Hepato-Cardio-Renal Syndrome.

Hepatorenal syndrome has conventionally been regarded as a multisystem syndrome in which pathophysiologic pathways that link cirrhosis with impairment in kidney function are followed by dysfunction of several organs such as the heart. The advances in cardiac studies have helped diagnose more subtle cardiac abnormalities that would have otherwise remained unnoticed in a significant subset of patients with advanced liver disease and cirrhosis. Accumulating data suggests that in many instances, the cardiac dysfunction precedes and predicts development of kidney disease in such patients. These obs

Facio-cardio-renal (Eastman-Bixler) syndrome.

We report on a 5-year-old boy with moderate mental retardation, horseshoe kidneys, tricuspid valve prolapse, and a characteristic face with broad nasal root, prominent ears, and a cleft palate. These manifestations suggested the diagnosis of the Eastman-Bixler syndrome. Our patient also had an isolated growth hormone deficiency which responded successfully to treatment.

Imaging of Congestion in Cardio-renal Syndrome.

PURPOSE OF REVIEW: Both cardiac and renal dysfunction can lead to water overload - commonly referred to as "congestion". Identification of congestion is difficult, especially when clinical signs are subtle. RECENT FINDINGS: As an extension of an echocardiographic examination, ultrasound can be used to identify intravascular (inferior vena cava diameter dilation, internal jugular vein distension or discontinuous venous renal flow) and tissue congestion (pulmonary B-lines). Combining assessment of cardiac structure, cardiac and renal function and measures of congestion informs the management of

The relevance of congestion in the cardio-renal syndrome.

Worsening renal function (WRF) during the treatment of acute decompensated heart failure (ADHF) occurs in up to a third of patients and is associated with worse survival. Venous congestion is increasingly being recognized as a key player associated with WRF in ADHF. Understanding the hemodynamic effects of venous congestion and the interplay between venous congestion and other pathophysiological factors such as raised abdominal pressure, endothelial cell activation, anemia/ iron deficiency, sympathetic overactivity, and stimulation of the renin-angiotensin-aldosterone system will help in devis

Management of Cardio-Renal Syndrome and Diuretic Resistance.

Diuretic resistance in acute heart failure has emerged as a powerful predictor of adverse outcome, which is often independent of underlying glomerular filtration rate (GFR). Metrics of diuretic efficacy differ in their accuracy, convenience, and biological plausibility, which should be taken into account when interpreting their results. Loop diuretic efficacy depends on adequate delivery of both the pharmacological agent itself and its substrate (i.e., sodium chloride) to the loop diuretic site of action at the luminal side of the thick ascending limb of Henle's loop. This requires an adequate

Chronic cardiorenal syndrome: cardio-renal protective effect of SGLT2i.

OBJECTIVES: Chronic cardiorenal syndrome (types 2 and 4) arises from chronic dysfunction of either the heart or kidneys, leading to progressive deterioration of the other organ. The coexistence of cardiac and renal dysfunction significantly complicates treatment strategies and is associated with high morbidity and mortality, with currently limited effective therapeutic options. Given the demonstrated cardiorenal protective effects of sodium-glucose cotransporter 2 inhibitors (SGLT2i), it is important to explore their potential mechanisms and clinical application value in the treatment of non-d

Preclinical models of cardio-renal syndrome: a systematic review.

The prevalence of heart failure (HF) and of chronic kidney disease (CKD) is continuously rising. Both diseases require significant management efforts, and more importantly, HF is often associated with CKD, aggravating the clinical scenario and leading to "cardiorenal syndrome" (CRS). Although clinical studies suggest a bidirectional interaction between HF and CKD, the pathophysiological understanding of CRS remains incomplete. Several mechanisms are involved in CRS, including changes in systemic and renal hemodynamics, endothelial dysfunction, inflammation, and activation of the renin-angioten

Cardio-Renal Syndrome: Latest Developments in Device-Based Therapy.

Background: Cardio-renal syndrome (CRS) is a complex condition involving bidirectional dysfunction of the heart and kidneys, in which the failure of one organ exacerbates failure in the other. Traditional pharmacologic treatments are often insufficient to manage the hemodynamic and neurohormonal abnormalities underlying CRS, especially in cases resistant to standard therapies. Device-based therapies have emerged as a promising adjunct or alternative approach, offering targeted intervention to relieve congestion, improve renal perfusion, and modulate hemodynamics. This study aimed to evaluate t

Cardio-renal syndrome type 4: epidemiology, pathophysiology and treatment.

