Quantitative morphological analysis framework of infant cranial sutures and fontanelles based on CT images.

Characterizing the suture morphological variation is a crucial step to investigate the influence of sutures on infant head biomechanics. This study aimed to establish a comprehensive quantitative framework for accurately capturing the cranial suture and fontanelle morphologies in infants. A total of 69 CT scans of 2-4 month-old infant heads were segmented to identify semilandmarks at the borders of cranial sutures and fontanelles. Morphological characteristics, including length, width, sinuosity index (SI), and surface area, were measured. For this, an automatic method was developed to determi

Renal tubular dysgenesis and very large cranial fontanels in a family with acrocephalosyndactyly S.C. type.

We describe the first case of renal tubular dysgenesis in a newborn female who survived 15 days. The immunohistochemical and lectin binding studies confirmed the lack of proximal tubule differentiation. Electron microscopy showed undifferentiated tubular epithelium. Widely patent cranial fontanels were present in the proposita. The father and sister showed acrocephalosyndactyly Saethre-Chotzen type. The parents were not related and there was no familial history of renal pathology.

Cranial nerve palsy as a delayed complication of attempted infanticide by insertion of a stylet through the fontanel. Case report.

A child suffered a sixth and seventh cranial nerve palsy due to intracerebral insertion of a stylet. The stylet was introduced through the anterior fontanel, most probably in an attempt at infanticide. The migration of the stylet through the brain was monitored because the child was first examined 6 years earlier. At operation the cranial part of the stylet lay in the fourth ventricle, compressing the facial nerve as well as the nucleus of the abducens nerve. The lower part of the stylet had reached the C-5 level.

Pediatric ventriculoperitoneal shunt: a comparative study between anterior fontanel ultrasound-guided versus conventional cranial end insertion.

PURPOSE: Ventriculoperitoneal (V-P) shunt is one of the most common neurosurgical procedures in pediatrics for the treatment of hydrocephalus. Shunt failure is one of the common mechanical complications which lead to major morbidities. This study aims to compare between cranial part insertions of the V-P shunts guided by trans-anterior fontanel ultrasound versus conventional insertion. METHODS: A prospective comparative randomized study was conducted on 60 pediatric patients aged ≤ 2 years who suffered hydrocephalus and allocated into 2 groups. In the first group (n = 30), the cranial parts o

Ultrasound detection of posterior fossa abnormalities in full-term neonates.

Routine cranial ultrasonography, using the anterior fontanelle as acoustic window enables visualization of the supratentorial brain structures in neonates and young infants. The mastoid fontanelle enables a better view of the infratentorial structures, especially cerebellar hemorrhage in preterm infants. Reports on the usefulness and reliability of cranial ultrasonography using the mastoid fontanelle approach for the detection of posterior fossa abnormalities, focusing only on full-term neonates are limited. This article describes the technique of mastoid fontanelle ultrasonography in full-ter

Delayed closure of the anterior fontanelle in Iron Age children from Zaghunluq, northwestern China.

Delayed closure of the anterior fontanelle is often associated with various disorders. However, the phenomenon might be a normal variation. In this study, the anterior fontanelle was investigated in children from the Iron Age Zaghunluq cemetery in Xinjiang, northwestern China. Age at death was estimated by tooth development/eruption in the children. The anterior fontanelle was visually assessed for delayed fusion. Three out of nine children aged between 3-6 years old from the cemetery had signs of open anterior fontanelle. However, there were no signs of craniofacial pathology or developmental

The false fontanel.

In the neonate, palpation of the anterior fontanel is recognized as a simple and reliable means for estimating intracranial pressure. With closure of the fontanel this aspect of the clinical examination is lost. The authors report a series of 15 shunted infants and children in whom a false fontanel was created by making a 2-cm craniectomy in the right parietal region and excising the underlying internal pericranium. This produces a cranial opening which, like the anterior fontanel, can be used for palpation and real-time ultrasound imaging of the brain. By removing the internal pericranium, re

Diagnosis and Management of Suture-Related Concerns of the Infant Skull.

The cranial fontanelles and sutures have several benign variations, including most cases of "early" or "late" closure of the anterior fontanelle, bathrocephaly, overriding sutures, and benign metopic ridging. However, recognizing true craniosynostosis and referring the patient to a craniofacial specialist in a timely fashion are imperative, as minimally invasive options can be offered to most patients younger than 6 months of age. Gaining comfort with the physical examination of an infant with an abnormal head shape is best achieved through experience and pattern recognition and will frequentl

Secondary metopic craniosynostosis after posterior cranial decompression in cloverleaf skull deformity.

