Post-Dialysis Syndrome: A Narrative Review.

Use of the term post-dialysis syndrome (PDS) was recently recommended to refer to the exacerbation of a debilitating, under-recognized cluster of physical, cognitive, psychological, and behavioral symptoms commonly affecting patients on maintenance dialysis after treatment sessions. This narrative review synthesizes existing and emerging literature to provide an overview of the characterization, pathophysiology, measurement, and management of PDS. Commonly referred to as post-dialysis fatigue, the term PDS more accurately captures the multidimensional nature and true burden of the condition an

Emotional dynamics during glucocorticoid treatment in chronic cluster headache - Insights from dynamic time warp analysis.

BACKGROUND: Cluster headache can affect mental health due to the severe attacks, with high rates of depression and anxiety. When prophylactic treatment fails, high dose prednisone can be used as bridging therapy, but may cause neuropsychological side effects. This prospective, exploratory, observational pilot study, aimed to assess the temporal associations between emotions during glucocorticoid treatment in people with cluster headache. METHODS: During prednisone treatment (67 days), ten participants with chronic cluster headache completed daily questionnaires consisting of ecological moment

A study design for modulating two components of the risk architecture of chronic pain: adverse life events and pain catastrophising.

BACKGROUND: Traumatic experiences and dysfunctional emotional responses to pain modulate pain processing via partly different neural circuits. Modulation of pain experience by brain mechanisms is partly conveyed via descending pathways through the midbrain and brainstem. We designed an intervention study using Eye Movement Desensitization and Reprocessing (EMDR) to desensitize (a) distressing memories of adverse life events (ALE), or (b) pain-related anxiety and emotion dysregulation such as pain catastrophising (PCAT). Our primary objective is to examine the extent to which EMDR efficacy on p

Fluctuations of dissociation and inner tension during inpatient dialectical behavior therapy: associations with self-injury and suicidal ideation.

BACKGROUND: In Dialectical Behavior Therapy (DBT), monitoring inner tension and state dissociation is considered helpful for understanding a patient’s affective and behavioral responses, especially regarding self-harming behaviors. This study aims to investigate how potentially aversive internal states including dissociation, inner tension and fluctuations of tension, i.e. affective instability, during the initial phase of inpatient DBT relate to self-injury, suicidal ideation, and symptom reduction. METHODS: 41 patients with personality disorder (borderline or combined), undergoing an 8-week

Temporal dynamics of daily diabetes-specific burdens using intensive longitudinal data: the central role of feelings of guilt within a multilevel network analysis.

AIMS/HYPOTHESIS: The aims were to: (1) analyse the day-to-day mechanisms through which daily diabetes-specific negative emotional experiences (burdens) accumulate and potentially affect emotional well-being; and (2) identify the specific triggers of these dynamics in people with type 1 and type 2 diabetes. METHODS: Data were derived from three prospective observational studies employing ecological momentary assessment for up to 17 days. Multilevel vector autoregression models were used to estimate within-person temporal networks of nine daily diabetes-specific burdens. Out-strength and in-str

Associations Between Social Determinants of Health, Mental Effort, and Frustration for Voice Therapy.

OBJECTIVE: Early life adversity and related social determinants of health (SDOHs) are risk factors for trauma, learning and regulation difficulties, and poor adult health outcomes, such as functional voice disorders (FVDs). It is unknown if learning new voice techniques is confounded by a history of adverse experiences. The purpose of this study was to investigate the relationship between SDOHs and cognitive effort in voice therapy. METHODS: Patients referred for voice therapy at the Emory Voice Center in Atlanta, GA were recruited to participate in a non-experimental prospective study using

A mechanism-based group psychotherapy approach to aggressive behavior (MAAP) in borderline personality disorder: a multicenter randomized controlled clinical trial.

