BACKGROUND: There is currently no clear evidence that nasal-biphasic positive airway pressure (n-BiPAP) confers any advantage over nasal-continuous positive airway pressure (n-CPAP). Our hypothesis was that preterm infants born before 30 weeks' gestation and <2 weeks old when extubated onto n-BiPAP will have a lower risk of extubation failure than infants extubated onto n-CPAP at equivalent mean airway pressure.
METHODS: We conducted an unblinded multicenter randomized trial comparing n-CPAP with n-BiPAP in infants born <30 weeks' gestation and <2 weeks old. The primary outcome variable was t

Automatic positive airway pressure devices are the most technologically advanced positive airway pressure devices available for use in OSA. Although heterogeneous, they have in common the ability to detect and respond to changes in upper airway resistance. Data cannot necessarily be extrapolated from one device to another, and the field is rapidly advancing. Most studies of APAP have been performed in a supervised setting, or patients have been carefully selected to have a high likelihood of OSA uncomplicated by disorders such as alveolar hypoventilation or central apnea or technical problems

Nasal continuous positive airway pressure (nCPAP) is an effective treatment of respiratory distress syndrome. Due to long-standing experience of early nCPAP as the primary respiratory support option in preterm infants, this approach is sometimes labeled 'the Scandinavian Model'. Mechanical ventilation is potentially harmful to the immature lungs and cohort studies have demonstrated that centers using more CPAP and less mechanical ventilation have reduced rates of bronchopulmonary dysplasia. However, there is a lack of evidence in the form of larger, randomized controlled trials to prove the su

A ventilator-dependent child had been in the paediatric intensive care unit (PICU) ever since birth. As a result, she had fallen behind considerably in her development. After 18 months, continuous positive airway pressure was successfully administered via a tracheostomy tube with a novel lightweight device. This enabled her to walk in the PICU. With this device, the child was discharged home where she could walk with an action range of 10 m. Subsequently, her psychomotor development improved remarkably. To the authors' knowledge, this is the first case report of a patient, adult or paediatric,

Premature neonates are predisposed to complications, including bronchopulmonary dysplasia (BPD). BPD is associated with long-term pulmonary and neurodevelopmental consequences. Noninvasive respiratory support with nasal continuous positive airway pressure (CPAP) has been recommended strongly by the American Academy of Pediatrics. However, CPAP implementation has shown at least a 50% failure rate. Enhancing nasal CPAP effectiveness may decrease the need for mechanical ventilation and reduce the incidence of BPD. Bubble nasal CPAP is better than nasal CPAP using mechanical devices and the bubbli

Obstructive sleep apnoea (OSA) is increasingly prevalent, particularly in the context of the obesity epidemic, and is associated with a significant social, health and economic impact. The gold standard of treatment for moderate to severe OSA is continuous positive airway pressure (CPAP). However compliance rates can be low. Methodology to improve patient tolerance to CPAP alongside with alternative, non-surgical and surgical, management strategies are discussed. All patients that fail CPAP therapy would benefit from formal upper airway evaluation by the otolaryngologist to identify any obvious

Continuous positive airway pressure (CPAP) is the treatment of choice for obstructive sleep apnoea syndrome (OSAS). However, CPAP is not tolerated by all patients with OSAS and alternative modes of pressure delivery have been developed to overcome pressure intolerance, thereby improving patient comfort and adherence. Auto-adjustable positive airway pressure (APAP) devices may be utilised for the long-term management of OSAS and may also assist in the initial diagnosis of OSAS and titration of conventional CPAP therapy. Newer modalities such as C-Flex and A-Flex also show promise as treatment o

Continuous positive airway pressure (CPAP) and noninvasive ventilation (NIV) hold much promise as means to protect the lungs of newborn infants who have respiratory distress from many causes. A wide variety of options are available to the clinician, including CPAP, bilevel CPAP, and both synchronized and unsynchronized noninvasive mechanical breaths. Limited data are available regarding the best ways to use CPAP and NIV in today's NICU environment. This article reviews current information on these modalities, including available options, possible risks, and unanswered questions.

