Between 1991-2001, 40 patients underwent esophagectomy without thoracotomy for: caustic esophageal stenosis (26 cases), cervical esophageal cancer (1), lower esophageal cancer (7), and acute post-caustic oesophagitis (2). Thirty-four patients underwent transhiatal esophagectomy, 3 patients had an esophagectomy by "stripping" and in 3 other patients a combination of stripping and transhiatal esophagectomy. Postoperative complications included: injuries of the laryngeal recurrent nerve (2), pulmonary complications (13), anastomotic leakage (5). Two patients died in the postoperative period one f

Results of 56 transhiatal esophagectomies performed for cardioesophageal cancer from 1985 to 1999 are evaluated. Operation was completed by one-stage plastic repair with gastric flap or colon. Manual transhiatal esophagectomy performed in 35 cases led to many intraoperative complications in location of the tumor in the middle and top part of thoracic esophagus, including two fatal. These complications make manual transhiatal esophagectomy in this location of the tumor undesirable. Of it is indicated only in location tumor in the bottom third of the esophagus. In such cases instrumental transhi

Transhiatal esophagectomy without thoracotomy was performed on 40 patients with cancer of the esophagus and two with corrosive stricture. The resection and esophagogastric anastomosis in the neck were done in a single stage. The 30-day mortality was 12%. Complications included intraoperative pneumothorax (71%), transient hoarseness (19%), anastomotic leak (17%) and pulmonary complications requiring respiratory assistance (7%). Two patients required surgery for internal hemorrhage, but there was no bleeding from the posterior mediastinum. Short operating time and avoidance of thoracotomy are th

Esophagectomy remains the gold standard curative therapy for the treatment of esophageal cancer. Despite 125 years of evolution, esophagectomy remains a demanding procedure associated with a 5% to 10% mortality and a 50% morbidity rate. Knowledge of the multitude of techniques possible for performing this complex procedure, as well as the host of associated complications, is vital for the practitioner aspiring to treat this challenging disease.

INTRODUCTION: Currently, the bases for the treatment of esophageal cancer are surgical resection and chemotherapy. Among the various surgical techniques used, minimally invasive esophagectomy (MIE) aims to reduce surgical aggression and cardiopulmonary complications while maintaining basic oncological principles. We present the results of our initial experience with this technique in the treatment of esophageal cancer.
MATERIAL AND METHOD: Fourteen patients with a diagnosis of esophageal cancer were selected to undergo MIE in three stages: right thoracoscopy, laparoscopy, and left cervicotomy

Minimally invasive esophagectomy can be safely performed in selected cases in centers specializing in minimally invasive esophageal surgery.Potential benefits include lessened physiologic insult, with decreased hospital stay and a more rapid recovery to full activity. Drawbacks include the cost of the disposable instrumentation and the steep learning curve. As thoracic surgeons continue to acquire expertise with this procedure, improved results may be expected. Prospective trials with longer follow-up will be required to confirm any advantages of MIE over conventional approaches. Open surgical

A 55-years-old man with a history of alcoholism, hypertension and obesity was diagnosed of epidermoid carcinoma of the middle third portion of the esophagus. He was treated with two cycles of cytostatics with cisplatin and 5-fluorouracil. Due to his poor general health an inability to swallow solids and liquids, he received parenteral nutrition for 20 days using a commercial formula lacking in vitamins and minerals. During distal esophagectomy we observed a tendency to hypotension and severe metabolic acidosis that was unexplained by the hemodynamic profile and that persisted throughout the fi

On the grounds of literature data and personal experience the authors evaluate manual transhiatal esophagectomy and suggest instrumental removal of the esophagus as a possible alternative. Experimental studies performed on 34 cadavers provide evidence that the topographic-anatomical properties of the esophagus allow it to be everted on a probe blindly without opening the thoracic cavity if its wall is not involved in the pathological process. This can be carried out either through the abdomen, i.e. "downwards" or through the mouth, i.e. "upwards" in which case the first method is preferable. T

