Betaxolol is a relatively cardioselective β-adrenoceptor blocking drug, with no partial agonist (intrinsic sympathomimetic) activity and weak membrane-stabilizing (local anesthetic) activity. Betaxolol selectively and competitively binds to and blocks beta-1 (β1) adrenergic receptors in the heart, thereby decreasing cardiac contractility and rate. This leads to a reduction in cardiac output and lowers blood pressure. When applied topically in the eye, this agent reduces aqueous humor secretion and lowers the intraocular pressure (IOP). In addition, betaxolol prevents the release of renin, a ho

In 352 patients with primary open-angle glaucoma or ocular hypertension, a multicenter double-masked, parallel-group clinical study compared the effects on intraocular pressure and ocular comfort of 0.5% betaxolol ophthalmic solution, a cardioselective beta-adrenergic blocking agent, with 0.25% betaxolol suspension. With twice-daily dosages, baseline intraocular pressure was significantly reduced (P = .0005), with no significant difference between the two groups, at Week 2 and at Months 1, 2, and 3. Further, the prevalence of ocular discomfort upon topical instillation was significantly lower

Betaxolol and its respective hydrochloride salt were studied in solution by computational calculations and infrared spectroscopy. The solution molecular conformations were taken to be the same as those exhibited by the compounds in the solid state given by X-ray diffraction and calculated after full geometry optimization by ab initio Hartree-Fock methods using the 6-31G(d) basis set. Infrared spectra of carbon tetrachloride solutions provide valuable information on the structure of the compounds in non-polar solvents at different concentrations.

Postoperative intraocular pressure (IOP) elevation is a well documented phenomenon after cataract surgery. Many pharmacological agents have been used to prevent this complication. We conducted a randomized double-blind, placebo-controlled study to evaluate the effect of topical betaxolol and timolol on postoperative IOP after extracapsular cataract extraction and intraocular lens implantation. One hundred and twenty patients were randomly divided into three groups. At the end of the operation, the patients were given one drop of 0.5% betaxolol, 0.5% timolol or placebo solution into the lower c

PURPOSE: We report two cases of periocular cutaneous hypopigmentation and one case of hyperpigmentation which appeared while the patients were on betaxolol. We propose several possible explanations for this phenomenon.
METHODS: Charts of three patients with glaucoma who developed periocular cutaneous pigmentary changes while on betaxolol were reviewed retrospectively.
RESULTS: Case #1 was a 47-year-old black man with primary open angle glaucoma, started on betaxolol 0.5% in both eyes in January 1981. Bilateral hypopigmentation of the eyelids was first documented in October 1987. Betaxolol wa

Since the approval of timolol for the treatment of open-angle glaucoma, beta-adrenergic blocking drugs have replaced epinephrine, a pure alpha and beta agonist, as the treatment of choice for glaucoma in aphakic patients. To date, there have been no reports of cystoid macular edema secondary to timolol, a non-selective beta-adrenergic blocker that apparently has little intrinsic sympathomimetic activity. Betaxolol is a new beta-adrenergic receptor blocker that preferentially blocks beta 1 receptors. We report a case of angiographically proven cystoid macular edema secondary to betaxolol therap

OBJECTIVE: To study the cardiopulmonary safety and ocular hypotensive efficacy of topical betaxolol hydrochloride 0.5% (a selective beta-1 blocker) in patients with unacceptably elevated intraocular pressure (IOP) and respiratory dysfunction.
METHODS: Thirty-one patients with poor control of elevated IOP were selected on the basis of coexistent respiratory disease which could be exacerbated by beta-antagonist administration and/or who had past adverse reactions to topical timolol. Using an open-label design, the ophthalmic, cardiovascular and pulmonary parameters of these patients were measur

Effect of instillations of 0.5% betaxolol solution (Alcon) on intraocular pressure, ocular hydro- and hemodynamics, function, systemic arterial pressure, and pulse was studied in 35 patients (57 eyes) with primary open-angle glaucoma. The effects of betoptic and arutimol on the eyes are compared. Betoptic is characterized by a marked hypotensive effect explained by depression of aqueous humor production. The drug does not influence ocular function and hemodynamics. Comparison of the hypotensive effects of betaxolol and arutimol has shown that arutimol more effectively reduces intraocular press

In a prospective, randomized, double-masked study, primary open-angle glaucoma patients were treated twice daily with either betaxolol 0.5% or timolol 0.5%. Patients received a pretreatment baseline examination following a 2 week washout period; they were then examined after 3 and 6 months of treatment. Each examination included visual field measurements with an Octopus automated perimeter and intraocular pressure measurement. The visual fields of the betaxolol group showed on the average a pronounced tendency toward improvement (MD decrease of 2.05 dB); visual fields in the timolol group show

