[The potentials and limits of the radiotherapy of laryngeal carcinoma].

Early stage glottic carcinoma can be treated with primary radiotherapy reaching equally good results compared to primary surgery; for supraglottic carcinoma surgery offers only slight advantages over primary radiotherapy. Tumor control for recurrences is considerably improved by salvage surgery. The use of primary radiotherapy allows improved or at least preserved voice and reduced rates of acute complications and late treatment sequelae. However, treatment duration is longer and probably more expensive compared to function preservation microlaryngoscopic surgery. Which treatment approach is c

Operative treatment of severe scoliosis and pelvic obliquity in patients with spinal muscular atrophy: assessment of outcomes and complications.

BACKGROUND: Few reports exist that focus on patients with spinal muscular atrophy (SMA) and severe spinal deformity. In this study, we aimed to report surgical outcomes and complications for SMA patients with severe scoliosis and pelvic obliquity. METHODS: A retrospective review of data on operatively treated SMA patients with severe scoliosis and pelvic obliquity (minimum major coronal curve Cobb angle > 100° and pelvic obliquity > 20°) was performed. Radiography findings, pulmonary function, motor status, the sitting function score, and perioperative and postoperative complications were the

Upper respiratory tract carcinoma with chromosomal translocation 15;19: evidence for a distinct disease entity of young patients with a rapidly fatal course.

BACKGROUND: Carcinoma of the upper respiratory tract is rare in childhood, and cytogenetic aberrations have not been characterized in this population. The chromosomal translocation 15;19 has been reported four times previously. All patients were young and had tumors arising in the thorax. The three reports that provide clinical follow-up all describe superior vena cava syndrome and death soon after presentation. All tumors were diagnosed as carcinoma (three undifferentiated, one mucoepidermoid), and the authors suggested thymus, lung, or germ cell origin. METHODS: The authors investigated the

Influence of chemoreceptor stimuli on genioglossal response to negative pressure in humans.

Genioglossal muscle (GG) activity is modulated by both chemoreceptive and mechanoreceptive reflexes that help stabilize airway patency. We assessed the effects of blood gas changes, within the range encountered during mild obstructive apnea-arousal cycles, on GG activity and the GG reflex to upper airway negative pressure. Eighteen healthy adults were studied while awake under 5 conditions: (1) baseline (PET(CO(2)) = 40 mm Hg, Sa(O(2)) = 99%); (2) hypercapnia (PET(CO(2)) = 45 mm Hg); (3) hypocapnia (PET(CO(2)) = 35 mm Hg, induced via hyperventilation with an iron lung ventilator); (4) hypoxia

A Sedation Protocol for Endoscopic Baton-Plate Adjustment in Neonates With Pierre Robin Sequence.

BACKGROUND: Robin sequence (RS) is characterized by upper airway obstruction leading to life-threatening desaturations in neonates. Procedural sedation in this population presents unique challenges due to anatomical airway anomalies, young patient age, and associated comorbidities. The pre-epiglottic baton-plate (PEBP) procedure is a non-invasive, endoscopically guided intervention that alleviates obstruction, facilitates oral feeding, and promotes mandibular growth. Sedation for PEBP adjustment requires the maintenance of spontaneous respiration while ensuring patient comfort and procedural t

Mandibular Advancement for Obstructive Sleep Apnea: Efficacy and the Trade-off of Expiratory Velopharyngeal Obstruction During Sleep Endoscopy - An Exploratory Observational Study.

PURPOSE: Mandibular advancement devices (MADs) are effective alternatives for obstructive sleep apnea (OSA), yet robust predictors of response are lacking, and the phenomenon of device-induced expiratory velopharyngeal obstruction (EVO) remains overlooked. This study conducted an exploratory clinical simulation to compare the efficacy and airway dynamics of a custom-fitted, maximally protrusive advanced MAD simulator versus a conventional edge-to-edge oral appliance (OA) during drug-induced sleep endoscopy (DISE). PATIENTS AND METHODS: In this prospective observational study, 75 adults with O

Effect of Novel Hypoglossal Stimulation on Airflow and Airway Collapsibility in OSA.

BACKGROUND: Hypoglossal nerve stimulation (HNS) to treat OSA currently requires placement of a cuff or "saddle" electrode around or adjacent to the hypoglossal nerve or nerves. Limitations for this therapy include cost, invasiveness, and variable efficacy. RESEARCH QUESTION: Can HNS applied via percutaneous implantation of a linear, multipair electrode array restore airflow to airway narrowing, obstruction, or both and improve airway collapsibility in people with OSA? STUDY DESIGN AND METHODS: Participants with OSA undergoing drug-induced sleep endoscopy with propofol were instrumented with

Anatomical Cadaver Study of Endolaryngeal Vascularization: Focus on the Glottis, Supraglottis, and Subglottis From the Transoral Microsurgical Point of View.

INTRODUCTION: Carbon dioxide laser coagulation during transoral laser microsurgery (TLM) for laryngeal cancer allows control of bleeding from vessels smaller than 0.5 mm. Therefore, larger arteries and veins must be carefully managed by clipping and/or monopolar cautery. The aim of this paper is to detail endolaryngeal vascular anatomy and identify areas of possible bleeding during TLM. METHODS: We performed an anatomical study on a series of 11 fresh-frozen human cadavers. After injection of a bicomponent red silicone into the innominate, left common carotid, and left subclavian arteries, 22

Three-Dimensional Map of Isoprognostic Zones in Glottic Cancer Treated by Transoral Laser Microsurgery as a Unimodal Treatment Strategy.

INTRODUCTION: The Union for International Cancer Control-American Joint Committee on Cancer TNM staging system for glottic squamous cell carcinoma (SCC) includes different types of lesions defined by the involvement of specific subsites in each T category. Our study aims to identify different subcategories according to tumor local extension and determine oncologic outcomes after treatment by transoral laser microsurgery (TLM) alone. METHODS: We retrospectively evaluated 410 patients affected by previously untreated pT1-pT3 glottic SCC treated by TLM alone from January 2005 to December 2015 at

Decreased expression of the NKG2D ligand ULBP4 may be an indicator of poor prognosis in patients with nasopharyngeal carcinoma.

INTRODUCTION: Tiotropium, a long-acting muscarinic antagonist, is commonly employed for the maintenance treatment of chronic obstructive pulmonary disease (COPD) and asthma. While its efficacy has been validated through numerous randomized controlled trials, safety concerns in real-world post-marketing settings necessitate further evaluation. AIM: This study aimed to analyze the adverse events (AEs) associated with tiotropium reported in the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) database to identify potential safety signals. METHODS: A retrospective analysi

Objective assessment of supraglottoplasty outcomes using polysomnography.

OBJECTIVE: Supraglottoplasty is well documented as an effective procedure in the management of moderate to severe laryngomalacia. Traditionally assessed outcomes in the treatment of laryngomalacia include: reduced stridor, improved feeding and a resumption of weight-gain. Previous authors have documented the use of polysomnography in assessing the severity of paediatric laryngeal abnormalities. The aim of the current study was to objectively assess the efficacy of supraglottoplasty for laryngomalacia by comparing data from pre- and post-operative polysomnography. METHODS: A retrospective revi
