The "mini brain": plasmacytoma in a vertebral body on MR imaging.

OBJECTIVE: Patients with solitary plasmacytoma in the spine frequently require a biopsy for diagnosis of their condition. We report an appearance of plasmacytoma in the spine, which is sufficiently pathognomonic to obviate biopsy. CONCLUSION: Identification of a "mini brain" in an expansile lesion in the spine is characteristic of plasmacytoma. It is important that radiologists note this characteristic because biopsy can be avoided in patients with this appearance. Although biopsy might still be required at many institutions, at our institution, surgeons find this appearance sufficiently path

Preoperative estimation of screw fixation strength in vertebral bodies.

STUDY DESIGN: Bone mineral density and bone cross-sectional area of human cadaveric vertebral bodies were investigated radiologically and histologically, respectively. After ventral instrumentation with ventral derotation spondylodesis screws, axial pullout force was measured and compared with radiologic and histologic data. OBJECTIVES: To elucidate how well ventral derotation spondylodesis screw fixation strength can be estimated before surgery by specified applications of dual-energy x-ray absorptiometry, quantitative computed tomography, T2*-relaxation time in magnetic resonance imaging, a

Gender differences in vertebral body sizes in children and adolescents.

PURPOSE: To determine whether differences in vertebral bone densities or sizes account for gender differences in skeletal mass during growth. MATERIALS AND METHODS: Quantitative computed tomography (CT) was used to measure the densities of cortical and cancellous bone and dimensions of the lumbar vertebral bodies in 196 healthy children and adolescents, ages 4-20 years. RESULTS: Neither cancellous nor cortical bone densities differed between boys and girls with age or level of sexual development. In contrast, the cross-sectional areas of the vertebral bodies were greater in boys than girls t

Fracture of the S1 vertebral body in a patient with ankylosing spondylitis.

We report an unusual case of spinal fracture in ankylosing spondylitis. The fracture occurred through the S1 vertebral body and resembled a traumatic spondylolisthesis. Treatment consisted of prompt operative intervention after extensive diagnostic evaluation. The case emphasizes the difficulty of diagnosing spinal fractures in those with ankylosing spondylitis, the necessity for being alert to the possibility of fracture in this group, and the value of early surgical intervention for sacral fractures with neural deficit.

[The intravertebral vacuum phenomenon or gaseous dissection of the vertebral body].

The intravertebral vacuum phenomenon is rather uncommon. It is determined by the collapse of the body of the vertebra with successive aspiration ex vacuo of gas (nitrogen) from the surrounding tissues, which collects under the endplates, causing them to appear disconnected. To date, the one who has reported the highest number of cases is Maldague, with sixty examples. Our report concerns four cases seen during the past five years. According to the most widely accepted pathogenetic theory, this phenomenon represents, together with vertebral collapse and increased bone density, a pathognomonic s

[Risk factors for the collapse of cemented vertebral bodies after the minimally invasive vertebral augmentation procedures].

OBJECTIVE: To evaluate the incidence rate, the independent factors, and the prevention measures of the collapse of cemented vertebral bodies after minimally invasive vertebral augmentation procedures including percutaneous vertebroplasty (PVP) and percutaneous kyphoplasty(PKP). METHODS: From January 2012 to June 2013, 154 patients with single segmental osteoporotic vertebral compression fractures(OVCF)were treated with PVP or PKP. There were 65 males and 89 females, ranging in age from 57 to 90 years old, with a mean of (76.20±9.35) years old. All the patients were followed up, and the durati

Vertebral artery location in relation to the vertebral body as determined by two-dimensional computed tomography evaluation.

STUDY DESIGN: This study analyzed the precise two-dimensional location of the vertebral artery within cervical vertebrae as determined by measurements obtained from axial computed tomographic images of the cervical spine. OBJECTIVE: To determine the margin of safety necessary to avoid vertebral artery laceration during central decompression and lateral nerve root decompression for cervical spinal stenosis. SUMMARY OF BACKGROUND DATA: Laceration of the vertebral artery is a rare but potentially catastrophic complication of anterior decompressive surgery of the cervical spine. METHODS: The me

Bilateral anterior lumbar vertebral body tethering: a feasibility cohort study.

