Chagas disease is a chronic parasitic infection caused by Trypanosoma cruzi. Its most severe complication is chronic Chagas cardiomyopathy, which may lead to heart failure, malignant ventricular arrhythmias, and sudden cardiac death. Because the disease often evolves silently for decades, early detection of myocardial involvement remains challenging. Cardiovascular imaging plays a key role in diagnosis and risk stratification. Echocardiography provides a functional assessment and detects structural abnormalities, whereas cardiac magnetic resonance allows for tissue characterization and identif

Chagas cardiomyopathy, caused by chronic infection with Trypanosoma cruzi, is a leading cause of non-ischaemic heart failure in Latin America and is increasingly recognised worldwide due to migration. Its pathogenesis is multifactorial, involving persistent parasitism, immune-mediated myocardial injury, microvascular dysfunction and progressive fibrosis, leading to a heterogeneous clinical spectrum. Four main phenotypes are recognised, including subclinical, arrhythmic, heart failure and thromboembolic, which often overlap and evolve throughout the disease course.Diagnosis relies on serologica

Chagas' disease (ChD), caused by the protozoa Trypanosoma cruzi (T. cruzi), was discovered and described by the Brazilian physician Carlos Chagas in 1909. After a century of original description, trypanosomiasis still brings much misery to humanity and is classified as a neglected tropical disease prevalent in underdeveloped countries, particularly in South America. It is an increasing worldwide problem due to the number of cases in endemic areas and the migration of infected subjects to more developed regions, mainly North America and Europe. Despite its importance, chronic chagas cardiomyopa

Chagas cardiomyopathy (CC) is one of the chronic manifestations of Trypanosoma cruzi (T. cruzi) infection and is a major public health disease in Latin America. Since it is a chronic systemic infection, Chagas disease was long considered a potential contraindication for transplantation because of the risk of recurrence with immunosuppression. However, early South American experience in the 1980's established the feasibility of heart transplantation (HT) in patients with Chagas disease. Indeed, the first cardiac transplant for a recipient with CC was performed in 1985 in Brazil. Chagas etiology

In this study the histopathologic features of Chagas' disease and idiopathic dilated cardiomyopathy were compared in 38 patients with congestive heart failure, classes II or III. Biopsy specimens were obtained with the Stanford model needle for right ventricle. Twenty-two patients presented non-chagasic dilated cardiomyopathy, and 16 were carriers of Chagas' disease. Twenty-three were male and 15 were female; their ages varied 18 and 49 years (mean 35.5). Fibrosis was slightly more frequently and intense in Chagas' disease than in dilated cardiomyopathy. Myocarditis aspect also was more freque

BACKGROUND: Chagas cardiomyopathy (CHCM) is the most important clinical manifestation of Chagas disease. The analysis of cardiac miRNAs may contribute to predicting the progression to CHCM in Chagas indeterminate phase and/or to the differential diagnosis for cardiomyopathy.
METHODS: We carried out a case-control study to identify circulating miRNAs associated with CHCM. We assigned 104 participants to four groups: healthy controls (HC), Chagas non-cardiomyopathy controls, CHCM cases, and ischemic cardiomyopathy controls. We performed a clinical, echocardiographic, and laboratory evaluation a

More than 110 years after Chagas disease discovery, some aspects of the mechanisms involved in the physiopathology of heart damage are still unknown. Previously, Trypanosoma cruzi was considered to be the only cause of myocardial injury. Currently, there are other known mechanisms involved in the physiopathology of Chagas heart disease, including the immune-inflammatory response, autoimmunity, microvascular abnormalities and nerve damage, which are interrelated and contribute to cardiac damage. Nowadays, the parasite is the main cause of cardiac damage during the acute stage of Chagas disease

The mechanisms by which Trypanosoma cruzi causes cardiomyopathy and induces neuronal destruction are discussed in this paper. The results suggest that autoimmunity in the chronic phase is the main cause of the progressive cardiac destruction, and that autoreactivity is restricted to the CD4+ T cell compartment. During the acute phase, the neuronal and cardiac fiber destruction occurs when ruptured parasite nests release T. cruzi antigens that bind to the cell surface in the vicinity which become targets for the cellular and humoral immune response against T. cruzi. The various factors involved

BACKGROUND: Chagas disease, caused by Trypanosoma cruzi parasites, is a common cause of heart failure (HF) in Latin America and has recently been declared endemic in the United States. The authors compared outcomes in Chagasic HF vs ischemic and other nonischemic etiologies of HF with reduced ejection fraction (HFrEF).
OBJECTIVES: The aim of this study was to compare clinical outcomes of Chagasic HFrEF vs ischemic and other nonischemic etiologies.
METHODS: Investigator-reported etiology of HFrEF in the ATMOSPHERE, PARADIGM-HF, and GALACTIC-HF trials was categorized as ischemic, valvular, alc