Esophagectomy without thoracotomy is performed by an abdomino-cervical blunt dissection. A cancer-free margin after resection is only possible after total esophagectomy. The proximal stomach including the lymph nodes at the right and left paracardia and the whole of the lesser curvature are removed with the esophagus. The food canal is restored by splitting the stomach longitudinally and forming a stomach tube from the greater curvature. This stomach tube is pulled up to the neck through the now empty mediastinum and anastomosed. This movement is easy because the blood supply of the esophagus

PURPOSE: The aim of this study is to report late postoperative complications occurring after oesophagectomy for cancer over a 12-month period and to compare the incidence of these complications according to the level of the anastomosis.
PATIENTS AND METHOD: This study included 106 consecutive patients 51% with subtotal oesophagectomy (thoracic anastomosis), and 49% with total oesophagectomy (cervical anastomosis). The two groups were comparable for age, mean weight loss before surgery, life expectancy, number of positive margins, TNM grading, size and tumour differentiation.
RESULTS: Late mo

Esophagectomy is the mainstay for treating esophageal cancers and other pathology. Even with refinements in surgical techniques and the introduction of minimally invasive approaches, the overall morbidity remains formidable. Complications, if not quickly recognized, can lead to significant long-term sequelae and even death. Vigilance with a high degree of suspicion remains the surgeon's greatest ally when caring for a patient who has recently undergone an esophagectomy. In this review, we highlight different approaches in dealing with anastomotic leaks, chyle leaks, cardiopulmonary complicatio

BACKGROUND: We have initially published our experience with the robotic transthoracic esophagectomy in 32 patients from a single institute. The present paper is the extension of our experience with robotic system and to best of our knowledge this represents the largest series of robotic transthoracic esophagectomy worldwide. The objective of this study was to investigate the feasibility of the robotic transthoracic esophagectomy for esophageal cancer in a series of patients from a single institute.
METHODS: A retrospective review of medical records was conducted for 83 esophageal cancer patie

Lewis described a technique for resection of cancer of the midthoracic esophagus in a staged manner in 1946. The same procedure done at one stage has remained a standard technique for resection of a carcinoma involving the thoracic esophagus.

The operation of transhiatal esophagectomy has been revived in recent years, and several series have reported upon some of the advantages of this procedure over esophagectomy by way of a thoracotomy. Because part of the technique involves a "blind" dissection, it is important that the surgeon is completely familiar with the anatomic relations of the esophagus. A careful anatomic review of these relations was undertaken and important anatomic landmarks have been highlighted with the aid of anatomic dissection. Areas of anatomic fixation are found particularly at the tracheal bifurcation, the do

It should be understood that esophagectomy without thoracotomy is not a surgical procedure for the treatment of a particular esophageal lesion but is only a surgical aid. Therefore, it is extremely important to discuss the specific indications for using this technique to treat a particular esophageal disorder. The technique of esophagectomy without thoracotomy should not be used for malignant lesions of the thoracic esophagus except under particular circumstances, such as resection of a carcinoma in an extremely early stage or palliative resection of advanced carcinoma of the thoracic esophagu

Blunt esophagectomy without thoracotomy has been performed in 26 patients: four with benign disease and 22 with carcinomas involving various levels of the esophagus (10 cervicothoracic, one upper third, five middle third, and six distal third). Continuity of the alimentary tract was restored by anastomosing the pharynx or cervical esophagus either to stomach (19 patients) or to a colonic graft (seven patients). Esophageal resection and reconstruction were performed in a single stage in 25 patients, and the esophageal substitute was positioned in the posterior mediastinum in the original esopha

BACKGROUNDS: Esophageal cancer frequently metastasizes to lymph nodes along the recurrent laryngeal nerve (RLN).1 Therefore, it is essential to dissect RLN nodes for curative esophagectomy. Complete RLN node dissection without injury to the organs at the cervicothoracic junction requires experienced techniques. This report describes the cervicothoracoscopic approach, whereby the complete dissection can be safely performed.
METHODS: With this approach, both cervical and laparoscopic procedures are performed in the open-leg supine position before the thoracoscopic procedure is performed in the
