An ethnopsychology of idioms of distress in urban Kenya.

Idioms of distress have become a central construct of anthropologists who aspire to understand the languages that individuals of certain sociocultural groups use to express suffering, pain, or illness. Yet, such idioms are never removed from global flows of ideas within biomedicine that influence how cultural idioms are conceived, understood, and expressed. This article proposes a preliminary model of ethnopsychology described by urban Kenyans, which incorporates local (traditional) and global (biomedical) idioms of distress that are both distinct and overlapping in symptomology and experience

Discourses leading to diagnostic excess: an ethnopsychological study in Italian schools.

The increase in requests for certifications has created uncertainty and changes in the way minors are perceived in the school environment. The way in which schools categorize minors can influence their developmental path, facilitating or compromising their psychological and sometimes even clinical development. This article explores how teachers, parents, and child neuropsychiatry services in Italy identify and communicate a student's difficulty that may lead to a diagnostic certification of special educational needs (SEN). Drawing on an ethnopsychological perspective and employing semi-structu

Experience of nervousness and anxiety disorders in Puerto Rican women: psychiatric and ethnopsychological perspectives.

Analyses of case materials describe variations in the experiences of Puerto Rican women diagnosed as having an anxiety disorder, in treatment with mental health clinicians, physicians, or traditional healers. Their common complaints are examined as core symbolic elements in culturally patterned complexes of meanings focused around personal trauma, stressful life events, personal and social reactions, expectations about treatment, and the course of illness. Many of these women report themselves to be "nervous," to be "sick from nerves," or to have had an "ataque de nervios." "Nervousness" is th

Indigenous-Amazonian Traditional Medicine's Usage of the Tobacco Plant: A Transdisciplinary Ethnopsychological Mixed-Methods Case Study.

Harmful usage of tobacco is a global public health problem associated with adverse health effects and addiction. Yet, in the Peruvian Amazon, the native region of Nicotiana rustica L., this plant is used in remarkably different manners: it is considered a potent medicinal plant, applied in liquid form for oral ingestion to treat mental health problems, a common and ancient healing practice in this region. Using a transdisciplinary field research approach with mixed ethnopsychological methods, this work aimed to report for the first time a case study in this context. The intervention took place

From DSM-I to III-R; voices of self, mastery and the other: a cultural constructivist reading of U.S. psychiatric classification.

The continual process of mental disease classification in U.S. psychiatry is assumed to reflect advancing professional knowledge of these disorders. To date, the American Psychiatric Association has developed four standard classifications, or nosologies, called Diagnostic and Statistical Manuals ('DSMs'). DSM-I, the earliest, appeared in 1952 while the most recent, DSM-III-R, appeared in 1987. This paper employs a cultural constructivist perspective to deconstruct these nosologies and the classificatory process itself. Constructivism's premises, which emphasize culture, history, meaning and th

Rewind, Replay, Rebound - Reflections on Toomela's Call for a Unified Theory of the Psyche Revisiting Vygotsky, Anokhin, and Luria.

Toomela (2025) have provided a useful constructive critique of why Cultural Psychology has not yet succeeded in becoming a "general human psychology". This commentary includes a summary of the points Aaro Toomela states in his article and moves further into the discussion and balance between understanding Cultural Psychology as a system, in terms of Luria, Vygotsky and Anokin, as Toomela suggests, and suggesting importance of "bricoleurs" in terms of Smedslund (2012) in flexible and improvising manners. The main content of Aaro Toomela´s article includes (1) clear definitions on culture and ps

Cultural Countertransference of Young Clinicians Facing a State of Possession: A Cultural Case Study.

Possession states are a well-known but a complex clinical aspect of transcultural psychiatry. How do young clinicians react when facing such symptoms? Starting from the case of a Somali patient suffering from possession states, treated in a transcultural consultation group, we will attempt to explain the cultural countertransference experienced by young clinicians who witnessed a trance. Using the Cultural Formulation Interview, we observed an evolution of the countertransference, and therefore an evolution of the symptom's understanding and of the patient's condition. This clinical case thus

"Thinking too much": A systematic review of a common idiom of distress.

Idioms of distress communicate suffering via reference to shared ethnopsychologies, and better understanding of idioms of distress can contribute to effective clinical and public health communication. This systematic review is a qualitative synthesis of "thinking too much" idioms globally, to determine their applicability and variability across cultures. We searched eight databases and retained publications if they included empirical quantitative, qualitative, or mixed-methods research regarding a "thinking too much" idiom and were in English. In total, 138 publications from 1979 to 2014 met i

Culture, Psychiatry and Epigenetics.

