKGROUND: Noninvasive neurally adjusted ventilatory assist (NIV-NAVA) provides synchronized respiratory support based on diaphragmatic electrical activity and has shown efficacy in preterm neonates. However, its efficacy in late preterm and term neonates with transient tachypnea of the newborn (TTN) remains unclear. METHODS: We retrospectively analyzed 183 neonates (≥ 34 weeks' gestation) with TTN between 2016 and 2024 who received initial respiratory support with either NIV-NAVA (n = 68) or sigh positive airway pressure (SiPAP, n = 115). After propensity score matching (PSM), we compared p

A lung ultrasound B-line score to stratify oxygen therapy in transient tachypnea of the neonate: a prospective cohort study.

BACKGROUND: Transient tachypnea of the neonate (TTN) is the most common cause of respiratory distress in newborns. While often self-limiting, the severity of TTN varies significantly, ranging from mild tachypnea to severe respiratory failure requiring non-invasive ventilation. Early and objective grading of TTN severity remains difficult in routine practice. Clinical scores are subjective, and chest radiography is insensitive to the extent of lung fluid. This study evaluated a semi-quantitative lung ultrasound (LUS) B-line score as a predictor of oxygen requirement in neonates with TTN. METHO

Evaluating the efficacy of LDH and inflammatory indices in discriminating neonatal respiratory distress syndrome from transient tachypnea of the newborns.

BACKGROUND: Early discrimination between transient tachypnea of the newborn (TTN) and respiratory distress syndrome (RDS) is critical for establishing timely targeted therapies. Our aim was to assess the efficacy of LDH, platelet indices, and systemic inflammatory indices in distinguishing neonatal RDS from TTN early. METHODS: In total, 300 neonates were enrolled in this case-control study. Lactate dehydrogenase (LDH) levels were estimated. Platelet and systemic inflammatory indices were calculated using complete blood count. RESULTS: The RDS group exhibited substantially higher serum levels

The role of plasma N-terminal pro-B-type natriuretic peptide in predicting the severity of transient tachypnea of the newborn.

BACKGROUND/AIM: Transient tachypnea of the newborn (TTN) is a consequence of inadequate neonatal lung fluid clearance. Natriuretic peptides play an important role in the regulation of extracellular fluid volume. The aim of the study was to investigate the relation between plasma N-terminal pro-B-type natriuretic peptide (NT-proBNP) levels and TTN, and to find out its role in predicting disease severity. METHODS: A prospective controlled study involving 67 infants with TTN and 33 controls ≥34 weeks gestational age was conducted. Study and control groups were compared for plasma NT-proBNP level

Lung ultrasound in early diagnosis of neonatal transient tachypnea and its differentiation from other causes of neonatal respiratory distress.

BACKGROUND: Transient tachypnea of the newborn (TTN) is one of the most common causes of neonatal respiratory distress (RD). However, distinguishing TTN from other causes of RD may be difficult during the first 12:24 h after birth. Lung ultrasonography (LUS) has been successfully utilized in the diagnosis and differential diagnosis of neonatal RD. This study aimed to investigate the diagnostic value of LUS for early diagnosis of TTN as well as differentiate it from other causes of neonatal RD in near and full term Egyptian neonates. METHODS: LUS was performed in 65 near and full term neonates

Using Late C-reactive Protein Measurement in Transient Tachypnea of the Newborn to Predict Bacteremia and Reduce Blood Sampling Frequency.

Background Frequent blood draws in neonates pose significant challenges, including procedural pain, risk of anemia, and increased healthcare burden. Hospitalized neonates with respiratory distress often undergo multiple blood tests to rule out bacterial infections, despite the low incidence of neonatal bacteremia. C-reactive protein (CRP) levels rise 10-12 hours post-infection, suggesting that early testing may not be optimal for predicting bacteremia. This study evaluates whether delaying the initial blood test and relying on a single blood draw can effectively predict neonatal bacteremia, ai

A randomized controlled trial of restricted versus standard fluid management in late preterm and term infants with transient tachypnea of the newborn.

BACKGROUND: Transient tachypnea of the newborn(TTNB) is the most common respiratory morbidity in late preterm and term babies and is pathophysiologically related to delayed lung fluid clearance after birth. Mimicking low physiological fluid intake in the initial period of life may accelerate the recovery from TTNB. In a randomized controlled trial, we compared the roles of restricted versus standard fluid management in babies with TTNB requiring respiratory support. METHODS: This parallel group,non-blinded, stratified randomized controlled trial was conducted in a level III neonatal unit of e
