Histologic and biomechanical comparison of fascia lata autograft, acellular dermal xenograft, and synthetic patch for bridging massive rotator cuff tear in a rabbit model.

BACKGROUND: Bridging repair has emerged as a promising and reliable treatment strategy for the massive rotator cuff tears (MRCTs). However, there remains a lack of evidence on which bridging graft provides the better repair results, and a dearth of animal studies comparing bridging repairs with different grafts. The purpose of this study was to evaluate the histological and biomechanical outcomes of commonly used grafts (autologous fascia lata (FL), acellular dermal matrix graft (ADM), and polyethylene terephthalate (PET) patch). METHODS: A total of 66 male New Zealand White Rabbits were used

A human retinal microvascular endothelial-pericyte co-culture model to study diabetic retinopathy in vitro.

This human primary co-culture model using human retinal microvascular endothelial cells (hREC) and human retinal pericyte cells (hRP) aims to improve current understanding of the cellular changes occurring in the retinal microvasculature during diabetic retinopathy (DR). Currently, patients often present in clinic with late-stage DR, only when vision becomes impaired. Therefore, new strategies for earlier detection in clinic, combined with novel pharmaceutical and cellular interventions are essential in order to slow or halt the progression of DR from background to sight-threatening stage. Case Report: En Bloc Resection of Pancoast Tumor with Adjuvant Aortic Endograft and Chemoradiation.

"Pancoast" tumors frequently require a multidisciplinary approach to therapy and are still associated with high morbidity and mortality. Due to their sensitive anatomic location, complex resections and chemoradiation regimens are typically required for treatment. Those with signs of aortic invasion pose an even greater challenge, given the added risks of cardiopulmonary bypass for aortic resection and interposition. Placement of an aortic endograft can facilitate resection if the tumor is in close proximity to or is invading the aorta. Prophylactic endografting to prevent radiation-associated

Bacterial translocation as a source of Dacron-graft contamination in experimental aortic operation: the importance of controlling SIRS.

BACKGROUND: Several experimental studies have shown the beneficial effects of nitric oxide (NO) in the modulation of the systemic inflammatory response syndrome (SIRS). Nitric oxide is involved in and affects almost all stages in the development of inflammation. We have attempted to ascertain whether the nitric oxide donor molsidomine prevents aortic graft contamination through control of the SIRS and a decrease in bacterial translocation (BT). METHODS: Twenty-four mini-pigs were divided into 4 groups. The animals were subjected to suprarenal aortic/iliac cross-clamping (for 30 minutes) and b

Proof of fallout endothelialization of impervious Dacron grafts in the aorta and inferior vena cava of the dog.

PURPOSE: The purpose of this study was to prove directly whether cells from the blood stream contribute to endothelialization of isolated, impervious Dacron vascular grafts in the dog. METHODS: We designed an 18 cm, three-component graft with two parallel central Dacron limbs; one was made impervious with silicone rubber, and the other was preclotted. This model was implanted in the canine descending thoracic aorta with 30 microns polytetrafluoroethylene grafts anastomosed at each end. An 8 cm, three-component graft completely coated with silicone rubber was implanted in the canine abdominal

Superior late patency of small-diameter Dacron grafts seeded with omental microvascular cells: an experimental study.

The purpose of the study was to investigate the effect of omental microvascular cell seeding on the patency of small-diameter Dacron prostheses usable for coronary artery bypass grafting. In a canine carotid artery model, each dog (n = 64) received one seeded and one similar nonseeded Dacron prosthesis (internal diameter = 4 or 6 mm). Enzymatically harvested omental microvascular cells (omentum = 27.6 +/- 5.9 g [+/- the standard deviation]; range, 17 to 50 g) were seeded prior to implantation. The seeding density was 1.91 +/- 0.26 [+/- the standard error] x 10(6) cells/cm2 of graft surface. Di

The unidirectional valve patch provides no benefits to early and long-term survival in patients with ventricular septal defect and severe pulmonary artery hypertension.

OBJECTIVE: Our aim was to test whether a unidirectional valve patch would provide benefit to early and long-term survival for patients with ventricular septal defect and severe pulmonary artery hypertension. METHODS: Eight hundred seventy-six cases of ventricular septal defect with severe pulmonary artery hypertension were closed with or without a unidirectional valve patch and were classified as the unidirectional valve patch (UVP) group (n = 195) and nonvalve patch (NVP) group (n = 681), respectively. Propensity scores of inclusion into the UVP group were used to match 138 pairs between the

Relationship between the diameter of the vein graft and postoperative ankle brachial pressure index following femoro-popliteal bypass.

AIM: We studied whether the diameter of the saphenous vein graft affects the result of femoro-popliteal bypass surgery. METHODS: Thirty-eight patients with bypasses from the femoral artery to the above knee popliteal artery were studied. Bypasses without a patent anterior or posterior tibial artery were excluded. The great saphenous vein was used as a bypass graft in 20 extremities and Dacron grafts (6 mm or 8 mm diameter) were used in 18 arteries. The smallest diameter of the saphenous vein was measured preoperatively with ultrasonography. Vein grafts were divided into two groups: small vein

Truncus arteriosus with interrupted aortic arch: successful repair using modified cardiopulmonary bypass and surgical techniques.

A neonate with truncus arteriosus (type 1) and interrupted aortic arch (type A) associated with an excessively large gap between interrupted aortic segments underwent successful reconstruction of the aortic arch with a partial subclavian flap aortoplasty plus bovine jugular patch (Contegra 200, Medtronic, Inc, Minneapolis, MN) using continuous selective low-flow cardiopulmonary bypass without circulatory arrest. The ventricular septal defect was closed with a Dacron (IMPRA, Inc, Tempe, AZ) patch, and a Contegra prosthesis was used to re-establish right ventricle to pulmonary artery continuity.

[Aorto-duodenal fistula--direct suture and pedicled omentum flap-plasty].

Between 1985 and 1993, six patients underwent emergency operation at the Department of General Surgery, Essen University Hospitals, for a secondary aorto-duodenal fistula. In all patients, a Dacron tube- or bifurcation prosthesis has been implanted 1-10 years previously to repair an arteriosclerotic aneurysm of the abdominal aorta. The main symptom of the aorto-duodenal fistula was massive gastrointestinal hemorrhage, with manifest shock in two cases. The most reliable diagnostic procedure, in addition to ultrasonography, was found to be computed angio-tomography. In three cases where there wa

Construction of NO-releasing coating on PET via layer-by-layer self-assembly.

Nitric oxide (NO) releasing coating is promising for implantable medical devices due to the role of NO in the vascular system. In the study, a multilayered NO-releasing coating was constructed via layer-by-layer (LBL) self-assembly to improve the hemocompatibility of poly(ethylene terephthalate)(PET). A keratin-based NO donor (KSNO) was synthesized and used as a negatively-charged monolayer. Polyethyleneimine (PEI) acted as a positively-charged monolayer. The LBL coating has an adjusted KSNO payload capacity, achieving a tunable NO release in the presence of ascorbic acid (Asc). The coated PET

Life Saver: Aortic Wrapping.

Reoperative aortic root reconstruction remains a formidable surgical challenge. Thereof, the wrapping of Dacron graft using remnants of native aorta may be reasonable to avoid complications such as bleeding and infection. Our case that had a modified Bentall procedure at our institute was reoperated because of discontinuity between left ventricular outflow and valved aortic conduit. During reoperation, proximal portion of the valved conduit was partially separated from its attachment in the left ventricular outflow tract. The residual native aorta that was wrapped around the Dacron graft was l
