The Macarthur GP After-hours Service: a model of after-hours care for Australia.

INTRODUCTION: The Macarthur GP After-hours Service (MGPAS) was established to streamline the provision of after-hours medical care in an outer-urban community. This paper reports on a process evaluation of the MGPAS. METHODS: A mixed methods approach involving surveys, stakeholder interviews and analysis of administrative data was used. RESULTS AND DISCUSSION: This model of care was well accepted and regarded by general practitioners, Macarthur Health Service staff and the community. The MGPAS was found to be an acceptable and efficient model of after-hours medical care. Areas that required

Neuroprotective Care of Extremely Preterm Infants in the First 72 Hours After Birth.

Birth at extremely low gestational ages presents a significant threat to infants' survival, health, development, and future well-being. After birth, a critical period of brain development must continue outside the womb. Neuro-supportive and neuroprotective family centered developmental care for and standardized care practices for extremely preterm infants have been shown to improve outcomes. Neuroprotective interventions must include a focus on the emotional connections of infants and their families. Being in skin-to-skin contact with the mother is the developmentally expected environment for

Typology of after-hours care instructions for patients: telephone survey and multivariate analysis.

OBJECTIVE: To develop a typology of after-hours care (AHC) instructions and to examine physician and practice characteristics associated with each type of instruction. DESIGN: Cross-sectional telephone survey. Physicians' offices were called during evenings and weekends to listen to their messages regarding AHC. All messages were categorized. Thematic analysis of a subset of messages was conducted to develop a typology of AHC instructions. Logistic regression analysis was used to identify associations between physician and practice characteristics and the instructions left for patients. SETT

After-hours presentations to community-based primary care in the Australian Capital Territory.

This study describes patient characteristics and presentations to community-based primary care medical services in the after-hours period. A cross-sectional survey completed by practitioners was voluntarily conducted over one weeknight and weekend in 2019 at 31 of 51 extended hours general practices (GP), three of three medical deputising services and three of three nurse-led walk-in clinics (WICs). Of 3371 community-based after-hours presentations, 934 consultation records were obtained (overall response: 28%). Emergency departments reported 524 triage code 4 and 5 patients within the study p

Development and implementation of the TrAC (Tracking After-hours Calls) database: a tool to collect longitudinal data on after-hours telephone calls in long-term care.

INTRODUCTION: Effective telephone communication between long-term care (LTC) nurses and physicians is an integral part of high-quality care, yet little is known about this key aspect of LTC practice. The authors describe the development and implementation of the TrAC (Tracking After-hours Calls) system, an electronic database designed to collect longitudinal data on after-hours telephone calls from LTC facilities. DEVELOPMENT: A relational database was developed to systematically record key characteristics of calls received from LTC facilities, including the date, time, location and reason fo

The Development and Performance of After-Hours Primary Care in the Netherlands: A Narrative Review.

In many Western countries, hospital emergency departments are overcrowded, leading to the desire to strengthen primary care, particularly after hours. To achieve this goal, an increasing number of Western nations are reorganizing their after-hours primary care systems into large-scale primary care physician (PCP) cooperatives. This article provides an overview of the organization, performance, and development of PCP cooperatives in the Netherlands. The Dutch after-hours primary care system might offer opportunities for other countries facing problems with after-hours care and inappropriate eme

Follow-up care by patient's own general practitioner after contact with out-of-hours care. A descriptive study.

BACKGROUND: Little is known about the care process after patients have contacted a GP cooperative for out-of-hours care. The objective of this study was to determine the proportion of patients who seek follow-up care after contact with a GP cooperative for out-of-hours care, and to gain insight into factors that are related to this follow-up care. METHODS: A total of 2805 patients who contacted a GP cooperative for out-of-hours care were sent a questionnaire. They were asked whether they had attended their own GP within a week after their contact with the cooperative, and for what reason. To

Are You PREPAREd? A Structured Approach to Support Safe and Confident Transitions to After-Hours Care.

BACKGROUND AND NEED FOR INNOVATION: Early after-hours work represents a demanding transition for newly graduated doctors. Preparation is often fragmented, with limited attention to the integrated clinical, organisational, and supervisory demands of after-hours care, contributing to uncertainty that may affect both patient safety and professional development. GOAL OF INNOVATION: PREPARE (PREParing for After-hours caRE) is an eight-week programme designed to support new graduates during their early postgraduate transition by enabling safe and confident participation in after-hours care. STEPS

Successful Heart Transplant after Ten Hours Out-of-body Time using the TransMedics Organ Care System.

