The elusive diagnosis of thoracic outlet syndrome and its controversial relationship with carpal tunnel syndrome is examined by a retrospective study of 1000 cases of carpal tunnel syndrome and 63 cases of thoracic outlet syndrome. Thoracic outlet syndrome is rare and difficult to accurately diagnose. The signs and symptoms of thoracic outlet syndrome lack specificity. The ancillary tests used to evaluate patients with expected thoracic outlet compression also add little objectivity to the diagnostic process. Carpal tunnel syndrome is still as inaccurately diagnosed as thoracic outlet syndrome

BACKGROUND: Thoracic outlet syndrome (TOS) arises from compression of neurovascular structures as they traverse the thoracic outlet at the base of the neck, with 3 main types: neurogenic, venous, and arterial. Established risk factors include acute trauma, repetitive upper extremity use, or congenital anatomical variations. Birth trauma, although relatively rare, remains a recognized cause of injury to the brachial plexus, yet its role in TOS development has not been investigated. This study aims to explore the prevalence of reported birth trauma among patients with clinically diagnosed and su

Thoracic outlet syndrome (TOS) involves inconsistent symptoms, presenting a challenge for medical providers to diagnose and treat. Thoracic outlet syndrome is defined as a compression injury to the brachial plexus, subclavian artery or vein, or axillary artery or vein occurring between the cervical spine and upper extremity. Three common subcategories are now used for clinical diagnosis: neurogenic, arterial, and venous. Postural position and repetitive motions such as throwing, weightlifting, and manual labor can lead to symptoms. Generally, TOS is considered a diagnosis of exclusion for athl

Venous thoracic outlet syndrome (TOS) is uncommon but occurs in young, healthy patients, typically presenting as subclavian vein (SCV) effort thrombosis. Venous TOS arises through chronic repetitive compression injury of the SCV in the costoclavicular space with progressive venous scarring, focal stenosis, and eventual thrombosis. Diagnosis is evident on clinical presentation with sudden spontaneous upper extremity swelling and cyanotic discoloration. Initial treatment includes anticoagulation, venography, and pharmacomechanical thrombolysis. Surgical management using paraclavicular decompress

Robotic resection of the "offending portion" of the first rib in patients with thoracic outlet syndrome (TOS) has been associated with excellent results. The results have been due to (I) a better understanding of the pathogenesis of TOS, and (II) the technical advantages of the robotic platform. This article outlines the recent understanding of the pathogenesis of TOS, and reports the experience with robotic resection of the "offending portion" of the first rib in patients with neurogenic and venous TOS. Patients diagnosed with TOS underwent robotic first rib resection. Diagnosis of TOS was ma

OBJECTIVE: Thoracic Outlet Syndrome (TOS) is caused by a compression of the brachial plexus and the subclavian vessels in their passage to the upper limb. It mostly occurs in women aged 20-50, so it is infrequent in children. We present our results in the diagnosis and management of pediatric TOS.
MATERIAL AND METHODS: Retrospective study of patients diagnosed with TOS between December 2017 and June 2018. Clinical, radiological, surgical, and evolution variables were assessed.
RESULTS: Five TOS were diagnosed in 4 patients - one TOS was bilateral. Mean age at diagnosis was 12.5 years (7-15),

Thoracic outlet syndrome (TOS) is an infrequent entity encountered by medical practitioners attributed to compression of the neurovascular structures passing through the thoracic outlet. Here, we report the presentation of a young adult who was referred for workup of cervical radiculopathy and was planned to undergo an anterior cervical discectomy and fusion. A dynamic cerebral angiogram was performed and confirmed the diagnosis of arteriogenic TOS. The etiology, presentation, and diagnostic workup of TOS are briefly discussed with emphasis placed on maintaining a healthy suspicion in order to

Thoracic outlet syndrome (TOS) is a condition arising from compression of the subclavian vessels and/or brachial plexus as the structures travel from the thoracic outlet to the axilla. Despite the significant pathology associated with TOS, there remains some general disagreement among experts on the specific anatomy, etiology, and pathophysiology of the condition, presumably because of the wide variation in symptoms that manifest in presenting patients, and because of lack of a definitive gold standard for diagnosis. Symptoms associated with TOS have traditionally been divided into vascular an

This study aims to quantify the management and outcomes of patients treated for venous thoracic outlet syndrome (vTOS) over a seven-year period. A retrospective case-note review of all patients undergoing first rib resection at a regional vascular unit between January 1, 2002 and December 31, 2009 was performed. Treatment pathways and outcomes recorded as freedom from symptoms and venous patency were analyzed. Thirty-five patients were identified with vTOS. Ninety-one percent of patients had patent veins at discharge from clinical follow-up and were symptom-free at a median of 44 months. Patie

