Pharmacological characterization of dihydromorphine, 6-acetyldihydromorphine and dihydroheroin analgesia and their differentiation from morphine.

The present study examined the pharmacology of dihydromorphine, 6-acetyldihydromorphine and dihydroheroin (3,6-diacetyldihydromorphine). Like morphine, dihydromorphine and its acetylated derivatives all were highly selective mu-opioids in receptor binding assays. All the compounds were potent mu-selective analgesics, as shown by their sensitivity towards the mu-selective opioid receptor antagonists naloxonazine and beta-funaltrexamine. However, the actions of dihydromorphine and its analogs were readily distinguished from those of morphine, differences that were surprising in view of the very

Practical and high-yield syntheses of dihydromorphine from tetrahydrothebaine and efficient syntheses of (8S)-8-bromomorphide.

A practical method for the conversion of tetrahydrothebaine to dihydromorphine in 92% yield is described. The procedure should allow more efficient production of opium products and may be easily modified for large-scale synthesis. The conversion of codeine to (8S)-8-bromomorphide, a potentially valuable intermediate to 6-demethoxyoripavine and derivatives, is also described. The absolute configuration of (8S)-8-bromomorphide was determined by a single-crystal X-ray diffraction study of the hydrobromide salt.

Contribution of dihydrocodeine and dihydromorphine to analgesia following dihydrocodeine administration in man: a PK-PD modelling analysis.

AIMS: It is not clear whether the analgesic effect following dihydrocodeine (DHC) administration is due to either DHC itself or its metabolite, dihydromorphine (DHM). We examined the relative contribution of DHC and DHM to analgesia following DHC administration in a group of healthy volunteers using a PK-PD link modelling approach. METHODS: A single oral dose of DHC (90 mg) was administered to 10 healthy volunteers in a randomised, double-blind, placebo-controlled study. A computerized cold pressor test (CPT) was used to measure analgesia. On each study day, the volunteers performed the CPT b

Postmortem distribution of dihydrocodeine and metabolites in a fatal case of dihydrocodeine intoxication.

A report of a fatal dihydrocodeine ingestion under substitution therapy is given. Quantitation of dihydrocodeine, dihydromorphine, N-nordihydrocodeine, dihydrocodeine-6-, dihydromorphine-6- and dihydromorphine-3-glucuronide was performed simultaneously after solid-phase extraction prior to HPLC analysis, and the analytes were detected using their native fluorescence. Postmortem concentrations of blood samples from different sampling sites as well as from liver, kidney and cerebrum are reported. A hair sample was investigated to prove long-term use of the substitute drug. Site-to-site differenc

Dihydrocodeine Overdoses in a Neonate and in a 14-year-old Girl Who Were Both Genotyped as Cytochrome P450 2D6*1/*10-*36: Comparing Developmental Ages and Drug Monitoring Data With the Results of Pharmacokinetic Modeling.

A high activity of cytochrome P450 2D6 (CYP2D6) reportedly leads to toxicity of dihydrocodeine/codeine by increasing toxic potential of their metabolite dihydromorphine/morphine, which are further metabolized to highly active dihydromorphine 6-O-glucuronide and the less active morphine 3-O-glucorinide but rapidly excreted into urine as water-soluble forms. A case of acute respiratory depression after administration of prescribed dihydrocodeine phosphate (2.0 mg/d divided twice a day for 2 days) to a 1-month-old baby boy genotyped as CYP2D6*1/*10-*36 is described. The case is compared with that

Identification and synthesis of norhydromorphone, and determination of antinociceptive activities in the rat formalin test.

The main objective of this paper is to report the identification and synthesis of norhydromorphone, a novel metabolite of hydromorphone, and its antinociceptive activities when tested in the formalin test as compared to other known analgesics. In addition, we are reporting for the first time the lack of antinociceptive activities of hydromorphone-3-glucuronide, dihydromorphine-3-glucuronide and dihydroisomorphine-3-glucuronide in the rat formalin test. Norhydromorphone was isolated and identified as a metabolite of hydromorphone in a cancer patient's urine. An authentic standard of norhydromor

The interactions of 1-alkyl-4,4-diphenylpiperidines with opiate receptors.

1-Alkyl-4,4-diphenylpiperidines inhibit the binding of mu- and delta-opiate receptor agonists to crude synaptosomal membranes of rat brain. Opiate receptor affinity is dependent on the nature of the 1-alkyl substitution. In displacing [3H]etorphine, [3H]dihydromorphine and [3H][D-Ala2,D-Leu5] enkephalin, the 1-methyl-derivative was most effective (IC50 [3H]etorphine = 9 microM, IC50 [3H]dihydromorphine = 0.71 microM, IC50 [3H][D-Ala2,D-Leu5]enkephalin = 2.1 microM). 1-t-Butyl-4,4-diphenylpiperidine and its 1-isopropyl analogue, used as antiparkinsonism drugs, exhibit distinctly lower inhibitin

The detection of dihydrocodeine and its main metabolites in cases of fatal overdose.

The levels of dihydrocodeine found in impaired individuals and in fatalities show a wide overlap in the ranges. Among other factors, the genetically controlled metabolism of dihydrocodeine should play an important role in dihydrocodeine toxicity. For the first time, the most important metabolites of dihydrocodeine were investigated in femoral blood from three fatal cases by simultaneous determination using HPLC and native fluorescence for detection. The amount of parent drug always exceeded dihydrocodeine-glucuronide formation and dihydromorphine concentrations ranged from 0.16-0.21 mg/L. The

Search for an endogenous ligand for the opiate receptor.