BACKGROUND: Chagas cardiomyopathy is a chronic sequela of infection by the parasite, Trypanosoma cruzi. Advanced cardiomyopathy is associated with a high mortality rate, and clinical characteristics have been used to predict mortality risk. Though multiple biomarkers have been associated with Chagas cardiomyopathy, it is unknown how these are related to survival.
OBJECTIVES: This study aimed to identify biomarkers associated with mortality in individuals with severe Chagas cardiomyopathy in an urban Bolivian hospital.
METHODS: The population included individuals with and without T. cruzi inf

A woman (49 years) with Chagas' disease showed: ECG, right bundle-branch block and left anterior-superior fascicular block; V1 has unusual R > R', and elevated ST segment from V2 to V6 . Additional imaging revealed concomitant HCM and Chagas, which is uncommon. Overlapping of ECG findings can be explained by this rare association of diseases.

UNLABELLED: Chagas disease (CD) remains a major cause of stroke in developing countries, but cognitive repercussion of CD has not been well studied.
OBJECTIVE: To compare the frequency and pattern of cognitive dysfunction in patients with CD cardiomyopathy (CDC) and other cardiomyopathies (OC).
METHODS: We studied 37 patients with CDC and 42 patients with OC with similar age, educational level and cardiac systolic function. Cognitive tests were applied to both groups by a single examiner blinded to CD status. Logistic regression multivariable models were constructed to ascertain predictors o

The natural history of Chagas' disease and its manifestations when the heart is involved are detailed clinically and pathologically. Three phases are recognized: the acute phase, lasting from 1-3 months, the latent phase, which may last from 10-20 years, and the chronic phase, which has the most serious manifestations. This phase is subdivided into three clinical stages. An analysis of the varied cardiac manifestations on 235 patients is included.

PURPOSE OF REVIEW: Chagas cardiomyopathy (CCC) is a major cause of heart failure (HF) in Latin America, yet sex-specific differences in HF presentation and outcomes remain underexplored. This review summarizes current knowledge on clinical manifestations, HF phenotypes, and molecular mechanisms in women and men with CCC.
RECENT FINDINGS: Men with CCC more frequently present with severe HF, arrhythmias, and sudden cardiac death, whereas women often exhibit nonspecific symptoms such as fatigue and palpitations. HF prevalence in women is variable, with some cohorts showing lower left ventricular

BACKGROUND: The role of trypanocidal therapy in patients with established Chagas' cardiomyopathy is unproven.
METHODS: We conducted a prospective, multicenter, randomized study involving 2854 patients with Chagas' cardiomyopathy who received benznidazole or placebo for up to 80 days and were followed for a mean of 5.4 years. The primary outcome in the time-to-event analysis was the first event of any of the components of the composite outcome of death, resuscitated cardiac arrest, sustained ventricular tachycardia, insertion of a pacemaker or implantable cardioverter-defibrillator, cardiac tr

BACKGROUND: Chagas disease (ChD), a neglected tropical disease caused by Trypanosoma cruzi, affects around 7.5 to 10 million people globally, primarily in Latin America. Chronic Chagas cardiomyopathy (CCM) is the most severe clinical form, leading to substantial cardiovascular morbidity and mortality. Despite existing guidelines, fragmented health systems, low provider awareness, and limited access to care hinder effective disease management.
OBJECTIVES: We aimed to define the key components of the CCM quality of care (structure, process, and outcomes) for main clinical activities at the thre

The initial infection of Chagas disease is typically asymptomatic, but approximately 30% of people will progress to a chronic cardiac form, and others develop the gastrointestinal form. Death is often sudden due to arrhythmias or progressive heart failure. Prevention through vector control programs and blood bank screening, along with strengthened surveillance systems and rapid information sharing, are key to decreasing disease burden globally. The epidemiology, diagnostic evaluation, diagnosis, and treatment of acute and chronic Chagas cardiac disease are discussed with focus on educating the

Chagas disease, or American trypanosomiasis, is a parasitic disease caused by the protozoan Trypanosoma cruzi, which is native to the Americas. We report the case of a 35-year-old woman in her 37th week of pregnancy who required a cesarean section after being diagnosed with chronic-stage Chagas disease involving the heart and the digestive system. We discuss the anesthetic technique used, the principal pathophysiologic features of the cardiomyopathy and megaesophagus seen in this disease, and their implications for anesthetic management. We review the different clinical stages of the disease,