BACKGROUND: High levels of trait anger and aggressive behavior are common and problematic phenomena in patients with borderline personality disorder (BPD). In BPD, patterns of reactive aggression often lead to functional impairment affecting important areas of life. Despite the high burden on individuals and their social environment, there are no specific, cost-effective treatments to reduce aggression in BPD. In previous studies, we and others have been able to infer specific biobehavioral mechanisms underlying patterns of reactive aggression in BPD that can be used as potential treatment tar

Heart Rate Variability Biofeedback for Substance Use Disorder: A Randomized Clinical Trial.

IMPORTANCE: Preliminary studies suggest heart rate variability biofeedback (HRVB) may reduce craving and negative affect in individuals with substance use disorder (SUD), but few studies have evaluated whether this translates into improved substance use outcomes, and no prior studies have examined second-generation wearable HRVB technology in this context. OBJECTIVE: To evaluate the effects of second-generation HRVB on negative affect, positive affect, craving, and alcohol and other drug (AOD) use in adults with SUD. DESIGN, SETTING, AND PARTICIPANTS: This phase 2 randomized clinical trial i

An Ecological Examination of Indica Versus Sativa and Primary Terpenes on the Subjective Effects of Smoked Cannabis: A Preliminary Investigation.

Background: The legal cannabis landscape has greatly outpaced scientific knowledge. Many popular cannabis claims, such as cultivar (colloquially referred to as strain) classification and terpene content producing different subjective effects, are unsubstantiated. This study examined, for the first time, whether cultivar classification (sativa/indica) and terpene content (caryophyllene, limonene, myrcene, pinene, and terpinolene) were associated with subjective cannabis effects (i.e., pain levels, low-arousal ["indica-like"] effects, high-arousal ["sativa-like"] effects, and negative effects).

Internet-based treatment of gambling problems: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND AND AIMS: The effect of internet-based psychological treatment for gambling problems has not been previously investigated by meta-analysis. The present study is therefore a quantitative synthesis of studies on the effects of internet-based treatment for gambling problems. Given that effects may vary according to the presence of therapist support and control conditions, it was presumed that subgroup analyses would elucidate such effects. METHODS: A systematic search with no time constraints was conducted in PsycINFO, MEDLINE, Web of Science, and the Cochrane Library. Two authors ind

[Network model of mental disorders: Application and interest in post-stroke depression].

In contrast to the classic models in psychopathology, the network model considers that the temporal interactions between symptoms are the causes of their occurrence. This model could also be particularly suitable for understanding the processes involved in post-stroke depression. The aim of this paper is to perform a network analysis in order to describe the temporal dynamic of the links existing between depression symptoms during the acute phase after stroke. Twenty-five patients (64% male, mean age 58.1±14.9 years old) hospitalized for a minor stroke (no neurocognitive or motor impairment) w

Prospective cohort study of early biosignatures of response to lithium in bipolar-I-disorders: overview of the H2020-funded R-LiNK initiative.

BACKGROUND: Lithium is recommended as a first line treatment for bipolar disorders. However, only 30% of patients show an optimal outcome and variability in lithium response and tolerability is poorly understood. It remains difficult for clinicians to reliably predict which patients will benefit without recourse to a lengthy treatment trial. Greater precision in the early identification of individuals who are likely to respond to lithium is a significant unmet clinical need. STRUCTURE: The H2020-funded Response to Lithium Network (R-LiNK; http://www.r-link.eu.com/ ) will undertake a prospecti

Smartphones for Real-time Assessment of Adherence Behavior and Symptom Exacerbation for High-Risk Youth with Asthma: Pilot Study.

BACKGROUND: Youth with asthma who have poor medication adherence, have limited access to care, and are frequently seen in the acute care setting are often termed "high risk." OBJECTIVE: This study aimed to design and test the feasibility of using smartphone technology to assess contextual factors that may impact changes in daily medication adherence and to identify new symptom episodes among high-risk youth with asthma in their home environment. METHODS: Youth aged 8-17 years with high-risk asthma from 2 children's hospitals were eligible for the 2-month study. An app was downloaded on parti

Caudate dopamine predicts cognitive effort decisions both in the lab and daily life.