In a double-masked study, 40 patients with primary open-angle glaucoma were randomly assigned to treatment with betaxolol 0.5% or timolol 0.5% in both eyes twice daily. Visual fields and intraocular pressures were studied. Perimetry was performed with the Octopus G1 program two weeks after a washout period and three, six, 12, and 18 months after initiation of treatment. Both drugs reduced intraocular pressure. The reduction in intraocular pressure in the timolol-treated group was more pronounced than that in the betaxolol-treated group; the difference, however, was not statistically significan

The mechanisms of drug clearance from the aqueous humor are poorly defined. In this study, a cocktail approach was used to simultaneously determine the pharmacokinetics of three β-blocker agents after intracameral (ic) injection into the rabbit eyes. Aqueous humor samples were collected and analyzed using LC-MS/MS to determine drug concentrations. Pharmacokinetic parameters were obtained using a compartmental fitting approach, and the estimated clearance, volume of distribution, and half-life values were the following: atenolol (6.44 μL/min, 687 μL, and 73.87 min), timolol (19.30 μL/min, 937 μ

Twenty two pregnant women with mild to moderate hypertension were treated with betaxolol (10-40 mg/day), a cardioselective beta adrenoceptor blocking agent. The analysis of the changes from the baseline confirmed the antihypertensive effect of the drug with a mean decrease in SBP of 11.8 mm Hg and in DBP of 8.3 mm Hg. A diastolic BP less than 90 mm Hg was obtained in 20 patients after the first day of therapy. Fetal safety, assessed by ultrasonography and cardiotocographic recording was excellent. The 22 mothers gave birth to 23 live born babies (one twin pregnancy). Mean Apgar scores were 8.3

In a randomized, double-masked study, 41 patients with primary open-angle glaucoma or ocular hypertension were treated with betaxolol 0.5% or timolol 0.5% drops for 26 weeks. The average decrease in intraocular pressure (IOP) over the total study period was significant with both betaxolol (-6.3 mmHg) and timolol (-7.2 mmHg) in patients receiving no adjunctive therapy. There was no difference between betaxolol and timolol with respect to changes from baseline IOP. Significantly decreased mean brachial arterial pressure (MAP) was seen only with timolol, although the difference between the two gr

The absolute bioavailability and dose proportionality of betaxolol [(+/-)-1-(p-[2-cyclopropylmethoxy)ethyl]phenoxy]-3- (isopropylamino)-2-propanol hydrochloride], a cardioselective beta-adrenergic antagonist effective in the treatment of angina and hypertension, was studied in 12 healthy male subjects using a four-way crossover Latin Square design. Each subject received a 10-mg iv dose administered by constant-rate infusion over a period of 30 min and three oral doses (10, 20, and 40 mg). Blood and urine were collected over a 48-h period and analyzed for betaxolol using gas-liquid chromatograp

1-[4-(2-Cyclopropylmethoxyethyl)phenoxy]-3-[1-p-(bromoacetamidophe nyl) -2-methyl-2-propylamine]-2-propranol (8), which is a derivative of the beta 1-adrenergic agent betaxolol, was synthesized. Compound 8 showed less potent beta-adrenergic blocking activity than betaxolol in an in vitro test with guinea pig tracheal muscle and left atrium but retained high beta 1-selectivity. Irreversible beta-adrenoceptor antagonism of 8 was assessed by the blockade of the isoproterenol response of the guinea pig atria and by ligand binding studies with rat cerebral cortex.

We present a case of successful treatment of near-fatal beta-blocker self-poisoning but requiring extracorporeal circulatory support with severe complications. A 38-year-old woman ingested a mixture of tablets including betaxolol (5.32 g). Despite intensive treatment with fluid, dobutamine, isoprenaline, epinephrine, nor-epinephrine and glucagon, sustained cardiogenic shock occurred with almost complete hypokinesia of the left ventricular 14 h later. Therefore, a cardiac support was performed with a percutaneous cardiopulmonary bypass device at bedside. We review the literature with emphasis o

We investigated the effect of betaxolol on the decrease of mitochondrial aspartate aminotransferase (mAAT) activity in rat retinas induced by hypoxia in vitro. It is reported that mAAT decreases in ischemic or hypoxic retina and that the decrease is caused by Ca(2+)-dependent proteases such as calpain. Betaxolol is a compound that has beta(1)-adrenergic receptor blocking and voltage-dependent calcium channel blocking properties. The rat eye cups were maintained with Locke's solution saturated with 95% air - 5% CO(2). The eye cups were immersed in glucose-free Locke's solution saturated with 95