OBJECTIVE: To investigate if uvulopalatopharygopalasty (UPPP) compromised continuous positive airway pressure (CPAP) therapy.
METHODS: Automated continuous positive airway pressure(Auto CPAP) was applied to treat 15 patients with obstructive sleep apnea syndrome(OSAS) whose apnoea-hypopnoea index (AHI) was still higher than 20 after UPPP, and also it was used in the treatment of 32 randomly selected OSAS patients who had not had any treatment before. The treatment results of these 2 groups were compared.
RESULTS: It was found that AHI and apnoea index(AI) between the 2 groups after Auto CPAP

The treatment of choice for obstructive sleep apnoea (OSA) is continuous positive airway pressure therapy (CPAP). Since its introduction in clinical practice, CPAP has been used in various clinical conditions with variable and heterogeneous outcomes. In addition to the well-known effects on the upper airway CPAP impacts on intrathoracic pressures, haemodynamics and blood pressure (BP) control. However, short- and long-term effects of CPAP therapy depend on multiple variables which include symptoms, underlying condition, pressure used, treatment acceptance, compliance and usage. CPAP can alter

Despite the large body of information regarding the beneficial effects of continuous positive airway pressure (CPAP) in infants with respiratory distress syndrome (RDS) data are insufficient at this time to support or refute its use during neonatal resuscitation. An individualized approach to infants with respiratory distress is recommended.

The aim of the study was to measure the difference between the set continuous positive airway pressure (CPAP) value and the pharyngeal pressure reading during CPAP in premature infants with mild respiratory distress syndrome, using two different devices: hood CPAP and the conventional nasal system. The preliminary results suggest that hood CPAP may produce more stable pharyngeal pressure than the conventional nasal device.

PURPOSE OF REVIEW: To present recent data on the role of noninvasive ventilation in the respiratory management of newborn infants.
RECENT FINDINGS: Noninvasive ventilation is growing in popularity but is applied using widely varying devices and settings. Although short-term physiological advantages were reported for bubble and variable-flow continuous positive airways pressure, neither has convincingly shown superior clinically important outcomes. Continuous positive airways pressure may be used as the initial mode of support for very preterm infants but increased rates of pneumothorax in inf

Nasal continuous positive airway pressure (nCPAP) improved arterial oxygenation in patients with sleep apnoea as well as those with acute pulmonary processes such as Pneumocystis carinii pneumonia. Despite an expanding pool of clinical information, little if any attempt seems to have been made to see whether nCPAP alters ventilatory patterns. The effect of nCPAP was assessed by respiratory inductance plethysmography in 14 healthy males. nCPAP reduced respiratory rate (14.3 +/- 1.47 to 9.7 +/- 1.98, p less than 0.0001) but increased tidal volume (0.483 +/- 0.090 to 0.602 +/- 0.140 l, p = 0.01).

BACKGROUND: In 2011, the Neonatal Resuscitation Program (NRP) added consideration of continuous positive airway pressure (CPAP) for spontaneously breathing infants with labored breathing or hypoxia in the delivery room (DR). The objective of this study was to determine if DR-CPAP is associated with symptomatic pneumothorax in infants 35 to 42 weeks' gestational age.
METHODS: We included (1) a retrospective birth cohort study of neonates born between 2001 and 2015 and (2) a nested cohort of those born between 2005 and 2015 who had a resuscitation call leading to admission to the NICU and did n

We studied the acceptability of nasal continuous positive airway pressure (CPAP) therapy in 74 patients with obstructive sleep apnea-hypopnea (OSAH). CPAP was conducted in multidisciplinary treatment, combined with surgery and oral appliances. OSAH was defined by an apnea-hypopnea index > 20/h. Follow-up was 3 to 21 months. Automated titration during full polysomnography was conducted in the hospital using auto-CPAP devices (Autoset T, RESMED Co, Australia). CPAP alone was done in 37 (50%); CPAP with conservative nasal treatment in 25 (34%); CPAP with nasal surgery, endoscopic nose and/or sinu

Sleep apnoea is associated with increased mortality and morbidity. The treatment goal is to reduce the neurocognitive and cardiovascular sequelae. CPAP therapy in sleep apnoea is discussed in two parts in the article. The first part will consider CPAP therapy in the more common form of sleep apnoea (i.e. obstructive or mixed sleep apnoea) and the second part will consider CPAP therapy in central sleep apnoea. Alternative positive airway pressure modalities are discussed. CPAP therapy has been extensively studied and it remains the mainstay of treatment in obstructive sleep apnoea, as it is sti

While continuous positive airway pressure (CPAP) is being increasingly employed for many forms of acute respiratory failure and postoperative hypoxaemia, most CPAP systems are either 'home made' or incorporated in sophisticated ventilators used in intensive care units. This paper describes the development of a continuous low flow CPAP system using a large latex reservoir bag and venturi generated positive end expiratory pressure (PEEP). The system has been successfully used for over twelve months in the general wards, the intensive care unit, accident and emergency department, the recovery war
