Antifibrotic agents for liver disease.

Complications from chronic hepatitis C (HCV) and recurrent HCV post-transplant are responsible for significant morbidity and mortality in the United States and Europe. Current antiviral therapies are at best, effective in up to 50% of patients in the pre-transplant setting, and in the post-transplant setting are associated with more limited efficacy and increased toxicity. With this reduced efficacy of antiviral strategies in the post-transplant setting, new approaches are urgently needed. Substantial progress has been made in understanding the pathogenesis of hepatic fibrosis over the last 20

Liver fibrosis: Direct antifibrotic agents and targeted therapies.

Liver fibrosis and in particular cirrhosis are the major causes of morbidity and mortality of patients with chronic liver disease. Their prevention or reversal have become major endpoints in clinical trials with novel liver specific drugs. Remarkable progress has been made with therapies that efficiently address the cause of the underlying liver disease, as in chronic hepatitis B and C. Highly effective antiviral therapy can prevent progression or even induce reversal in the majority of patients, but such treatment remains elusive for the majority of liver patients with advanced alcoholic or n

Design and Synthesis of Betulinic Acid Derivatives as Antifibrotic Agents for Liver Fibrosis.

Hepatic fibrosis is a major contributor to liver-related mortality and overall mortality. Currently, there is no established treatment for hepatic fibrosis. Research indicates that the transforming growth factor (TGF-β) signaling pathway is one of the key pathways involved in hepatic stellate cell (HSC) activation and fibrosis progression. Consequently, inhibiting the TGF-β signaling pathway represents a potential therapeutic strategy to combat hepatic fibrosis progression. Betulinic acid (BA) was widely extractable from natural plants, exhibits significant antifibrotic therapeutic effects. Th

Interstitial Pneumonia Secondary to Hermansky-Pudlak Syndrome Type 4 Treated with Different Antifibrotic Agents.

Hermansky-Pudlak syndrome (HPS) is an autosomal recessive hereditary disease that may be complicated by progressive and potentially fatal interstitial pneumonia. We herein report a 64-year-old woman with interstitial pneumonia associated with HPS type 4 whom we treated with nintedanib after pirfenidone proved ineffective. To our knowledge, there have been no previous reports of nintedanib being used to treat a patient with HPS type 4. There is a need for clinical trials of antifibrotic agents, including nintedanib, pirfenidone, and new therapeutic agents with different mechanisms of action in

Pirfenidone as a Pleiotropic Antifibrotic Agent in Metabolic Steatohepatitis: From Mechanisms to Clinical Evidence.

Metabolic dysfunction-associated steatohepatitis (MASH) is a progressive phenotype of steatotic liver disease that can lead to advanced fibrosis, cirrhosis and liver-related complications. Pirfenidone is a small synthetic molecule that was originally approved for idiopathic pulmonary fibrosis. It has pleiotropic antifibrotic, anti-inflammatory, antioxidant, and immunomodulatory properties. In preclinical liver models, pirfenidone reduces hepatic stellate cell activation, collagen deposition, oxidative stress and proinflammatory cytokine expression, largely through the modulation of transformin

Antifibrotic Agents for Silicone Implant Capsular Contracture: A Study on Nintedanib and Pirfenidone in Rats.

BACKGROUND: Capsular contracture is a frequent complication after breast implantation, often requiring revision surgery with high recurrence. Fibrosis and chronic inflammation are central to its pathogenesis; thus, antifibrotic pharmacological modulation is a promising preventive strategy. This study evaluated the effects of pirfenidone and nintedanib on capsular contracture in a rat model. METHODS: Twenty-seven female Wistar albino rats were divided into control, pirfenidone, and nintedanib groups. Silicone block implants were placed under the panniculus carnosus. Pirfenidone (100 mg/kg/day)

Isosilybin B: a potential novel therapeutic agent with hepatoprotective, anticancer and antifibrotic properties.

BACKGROUND: Silybum marianum (milk thistle) is a plant for centuries well known for its hepatoprotective effects. The extract from seeds, silymarin, and its major compound, silibinin, are well studied for their hepatoprotective and antifibrotic effects. The role of other minor compounds, such as isosilybin B, remains underexplored. PURPOSE: This study aimed to compare the cytotoxic and antifibrotic properties of IB with those of silibinin and silymarin in vitro. It focuses on evaluating the cytotoxic effect of these substances on tumor and non-tumor liver cells. Moreover, antifibrotic potenti

Prediction and Demonstration of Retinoic Acid Receptor Agonist Ch55 as an Antifibrotic Agent in the Dermis.

The prevalence of fibrotic diseases and the lack of pharmacologic modalities to effectively treat them impart particular importance to the discovery of novel antifibrotic therapies. The repurposing of drugs with existing mechanisms of action and/or clinical data is a promising approach for the treatment of fibrotic diseases. One paradigm that pervades all fibrotic diseases is the pathological myofibroblast, a collagen-secreting, contractile mesenchymal cell that is responsible for the deposition of fibrotic tissue. In this study, we use a gene expression paradigm characteristic of activated my

The antifibrotic agent pirfenidone inhibits angiotensin II-induced cardiac hypertrophy in mice.

Pirfenidone (5-methyl-1-phenyl-2-[(1)H]-pyridone) is an effective drug for idiopathic interstitial pneumonia that can prevent and reverse tissue fibrosis in several organs. Therefore, we investigated whether pirfenidone has a potential role in preventing angiotensin II (Ang II)-induced cardiac hypertrophy. A cardiac hypertrophic mouse model was created using an Ang II infusion (200 ng kg(-1) min(-1)) in wild-type mice for 2 weeks. Mice were divided into the following three groups: a saline-infused (control) group, an Ang II infusion (vehicle) group and an Ang II infusion+pirfenidone-treated (P

The use of suramin, an antifibrotic agent, to improve muscle recovery after strain injury.

BACKGROUND: Muscle strain injuries are extremely common in sports medicine. Muscle healing often is hindered by scar tissue formation after injury. HYPOTHESIS: Suramin can prevent scar tissue formation and improve muscle healing after injury because of its ability to antagonize transforming growth factor-beta1, a fibrotic cytokine. STUDY DESIGN: Controlled laboratory study. MATERIALS AND METHODS: In vitro, muscle-derived fibroblasts (a potential cell source of muscle fibrosis) were incubated with suramin and/or transforming growth factor-beta1; a cell growth curve was obtained. Exploring novel A2AAR antagonists: Design, synthesis, and evaluation of 2,6,9-trisubstituted purine derivatives as promising antifibrotic agents.

A series of 2,6,9-trisubstituted purine derivatives were designed and synthesized with diverse chemical moieties. Through a comprehensive biological evaluation, we identified 4-(6-(methylamino)-2-(phenylethynyl)-9H-purin-9-yl)phenol (6a) as a promising A2AAR antagonist with potent antifibrotic properties. Compound 6a demonstrated significant efficacy in inhibiting CRE promoter activity and in reducing the expression of fibrogenic marker proteins and downstream effectors of A2AAR activation, surpassing the A2AAR antagonist ZM241385 and initial screening hits, 9-benzyl-N-methyl-2-(phenylethynyl)
