Antimicrobial resistance (AMR) remains one of the most significant threats to global health, largely driven by inappropriate antibiotic use. This review outlines the current principles and practices of antimicrobial stewardship (AMS) in the context of critical care. We address the global burden of AMR, the development of resistance and the implementation of AMS strategies from multidisciplinary, diagnostic and pharmacokinetic perspectives. The use of rapid diagnostics, biomarkers such as procalcitonin, and the integration of electronic health records are discussed as enablers of targeted thera

Due to the increase in antimicrobial resistance, strategies such as antimicrobial stewardship programs (ASP) have been developed to improve the clinical results, decrease the adverse effects and the development of resistances and ensure cost-effective therapies. Fosfomycin has a unique mechanism of action against Gram-positive and Gram-negative bacteria. Cross-resistance is uncommon; however, fosfomycin should be used in combination in severe infections to avoid selecting resistant mutations. Fosfomycin's oral formulation facilitates sequential treatment, has low toxicity and high tissue penet

Colonization with multidrug-resistant organisms (MDROs) is a risk factor for subsequent infection. Surveillance for MDROs, including methicillin-resistant Staphylococcus aureus, vancomycin-resistant enterococci, extended-spectrum beta-lactamase-producing Enterobacterales, and carbapenemase-producing organisms, is commonly conducted in hospitals to prevent spread of MDROs, in part to reduce the potential for additional infections. Although colonization is a risk factor for infection, data on colonization with various MDROs are often not considered when selecting anti-infective therapy. There ar

The emergency department (ED) is the hub of the US health care system. Acute infectious diseases are frequently encountered in the ED setting, making this a critical setting for antimicrobial stewardship efforts. Systems level and behavioral stewardship interventions have demonstrated success in the ED setting but successful implementation depends on institutional support and the presence of a physician champion. Antimicrobial stewardship efforts in the ED should target high-impact areas: antibiotic prescribing for nonindicated respiratory tract conditions, such as bronchitis and sinusitis; ov

Because of the increasing plague of antimicrobial resistance and antibiotic misuse, antimicrobial stewardship programs (ASPs) are now a mandatory entity in all US hospitals. ASPs can use technological advances, such as the electronic medical record and clinical decision support systems, to impact a larger patient population with more efficiency. Additionally, through the use of mobile applications and social media, ASPs can highlight and propagate educational information regarding antimicrobial utilization to patients and providers in a widespread and timely manner. In this article, the author

Infections caused by organisms resistant to multiple drugs are associated with morbidity and mortality, prolonged hospital stay and increased costs. Since antibiotic use is the main factor in the development of resistance, it is hoped that the judicious use of antibiotics will reduce the emergence of antimicrobial resistance. This article addresses antibiotic misuse in the inpatient setting and outlines the role of the nurse in optimising antibiotic therapy through antimicrobial stewardship.

Antimicrobial resistance (AMR) has been declared a global public health emergency, necessitating the involvement of the whole of society to address this increasingly serious threat. AMR causes prolonged illness, greater risk of infection spread, increased morbidity, and higher mortality rates, which result in increased expenses to the government, healthcare services, and individuals. Inappropriate use of antimicrobials has been indicated as the primary cause for the global expansion of AMR. Under these circumstances, various countries have formulated suitable national action plans to curb AMR.

OBJECTIVES: For perforated appendicitis in children, microbiological cultures should always be sought if an adequate sample is available. Knowledge of local epidemiology is important for optimal selection of antimicrobial therapy. The aim of this study was to evaluate the aetiology and susceptibility of pathogens in paediatric patients with perforated appendicitis.
METHODS: Microbiological results of tissue samples obtained at surgery from children with acute appendicitis over 24 months were evaluated retrospectively.
RESULTS: Among 209 children operated for acute appendicitis, 62 (29.7%) we

Antimicrobial resistance is increasingly recognized in veterinary ophthalmology, yet its development remains largely unnoticed in daily clinical practice. Although resistant ocular pathogens are reported worldwide, antimicrobial prescribing decisions are frequently made without feedback on local resistance patterns. This Viewpoint discusses aggregated antibiograms as a practical tool to improve situational awareness and support antimicrobial stewardship in veterinary ophthalmology. While antibiograms do not quantify reductions in antimicrobial resistance, their systematic use can provide conte