Cloverleaf skull deformity or Kleeblattschadel syndrome is a severe condition where multiple cranial sutures are absent and prematurely fused, leading to a trilobate head shape. The remaining open sutures or fontanelles compensate for rapid brain expansion, while the constricted fused calvarium restricts brain growth and results in increased intracranial pressure. Recent data show that early posterior cranial and foramen magnum decompression positively affects infants with cloverleaf skulls. However, long-term sequelae are still rarely discussed. We hereby report a child who developed secondar

Factors Associated with Helmet Therapy Outcomes in Positional Plagiocephaly.

Background: Helmet therapy is considered to be a treatment for infants with positional plagiocephaly. Although some studies suggest that anterior fontanelle (AF) size may also affect treatment outcomes, evidence and influence remain unclear. The aim of this study is to assess the impact of anterior fontanelle size on the effectiveness of helmet therapy, with the goal of determining the optimal timing and patient criteria for treatment. Methods: We conducted a retrospective study of 94 infants treated with helmet therapy for positional plagiocephaly at Kwangju Christian Hospital between January

Three-dimensional CT imaging in pediatric calvarial pathologies.

In children with suspected cranial pathologies, three-dimensional (3D) computed tomography (CT) imaging is superior to other modalities. It can help differentiate actual pathology from normal or variant appearances. Sutures and fontanelles, synostosis, abnormalities of head shape without craniosynostosis, congenital calvarial defects, cranial fractures, bone tumors, and postoperative cranial vault can be assessed easily with 3D CT imaging. We aimed to discuss the common normal, variant, and pathological findings that 3D CT imaging can aid to diagnose as well as explain the usefulness of 3D CT

[A neonate without claviculae].

A 1-week-old girl, term born after a non-complicated parturition, presented with abnormal clavicles. At physical examination small and short clavicles were seen, as well as a widened anterior and posterior fontanelle. An X-ray of the thorax showed hypoplastic clavicles and an X-ray of the skull showed delayed ossification of the bones, hypoplasia of the maxilla and extra teeth. This led to the diagnosis 'cleidocranial dysplasia'.

Anterior Fontanelle Wormian Bone/ Fontanellar Bone: A Review of this Rare Anomaly with Case Illustration.

Wormian bones are a relatively rare skeletal anomaly that present as accessory bone(s) within the sutures of the cranium and even more rarely within the fontanelles. It is believed that they arise from the formation of abnormal cranial ossification centers. Although not extensively reported in the literature, this anomaly is thought to be associated with other anatomical anomalies such as osteogenesis imperfecta, rickets, and other bone dysplasias. When located within the fontanelles, the most likely site of occurrence is the posterior fontanelle. This case report describes a rare incidence of

Wide sutures and large fontanels in the newborn.

Newborn infants with wide cranial sutures and large anterior fontanels, but having normal head circumferences, were prospectively studied and compared with a group of normal controls. The study group with wide sutures was composed of two different groups--one with good calcification of the calvaria, fairly large anterior fontanels, and no Wormian bones; and another with distinct parietal prominences, very large anterior and posterior fontanels with prolonged patency, poor initial calcification of the calvaria, and Wormian bones. Normal head growth and physical and mental development were obser

Concurrent Sagittal Craniosynostosis and Anterior Fontanelle Wormian Bone: Radiological Findings and Surgical Management in an Infant With Epispadias.

Wormian bones are accessory ossicles that typically occur within cranial sutures. Their presence within fontanelles, particularly concurrent with craniosynostosis, represents an uncommon finding that poses unique diagnostic and management considerations. We present a one-month-old male infant born at 35 weeks of gestation, who presented with an abnormal skull configuration. Physical examination revealed scaphocephaly with a palpable sagittal ridge, bilateral temporal hollowing, and concurrent distal shaft epispadias. Computed tomography (CT) with three-dimensional (3D) reconstruction demonstra

The morphogenesis of wormian bones: a study of craniosynostosis and purposeful cranial deformation.

Wormian bones are accessory bones that occur within cranial suture and fontanelles, most commonly within the posterior sutures. They occur more frequently in disorders that have reduced cranial ossification, hypotonia or decreased movement, thereby resulting in deformational brachycephaly. The frequency and location of wormian bones varies with the type and severity of cranial deformation practiced by ancient cultures. We considered the hypothesis that the pathogenesis of wormian bones may be due to environmental variations in dural strain within open sutures and fontanelles. In order to explo
