Arthroscopic Extensor Carpi Radialis Brevis Tenotomy Under Local Anesthesia for Refractory Lateral Epicondylitis.

Refractory lateral epicondylitis poses significant challenges for patients and doctors. Whereas traditional surgical treatments involve open resection and extensor carpi radialis brevis (ECRB) muscle debridement, arthroscopic surgery advancements offer a more refined approach. Numerous surgical methods are used to treat lateral epicondylitis. Nevertheless, some patients experience discomfort after arthroscopic surgery. This article outlines our approach for arthroscopic ECRB muscle tenotomy under local anesthesia. Arthroscopic ECRB muscle tenotomy under local anesthesia is an important treatme

Biceps tenodesis versus tenotomy in isolated LHB lesions: a prospective randomized clinical trial.

INTRODUCTION: Currently there exists no clear evidence concerning the surgical treatment of LHB lesions with either tenotomy or tenodesis. The aim of the study is therefore to evaluate elbow flexion and forearm supination force as well as the biceps muscle distalization according to both techniques in isolated LHB lesions. METHODS: Consecutive patients aged 40-70 years with shoulder arthroscopies for isolated SLAP or biceps pulley lesions were prospectively randomized to arthroscopic suprapectoral intraosseous LHB tenodesis or tenotomy. Pre-, 6 and 12 months postoperatively, the SST, ASES, Co

Graded vertical rectus tenotomy for small-angle cyclovertical strabismus in sagging eye syndrome.

BACKGROUND/AIMS: Graded vertical rectus tenotomy (GVRT) is postulated as effective for small-angle vertical heterotropia. We aimed to determine the dosing recommendations for GVRT in sagging eye syndrome (SES). METHODS: This was a retrospective, observational study of surgical outcomes for GVRT from 2009 to 2014 in a single surgeon's academic practice. There were 37 (20 women) patients of average age 68±10 (SD) years with comitant or incomitant hypertropia ≤10Δ caused by SES. The main outcome measure was the dose-effect of GVRT required to correct intraoperative hypertropia. RESULTS: Preoper

Correlation of clinical and ultrasonographic findings after Achilles tenotomy in idiopathic club foot.

Achilles tenotomy is a recognised step in the Ponseti technique for the correction of idiopathic congenital talipes equinovarus in most percutaneous cases. Its use has been limited in part by concern that the subsequent natural history of the tendon is unknown. In a study of 11 tendons in eight infants, eight tendons were shown to be clinically intact and ten had ultrasonographic evidence of continuity three weeks after tenotomy. At six weeks after tenotomy all tendons had both clinical and ultrasonographic evidence of continuity.

Endoscopic Flexor Hallucis Longus Tenotomy for Post-traumatic Checkrein Deformity: A Case Report.

BACKGROUND: Checkrein deformity is a dynamic flexion deformity of the great toe, most often occurring after distal tibial trauma due to flexor hallucis longus (FHL) tethering or contracture. This report presents a rare case of post-traumatic checkrein deformity managed successfully with endoscopic FHL tenotomy. CASE REPORT: A 16-year-old male presented with dynamic hallux flexion deformity, 3 years after internal fixation for a distal tibia and fibula fracture. Clinical examination confirmed checkrein deformity. Endoscopic FHL tenotomy was performed via hindfoot endoscopy. Postoperatively, th

Biomechanics Explains Variability of Response of Small Hypertropia to Graded Vertical Rectus Tenotomy.

PURPOSE: Small angle hypertropia in sagging eye syndrome is conveniently treated by graded vertical rectus tenotomy, yet an adjustable technique under topical anesthesia has been recommended because of variability of effect. We performed graded tenotomy in an experimental model to elucidate the reason for variability of response to this surgical procedure. DESIGN: Experimental study. METHODS: Thirty-two fresh bovine rectus musculotendon specimens were prepared including continuity with insertional sclera, and extending for a total 40 mm length to the proximal muscle bellies, and trimmed to 1

Does endoscopic piriformis tenotomy provide safe and complete tendon release? A cadaver study.

BACKGROUND: Endoscopic piriformis release (EPR) is among the available treatments for piriformis syndrome. This procedure typically involves dividing the muscle near the sciatic nerve in the sub-gluteal space, which contains numerous blood vessels and nerves. The objectives of this prospective cadaver study were: 1) to assess the reproducibility and quality of endoscopic piriformis tenotomy near the greater trochanter; 2) to detect iatrogenic injuries to the lateral hip rotators, nerves, and vessels; 3) and to define the surgical safety margins relative to the sciatic nerve and inferior glutea

Surgical correction for curly toe using open tenotomy of flexor digitorum brevis tendon.

Curly toe is a common congenital deformity characterized by flexion and varus deformity of the interphalangeal joints. Because this minor deformity is seldom accompanied with any symptoms, treatment strategy has rarely been discussed in detail in the literature. Eight toes in seven patients with curly toe were treated by open tenotomy of the medial slip of the flexor digitorum brevis tendon. If sufficient correction was not obtained, the collateral ligament and the volar plate of the proximal interphalangeal joint were dissected. The skin defect at the plantar base of the toe was covered using

Extensor tenotomy: a technique for correction of posttraumatic distal interphalangeal joint hyperextension deformity.

Tenotomy of the dorsal apparatus over the middle phalanx was done in 13 digits for treatment of posttraumatic hyperextension of the distal interphalangeal joint associated with a boutonniere deformity. All but one patient was improved with distal interphalangeal joint flexion averaging 45 degrees at follow-up. Four digits had a 5 degree to 10 degree extensor lag. This procedure is simple and reliable and should be considered when increased distal interphalangeal joint flexion is desired.

The impact of associated tenotomies on the outcome of incomplete phalangeal osteotomies for lesser toe deformities.

BACKGROUND: Partial or incomplete osteotomy (IO) of the phalanx is recently described in the literature. However, the clinical outcome and the rate of complications when applied to lesser toe deformities (LTD) have been never addressed. This study aims to find out if the association of tenotomies to incomplete or partial phalanx osteotomies has a significant impact on the clinical outcomes, the occurrence of complications, and the recovery time after surgery. METHODS: A retrospective review of two cohorts of cases operated in our institution for hallux abductus valgus (HAV) and associated LTD

Tenotomy versus tenodesis in the treatment of the long head of biceps brachii tendon lesions.

BACKGROUND: The superiority of tenotomy vs. tenodesis for surgery on lesions of the long head of the biceps brachii tendon is still under debate. Indeed, high-quality evidence is lacking, mainly because of methodological problems, such as retrospective design, population sample size or lack of patient randomization. METHODS/DESIGN: The study will be a two-center, double-blind, randomized, controlled trial to compare patients treated with biceps tenotomy or tenodesis for lesions of the long head of the biceps brachii tendon over a 2-year follow-up period. The study participants will be 128 adu

Regeneration of the Achilles tendon after percutaneous tenotomy in infants: a clinical and MRI study.

We aimed to study the regeneration potential of tendo-achilles after percutaneous tenotomy in 34 clubfeet treated by Ponseti's technique. Clinical and MRI evaluation was done after 6 weeks and 6 months of tenotomy to assess the regeneration of the tendon. At the follow-up, Achilles tendon was palpated like a cord in all the feet. MRI study revealed continuity of the tendon in all cases at the end of 6 weeks and 6 months of the tenotomy. Thus, it could be concluded that tendo-achilles does regenerate following percutaneous tenotomy, used in the correction of clubfoot using Ponseti's technique.
