Transcutaneous blood gas monitoring during neonatal intensive care.

AIM: To evaluate the accuracy in transcutaneous (Tc) blood gas monitoring in newborn infants, including extremely low birth weight infants, during neonatal intensive care. METHODS: Tc PO(2) /PCO(2) was monitored in the neonatal intensive care unit (NICU) during stable infant conditions. In comparison, simultaneous arterial PO(2) and PCO(2) was measured. Sixty measurements were taken in 46 infants with median (range) birth weight of 0.93 (0.53-4.7) kg and at median (range) age of 8.5 (1-44) days. Comparison of measurements was performed using Bland-Altman plots, and the mean (95% CI) of the di

Modeling and Evaluation of a Rate-Based Transcutaneous Blood Gas Monitor.

OBJECTIVE: Current methods for transcutaneous blood gas monitoring (TBM) - a common health monitoring method in neonatal care - comes with a suite of challenges like limited attachment opportunities, and risks of infections from burning and tearing of the skin, which limits its use. This study presents a novel system and method for rate-based transcutaneous CO2 measurements with a soft, unheated skin-interface that can address many of these problems. Additionally, a theoretical model for the gas transport from the blood to the system's sensor is derived. METHODS: By simulating CO2 advection a

Transcutaneous blood gas monitoring and tissue perfusion during common femoral thromboendarterectomy.

BACKGROUND: Improving tissue perfusion can improve clinical outcomes in surgical patients, where monitoring may aid clinicians in detecting adverse conditions and guide interventions. Transcutaneous monitoring (TCM) of oxygen (tcpO2) and carbon dioxide (tcpCO2) is a well-proven technology and could potentially serve as a measure of local circulation, perfusion and metabolism, but the clinical use is not thoroughly explored. The purpose of this proof-of-concept study was to investigate whether TCM of blood gasses could detect changes in perfusion during major vascular surgery. METHODS: Ten pat

Compliance of a microstructured, soft sampling device for transcutaneous blood gas monitoring.

Premature neonates are too small for repeated blood sampling, but still require precise monitoring of blood gas levels. The standard method therefore involves transcutaneous blood gas monitoring (TBM), i.e. analyzing gas that permeates the skin. The method involves skin heating and requires frequent relocation of a rigid sensor that is adhesively mounted to the skin, which makes the monitoring intermittent and can cause tissue damage. To mitigate this, this paper introduces a TBM concept that replaces the sensor with a small, non-adhesive, flexible, polydimethylsiloxane patch, routing the gase

Evaluation of Transcutaneous Non-Invasive Blood Gas Analysis for Monitoring Gas Exchange in Pediatric Cardiac Surgical Patients Post Extubation.

INTRODUCTION: Pediatric cardiac surgery patients need close post-extubation monitoring for ventilation. Non-invasive transcutaneous partial pressure of oxygen (TcPO2) and transcutaneous partial pressure of carbon dioxide (TcPCO2) offer continuous insights and in improving care. OBJECTIVE: To investigate the correlation of transcutaneous blood gases (TcPO2, TcPCO2) with arterial blood gases i.e. arterial partial pressure of oxygen (PaO2) and arterial partial pressure of carbon dioxide (PaCO2). METHODS: We conducted a study on 30 pediatric post-cardiac surgery patients (four months to three ye

[Are transcutaneous blood gas measurement monitors (SenTec Digital Monitor system) useful for the respiratory management of chronic respiratory failure?].

BACKGROUND: The level of arterial carbon dioxide partial tension (PaCO2) is a sine qua non for the determination of the adequacy of ventilation. The aim of this study was to assess the usefulness of SenTec Digital Monitor System in measuring transcutaneous arterial carbon dioxide partial tension (PCO2) non-invasively for the management of chronic respiratory failure. METHODS: Three ICU patients suffering from chronic respiratory failure were enrolled. We recorded the data of PCO2 measured by a transcutaneous ear sensor (V-Sign : SenTec Inc.) and the data of PaCO2 (GASTAT-603ie Techno Medica C

Early detection of microcirculatory changes using simultaneous heated and unheated transcutaneous blood gas monitoring: a proof-of-concept study in a porcine lipopolysaccharide model of systemic inflammation.

INTRODUCTION: Early detection of microcirculatory failure in septic patients has the potential to improve clinical outcomes. The microcirculation regulates the oxygen (O2) supply and carbon dioxide (CO2) washout in tissues and changes early during severe inflammation and sepsis. Transcutaneous blood gas sensors measure tissue O2 and CO2 levels at the skin surface, or when warmed use the vasodilatory response to approximate arterial levels. In addition to differences in measured O2 and CO2 values, systemic inflammation is likely to affect this vasodilatory effect of skin warming. This study inv

Agreement Between Transcutaneous Monitoring and Arterial Blood Gases During COPD Exacerbation.

BACKGROUND: Transcutaneous measurements of CO2 and O2 ([Formula: see text], [Formula: see text]) are noninvasive and allow for continuous monitoring in adults with exacerbation of COPD, but substantial accuracy issues may exist. We investigated agreement between results of arterial blood gas analysis and transcutaneous measurements of CO2 and O2 in patients with COPD. METHODS: Adult subjects were monitored after acute admission to a respiratory intermediate care unit or ICU due to exacerbation of COPD and with ongoing noninvasive ventilation or immediately following extubation. Monitored vari

Simultaneous transcutaneous carbon dioxide and transcutaneous oxygen monitoring in neonatal intensive care.