PURPOSE: Anterior vertebral body tethering (VBT) is a viable option for children with idiopathic scoliosis. Treating double major curves with bilateral VBT may allow patients to avoid spinal fusion while improving the coronal Cobb angle. METHODS: A single center retrospective study was conducted to identify all patients who underwent bilateral VBT (lowest instrumented vertebra L3 or 4) with minimum 2-year follow-up. Clinical and radiographic parameters were collected, including complications and reoperations. Statistical analysis was performed utilizing Students' t-test. RESULTS: Seventy-thr

Does screw alignment affect tether breakage in vertebral body tethering?

PURPOSE: Vertebral body tethering (VBT) is a non-fusion surgery for treating idiopathic scoliosis, with tether breakage as a common complication that occurs in 19–52% of patients. It has been reported in mechanical studies that screw malalignment > 1 mm may cause tether breakage. This is a clinical study aiming to assess the effect of screw alignment on tether breakage. METHODS: This was a retrospective multicenter cohort study of 115 patients with idiopathic scoliosis who underwent VBT. Clinical data and radiographic parameters such as Cobb angle, screw malalignment, and inter-screw angle wer

Coronal decompensation following thoracic vertebral body tethering in idiopathic scoliosis.

PURPOSE: Post-operative coronal decompensation (CD) continues to be a challenge in the treatment of adolescent idiopathic scoliosis (AIS). CD following selective spinal fusion has been studied. However, there is currently little information regarding CD following Vertebral Body Tethering (VBT). Thus, the goal of this study is to better understand the incidence and risk factors for CD after VBT. METHODS: Retrospective review of a prospective multicenter database was used for analysis. Inclusion criteria were patients undergoing thoracic VBT, a minimum 2-year follow-up, LIV was L1 or above, ske

Biomechanical modeling and assessment of lumbar vertebral body tethering configurations.

PURPOSE: Vertebral body tethering (VBT) is a fusionless spinal growth modulation technique, which shows promise for pediatric idiopathic scoliosis (IS) curve correction. This technique, mainly used for thoracic curves, is increasingly being used to treat lumbar curves in order to preserve spine flexibility. It remains necessary to adequately define the cord tension to be applied during the operation and the instrumented levels to biomechanically predict correction over time for the lumbar spine. METHODS: Twelve pediatric patients with lumbar IS, treated with lumbar-only or lumbar and thoracic

Radiographic outcome after vertebral body tethering of the lumbar spine.

INTRODUCTION: Multiple studies have analyzed the outcome after thoracic Vertebral Body Tethering (VBT). The results seem reproducible with most studies reporting coronal correction rates around 50% and a tether breakage rate near 20% at two years follow-up. There is a paucity of data on lumbar VBT, and no study has yet analyzed the radiographic outcome after lumbar VBT in a double tether technique at two years follow-up, which was the aim of this study. METHODS: This is a retrospective, single surgeons' data analysis of all consecutive immature patients who have had VBT of the lumbar spine (t

Evidence-based Indications for Vertebral Body Tethering in Spine Deformity.

Posterior spinal fusion has long been established as an effective treatment for the surgical management of spine deformity. However, interest in nonfusion options continues to grow. Vertebral body tethering is a nonfusion alternative that allows for the preservation of growth and flexibility of the spine. The purpose of this investigation is to provide a practical and relevant review of the literature on the current evidence-based indications for vertebral body tethering. Early results and short-term outcomes show promise for the first generation of this technology. At this time, patients shou

Vertebral Body Avascular Osteonecrosis: An Unusual Late Complication of Tracheostomy.

Vertebral body erosion is a rare late complication of tracheostomy. Here we present the case of a 30-year-old female patient, in a vegetative state after severe brain injury, with a permanent tracheostomy and prolonged mechanical ventilation, who suffered from recurrent episodes of severe air leakage with oxygenation drop due to a puncture in the tracheostomy tube cuff. A neck computed tomography (CT) detected destruction of two vertebral bodies, C7 and Th1, and a bony fragment - a remnant of C7 penetrated the trachea and probably repetitively punctured the inflated cuff. A biopsy of the C7 ve