Bovine respiratory disease has historically been, and continues to be, a significant issue facing the beef stocker industry. This single disease syndrome is responsible for nearly 90% of all morbidity and mortality in stocker cattle and has significant effects on animal welfare, animal performance, and overall farm profitability. Multiple antimicrobials in different antimicrobial classes are approved for metaphylaxis and, of those available for use, members of the macrolide class have demonstrated the greatest efficacy. Unfortunately, antimicrobial resistance is an emerging threat to antimicro

Neonatal infections result in significant morbidity and mortality. Antibiotics are vital for the treatment of infections but disrupt the neonatal microbiome, put the infant at risk for an adverse drug reaction, and may lead to the development of antibiotic resistance. Immediately after birth, clinicians must determine which infants require empiric antibiotics. Online risk stratification tools may provide a superior approach to decision trees. In infants who require empiric therapy for early-onset sepsis, ampicillin and an aminoglycoside with dosing based on recent pharmacokinetic studies repre

The global rise of antimicrobial resistance (AMR) poses a profound threat to human, animal and environmental health. Although antimicrobials have revolutionized modern medicine, their overuse and misuse have accelerated AMR, necessitating urgent, multisectoral action. Antimicrobial stewardship (AMS), a set of coordinated strategies that promote responsible antimicrobial use, has emerged as a key intervention in managing AMR. In this Review, we explore AMS within a One Health framework, emphasizing interconnectedness across sectors. We examine clinical, economic, sociocultural and environmental

Older adults hospitalized in internal medicine wards or long-term care facilities (LTCF) are progressively increasing. Older adults with multimorbidity are more susceptible to infections, as well as to more vulnerable to adverse effects (and interactions) of antibiotics, resulting in a need for effective and safer strategies for antimicrobial stewardship (ASM), both in hospitalization wards and long-term care facilities. Studies on antimicrobial stewardship in older patients are scarce and guidelines are required. Given the peculiarities of the optimization of antimicrobial prescription in ind

Dentistry accounts for approximately 10% of all antibiotic prescriptions globally, with evidence consistently demonstrating that the majority of these prescriptions are inappropriate. Current dental antimicrobial stewardship programs are defined almost exclusively around prescribing behavior-optimizing antibiotic choice, dose, and duration. This pharmacological framing, while necessary, is structurally incomplete. It overlooks the primary clinical intervention that renders antibiotics unnecessary in the first place: surgical source control through incision and drainage or tooth extraction. In

The duration of antimicrobial treatment for bacteremia has changed radically over the last decade, with shorter regimens now being the norm, the duration is now seven days in most cases. This change, which has brought countless clinical and ecological benefits, has been made possible by independent clinical research led by PROA teams in their tireless efforts to improve antimicrobial use and combat AMR. The evidence is so strong in Enterobacterales that it is now considered inappropriate to treat uncomplicated bacteraemia caused by Escherichia coli, for example, for more than seven days. In ot

Antimicrobial Stewardship Programs (ASPs) are a cornerstone strategy to mitigate the global threat of antimicrobial resistance (AMR), primarily driven by inappropriate antimicrobial use. ASPs aim to optimize antimicrobial therapy by ensuring appropriate indication, agent selection, dosing, route of administration, and duration of treatment. Through these interventions, ASPs improve clinical outcomes, reduce adverse drug events, decrease selective pressure for resistant organisms, and contribute to healthcare cost containment. Effective implementation requires a multidisciplinary approach invol

BACKGROUND: While antimicrobial stewardship (AMS) is essential for combating antimicrobial resistance (AMR), TB-specific AMS strategies remain poorly defined.
METHODS: An international panel of 62 experts participated in a Delphi process. Using a 5-point Likert scale (5 = strong agreement; 1 = strong disagreement), participants evaluated 10 draft clinical standards developed by a core coordination team. A standard was adopted if ≥90% of respondents rated it three or higher, according to a predefined consensus threshold.
RESULTS: All 10 standards reached the consensus threshold and were adopt