Continuous transcutaneous carbon dioxide partial pressure (tc PCO2) and continuous transcutaneous oxygen partial pressure (tc PO2) was monitored simultaneously in 10 consecutive neonatal intensive care patients treated for respiratory problems or immaturity. During measurement the electrode temperature was 44 degrees C while during the resting periods--with the electrodes left in situ--the electrode temperature was 37 degrees C. Measurements were performed for periods up to 31 hours. It was possible to discover changes in central blood gas partial pressures also at the lower electrode temperat

Noninvasive measurement of arterial oxyhemoglobin saturation with a heated and a non-heated skin reflectance pulse oximeter sensor.

The feasibility of measuring arterial oxyhemoglobin saturation (SaO2) noninvasively using a skin reflectance pulse oximeter sensor attached to the scalp, neck, and thigh regions of anesthetized swine was investigated. The optical reflectance sensor used consisted of a pair of red and infrared light-emitting diodes and a concentric array of six identical photodiodes. Two prototype sensor assemblies were evaluated: one assembly housed only the optical sensor, whereas the other also included a miniature heater. Measurements made from the scalp and neck regions were obtained with the non-heated sk

A Transcutaneous Carbon Dioxide Monitor Based on Time-Domain Dual Lifetime Referencing.

The partial pressure of arterial carbon dioxide plays a critical role in assessing the acid-base and respiratory status of the human body. Typically, this measurement is invasive and can only be taken momentarily when an arterial blood sample is drawn. Transcutaneous monitoring is a noninvasive surrogate method that provides a continuous measure of arterial carbon dioxide. Unfortunately, current technology is limited to bedside instruments mainly used in intensive care units. We developed a first-of-its-kind miniaturized transcutaneous carbon dioxide monitor that utilizes a luminescence sensin

Changes in arterial and transcutaneous oxygen and carbon dioxide tensions during and after voluntary hyperventilation.

The purposes of our study were (1) to investigate whether a 3-min short-term hyperventilation leads to posthyperventilatory hypoxemia and (2) to assess the role of transcutaneous blood gas measurements for monitoring oxygen and carbon dioxide changes during the after the test. In 10 male volunteers arterial and transcutaneous blood gases were measured simultaneously before, during and after a 3-min voluntary hyperventilation maneuver. Baseline arterial PO2 increased from 13.7 +/- 0.4 kPa (103 +/- 3 mm Hg) to 18.6 +/- 0.3 kPa (139 +/- 2.3 mm Hg; p < 0.005 compared to baseline) during hyperventi

Validation study of a transcutaneous carbon dioxide monitor in patients in the emergency department.

BACKGROUND: Transcutaneous carbon dioxide tension (TcPco(2)) monitors offer a non-invasive method of continuously measuring arterial carbon dioxide tensions. The concordance between the TcPco(2) measurement and the value obtained from arterial blood gas sampling (Paco(2)) was measured in patients attending the emergency department. METHODS: A prospective observational cohort study was performed in 49 adult patients who were undergoing arterial blood gas sampling as part of their assessment. Blood gas sampling and recording of the TcPco(2) level from the monitor was done simultaneously. Concor

The effect of succinylcholine on malignant hyperthermia events in susceptible swine.

BACKGROUND: While the impact of volatile anaesthetics to induce malignant hyperthermia (MH) is abundantly clear, the role of succinylcholine still remains controversial. To evaluate the influence of succinylcholine on porcine MH events, the authors investigated the hemodynamic and metabolic responses in MH susceptible (MHS) and non-susceptible (MHN) swine following either succinylcholine or halothane application alone or a combination of both substances. METHODS: With approval of the local animal care committee 27 MHS and 30 MHN pigs were anaesthetized and mechanically ventilated. Fiberoptic

Evaluation of a transcutaneous carbon dioxide monitor in patients with acute respiratory failure.

BACKGROUND: Non-invasive measurement of oxygenation is a routine procedure in clinical practice, but transcutaneous monitoring of PCO(2)(PtCO(2)) is used much less than expected. METHODS: The aim of our study was to analyze the value of a commercially available combined SpO(2)/PtCO(2) monitor (TOSCA-Linde Medical System, Basel, Switzerland) in adult non-invasive ventilated patients with acute respiratory failure. Eighty critically ill adult patients, requiring arterial blood sample gas analyses, underwent SpO(2) and PtCO(2) measurements (10 min after the probe was attached to an earlobe) simu

Optimizing Transcutaneous Oxygen Measurement Sites on Humans.

This study utilizes an optical method of transcutaneous oxygen sensing that has the potential to revolutionize at-home care. This technique is based on quenching the luminescence of a platinum porphyrin film. Since oxygen quenches luminescence, its lifetime is further measured to assess the partial pressure of transcutaneous oxygen diffusing through the skin. Unlike conventional transcutaneous oxygen monitors that use electrochemical sensors, the luminescence-based sensor allows the use of dry electrodes that do not require heating and reduce the risk of accidental skin irritations or burns. Prospective randomized evaluation of transcutaneous carbon dioxide monitoring during complex electrophysiological procedures under deep sedation: the TRACES trial.

BACKGROUND: The field of interventional electrophysiology is rapidly growing. For safe and efficient procedures sedation is needed for patient comfort and immobility. However, sedative and analgesic agents can induce respiratory depression, risking hypercapnia, hypoxia, and hypotension. This study evaluates the impact of adding transcutaneous carbon-dioxide (CO2) monitoring to standard monitoring practices in electrophysiological procedures under deep sedation. METHODS: This prospective, randomized study at Ulm University Heart Center included 726 patients undergoing treatment under deep seda