Cardiovascular diseases such as coronary artery disease, congestive heart failure, arrhythmia, and sudden cardiac death represent the leading causes of morbidity and mortality in patients with CKD, increasing sharply as patients approach end-stage renal disease. The pathogenesis includes a complex, bidirectional interaction between the heart and kidneys that encompasses traditional and nontraditional risk factors, and has been termed cardio-renal syndrome type 4. In this review, an overview of the epidemiology and scope of this problem is provided, some suggested mechanisms for the pathophysio

[Cardio-renal syndrome: the challenge in heart failure treatment].

Heart failure is a chronic and progressive disease that is estimated to affect approximately 20 million people worldwide and is one of the major public health problems. Its prevalence is reaching epidemic levels with about 550,000 new cases diagnosed annually, partly due to increased life expectancy in developed countries. And as it is a systemic disease, it can cause dysfunction in various organs, but especially in the kidney. The renal failure is often associated with heart failure and, when present together, make the treatment more complex and the prognosis is worse. This is the cardio-rena

Renal perfusion index reflects cardiac systolic function in chronic cardio-renal syndrome.

BACKGROUND: Cardiac dysfunction can modify renal perfusion, which is crucial to maintain sufficient kidney tissue oxygenation. Renal cortex perfusion assessed by dynamic ultrasound method is related both to renal function and cardiac hemodynamics. The aim of the study was to test the hypothesis that Renal Perfusion Index (RPI) can more closely reflect cardiac hemodynamics and differentiate etiology of chronic cardio-renal syndrome. MATERIAL AND METHODS: Twenty-four patients with hypertension and chronic kidney disease (CKD) at 2-4 stage (12 with hypertensive nephropathy and 12 with CKD prior

Extracorporeal ultrafiltration in heart failure and cardio-renal syndromes.

Acute decompensated heart failure and fluid overload are the most common causes of hospitalization in heart failure patients and they often contribute to disease progression. Initial treatment encompasses intravenous diuretics, although there might be a percentage of patients refractory to this pharmacologic approach. New technologies have been developed to perform extracorporeal ultrafiltration in fluid-overloaded patients. Newer simplified devices permit ultrafiltration to be performed with peripheral venous access and low blood flows, making ultrafiltration feasible at most hospitals and ac

The complexity of the cardio-renal link: taxonomy, syndromes, and diseases.

Bidirectional mechanisms exist that link diseases affecting the heart and kidney. This link is complex and remains poorly understood; therefore, charting the shared territory of cardiovascular (CV) and renal medicine poses major problems. Until now, no convincing rationale for delineating new syndromes existed. The multiple connections of the arterial system and the heart and kidney with other systems, from energy and protein balance to the musculoskeletal, clearly require special focus and rigorous framing. Nephrologists have yet to fully understand why the application of dialysis has had onl

Role of iron deficiency and anemia in cardio-renal syndromes.

Chronic heart failure is a common disorder associated with unacceptably high mortality rates. Chronic renal disease and anemia are two important comorbidities that significantly influence morbidity and mortality in patients with chronic heart failure (CHF). Progress in CHF again may cause worsening of kidney function and anemia. To describe this vicious cycle, the term cardio-renal anemia syndrome has been suggested. Iron deficiency is part of the pathophysiology of anemia in both CHF and chronic kidney disease, which makes it an interesting target for treatment of anemia in cardio-renal anemi

Reversible Cardio-Renal-Cerebral Syndrome in a Dog: A Case Report.

A 14-year-old miniature Pinscher was presented with azotemia, severe hyperkalemia, and oliguria caused by decompensation of chronic renal disease, along with bradycardia resulting from third-degree atrioventricular (AV) block. Supportive medical treatment was instituted for the oliguria, azotemia, and hyperkalemia. Within 12 h of hospitalization, multifocal central neurologic signs developed, including nystagmus, quadriparesis, decreased mentation, and ventral neck flexion. A diagnosis of cardiorenal cerebral syndrome was made. A transvenous permanent pacemaker was placed to improve cardiac ou

Cardio-Renal Metabolic Syndrome: An Integrated Approach to Prevention and Management.

Cardio-renal metabolic syndrome (CRMS), also termed cardiovascular-kidney-metabolic (CKM) syndrome, represents the convergence of metabolic risk factors, chronic kidney disease (CKD), and cardiovascular disease (CVD), underpinned by insulin resistance (IR), neurohormonal activation, oxidative stress, and chronic inflammation. This narrative review synthesizes current evidence on the epidemiology, pathophysiology, diagnostic approaches, prevention, and management of CRMS. Globally, CRMS prevalence continues to rise, driven by aging populations, urbanization, obesity, and diabetes, with dispropo