OBJECTIVE: We report the successful transplantation of a heart following an out-of-body time of 611 minutes into a recipient with dilated cardiomyopathy and left ventricular assist device implant. PATIENTS: Our patient was urgently waiting for a cardiac transplant whilst receiving LVAD support. Recurrent VF and repeated AICD shocks necessitated this action. RESULTS: Although requiring ECMO and inotropic support in the first 17 hours post-transplant, the patient was discharged from hospital on day 15 post-transplant with normal cardiac function. CONCLUSION: We report some of the salient poin

Recurarization in the Post-anesthesia Care Unit One Hour After Sugammadex Administration: A Case Report.

Postoperative recurarization has been a well-documented phenomenon since the advent of neuromuscular blockade drugs in clinical practice. Here, we present a case of recurarization after the use of rocuronium bromide in the post-anesthesia care unit (PACU), despite sugammadex administration and neuromuscular monitoring. A 68-year-old man with known psychiatric and obstructive pulmonary pathology underwent emergency surgery for abdominal evisceration correction. General anesthesia was induced with fentanyl, propofol, and rocuronium. At the end of the procedure, neuromuscular blockade was reverse

PO-42 - A novel approach to a common problem: after-hours care of cancer patients diagnosed with VTE.

INTRODUCTION: Oncologists often receive phone calls from radiologists after regular working hours or while on call, informing them that a cancer patient has been diagnosed with a blood clot. In these situations, there may not be nursing staff available to contact the patient and provide teaching for Low Molecular Weight Heparin (LMWH) injections. As a result, patients are often sent to the emergency for injection and teaching, taxing an already overburdened emergency department. This problem constitutes an important care gap. AIM: In Alberta, Canada, pharmacists are able to prescribe medicati

Emergency department tachypnea predicts transfer to a higher level of care in the first 24 hours after ED admission.

OBJECTIVES: The authors hypothesized that vital sign abnormalities detected in the emergency department (ED) can be used to forecast clinical deterioration occurring within 24 hours of hospital admission. METHODS: This was a retrospective case-control study performed after implementation of a hospitalwide rapid response team (RRT) system. Inclusion criteria for study patients consisted of age > or = 18 years, admission to the general floor though the ED, and RRT activation and subsequent transfer to a higher level of care in the first 24 hours. Control patients were > or =18 years, were admit

An After-Hours Virtual Care Service for Children With Medical Complexity and New Medical Technology: Mixed Methods Feasibility Study.

BACKGROUND: Family caregivers (FCs) of children with medical complexity require specialized support to promote the safe management of new medical technologies (eg, gastrostomy tubes) during hospital-to-home transitions. With limited after-hours services available to families in home and community care, medical device complications that arise often lead to increased FC stress and unplanned emergency department (ED) visits. To improve FC experiences, enable safer patient discharge, and reduce after-hours ED visits, this study explores the feasibility of piloting a 24/7 virtual care service (Conn

Paediatric Presentations to an Emergency Department Providing Urgent and After-Hours Outpatient Care in Cambodia: A 10-Year Retrospective Review.

BACKGROUND: In Cambodia, emergency rooms (ERs) often serve as the primary access point for paediatric care, reflecting limited outpatient capacity and health system constraints. METHODS: A retrospective descriptive analysis of presentations at Angkor Hospital for Children between 2012 and 2021 was conducted. Diagnoses were harmonised using ICD-10 definitions. The primary cohort included all eligible presentations, while a secondary cohort excluded patients aged outside the hospital age range, procedural-only visits, unclassifiable diagnoses, and referrals. Analyses focused on the 10 most comm

No Variation in Patient Care Outcomes After Implementation of Resident Shift Work Duty Hour Limitations and a Hospitalist Model System.

This study examines whether implementing a resident shift work schedule (RSWS) alone or combined with a hospitalist-led model system (HMS/RSWS) affects patient care outcomes or costs at a pediatric tertiary care teaching hospital. A retrospective sample compared pre- and postintervention groups for the most common primary discharge diagnoses, including asthma and cellulitis (RSWS intervention) and inflammatory bowel disease and diabetic ketoacidosis (HMS/RSWS intervention). Outcome variables included length of stay, number of subspecialty consultations, and hospitalization charges. For the RSW