OBJECTIVE: To evaluate the sequences and maneuvers recommended for the study of the thoracic outlet syndrome (TOS) and the usefulness of magnetic resonance imaging (MRI) in demonstrating its etiology.
MATERIAL AND METHODS: We present a study of eight patients with clinical presentation suggestive of TOS. All underwent MRI, gadolinium-enhanced angio-MRI with the arms extended along the body and with postural maneuvers of abduction and elevation of the arms, plain-film chest x-rays, and digital angiography. The anatomic characteristics of the superior aperture of the thorax were analyzed on bot

The treatment of thoracic outlet syndrome (TOS) is controversial and long-term results are poorly documented. This retrospective study was carried out to assess clinical outcome 2 years after TOS surgery and to determine predictive factors of outcome. Between 1979 and 1999, 155 patients underwent TOS surgery. Of these patients, 140 (90.3%) had a minimum follow-up of 2 years. Thirty-six (25.7%) patients underwent bilateral procedures. A total of 176 procedures were reviewed and served as a basis for study. Presenting symptoms were neurologic in 15 cases (8.5%), arm or hand ischemia occurred in

Sixty patients were treated by physical therapy for thoracic outlet syndrome (TOS) between 1995 and 1999. Females were predominant (76%), the average age was 34.5 years (18 to 60). Risk factors were: hypermobility, cervical traumatism and profession. Neurological signs were predominant and positive provocation tests were found: Adson (48%), sonnette (62%), E.A.S.T. (51%). Thoracic pain inaugurated the symptoms in 3 times: pseudo-angina (2), dyspnea (1). Radiological results were positive for: cervical rib (8), apophysomegaly of C7 (25), clavicular traumatism (1). Electromyogram was positive in

We report a case of first rib hemangioma that caused thoracic outlet syndrome. A 50-year-ole woman who was admitted to our hospital with a clinical diagnosis of thoracic outlet syndrome presented with fullness and easy fatigue of her right arm. Her right arm discomfort was associated with intermittent engorgement of superficial veins over the shoulder girdle. A chest radiograph revealed an enlargement of the anterior aspect of the first rib with fine bony trabeculations. Computed tomography scan showed contrast enhancement over the enlarged rib. Our tentative preoperative diagnosis was a benig

In 123 patients the peculiarities of the blood circulation disorder of upper extremities in presence of the compression thoracic outlet syndrome were studied. The m. scaleni syndrome was noted in 61 (49.5%) patients, cervical rib syndrome--in 9 (7.3%), costoclavicular syndrome--in 22 (18%), hyperabduction syndrome--in 31 (25.2%). The dependence of the blood circulation disorder from the kind of compression was established, permitting to choose the method of treatment differentially and to improve its results.

The data from the nationwide hospital discharge register was used for collecting the diagnoses of the thoracic outlet syndrome (TOS) which were combined with the procedure numbers of first rib resection and scalenotomy. During the years 1987-1993 the total number of operative operative periods for patients with a TOS diagnosis was 483 for 464 patients. Each year first rib resections were done significantly (P = 0.001) more often (55.7 SE 7.1; 1.11/100,000) than scalenotomies (13.4 SE 1.8; 0.27/100,000). The operation for TOS was most commonly combined with the diagnosis of TOS with brachial pl

The major developments in the evolution of understanding the thoracic outlet syndromes is presented in a chronologically historical record beginning with the first mention of a cervical rib by Galen in the second century A.D. Appreciation of the vascular and neurologic types of TOS progressed slowly through the centuries until detailed clinical studies were presented in the early 1900s. Interest in these syndromes increased steadily since then with description of the effectiveness of first rib resection by Murphy in 1910, and scalenotomy without cervical rib resection favored by Adson in 1927.

Arterial complications of thoracic outlet syndrome (TOS) were surgically treated in 11 patients (12 limbs) and venous complications in five (6 limbs). Arteriography showed total occlusion or significant stenosis of the subclavian artery in eight patients (bilateral in 1), with complicating peripheral thrombosis in three. Two patients had unilateral subclavian artery aneurysm: One was the patient with bilateral subclavian occlusion, and the other also had brachial artery embolism. Yet another patient had brachial thrombosis. Treatment included reconstructive surgery (3 limbs), thoracic sympathe

BACKGROUND: Thoracic outlet syndrome (TOS) treatment remains a challenge in this context of functional pathology. After an initial surgical treatment, TOS may recur, and its management becomes a challenge again. We report our experience about recurrent-TOS after complete first rib resection and scalenotomy. After a new diagnostic assessment, and failure of physiotherapy, we performed a second surgical treatment. CASE PRESENTATION: A 32-year-old patient presented with symptoms consistent with left-sided neurogenic thoracic outlet syndrome (NTOS). After the failure of physiotherapy, a first rib