Regarding the methodology of ethnopsychiatric research, for example, on the Vietnamese Mo Mường's burial rituals or the health-promoting and healing effects of the Chinese Tujia dances, three different approaches can be distinguished: phenomenological-descriptive methods, hermeneutic-interpretive techniques and studies on underlying mechanisms such as hypnotherapeutic or neuroendocrine dynamics. Since epigenetics has shown that cultural and artistic experiences can influence gene expression and genetic functioning, even across generations, the article argues for greater consideration of epigen

Neurasthenia, Generalized Anxiety Disorder, and the Medicalization of Worry in a Vietnamese Psychiatric Hospital.

This article examines two forms of the medicalization of worry in an outpatient psychiatric clinic in Ho Chi Minh City, Vietnam. Biomedical psychiatrists understand patients' symptoms as manifestations of the excessive worry associated with generalized anxiety disorder (GAD). Drawing on an ethnopsychology of emotion that reflects increasingly popular models of neoliberal selfhood, these psychiatrists encourage patients to frame psychic distress in terms of private feelings to address the conditions in their lives that lead to chronic anxiety. However, most patients attribute their symptoms to

Cultural continuity, identity, and resilience among Indigenous youth: Honoring the legacies of Michael Chandler and Christopher Lalonde.

This essay is an introduction to the thematic issue of Transcultural Psychiatry in honor of the work of Michael Chandler and Christopher Lalonde, developmental psychologists who made essential contributions to the study of identity and wellness among Indigenous youth in Canada and internationally. We outline their major contributions and illustrate the ways their innovative theory and methods have inspired decades of research, including the recent work presented in this issue, which addresses four broad themes: (1) the importance of a developmental perspective in mental health research; (2) th

"It should not have happened": metaphorical expressions, idioms, and narrative descriptions related to trauma in an indigenous community in India.

Purpose: Psychological trauma can be viewed as a metaphor which originates from somatic medicine and comes from the Greek word "wound". To gain a better understanding of trauma in a culturally sensitive way, the present project aimed to explore alternative metaphors used to describe extreme aversive or catastrophic events. Methods: This ethnopsychological study was carried out among the Adivasis indigenous people in tribal communities in Pune, India. We performed 28 interviews with lay persons and key informants, focusing on collectively shared metaphors. The data were examined using systemati

Tension Among Women in North India: An Idiom of Distress and a Cultural Syndrome.

The existing literature on Indian ethnopsychology has long asserted that somatization is a key aspect of experiences of distress. The study of idioms of distress arose out of work done in India (Nichter in Cult Med Psychiatry 5(4):379-408, 1981), but ironically, little subsequent work has systematically explored idioms of distress in this part of the world. This ethnographic study focused on the term tension (tenśan) and its relation to a cultural syndrome among women in urban North India. This syndrome appears to involve rapid-onset anger, irritation, rumination, and sleeplessness as key symp

Caring for Families Separated by Changing Immigration Policies and Enforcement: A Cultural Psychiatry Perspective.

Recent changes in U.S. immigration policies and enforcement have precipitated a 300% rise in arrests and planned deportations. Although some family members face deportation, other family members may have state-sanctioned status. Such mixed status puts hundreds of thousands of families at risk of forced separation and associated mental health problems. Building on cross-cultural work with refugee families and other groups and on work with families separated by parental incarceration, the authors provide recommendations to guide clinicians working with families who are separated or who fear sepa

Deconstructing culture-bound syndromes.

Ethnopsychiatry comprises a large field of literature written from diverse perspectives, disciplines and orientations. Its status as an interdiscipLinary activity presents formidable obstacles to researchers interested in comparative problems and the development of generalizations. These difficulties can be demonstrated by even a cursory examination of the material written on what has come to be called 'culture-bound syndromes'. In spite of the considerable body of specific studies written about these 'ethnic psychoses', as Devereux termed them, debates continue to swirl over how to define the

Reflechi twòp--thinking too much: description of a cultural syndrome in Haiti's Central Plateau.

A rich Haitian ethnopsychology has been described, detailing concepts of personhood, explanatory models of illness, and links between mind and body. However, little research has engaged explicitly with mental illness, and that which does focuses on the Kreyòl term fou (madness), a term that psychiatrists associate with schizophrenia and other psychoses. More work is needed to characterize potential forms of mild-to-moderate mental illness. Idioms of distress provide a promising avenue for exploring common mental disorders. Working in Haiti's Central Plateau, we aimed to identify idioms of dist

Understanding mental distress in Arcahaie, Haiti: Heterogeneous uses of idioms of distress in communicating psychological suffering.

Research on mental health in specific communities requires careful attention to cultural context and language. Studies on global mental health have increasingly analyzed idioms of distress, or culturally situated ways of conceptualizing, experiencing, and expressing distress. This study examines how idioms of distress are used and understood in Arcahaie, Haiti. The goal was to enrich current understanding of mental health conceptualization and communication by exploring the heterogeneity of common idioms of distress. Interviews with community members (N = 47) explored meanings and perceived ca