UNLABELLED: Making choices about whether and when to engage cognitive effort is a common feature of human everyday experience with important consequences for academic, career, and health outcomes. Yet, despite its importance, little is understood about the neurochemical mechanisms that underlie individual differences in the willingness to engage in cognitively effortful activities. To examine this question, we employed a multi-modal neuroimaging approach in which participants performed the Cognitive Effort Discounting paradigm (Cog-ED) while simultaneous PET-fMRI was acquired. We assessed both

Febrile Seizures in an App-Based Children's Fever Registry: Mixed Methods Study.

BACKGROUND: Febrile seizures, although typically benign, can cause significant emotional distress for parents. Their diverse etiological risk factors underscore the need for further research. Ecological momentary assessment (EMA) offers a cost-effective and timely method for real-time data collection. The FeverApp, an EMA-based registry for fever management, enables parents to document febrile seizures as they occur. OBJECTIVE: This study systematically investigates febrile seizure records from the FeverApp registry to assess their characteristics and explore the clinical implications of the

Testing a Smartphone-Based Intervention Targeting Anxiety Sensitivity Among Women Presenting for Emergency Care After Sexual Assault: Pilot Randomized Controlled Trial.

BACKGROUND: Anxiety sensitivity (AS), defined as the fear of anxious arousal, is a promising therapeutic target for reducing posttraumatic stress disorder (PTSD) symptom development after trauma exposure. Initial research suggests that smartphone-based AS interventions may be acceptable to sexual assault survivors at risk for PTSD symptoms and effective for symptom reduction, but only small one-arm proof-of-concept studies have been conducted. OBJECTIVE: The goal of this study was to extend prior proof-of-concept work by conducting a pilot randomized controlled trial. The aims were to evaluat

Testing an integrated model of population mental health: study protocol for the epidemiological, two-arm Mannheim incidence and inception cohort study (MAKO).

INTRODUCTION: Monitoring of population mental health is now considered to play a vital role in health, risk factor and demographic surveillance systems across the globe. Adopting a public mental health perspective on monitoring and surveillance requires us to consider the entire spectrum of mental health, which broadly ranges from positive mental health and well-being to severe mental disorders, including at-risk populations. This population mental health approach has become the mainstay of public mental health provision in many European countries but there is a dearth of research on this issu

Teens and opioids postsurgery (TOPS): protocol for a prospective observational study describing associations between sleep deficiency and opioid use following outpatient surgery in adolescents.

INTRODUCTION: The opioid crisis is a significant burden on adolescent public health in the USA. Medical use of prescription opioids is a pathway via which adolescents transition to opioid misuse, opioid use disorder and overdose. More than half of all opioids prescribed to adolescents are for pain management following surgery. Yet, little is known about the critical period following surgery during which adolescents initiate opioid misuse or the modifiable mechanisms (such as sleep deficiency) contributing to this process. This prospective observational study will broaden our knowledge by exami

Acceptability of a personalised single-session feedback intervention for eating disorders: a qualitative content analysis.

BACKGROUND: Eating disorders (EDs) are prevalent, serious conditions that often require specialised care. Barriers to treatment are numerous and include cost, waitlists, geographic limitations, and lack of services for earlier-stage illness. Single-session interventions (SSIs) hold promise as accessible, cost-effective approaches to support individuals with EDs who may otherwise be unable to access care. However, to ensure uptake and use in real-world contexts, SSIs must be acceptable to their target population. This qualitative study investigated the anticipated acceptability of an SSI for ED

Would a comprehensive hearing aid fitting process lead to placebo effects compared to a simple process?

OBJECTIVES: Placebo effects refer to the impact of a treatment on health outcomes that cannot be attributed to the treatment itself. The current study aimed to investigate whether a comprehensive hearing aid fitting process would induce placebo effects compared to a simple process, and whether personal attributes such as personality traits could predict susceptibility to these effects. DESIGN: Thirty adults with hearing loss completed the study. The study began with a fitting session in which the field trial hearing aid configuration (the actual fitting) was set, followed by two experimental