It was investigated whether there is an endogenous factor in the brain which binds to the opiate receptor in neural tissue. Extracts from rat brain were processed in different ways; fractions were assayed for ability to inhibit the receptor binding of dihydromorphine. There was no evidence for high-molecular weight substances or lipid soluble substances with such ability. On the other hand, processing of an acid water extract of brain except the cerebellum (which was negative) yielded an active fraction with receptor blocking activity. This fraction was heat stable, polyionic and had an appare

Opiate receptor mediated effects of IFN-alpha and lymphocyte derived endorphin-like peptides.

Viral infection of lymphocytes induces the synthesis of interferon (IFN)-alpha and immunoreactive corticotropin (irACTH). We have previously shown the irACTH to be antigenically, structurally, and functionally related (if not identical) to pituitary ACTH. Virus infected lymphocytes also synthesize in endorphin as measured by immunofluorescence. The endorphin-like activity was found to be associated directly with IFN-alpha as well as other, lower molecular weight peptide(s) having no antiviral activity. Crude IFN-alpha at 1000 U/ml exhibited opiate receptor binding activity by inhibiting 47.2%

Comparison of mu, delta, and kappa opiate binding sites in rat brain and spinal cord.

The binding characteristics of mu, delta, and kappa opiate sites were studied in rat brain and spinal cord membrane homogenates. Scatchard analysis of 3H-Dihydromorphine, 3H-D-Ala2-D-Leu5-Enkephalin (in the presence of morphiceptin), and 3H-Ethylketocyclazocine (in the presence of morphiceptin and D-Ala2-D-Leu5-Enkephalin) binding sites revealed similar high affinities of these ligands for their respective sites in brain and spinal cord. The majority of binding in brain and spinal cord was attributed to mu and delta sites, with only about 10% of the combined total binding capacity being kappa.

Screening for drug abuse: use of NaCl to increase drug recovery from papers coated with ion-exchange resin.

Use of papers loaded with ion-exchange resins to adsorb drugs from urine specimens resulted in large losses during the procedure. The first step, removal of drugs from urine specimens, was 25-85% efficient. The second step, elution of drugs from paper for further processing, was approximately 40-70% complete. The efficiency of the first step was decreased and the efficiency of the second step was increased by addition of NaCl, except in the case of barbiturates. Presence of salt during elution increased the yield of dihydromorphine by 20%, of methadone by 16%, of amphetamine by 34%, and of chl

Alcohol interactions with brain opiate receptors.

Ethanol, added in vitro to mouse caudate membranes, inhibited high-affinity binding of 0.2 nM 3H-dihydromorphine (3H-DHM) over an ethanol concentration range of 250-1,000 mM. At lower, physiologically-attainable ethanol concentrations (e.g.: 50 mM), 3H-DHM binding was significantly increased. Over the concentration range of 50-1,000 mM, ethanol inhibited 0.5 nM 3H-[D-Ala2,D-Leu5]enkephalin (3H-ENK) binding to mouse caudate tissue, and no stimulation of 3H-ENK binding was noted at any of these concentrations of ethanol. Ethanol inhibits opiate binding in a pseudo-competitive manner and, therefo

Dihydrocodeine: safety concerns.

Dihydrocodeine (DHC) is a semi-synthetic analogue of codeine, which was formed by the hydrogenation of the double tie in the main chain of the codeine molecule - instead of a double bond between carbons 7 and 8 DHC possesses a single bond. DHC is used as an analgesic and antitussive agent and for the management of dyspnea and opioid addiction. Limited data is available on the potency of DHC to other opioids. The analgesic effect of DHC is similar to codeine and approximately twice as potent as tramadol for an oral route. In contrast to codeine and tramadol, DHC analgesia seem to be irrespectiv

Opiate binding sites in the chick, rabbit and goldfish retina.

The characteristics of opiate binding sites in the retina of the chick, rabbit and goldfish have been investigated. In the newly hatched chick retina, 131 fmol/mg of binding sites for [D-Ala2-D-Leu5]-[3H]enkephalin are present; competition studies with the delta selective peptide [D-Thr-Leu5]-enkephalin (DTLET) and the mu selective peptide morphiceptin show that all of the [D-Ala2-D-Leu5]-[3H]-enkephalin binding sites are of the delta subtype. Dihydro[3H]morphine binds poorly to the chick retina; 13.2 fmol/mg of this binding is displaceable by morphiceptin and corresponds to mu binding sites.

Radioimmunoassay of hydromorphone and hydrocodone in human plasma.

A radioimmunoassay for hydromorphone in human plasma was developed using a commercially available morphine-6-antiserum and tritiated dihydromorphine. In the assay, free and bound drug are separated using dextran-coated charcoal. The method quantitates hydromorphone in the 2.5-20-ng/ml range with minimum sensitivity at 1.0 ng/ml. Within-run precision for hydromorphone as shown by the standard error of the estimate of the linear regression equation is +/- 1.01 ng/ml. Between-run precision data for hydromorphone at the 2,5-, 10-, and 20-ng/ml levels gave percent relative standard deviations of 22
